1.An effect of cerebral blood flow on Broca aphasia patient of cerebrovascular disease by early rehabilitative intervention

Objective To observe the effect of cerebral blood flow on Broca aphasia patient of cerebrovascular disease by early rehabilitative intervention, to evaluate the result of early rehabilitative intervention, to ques the marker evaluating early effect. Methods 41 Broca aphasia patients were divided to rehabilitation groups (23 patients) and control groups(18 patients) according to the sequence of hospitalization, and meanwhile 37 patients of health examination were chosen as health control groups, the patients of rehabilitation groups were treated with drug and rehabilitative intervention but the patients of control group were treated only with drug, the average cerebral blood flow of left middle cerebral artery of both groups and health control group was examined with transcranium doppler before and after treatment. Results The average cerebral blood flow of left middle cerebral artery of rehabilitation group and control group was not different before treatment, the rehabilitation group and control group was less than health control group; the average cerebral blood flow of left middle cerebral artery of rehabilitation group and control group was increased obviously after treatment than that before treatment; the average cerebral blood flow of left middle cerebral artery was not different between rehabilitation group and health control group after treatment, it was different between control group and health control group; the average cerebral blood flow of left middle cerebral artery was increased obviously on rehabilitation group than that on control group after treatment. Conclusions the early rehabilitative intervention can increase the average cerebral blood flow of left middle cerebral artery of Broca aphasia patient accelerate the functional restoration; TCD can be regarded as an early marker to evaluate early effect.
2.Observation of the Effect of Using Ginseng and Astragalus Arbran Decoction with Western Medicine on Type-2 Diabetes

Objective Observe the effect of using ginseng and astragalus arbran decoction with western medicine on type-2 diabetes. Method Divide fifty type-2 diabetes sufferers into control group and treatment group randomly.Sufferers from control group take diformin tablets and acarbose orally; Those from treatment group take ginseng and astragalus arbran decoction additionally on that basis for eight weeks continuously.Compare the changes of blood sugar of the two groups before and after the treatments in the situation of limosis and two hours after dinner. Results control group and treatment group each has a total effective rate of 72% and 98% respectively, significant difference exists (P<0.05). Conclusion Using ginseng and astragalus arbran decoction with western medicine has a better effect on controlling blood sugar.
3.Study on correlation between calsification of ligamentum nuchae and degeneration of ligamentum flavon

[Objective]To study the relationship between ossification of ligamentum nuchae(LN) and degeneration of ligamentum flavon(LF) of cervical spondylotic myelopathy(CSM).[Method]Fifty-six cervical LF samples from CSM patient of C_(3~7) in relation to the stage of ossification of LN samples(experimental group,EG) and 38 samples of corresponding ligament from patient with cervical vertebral trauma(CVT)(control group,CG) were obtained.Thickness of LF was measured,their pathological change was examined by microscopy,content of collagen and kydrolyproline and the ratio of type Ⅰ/ type Ⅱ collagen were determined by Woessners method and setting Salting out method.The correlation among the thickness of degenerative LF,the ratio of type Ⅰ/ type Ⅱ collagen,the content of collagen and hydrokyproline,and arrangement disorder of LF in relation to the ossification stage of LN were analyzed.[Result]In EG,as compared with in CG,there showed a decrease of elastic fibre,decrease of ratio type Ⅰ/ type Ⅱ collagen due to significant increase of type Ⅱ collagen,increase collagen content and disorder arrangement of LF.In relation to the ossification of LN,above changes were more marked in LF of C_(4、5),C_(5、6),than that of C_(3、4),C_(6、7)(P
4.Neurologic evaluation and curative effect facts of cervical spondylotic myelopathy anterior decompression and fusion

[Objective]To study the neurologic criterion and curative effect facts of cervical spondylotic myelopathy.[Method]A prospective study was carried out and 21 cases of cervical spondylotic myelopathy were involved in this investigation to be classified by Nurick grade,JOA score and 40 cent classification.The possibility curative effect facts were analysed.[Result]The patients were followed up for average 11 months.Spinal cord function score were improved by 2~3 grades according to Nurick grade;1~6 cent to JOA score and 7~12 cent to 40 cent postoperation.Height of vertebral,cervical lordosis,centrum posterior decompression being unenough,excess height of centrum were curative effect facts of cervical spondylotic myelopathy.[Conclusion]Nurick classify allow even more contrast,but unsuit to some Cervical spondylotic myelopathy,JOA is simple and convenit,efficienf and across-the-aboard;40 cent assess measure is impersonality but cockamamie.Height of vertebral and centrum posterior decompression being unenough are most important facts about surgery.
5.Effects of RNAi targeting survivin gene on A549 cell radiosensitivity

