1.Clinical Analysis of 78 Cases of Acute Pulmonary Thromboembolism

The clinical data of 78 inpatients with PTE were reviewed.The most of PTE patients had the lower limb deep phlebothrombosis.The most common risk factors were age over 60 years old,chronic diseases of heart and lung,surgical operation,diabetes,etc.PTE has a wide range of clinical presentation and non-specific.D-dimmer measure had high sensitivity.The spiral computed tomography showed the positive rate of PTE was 95.7%.Positive treatment of thrombolysis and anticoagulation could reduce the mortality.
2.Effect of elemene on patients with advanced liver cancer of leptin, AFP and liver function

Objective To investigate the effect of elemene on leptin, AFP and liver function of patients with advanced liver cancer.Methods 73 cases with advanced liver cancer of our hospital were selected and according to the random number table method were randomly assigned for two groups:control group (37 cases) were given routine clinical therapy;experimental group (36 cases) were treated on the basis of the control group with elemene injection.leptin, AFP ,CRP and liver function were compared after the treatment.ResuIts Compared with control group post-treatment, the leptin level in the experimental group was higher, CRP was lower(P<0.05); the serum level of AFP, CEA and CA199 were lower (P<0.05), the ALT, TBIL, AST, PT were lower ( P<0.05 ) , and the total effective rate was significantly higher ( P<0.05 ) .ConcIusion Elemene injection can improve liver function, elevate serum leptin level, reduce the levels of CRP, tumor marker levels and improve the quality of life .
3.The Influence of Pregnancy Outcomes with Estradiol Valerate in Addition during Luteal Phase in IVF- ET Cycles

Objective To evaluate the influence of pregnancy outcomes with different dosage of estradiol valerate in addition to progesterone for luteal supplementation in IVF-ET cycles. Methods From February 2011 to October 2012, 362 patients undergoing IVF-ET in the second affiliated hospital of Kunming Medical University were selected. The patients were randomly assigned into three groups. Since oocyte retrieval, all of them were injected luteal supplementation with progesterone in oil 60 mg/d, im. Group A as control group (112 cases) did not add with pentanoic acid estradiol;Group B for the experimental group (122 cases), added pentanoic acid estradiol 4 mg/d after embryo transfer;Group C for the experimental group (128 cases) added pentanoic acid estradiol 6 mg/d after embryo transfer. Finally the pregnancy outcome and implantation rate were compared for each group. Results There were no significant differences between group A and group B, group B and group C in the rate of embryos grow and pregnancy rate ( >0.05) . The embryo planting rate and pregnancy rate have significant difference between Group A and group C ( < 0.05) . Conclusion 6 mg/d fill estradiol valerate for luteal supplementation may improve implantation rate and pregnancy rate after IVF.
4.Progress of treatment for status epilepticus in children

Status epilepticus(SE)is one of the most common medical emergencies in children.There has been some recent understanding of this fife-threatening condition with reference to the definition,forms of ignorable seizure,drug treatment and role of EEG monitoring.This review discusses the current information regarding the definition,diagnosis,treatment of status epilepticus and the latest advance on management of refractory SE in children.
5.Establishment of HPLC method for determination of SN38-PA and SN38 in rat plasma

Objective To develop an HPLC method for the simultaneous determination of 7-ethyl-10-hydroxycamptothecin-10-palmitic acid ester(SN38-PA)and its active metabolite 7-ethyl-10-hydroxycamptothecin(SN38)in rat plasma. Methods The inter standard was 10-hydroxycamptothecin. The protein in plasma was precipitated with methanol after acidification with formic acid. SN38-PA and SN38 were separated on Agilent C18 column(4.6 mm×250 mm,5μm) with gradient elution by using the mobile phase of methanol-0.2% formic acid solution. The flow rate was 1 ml/min. The detection wavelength was set at 372 nm. The column temperature was maintained at 30℃. Results The linear ranges for SN38-PA and SN38 were 0.25-62.5(r=0.9998) and 0.05-12.5 μg/ml (r=0.9997) respectively. The limits of quantification were 0.18 and 0.04 μg/ml, respectively. The average relative recovery of SN38-PA and SN38 were 95.89% and 97.03%. The average absolutely recovery of SN38-PA and SN38 were 99.54% and 99.84%. The RSD for intra-day and inter-day were both less than 3%. Conclusion The method is fast, convenient, accurate and sensitive, so it can be used for determination of SN38-PA and SN38 in vivo.
6.Protective effect of apigenin in acute cerebral injury of rats

