1.Effect of micro ecological preparation on imbalance of intestinal flora in pediatric blood cancer after chemo-therapy

Objective To investigate the effect of micro ecological preparation on imbalance of intestinal flora in pediatric blood cancer after chemotherapy.Methods The clinical data of 60 patients with blood cancer were retrospectively analyzed.All patients were divided into the observation group and control group by different trentment methods,30 cases in each group.The control group were received conventional chemotherapy,the observation group were given Lactobacillus triple viable Bifidobacterium piece(two piece once,tid,4 weeks for a course)on the basis of the control group,the intestinal flora(Enterobacteriaceae,Enterococcus,Bifidobacterium,Lactobacillus,Escherichia coli)were detected before and after chemotherapy,made a record of respiratory,urinary,digestive,skin infections after chemotherapy.Results The Bifidobacterium and Lactobacillus of the observation group after treatment were signifi-cantly higher than before treatment(t =3.947,3.636,all P <0.05),there were no difference in Enterobacter,Entero-coccus,Escherichia coli in the observation group before and after treatment(all P >0.05).The Enterobacter,Entero-coccus,Bifidobacterium,Lactobacillus,Escherichia coli of the control group were lower after treatment than before treatment(t =3.712,3.641,3.722,3.543,3.616,all P <0.05),the Enterobacter,Enterococcus,Bifidobacterium, Lactobacillus,Escherichia coli of the observation group after treatment were higher than that of the control group(t =3.327,3.641,6.378,7.367,3.364,all P <0.05).The respiratory tract infection,cholecystitis,colitis,urinary tract infections incidence of the observation group were 50.00%,6.67%,16.67%,23.33%,respectively,which were low-er than 93.33%,30.00%,63.33%,60.00% of the control group(χ2 =13.871,5.455,13.611,8.297,all P <0.05).Conclusion The intestinal flora may be caused for treatment of chemotherapy,which are the importmant rea-son of various infections after chemotherapy,the Probiotics can reconstruct the ecological balance of the gut,increase the number of probiotics,foster the growth of normal flora,adjust the intestinal flora,reduce the chance of infection, conducive to the successful completion of chemotherapy,which is worthy of clinical application.
2.Efficacy of Enteric-coated Capsule Containing Four Viable Organisms (Bafidobacterium,Lactobacillus,Enterococcus and Bacilluscereus) for Chronic Diarrhea

OBJECTIVE: To validate the efficacy and safety of the enteric-coated capsule of Four Viable Organisms(bafidobacterium,lactobacillus,enterococcus and bacilluscereus) in the treatment of chronic diarrhea.METHODS: In this multi-center,randomized,blind control study,124 patients(trial group) with chronic diarrhea were randomly assigned to receive the enteric-coated capsule containing bacillus bifidus,lactobacillus,enterococcus and bacilluscereus,while the control group(n=80) to receive the capsule of bacillus bifidus,bacillus acidophilus and enterococcus faecalis for 14 days.The days spent for the relief of diarrhea relief and the concomitant symptoms of diarrhea such as pain,bloating,and tenesmus were observed.RESULTS: At 3 days,the remission rate in trial group(41.13%) was higher than in control group(11.25%)(P
3.Diagnosis and differential diagnosis of gliomatosis cerebri with magnetic resonance imaging

ObjectiveTo characterize gliomatosis cerebri on magnetic resonance imaging (MRI) and analyze differential diagnosis.MethodsMRI studies of 12 patients with gliomatosis cerebri were reviewed.ResultsTumors involved at least two lobes of the brain in all patients. Widespread invasion with hyperintensity was noted on T2-weighted MR images. No contrast enhancement occurred.Tumors were confirmed with surgery or biopsy.ConclusionGliomatosis cerebri is best detected with MRI.
4.Three-dimensional reconstruction reduction and design of digital plates for proximal tibia fracture

