1.CLINICAL ANALYSIS OF STRESS FRACTURE OF THE PUBIC RAMUS CAUSED BY MILITARY TRAINING: A REPORT OF 66 CASES

To investigate the clinical diagnosis and treatment of stress fracture of the pubic ramus caused by military training, clinical data of 66 cases (18 males and 48 females, ranging from 18~21 years, mean age 19 6 years ) with stress fracture of the pubic ramus were retrospectively analysed. 55 cases were military recruits undergoing basic training, 11 army veterans undergoing intensive goose step training. 66 cases complained of chronic groin pain which increased in activity and relieved while at rest. The symptom occurred 3~10 weeks after the training began. The pain often lasted 1 week ~14 months. 22 cases who sought medical advice in medical team were mistaken for muscular injury. There were 76 stress fractures of the pubic ramus in 66 soldiers. 56 had stress fractures of one inferior pubic ramus, 6 both inferior pubic rami, and four had ipsilateral inferior and superior pubic rami fractures. Stress fracture of the pubic ramus is not rare, but it's quite easy to be neglected or misdiagnosed. The key to its diagnosis is getting a deeper understanding and making an adequate and early imaging examination in light of the clinical data.
2.Characteristics of menopausal symptoms in menopausal women and its influencing factors

Objective To analyze the features of menopausal syndrome and its risk factors in patients from menopausal outpatient clinic,and further to evaluate menopausal syndrome and standardization managenent pathway of treatment.Methods The objects were enrolled in the menopausal outpatient clinic from Apr.2010 to May.2015,completing the self-designed questionnaires through one-by-one interview,including demographic information,and modified Kupperman Index.Results The average age of 907 menopausal women was 51.74+4.94 (aging from 30 to 70 years old).The incidence of (peri-)menopause syndrome was 95.7%,increasing with the prolonging of menopausal duration.The incidence and severity of surgery-related menopausal syndrome were higher than those of natural menopause.The five most frequent symptoms were:fatigue (84.6%) > insomnia (77.3%) > hot flashes sweating (76.3%) > mood swings (72.3%) > joint pains and heart palpitations (72.1%).Other factors such asage,menopausal status etc.also affected the menopausal symptoms.Conclusions Prevalence of (peri-) menopausal syndromes was high and it has many influencing factors.The incidence and severity of surgery-related menopausal syndrome are higher than those of natural menopause.
3.The clinical significance of determining serum paraquat by spectrometry

ObjectiveTo evaluate the reliability and clinical value for detecting paraquat (PQ)concentration in serum by spectrometry. MethodsThe determinations of wave length for detecting serum PQ concentration by ordinary spectrometry and second-derivative spectrometry were carried out. When the second-derivative spectrometry was used for detecting PQ in serum, the linear range and precision for PQ concentration were well defined. The results of serum PQ concentration determined by second-derivative spectrometry and by HPLC (high performance liquid chromatography) were compared in 8 patient with PQ poisoning. A total of 21 patients with acute poisoning after PQ ingestion over 4 hours admitted from October 2008 through September 2010 were retrospectively studied. Patients were divided into two groups as per the serum concentrations more than 1.8 μg/mL or less than that by second-derivative spectrometry on the day of admission. The severity of clinical manifestations between two groups was analyzed with t-test or Fisher's exact probabilities analysis. Results ( 1 ) The absorption peak of 257 nm could not be found by using ordinary spectrometry to detect the PQ concentration in serum. (2) The calibration curve in the 0. 4 ~ 8.0μg/mL range for detecting PQ concentration by second-derivative spectrometry observed the Beer's law (r =0. 996) . The average retrieval rate of PQ was within the range of 95.0%~ 99. 5% with relative standard deviation (RSD) within 1.35%~ 5.41% ( n = 6), and the lowest detection limit was 0. 05μg/mL. (3) The results of PQ concentrations from 8 patients with PQ poisoning detected by second-derivative spectrometry were consistent with those of the quantitative determinations by HPLC ( r = 0. 995,P<0. 01 ) . (4) The survival rate of patients with serum PQ concentration more than 1.8 μg/mL was 22. 2% ,and the incidences of acidosis, oligouria and pneumomediastium in these patients were 55.6%,55. 6% and 77.8%, respectively. These clinical manifestations were significantly different from those in patients with serum PQ concentration less than 1.8 μg/mL ( P < 0. 05 ) . Conclusions ( 1 ) It was inappropriate to take 257 nm as the determination wave length for detecting serum PQ concentration by ordinary spectrometry. (2) The method of second-derivative spectrometry was reliable for detecting serum PQ concentration. (3) Serum PQ concentration detected by second derivative spectrometry could be used to predict the severity of clinical manifestations of patients with PQ poisoning and was an important predictive factor for poor prognosis if the serum PQ concentration more than 1.8 μg/mL after PQ ingestion over 4 hours.

