BACKGROUND:The incidence of femoral head necrosis after internalfixation in femoral neck fracture is at highlevel, but the risk factors are stil under discussion. OBJECTIVE:To retrospectively propose and analyze the relative risk factors of femoral head necrosis after internal fixationwithfemoral neck fracture in patients younger than60years old. METHODS:Data from199 patients with femoral neck fractures, younger than60years,undergoingclosed reduction and internal fixation were analyzed retrospectively.Seven factors including age, sex, body mass index, Garden classification, fracture seasons, reduction quality and weight-bearing time after internal fixation were statisticalyanalyzedto evaluate their correlation with femoral head necrosis. RESULTS AND CONCLUSION:The incidence of femoral head necrosis was 13.1%amongthe199 cases. Body mass index, Garden classification, fracture seasons, reduction quality were significantly associated with femoral head necrosis. The age, sex and weight-bearing time wereinsignificantly associated with femoral head necrosis. In multivariate logistic regression analysis, Garden classification, reduction quality and fracture seasons (winterversussummer) hadoverteffectson development of femoral head necrosis. These results suggest that Garden classification and reduction quality make an important effect on the healing of femoral neck fracture. In addition, fracture in winter may be the risk factor of the femoral head necrosis after internal fixation in femoral neck fracture.

Objective To investigate the changes of the activity and the distribution of the rat cardiac calcineurin (CaN) which is the signalling molecule of the hypertrophy induced by the external aldosterone(Ald), and to detect the mechanism of the regulation of CaN. Methods 21 male wister rats were randomly divided into 3 groups: group Ald(7 rats), treated with Ald 18?g/d, peritoneal injection (i p) for 4 weeks; group spironolactone (spiron) (7 rats), treated with Ald and with spiron 20mg?kg -1 ?d -1 oral for 4 weeks; and group control (7 rats). The plasma concentrations of Ald, angiotensin Ⅱ(Ang Ⅱ)and endothelin(ET), blood levels of NO, the tissue activity of the CaN and distribution of CaN A? in the myocardium of the left ventricle, were detected in the rats by the radioimmunoassay , by the nitrate assay, by the chromophoric assay and by the immunohistochemistry, respectively. Results The concentration of the plasma Ald increased by 1 62 times, the blood NO - 3 concentration was deceased, the ratio of Ald over NO - 3( Ald/ NO - 3) increased by 2 95 times, and the cardiac tissue levels of CaN activity was were 1 47 times higher, in the rats treated with ald than in the normotensive rats. CaN A? distributed over cardiac cytoplasm, and the positive dyeing of the local CaN A? in the cytoplasm was thick near the cell membrane in the rats treated with Ald. The treatment with spiron could decreased the blood levels of the AngII?Ald/ NO - 3?ET/NO - 3?AngII/ NO - 3 and the tissue activity of myocardial CaN, positive dyeing of the local CaN A? in the cytoplasm, and increased blood NO - 3 concentration. Conclusion The aldosterone may be the main stimuli factor which can damage the balance of the blood active factors, and then make a novel cardiac hypertrophic signalling molecule calcineurin activated and redistributed in the cytoplasm of the rat left ventricular myocardium

BACKGROUND: Patient subjective satisfaction is of great significance for the clinical use of implant denture.OBJECTIVE: To investigate the patient satisfaction with single or bridge crown for repair of consecutive missing posterior teeth in the maxilla.METHODS: We selected 203 patients with consecutive missing of maxillary first and second molar teeth, who had received single (n=99) or bridge (n=104) crown for implant restoration 1 year ago. All patients were surveyed with a subjective satisfaction questionnaire and visual analogue scale.RESULTS AND CONCLUSION: There was no significant difference in sex and age (35-64 years) between the two groups. Food impaction ratio and crown off ratio were higher in the bridge crown group than the single crown group.Noticeably, most patients did not have the habit of using dental floss. The overall satisfaction scores were 88.96% in the single crown group and 85.32% in the bridge crown group, respectively. Four indexes with less than 80 scores in the two groups were that I dare to use the implant teeth to bite hard food, my implant teeth are easily cleaned, it takes me less time to clean my implant teeth than natural teeth, and I think the dental implant price is acceptable. Results from the analysis of variance showed that food impaction influenced chewing effect (P=0.047), made the use of dental floss difficult (P=0.031), made patients discomfort (P=0.038), and the use of dental floss could enhance the cleaning effect on dental implant (P=0.019), and largely reduced brushing bleeding (P=0.029). In conclusion, patients with missing maxillary posterior teeth can obtain higher satisfaction after single crown or bridge crown repair. However, food impaction and crown off become more serious after repair with bridge crown, while the repair with single crown exhibits better performance.