BACKGROUND:In three-dimensional CT reconstruction model, fracture virtual reduction and internal fixation cannot be achieved, and reasonable operation scheme cannot be formulated. It cannot fuly meet the needs of clinical orthopedics physicians. OBJECTIVE:To explore the methods to perform three-dimensional reconstruction, reduction on proximal tibial fracture and to design digital plate using software. METHODS:Nine specimens of knee joint were built to be A1-3, B1-3 and C1-3 types of proximal tibial fracture according to the AO standard. Al specimens were treated with CT scan. Three-dimensional reconstruction and reduction on nine types of bone fracture models were performed in Mimics. The digital plates and screws were designed in Solidworks. Virtual internal fixation on the reduced three-dimensional models was performed. RESULTS AND CONCLUSION:Al bone fracture models received three-dimensional reconstruction and reduction, and the design for internal fixational plates and screws as wel as virtual internal fixation were accomplished. A3, B3 and C3 types were presented. These results suggest that the digital plates for proximal tibial fracture could be designed in Mimics and Solidworks with a personal computer. Our experience could be used in the real operation.
5.Key techniques of internal fixation for proximal tibial fractures based on digital design and three-dimensional printing

BACKGROUND:Preliminary experiments have performed three-dimensional (3D) reconstruction of proximal tibial fractures, digital steel plate design, the establishmentof the proximal tibia plate standard parts library and the operation simulation of 3D printing. OBJECTIVE:To explore feasibility and accuracy of standard parts library plates and screws in the proximal tibial fracture internal fixation navigation in digital design combined with 3D printing model on the basis of preliminary studies. METHODS:Dicom format images of continuous thin layer CT scanning were colected in 20 cases of proximal tibial fractures, and uploaded in Mimics software for 3D reconstruction and fracture reduction. Plate and screw selected from standard part library wereusedfor virtual fixation. Navigation module with screw channel was designed. 3D printing skeleton, bone plate, and navigation module were used for skeleton model and internal fixation. Screw and plate were placed by navigation. Navigation module card, nail and board position were observed. Postoperative appearance and CT scanning were utilized to assess outcomes. RESULTS AND CONCLUSION:After CT scanning and reconstructionin 20 skeleton models, in combination with appearance, the position of plate, screw insertion point, the direction, length and diameter of the screw were consistent with that in Mimics software. The navigation module and the corresponding proximal end of the tibia were closely bonded with good fitting degree. In the application, card slots and stability were good, and could perfectly guide plate and screw placement. These reuslts suggest that with the aid of navigation module, standard parts library plate internal fixation for proximal tibia fracture has high accuracy. On the basis of digital design and 3D printing, digital internal fixation technology of standard parts library plate is expected to achieve good implant navigation in the department of orthopedics.
6.Digital design of standard parts database for distal femoral fractures treated with plating via three-dimensional printing

BACKGROUND:On CT reconstruction of three-dimensional (3D) model, fracture virtual reduction and internal fixation cannot be achieved, and reasonable operation scheme cannot be formulated. Digital design can fuly meet the needs of clinical orthopedics physicians. Standard parts database can provide the possibility to choose the ideal internal fixation. 3D printing makes the reasonable operation scheme accurate in clinical implementation. OBJECTIVE:To discuss the feasibility, accuracy and minimal invasion of internal fixation in treatment of the distal femoral fracture with digital design of standard parts database by 3D printing. METHODS: (1) Nine adult lower extremity specimens were selected to take continuously thin-layer CT scanning. After Dicom images were imported into the mimics software, the model was established. According to the AO classification, they were classified into A1-3, B1-3 and C1-3 types of distal femoral fracture by virtual design. Internal fixation with plate and screw formed standard parts database virtualy. It was printed out the pilot hole of the navigation module design by three-dimensional printing forming technique. Plate and screw were inserted assisted by the module. X-ray and CT scan were taken postoperatively to access the position. (2) 30 patients with distal femoral fracture were subjected to above fixation. The operation time, intraoperative blood loss and postoperative drainage were recorded. Imaging and curative effects were evaluated during folow-up. RESULTS AND CONCLUSION: (1) Nine samples underwent X-ray and CT scan. 3D reconstruction results revealed plate position, screw entry point, nail direction, length and diameter were consistent with presetting data in Mimics software. The navigation models were designed to fit the lateral bony structure of distal femur. There were good fitting degree, good card position and good stability when the navigation was applied. It could guide plant and screw implantation. (2) In 30 cases, the operation time was (104.63±26.12) minutes, intraoperative blood loss was (121.74±11.49) mL, and postoperative drainage volume was (30.29±6.38) mL. Al patients were folowed up. According to Schagzker criterion, the efficiency of 22 cases was excelent, 6 cases good and 2 cases average, and the excelent and good rate was 93%. The parameters of length, diameter, orientation and angle were consistent with that preoperatively. (3) Internal fixation formed by standard parts database assisted by 3D printing navigation model has advantages of high accuracy, short process, lessened blood loss, high safety, less complications, and precise fixation. Digital design of standard parts database via3D printing navigation module is expected to achieve implant navigation and application.
7.Epidemiological characteristics of brucellosis in Shanxi Province from 2011 to 2015