Pancreatic cancer remains a devastating disease with a 5-year survival rate of less than 5%.Recent advances in diagnostic methods and therapeutic approaches have increased the possibility of improving the existing poorprognosis.Recently,compounds from naturalsources receive ample attention as anti-cancer agents.Overwhelming in vitro evidence and clinical trials suggest that curcuminn,isothiocyanate,capsaicin,catechin may reduce the incidence of cancer.
5.Progress in Phospholipase A2 and Its Relevant Chinese Materia Medica

Phospholipase A2 (PLA2) is the speed limit enzyme that liberates the sn-2 fatty acyl chains from phospho-lipids to yield nonesterified fatty acids and lysophospholipids. Its metabolites have a wide range of biological activity in a series of physiological and pathological process such as gene expression, energy metabolism, plasmalemma re-configuration, signal transduction, inflammation, trauma, and etc. This review introduced members of the PLA2 super-family and their functions. And the effect on the activity and/or mass of PLA2 family was summarized with a purpose to promote related research, of which more than 100 items included prescriptions, herbs and their active ingredients in 150 articles were collected from the database.
6.Gene chip screen in mice kidney with acute paraquat poisoning and preliminary analysis of the differentially expressed genes

Objective To study the gene expression profile in mice kidney with acute paraquat (PQ) poisoning and identify key genes related to renal injury.Methods A total of 20 mice (C57BL/6) were randomly (random number) divided into four groups, namely control group (group A, n =5) and poisoned groups (groups B, C, D, n =5/group).In group A, mice were administrated with distilled water (0.01 mL/g weight) while in groups B, C, D were administered with equivalent volume of PQ solution (diluted from 20% to 0.05% with distilled water) dissolved in distilled water via a gastric tube.Mice of group A were sacrificed immediately and mice of groups B and C at 6 h and 24 h after administration of PQ.The gene expression profile changes of kidney tissue were measured by cDNA Arraychip technology.Mice of group D were observed for mortality rate 48 h later.Results The body weights of mice decreased significantly after administration of PQ.The mortality in group D at 48 h after PQ poisoning was 100%.Compared with the control group, totally 1 792 genes with differential expression variations were identified in 6 h group and 24 h group.There were 8 key genes selected through bioinformatics analysis and they were arranged in real-time PCR: Nlrc5 , Serpinb9 , Cd40 , Rnf135 , Dhx58 , Spl 10 , Fcgrl , and Arhgef12.And then, Nlrc5 , Serpinb9 and Rnf135 were under Western blot investigation.The results of PCR and Western blot showed no significant difference to those from bioinformatics genetic analysis.Conclusions The investigation based on genome wide chip in researching related genes of PQ kidney has offered a novel idea in studying pathogenesis of acute PQ intoxication.
7.Imaging findings of pancreatic multiple neuroendocrine tumor:A study of 12 cases

Objective To investigate the imaging features in CT/MR of pancreatic neuroendocrine tumors(PNETs) with multiple lesions and further deepen the understanding of this disease .Methods A retrospective review of 12 PNETs patients′radiological data with pancreatic tumors′numbers≥2 and confirmed by surgery or fine needle aspiration biopsy in Changhai Hospital were conducted .Five cases underwent pancreatic CT plain and enhanced scan , 2 cases underwent MRI plain and enhanced scan , and 5 cases underwent both CT and MRI scan .Results There were totally 46 lesions in 12 patients.There were 29 (63.0%) lesions located in the pancreatic head and neck , and 17(37.0%) lesions located in body and tail of pancreas.The sizes of the lesions ranged from 0.8 to 9.5 cm,and the median size was 2.9 cm.Forty-four (95.7%) of the tumors was round or oval , and 2 ( 4.3%) was lobulated;44 ( 95.7%) mass solid and 2 (4.3%) was cystic.CT plain scan detected punctate , crescent or nodular calcification in 8(17.4%) lesions;enhanced scan found 42 lesions(91.4%) were markedly enhanced in the arterial phase , 2 lesions (4.3%) were markedly enhanced in the pancreatic phase;2 lesions (4.3%) were slightly enhanced and the degree of enhancement was lower than that of the normal pancreas .Four cases (33.3%) had dilatation of pancreatic duct and/or the bile duct, 4 cases (33.3%) had distant organ metastasis, 2 cases (16.7%) had lymph node metastasis, and 3 cases (25.0%) had vascular invasion .Conclusions PNETs can be multiple and vary in the size.Most of the lesions are round or oval solid lesions and the malignant signs for organ metastasis can be found occasionally .In dynamic enhanced scanning , the obvious enhancement of the solid portion in the tumor and the higher enhancement degree than that of normal pancreas is the main characteristic .
8.Role of protocadherin 20 in spinal cord in development of bone cancer pain in rats

