2.Enhancement Patterns of Hepatic Metastasis from Stomach Cancer at Multi-phase Incremental Bolus Dynamic CT.

PURPOSE: The purpose of our study was to characterize the enhancing patterns of hepatic metastasis from gastric adenocarinoma using multi-phase incremental bolus dynamic CT with obtained both in early and late phase contrast scan and to evaluate the its advantages. METHODS AND MATERIALS: Of 33 cases of multi-phase incremental bolus dynamic CT with proved hepatic~metastasis from gastric adenocarinoma, we classified dynamic enhancement patterns as three types according to early phase contrast enhancement, and then analized the late phase contrast enhancement, more metastasis detection, segmental abnormal arterial perfusions and correlation between pathologic type. RESULTS: Type I (totally hypodense lesion) was in 18 patients(55%), type II (peripheral high density area) was seen in 13 patients (39%), and type III (near totally hyperdense lesion) was seen in two patients(6%). But in late phase, masses showed totally hypodense area in 26 patients(79%), central high with peripheral low density area(PLDA) in six patients(18%) and totally isodense in one patient(3%). More metastatic masses were detected with early phase in 11 patients(33%) as compared with late phase contrast. Segmental arterial hyperperfusion around the lesions, which could represent intrahepatic portal branch invasion, was seen in 10 patients(30%). There was no correlation between pathologic type and enhancement pattern of lesions. CONCLUSION: Multi-phase incremental bolus dynamic CT could represent variable hemodynamic changes of hepatic metastatic masses and was useful to evaluate the qualitative and quantitative analysis of hepatic metastases.

4.Investigation on the distribution of weak D and Del phenotype in unrelated blood donors with negative Rh(D)

Objective To investigate the distribution of weak D and Del phenotype in unrelated blood donors with negative Rh (D). Methods Four hundred and nineRhD (-) unrelated donors were screened by the saline agglutination method. Type weak D was detected by the indirect antiglobulin test, and type Del was detected by absorb radiation method. Results In 409 RhD (-) samples, 27 cases (6.61%) of type weak D were checked out, and 61 cases (14.91%) were type Del and 321 cases (78.48%) were corroborate for RhD (-), In the confirmed RhD (-) blood donors, phenotype ccee was most (49.14%), followed by Ccee (23.47%), People with phenotype ccEe accounted for 4.16%in type weak D, followed by Ccee (1.71%). Ccee accounted for 10.02%in type Del, and Ccee accounted for 1.71%. Conclusion RhD (-) donors screened by regular testing should adopt a more sensitive test for verifing type weak D or Del. In order to ensure the security of blood transfusion, people with type weak D and Del should be regarded as RhD positive blood donors, and the RhD negative people deemed to be recipients.
5.The Significance of Postangiographic CT for Differentiation of Hepatic Masses.

PURPOSE: To identify the longterm hemodynamics of various hepatic masses and to determine any differential findings by using postangiographic CT performed with increased amount of contrast media and time than conventional contrast CT. MATERIALS AND METHODS: 50 confirmed masses consisted of 22 hepatocellular carcinoma, 10 cholangiocarcinoma, 9 metastasis, and 9 cavernous hemangioma were included. The changes of the density of internal viable tumor portion relative to surrounding normal liver parenchyma in postangiographic CT from that in conventional CT were classified as 3 patterns; no specific changes, increase, or decrease. RESULTS: in 22 cases of hepatoceilular carcinoma, six cases showed no relative density change, four cases increase, and twelve cases decrease. In ten cases of cholangiocarcinoma, one case showed no change, nine cases increase. In nine cases of metastasis, four cases showed no change, five cases increase. In nine cases of hemangioma, all cases showed increase. CONCLUSION: In postangiographic CT which emphasize the significance of postequilibrium and delayed phase, other hemodynamic changes undetected in angiography could more easily be comprehended. Considering the differing amount of consumed contrast media and time duration, and with reference of other imaging modalities, differential diagnosis of hepatic masses based on longterm hemodynamics could easily be made.