Objective To evaluate the diagnostic value of dobutamine?stressed cardiac morphing ( CM) MPI ( CMMPI) for coronary artery disease ( CAD) in middle?aged patients. Methods A total of 97 middle?aged patients (52 males, 45 females, age 40-65 (52.98±7.74) years) from March 2011 to Febru?ary 2015 with chest distress, cardiopalmus and chest pain symptoms ( suspected CAD) were retrospectively studied. Informed consents were signed by patients before examination. All patients underwent one?day dobu?tamine?stressed/rest CMMPI and CAG within one month. The diagnostic value of CMMPI was assessed and compared with CAG. Results CMMPI revealed 42 abnormal cases and 9 normal cases in 51 cases with cor?onary artery stenosis by CAG. CMMPI found 11 abnormal cases and 35 normal cases in 46 cases with normal coronary artery by CAG. In comparison with CAG, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of CMMPI were 82.35%(42/51), 76.09%(35/46), 79.38%(77/97), 79.25%(42/53)and 79.55%(35/44) respectively. The side effect was slight and transient with an inci?dence of 82.47%(80/97). Conclusion The dobutamine?stressed CMMPI has an important value for the diagnosis of CAD and the assessment of myocardial damage in middle?aged patients.

BACKGROUND:Cel purification can eliminate the biological variability of cel s, providing new insight into cel regeneration therapy. OBJECTIVE:To study the Influence of CD133+cel s on human umbilical cord blood mononuclear cel transplantation for treatment of heart failure. METHODS:Human cord blood mononuclear cel s were isolated using lymphocyte separation medium method, and CD133+and CD133-cel s were sorted using immunomagnetic beads at a cel density of 1×108/L. Forty Sprague-Dawley rats were randomized into five groups:sham group, model group, CD133+cel group, CD133-cel group and mononuclear cel group. Animal models of heart failure were made using intraperitoneal injection of isoproterenol in al the groups except for the sham group. Rats in the CD133+cel group and CD133-cel group were given 1 mL CD133+cel s plus 1 mL PBS and 1 mL CD133-cel s plus 1 mL PBS via the tail vein, respectively. Rats in the mononuclear cel group were given 1 mL CD133+cel s plus 1 mL CD133-cel s via the tail vein, and those in the sham and model groups given 2 mL PBS via the tail vein. After 4 weeks, cardiac pathology, degree of myocardial fibrosis and colonization of CD133+cel s in myocardial tissues were observed. RESULTS AND CONCLUSION:Hematoxylin-eosin staining showed that myocardial tissues arranged disorderly in the model group, but regularly in the sham group;myocardial disorders were mildest in the CD133+cel group, successively fol owed by the mononuclear cel group, and severest in the CD133-cel group and model group. Masson staining showed that in the model group, col agen fibers were proliferated, arranged irregularly and even broken, while in the sham group, the col agen fibers were less in number and arranged in order. Additional y, there was less reduction in col agen fibers and milder myocardial disorders in the CD133+cel group compared with the other groups. Area of col agen fibers was increased significantly in al the groups except for the sham group (P<0.05), but this increment was the minimal in the CD133+cel group. Findings from immunohistochemistry and immunofluorescence staining showed that there were no CD133+cel s in the myocardial tissues of rats. Therefore, our data indicate that compared with the mononuclear cel transplantation, CD133+cel transplantation exerts superiorities in relieving myocardial damage and reducing myocardial fibrosis. However, CD133+cel s are not colonized in the myocardial tissue after transplantation.