Objective To explore epidemiological characteristics of brucellosis in Shanxi Province during 2011-2015,and to provide evidences for making control and prevention strategy.Methods Data and surveillance data of disease outbreaks in Shanxi Province from 2011 to 2015 were collected.The retrospective analysis method was used to analyze the population distribution,regional distribution,time distribution,outbreak situation,serological and pathogenic test results of brucellosis.Results During 2011-2015,totally 33 697 brucellosis cases were reported,with an average annual incidence of 18.65/100 000,and one death case was reported.Brucellosis distributed in 119 counties (districts) of 11 cities.The area with most reported brucellosis cases was Datong City.The incidence of the disease had seasonal fluctuation and the incidence peak was from March to July.The male/female ratio of the cases was 3.69 ∶ 1.00 (26 515/7 182).Totally 75.77% (25 531/33 697) were at the ages of 40-< 70 years.The yearly proportion of the cases occurred in the age groups was significantly different (H =146.263,P < 0.05).Farmer was the predominant occupation group among the cases [82.90% (27 935/33 697)].Totally 16 cases of human brucellosis outbreaks were occurred during the five years.The epidemic spread to seven counties in five cities.Totally 144 136 cases received serological test,the positive rate was 5.85% (8 426/144 136),and 6 759 cases were found.Totally 164 strains of Brucella melitensis were isolated in human blood culture samples.All of which were Brucella melitensis biotype 3.Conclusions In recent years,the epidemic situation of brucellosis in Shanxi is severe,the total incidence has presented a rising tendency.There are outbreaks of human brucellosis in some countries.Brucella melitensis biotype 3 is the epidemic strain in Shanxi.
8.Analysis of surveillance results of human brucellosis in Shanxi Province in 2012-2017

Objective To analyze the epidemiological surveillance results of human brucellosis in Shanxi Province from 2012 to 2017,to know the epidemic status of brucellosis,and to provide evidence for prevention and control of brucellosis.Methods Incidence date and surveillance date of disease outbreaks in Shanxi Province from 2012 to 2017 were collected,the retrospective analysis method was used to analysis the "three distribution" of brucellosis,outbreak situation and the results of serological and pathogenic surveillance in the 4 surveillance stations.Results A total of 36 220 brucellosis cases were reported from 2012 to 2017,the average incidence was 16.62/100 000;8 540 brucellosis cases were reported in 2014,with incidence 23.53/100 000;a total of 23 197 cases of brucellosis were reported mainly in Datong,Shuozhou,Jinzhong and Xinzhou,accounting for 64.04% of the province total.The onset was seasonal,and the peak of the epidemic was from March to August,accounting for 67.23% (24 350/36 220).The brucellosis cases were mainly youth (23 084),male (28 317),farmers and herdsman (32 616).In the 4 surveillance stations of the brucellosis,39 140 cases were investigated,of which 10 536 cases did serological test,in which 585 were positive for Brucella (5.55%).The highest positive rate of serological test was 9.50% (226/2 738) which was found in Tianzhen.A total of 626 samples carried out pathogen culture,in which 107 strains of brucellosis were detected,the detection rate was 17.09%,and 106 strains Brucella were melitensis biovar 3 of the total strains except 1 mutant.Conclusions The reported incidence in Shanxi Province is in a decline tendency,but the situation of brucellosis epidemic is still relatively serious.It is suggested that the surveillance work should be strengthened;the epidemic situation of brucellosis should be mastered in time and effectively controlled.
