1.Prevalence of smoking and drinking habits among Form Six students in Petaling District, Selangor.

Smoking and alcohol intake patterns may persist from adolescence to adulthood. The aims of this study are to determine the prevalence and factors associated with smoking and drinking habits among Form Six students. This was a cross-sectional study conducted in January 2003 among Form Six students from government schools in the Petaling District, Selangor. A hundred and thirty six self-administered questionnaires were distributed to students selected through multistage stratified sampling. Response rate in this study was 90.1% (136/151). The respondents were mainly Chinese 60 (44.1%) and female 88 (64.7%). The prevalence of smoking was 22.8%, whereas the prevalence of alcohol intake was 47.8%. Only 33.1% of the respondents practiced undetermined activities and 1.5% have undetermined characters. There were significant associations between smoking and males (Adjusted OR 2.56, 95% CI 1.02-6.43) and smoking and alcohol intake (Adjusted OR 2.74, 95% CI 1.11-6.78). Alcohol intake has significant negative association with Malays (Adjusted OR 0.83, 95% CI 0.03-0.27). Smoking habits among adolescents were associated with males and alcohol intake. However, only alcohol intake was negatively associated with Malays. Program interventions to reduce behavioral problems, particularly smoking and alcohol intake should be emphasized.

2.Risk Factors for Stroke in Akita Prefecture

In Akita Prefecture, there are nine hospitals established by the Akita Prefectural Federation of Agricultural Cooperatives for Health and Welfare (Koseiren). Half of the stroke patients in the prefecture were treated in these Koseiren hospitals, and half of the mass screening projects for the prevention of cardio vascular diseases were undertaken by these hospitals. A retrospective cohort study was done using mass-screening data (age, sex, past history of diabetes mellitus, blood pressure, body mass index, smoking and drinking habits) of 175,033 cases stored at these hospitals from 1988 to 1999, and the prefecture-wide stroke data of 2,520 initial stroke events registered from 1988 to 2003. The number of stroke cases was broken down into 1,428 cases of cerebral infarction (57%, CI), 693 cases of cerebral hemorrhage (27%, CH) and 399 cases of subarachnoid hemorrhage (16%, SAH). The subjects were also divided into five age groups:30-49, 50-59, 60-69, 70-79 and 80-89. Blood pressure (BP) was classified into six categories according to the JNC 6 criteria. Risk factors were determined using the Cox analysis. The hazard ratio for CI and CH was increasing with advancing age. CI showed a higher hazard ratio in men than women (hazard ratio for men was 1.8). The hazard ratio was increasing as BP became higher in any of three stroke subtypes, and especially CH showed the strongest correlation with BP. Uncontrollable risk factors were very closely associated with the attack of CI. On the other hand, BP (controllable risk) was closely linked with the attack of CH. Our results showed the prevention of CI was not easy. Controlling BP may be the most effective strategy for preventing hemorrhagic stroke (CH and SAH).

3.Job Satisfaction and Mental Distress among Japanese Farmers

Objective: Job satisfaction and mental distress may contribute to mental illness, particularly in regard to depression and even suicide. The aim of this study was to investigate the level of job satisfaction and its relationship to the level of mental distress among farmers in comparison to non-farming rural residents. Materials and Methods: A cross-sectional questionnaire survey of 7,000 rural residents was performed to assess the level of job satisfaction and mental distress. Mental distress was measured using the 12-item General Health Questionnaire (GHQ-12). The adjusted odds ratios and their 95% confidence intervals were calculated by means of multivariate logistic regression models. Results: A total of 5,364 rural residents (1,888 farmers and 3,476 non-farmers) responded, therefore yielding a response rate of 76.6%. Twenty-five percent of the farmers and 24 percent of non-farmers were either very dissatisfied or somewhat dissatisfied with their jobs. Twenty-five percent of the farmers and 29 percent of non-farmers had mental distress (GHQ score of 4 or above). The crude odds ratio of mental distress among dissatisfied farmers in comparison to satisfied farmers was 4.38 (95% CI = 3.49-5.51). Similarly, the crude odds ratio of mental distress among dissatisfied non-farmers in comparison to satisfied non-farmers was 5.01 (95% CI = 4.19-5.98). These statistically significant odds ratios remained even after controlling for age, gender and family structure. The most important cause of mental distress among farmers was related to family finances. Among non-farmers, however, the most important cause of mental distress tended to be human relations in the workplace. Conclusion: Although farmers experienced similar levels of job satisfaction and mental distress in comparison to non-farmers, the most important cause of mental distress differed between farmers and non-farmers. As the underlying risks for this level of job satisfaction, family finances are therefore considered to be a potentially hazardous factor in regard to mental distress for farmers.

4.Relation between Self-reported Weight Cycling History, Dieting and Bio-behavioral Health in Japanese Adult Males

Background: Epidemiological findings suggest that weight fluctuations are associated with unfavorable health outcomes compared with stable weight. However, the interrelationship between the weight cycling history and dieting status in a non-clinical male trial on the risk for bio-behavioral health is unclear. Objective: The purpose of this study was to examine the relation between weight cycling history as a result of intentional weight loss and bio-behavioral health in Japanese adult males. Method: A cross-sectional study was performed on a group of 146 Japanese working males (47.5±9.3 yr.). Each subject completed a series of self-reported questionnaires in which information about weight cycling history, current dieting practices, life-styles, and social background were assessed. Results of the physical check up were used to assess biological parameters. Self-reported weight cycling was defined as intentionally losing 10% of one's weight and regaining the lost weight. Results: Cyclers reported a significantly greater incidence of current dieting and recent weight gain compared with non-cyclers. Taking regular meals, eating breakfast everyday, and not eating snacks between meals every day were significantly less frequent among cyclers compared with non-cyclers after controlling for BMI. The adjusted odds ratio for AST abnormality was 5.46 (95%CI: 1.08 −27.67), ALT abnormality was 3.31 (95%CI: 1.24−8.78), and γ-GTP was 3.38 (95%CI: 1.07−10.67) among cyclers, compared with non-cyclers. Conclusion: These findings suggest that a history of weight cycling in men, regardless of current weight status, is associated with adverse bio-behavioral health. The risk for several liver enzyme abnormalities associated with weight cycling history was substantial, independent of relative body weight and lifestyle factors.