Objective To construct the RNAi vector targeting survivin gene, in order to observe its effect on lung adenoearcinoma A549 cell radiosensitivity, and explore the possible mechanism. Methods One pair of RNA interference sequence targeting survivin gene was designed according to the eDNA sequence, the recombinant RNAi plasmid pGenesil2-survivin was constructed. After certified by enzyme digestion and sequencing, the pGenesil2-survivin plasmid was transfected into A549 cells. Survivin mRNA and protein was measured by RT-PCR and Western blot, respectively. The apoptosis was analyzed with flow cytometry. The radiosensitivity was measured by clone formation assay. Results The pGenesil2-survivin vector was successfully established. After the pGenesil2-survivin was transfeeted into A549 cells for 48 h, surviving protein and mRNA in pGenesil2 group were not obviously changed compared with that in normal group. They were increased after 5 Gy X-ray irradiation, but obviously inhibited in pGenesil2-survivin group. The cell apoptosis in pGenesil2-survivin and 5 Gy X-ray groups was obviously increased (t1 = 10.63, P < 0.001 ; t2 = 3.75 , P < 0.05), the effect was more manifest in combined group(t = 4.83, P < 0.05). D0 and Dq in normal and pC, enesil2 group were not obviously different, but those in pGenesil2-survivin group were lower. Conclusions RNAi targeting survivin gene could inhibit survivin mRNA and protein expression, enhance the A549 cell apoptosis rate and cell radiosensitivity.

Objective To construct the vector of RNA interference(RNAi) targeting survivin gene and observe its effect combined with X-rays radiation on lung adenocarcinoma A549 cell apoptosis.Methods One pair of RNAi sequence targeting survivin gene were designed according to its cDNA sequence reported in GenBank,the recombinant RNAi plasmid pGenesil2-survivin was constructed.After identified by enzyme digestion and sequencing,the pGenesil2-survivin plasmid was trasfeced into A549 cells.In the experiment,normal group,pGenesil2 group,pGenesil2-survivin group,5 Gy irradiation group and pGenesil2-survivin + 5 Gy irradiation group were set up.The apoptosis of A549 cells was measured by flow cytometry with PI/Annexin Ⅴ and TUNEL,the survivin and caspase-3 expressions were measured by Western blotting.Results Two fragments about 389 bp and 4 206 bp were gotten by KpnⅠand EcoRⅠenzyme digestion,they are the same to expected result,the sequencing result was compared to oligonucleotide chain with DNAssist 2.0,they were equal,these indicated the identification of pGenesil2-survivin vector was right;pGenesil2-survivin was transfected into A549 cells for 48 h,the apoptotic percentage in pGenesil2-survivin and 5 Gy X-rays groups increased obviously(P
7.Hybrid metal-on-metal hip surface arthroplasty in 18 cases: 2 -year follow-up study