Objective To investigate the effects of protocadherin 20 (PCDH20) in the spinal cord in the development of bone cancer pain (BCP) in rats.Methods Thirty-six SPF female Sprague-Dawley rats,weighing 180-200 g,were randomly divided into 4 groups (n =9 each):sham operation group (group S),BCP group,lentivirus control group (group LC) and PCDH20 siRNA lentivirus group (group P).Control lentivirus and lentivirus containing PCDH20 siRNA 4 μl were injected into the ipsilateral spinal cord in groups LC and P,respectively.One week later,BCP was induced by injection of Walker 256 breast cancer cells into the upper segment of bone marrow of right tibia.The mechanical paw withdrawal threshold (MWT) was measured at 1 day before injection of lentivirus (T1),1 day before BCP (T2),and 7,14 and 21 days after BCP (T3-5).Three rats in each group were sacrificed after measurement of the MWT at 21 day after BCP and the tibia on the operated side was obtained for examination of invasion of the cancer cells with light microscope.The spinal cord was removed for determination of the expression of PCDH20 and postsynaptic density 95 (PSD95) protein (by Western blot) and mRNA (by RTPCR).Results In groups BCP,LC and P,the cancer cells grew out of the bone and destroyed the cortical bone seriously.Compared with group S,the MWT was significantly decreased at T3-5 in groups BCP,LC and P,the expression of PCDH20 and PSD95 protein and mRNA was up-regulated in groups BCP and LC,and the expression of PCDH20 was up-regulated in group P (P < 0.05).Compared with BCP group,no significant change was found in the MWT and expression of PCDH20 and PSD95 protein and mRNA in group LC (P > 0.05),and the MWT was significantly increased at T4,5 and the expression of PCDH20 and PSD95 protein and mRNA was down-regulated in group P (P < 0.05).Conclusion PCDH20 is involved in the development of BCP through regulating the expression of PSD95 in the spinal cord and adjusting the function of excitatory synapse in rats.
9.Observation of oral pharynx pain treated with durogesic in malignant tumor patients during radiochemotherapy

Objective To observe the efficacy and adverse reaction of Durogesic (thansdermal fenfanul) for analgesia in patients with malignant tumors, who occur moderate or severe pain from mucosal response during radiochemotherapy. Methods 50 patients were divided into two groups randomly. The treatment group received Durogesic and normal therapy, the contrast group received normal therapy. The analgesic effects, the quality of life and the side effects after medicine were observed. Results The pain remission rates of treatment group and control group were 100 % (25/25) and 52 % (13/25), respectively (χ2 =15.78, P < 0.01)[(9.66±0.35), (10.01±0.34) min]and[(18.12±0.46), (14.68±0.19) min].The quality of life in treatment group was improved significantly (t = 8.11, P <0.01).The main adverse reactions of Durogesic included constipation, itch of skin, drowsiness, nausea, vomiting and so on. All of adverse reactions were tolerable. Conclusion Durogesic for analgesia have high curative effect and slight adverse effects in pain of oral cavity and laryngeal pharynx in patients with malignant tumors during radiochemotherapy which make patients complete radiochemotherapy successfully.
10.Effects of Acute Exposure to Mild or Moderate Hypoxia on Human Psychomotor Performance and Visual-reaction Time

Objective The aim of this study was to determine whether psychomotor performance and visual reaction time were affected by acute exposure to mild or moderate hypoxia. Method Eighteen healthy male volunteers performed finger tapping, simple reaction time(SRT) and 4-choice reaction time(CRT) tests at simulated altitude of 300 m (control),2800 m, 3600 m and 4400 m for 1 h in a hypobaric chamber. Result SaO2 decreased from 98%(control) to 90%,82% and 74% respectively at the various altitudes. All the performance parameters showed no significant change after exposure to 2800 m for 1 h relative to ground level(P>0.05). However the mean reaction time of 4-CRT under 3600 m prolonged and performance decreased as compared with baseline value(P<0.05), and the performance decreased further under 4400 m(P<0.01). No significant difference was found in finger tapping and SRT even under exposure to 4400 m for 1 h.Furthermore, no decrease in correct rate were observed at any altitude (P>0.05). Conclusion The results from this study demonstrated that there were no measurable impairment of visual reaction time and psychomotor performance under exposure to an altitude of 2800 m for 1 h. However, adverse effects on psychomotor performance were observed under 3600 m and over.