Objective To investigate the clinical efficacy and safety of transurthral electrovaporization of the prostate(TUVP) for benign prostatic hyperplasia( BPH) at high risk.Methods Forty-eight patients with BPH at high risk were treated with transurthral electrovaporization of the prostate(TUVP).The clinical data and therapeutic results were measured.Results All patients went through the perioperiative period safely and had been followed up for 3 to 14 months.Postvoid residual ( PVR) , the International Prostate Symptom Score (IPSS) and quality of life (QOL) before operations were (97.5 ± 16.9) ml, ( 28.4 ± 2.3 ) score and (5.5 ±0.6) score respectively.Three months after operation ,PVR ,IPSS and QOL were( 30.2 ± 12.3 ) ml, (8.2 ± 1.3 ) score and( 1.9 ±0.5) score respectively,there was significant difference between them(t =22.31,53.16,31.94,all P<0.05).Conclusion TUVP is an effective and safe method in treating BPH patients at high risk.
9.The Experimental Studies on Cardiac Glycoside(Ouabain) and Electrolytes.

Acute digitalization with ouabain were performed 28 times in 20 intact Mongrel dogs, who were devided into four groups : i.e. 1) Control group; Acute digitalizations were performed to observe the changes of the plasma potassium and sodium concentrations and the concomittent electrocardiographic findings. 2) Group 1;-Electrolyte solution, either KCl or NaCl, was infused during acute digitalization to observe the influences of these ions on the actions of cardiac glycoside, especially arrhythmia producing action. 3) Group 2 ; -The K depleted group and the Na depleted group was each made by the measure of diet control and the usage of oral diuretics to observe the influences of the state of depleted electrolyte (K or Na) on the action of cardiac glycoside during acute digitalization. 4) Group 3 ; -The intravenous infusion of either KCl or NaCl solution was introduced as the cardiac arrhythmia by the ouabain was produced to observe the effects of these ions on the digitalis induced arrhythmia. The results obtained were summarized as follows. (1) The arterial plasma concentration of potassium was increased during acute digitalization. The rise occurred at early stage, and the maximal increase of potassium was observed at the ventricular tachycardia. The average increase was 0.65 mEq/L as compared to control value (p<0.05). The arterial plasma concentration of sodium was reduced during acute digitalization. The fall occurred following the rise of plasma K level. The maximal reduction of sodium was observed after ventricular tachycardia, and the average decrease was 5.2 mEq/L as compared to control value (p<0.05). (2) Rapid increase up to toxic level of the plasma potassium concentration occurred occasionally during acute digitalization in the group with KCl infusion. This result was best explained due to the inhibitory action of cardiac glycoside on the K transport. (3) The interesting change was on T wave, which showed the peaking at late portion. This change occured in 60.6% during digitalization with ouabain, and paralleled mostly, but not consistently and even sometimes inversely, with the shift of plasma potassium concentration. It is tempting to assume that the change of T wave was resulted from an altered potassium gradient across the myocardial cells rather than a ssuming the changes of the plasma K level. (4) Just prior to intoxication, the marked prolongation of PR interval and ST depression were observed in about 70~80% of the cases studied. These changes might be applicable to a clue of the cardiac glycoside overdosage. (5) The intravenous infusion of KCl suppressed markedly the arrhythmia producing action of the cardiac glycoside, and resulted in prompt and dramatic abolishing, of digitalis induced arrhythmia. The intravenous infusion of KCl solution, however, produced A-V block or dissociation occasionally. This finding would be likely resulted from the additive or synergistic action of K and cardiac glycoside in suppressing A-V conducting system. (6) The NaCl infusion affected nothing to the arrhythmia producing action of cardiac glycoside. (7) The K depletion reduced markedly the threshold of the heart to the toxic effects of cardiac glycoside. (8) Potent diuretics in the state of deficient diet would produce clinically significant hypopotassemia and hyponatremia or both. The inducced hypopotassemia may provoke serious cardiac arrhythmia in the digitalized patients or animals.