BACKGROUND: Metal-on-metal hip surface arthroplasty has improved the abradability for hip joint prosthesis and has the characteristics of normal biological stress transfer.OBJECTIVE: To observe the long-term state of hip-joint function of patients who underwent metal-on-metal hip surface arthroplasty.DESIGN: Follow-up study for cases.SETTING: Department of Orthopaedics, Jiangsu Provincial Corps Hospital of the Chinese People's Armed Police Force; Department of Orthopaedics, Affiliated Hospital of Nantong University.PARTICIPANTS: Eighteen cases (23 hips) who underwent a metal-on-metal hip surface arthroplastyprocedure in the Department of Orthopaedics, Jiangsu Provincial Corps Hospital of the Chinese People's Armed Police Force, and Department of Orthopaedics, Affiliated Hospital of Nantong University between September 2004 and July 2005 were recruited in this study. All cases, aged 28 to 54 years, include 11 males and 7 females. According to the classification of etiology, there were 13 cases of osteonecrosis(16 hips),3 cases of osteoarthritis( 4 hips ),1 case of congenital hip dysplasia (2 hips) and 1 case of posterior trauma arthritis(1 hip ). All cases applied the Conserve Plus resurfacing prosthesis (manufactured by Wright Medical Technology, USA), of which the pattern number of acetabular cup (press-fit depth: 1-2 mm) ranged from 38 mm to 56 mm in the inner diameter and from 44 mm to 62 mm in the outer diameter and that of femoral head cup ranged from 38 mm to 56 mm in the outer diameter. Preoperatively all patients signed the informed consent for the surgery, and the application of this technique also gave the approval of the Ethics Committee of the hospital.METHODS: ①After the epidural and lumbar combination anesthesia was satisfactory, the coxacava was exposed at first and the suitable size acetabular cup coated by hydroxyapatite ceramic was selected to be implanted, to be tightened and to be fixed by press-fit referring to the anatomical position. Subsequently to install the femoral head prosthesis, femoral cup was laid on the ready caput femoris and impacted by the presser to make the metal cup paste close-up with sclerotin when the concocted bone cement was overlaid on the prefabricated caput femoris surface and internal surface of prosthesis. Further, short-term of femoral cup should be kept the conformity with axial ray of the femoral neck. ②Patients were allowed to make the function exercise such as initiative stretch and contract of quadriceps muscle of thigh, passive motion of the knee joint and initiative motion of the knee joint under the non-weight loading on bed. Then they were encouraged to walk with two walking sticks two weeks after operation, progressing to get out of the two walking sticks six weeks postoperatively. All affected extremities were fixated with T-shaped tabula shoes in the abduction position after operation. ③All patients were reviewed with taking the anteroposterior radiographs of pelvis, evaluation of the clinically curative effect on the procedure of metal-on-metal hip surface arthroplasty and biocompatibility between the prosthesis and the host one year and two years after operation. Moreover, Harris score was assessed for all affected hips preoperatively, one year and two years postoperatively.MAIN OUTCOME MEASURES: ①The clinically curative effect on metal on metal hip surface arthroplasty; ②Biocompatibility;③The Harris score for the affected hips; ④The pain status of hip after operation.RESULTS: Eighteen cases were all brought into the outcome analysis at last.①The curative effect on the metal-on-metal hip surface arthroplasty: The femoral component of one case had a varus deformation of 10°six weeks after operation, but such complications as component loosening and femoral neck fractures, etc. Did not occur during the coming follow-up. The locations of rest prosthesis were satisfactory. Substantial radiolucencies were found at the rim of acetabular component (1 and 2 zone) in two hips, respectively one and two years after operation. But there was no evidence of radiolucencies around the short-stem of femoral component.②Biocompatibility: No patients were found to have obvious reactions including renal toxicity, pyretogen and rejection. No radiograph showed signs of loosening, dislocation, heterotopic bone formation, femoral neck narrowing, femoral head necrosis and prosthesis fixation failure, etc.③Harris score for the affected hips: The Harris score of all disease hips was improved from the mean 46 preoperatively to 85 one year after operation to 93 two years postoperatively. Of these,15 hips were excellent(> 90),6 hips good (80-89), and 2 hips fair(70–79).④The pain status of hip after operation: Two patients complained of slight pain, one patient of moderate pain, and no cases of severe pain happened.CONCLUSION: The long-term outcome for hip disease patients who undergo metal-on-metal hip surface arthroplasty is satisfactory.

9.Effects of rutaecarpine on ulcerative colitis in mice

AIM: To evaluate the effects of rutae-carpine on ulcerative colitis and intestinal movement in experimental animals. METHODS: The mouse model of acute colitis was induced by coionic instillation of dinitro-chlorobenzene (DNCB)/alcohol. Rutaecarpine (10, 30, and 100 mg?kg-1 ) were orally administrated to animals for 3 days. Diarrheas, colon weight index, macroscopic damage in the colon were observed and the activity of MPO and the content of PGE2 in colon were also examined . Meanwhile, the inhibitory effects of rutaecarpine on ileal contraction movement in Guinea pig which was induced by acetylcoline or histamine in vitro were examined. RESULTS: The diarrhea and the colon weight index were reduced and the macroscopic damage in the in-flammatory colon was effectively attenuated by rutae-carpine. The elevated level of MPO and PGE2 were also significantly decreased. Heal contraction movement induced by acetylcoline or histamine in vitro was strongly dose-dependent inhibited by rutaecarpine. CONCLUSION: Rutaecarpine can effectively inhibit the inflammatory infiltration, bleeding and proliferation in experimental ulcerative colitis and the movement of intestinal smooth muscle. Rutaecarpine may be an effective drug in treating inflammatory bowel diseases in clinic.