Objective To study the inhibition of iron-dependent lipid peroxidation(IDLPO)by apigenin on rat cerebral homogenate in vitro and to evaluate the neuroprotective effects of apigenin on acute transient focal cerebral ischemia-reperfusion injury in rats.Methods In vitro IDLPO on rat cerebral homogenate was induced with ferrous sulfate,the incubation mixture was observed at three various concentration of apigenin and deferoxamine(DFX),and malondialdehyde(MDA)level was assayed by the 2-thiobarbituric acid(TBA)test.In vivo,the transient focal cerebral ischemia-reperfusion model in rats was established with insertion of thread embolish into middle cerebral artery.In experiment groups the neurological behavior scores,TTC stain of brain slices,and neurocyte morphology were observed.The homogenate of left hemisphere was collected for investigating the effect of apigenin on content of MDA and activity of superoxide dismutase(SOD)in 24,48,and 72 h.Results MDA was reduced in three various concentration of apigenin on rat cerebral homogenate(P0.05).In vivo,abnormal neurological behavior scores existed and typical cortical infarct lesions were found by TTC stain in both apigenin and model groups.An obvious intracellular and intercellular edema and vacuolization were found in the cerebral cortexes and hippocampuses in model group.There was karyopycnosis in glias and neurons.However lesion was alleviated in apigenin group.The MDA contents in both apigenin and model groups were increased greatly compared to ones of Sham-operated group,respectively(P

9.Clinical manifestations, neuroimaging features, treatment, and follow-up in four pediatric patients with glutaric aciduria type Ⅰ

Objective To investigate the clinical features,magnetic resonance imaging (MRI),treatment,and follow-up of patients with glutaric aciduria type Ⅰ (GA-1).Methods Four pediatric patients with GA-1 diagnosed in our hospital were included in this study.They were treated with special diets and carnitine supplements.MRI and tandem mass spectrometry (MS/MS) were performed,and the mental development indices were measured.Results GA-1 was confirmed 2 months,13 months,4 months,and 7 months after birth.Seizure had been observed before the disease diagnosis in three patients and disappeared after treatment.In all four patients,T2-weighted brain MRI showed frontotemporal atrophy or hypoplasia and enlarged subarachnoid space in the sylvian fissures and anterior to the temporal lobes.Diffusion weighted imaging revealed high-density lesions over both the putamen and globus pallidus.The patients were followed up for 4 to 5 years.Plasma amino acids and acylcamitine profile were monitored every 3-5 months.The mean C5DC level and C5DC/C8 were kept the higher limits of the normal ranges,especially in case 3.During the follow-up,the body weight was at-2 SD-0 and the height at-1 SD-0.Intellectual development test showed that case 1 and case 4 had mildly abnormal intelligence,whereas case 2 and case 3 had extremely severe intellectual disability.Gene test confirmed the presence of gene mutations in all four cases,including IVS10-2A > C homozygous mutation in cases 1,3,and 4 and [IVS10-2A > C] + [c.245G >c(p.Arg82Pro)] hybrid mutation in case 2.Two female children were smoothly enrolled by local kindergarten,while two male children were unable to walk alone due to delayed motor development and spastic paralysis.Conclusions The phenotype of GA-1 patients is not remarkably correlated with its genotype correlation.Newborn screening is essential for identifying GA-1 patients.
