OBJECTIVES: This case report aims to (1) Present the differential diagnosis of chronic cough without neglecting uncommon causes (2) Increase awareness of diagnostic challenges that may arise in the face of a compromised airway (3) Elucidate the therapeutic complexity of airway management; (4) Emphasize the value of a multi-disciplinary team approach in such cases; Illustrate the vital role otolaryngologists play in the diagnosis and management of airway problems DESIGN: Case Report SETTING: Tertiary Care Hospital in Quezon City, Philippines PATIENTS: One Patient RESULTS: This is the case of an eleven-year old male that presented with chronic cough that was refractory to conventional empiric medical therapy. As his symptoms progressed, further evaluation revealed a mass that was obstructing a large portion of the airway. Timely and methodical investigation and intervention proved to be life-saving. CONCLUSION: A working knowledge of the differential diagnoses of chronic cough is crucial in the diagnosis and management of the compromised airway. A high index of suspicion, systematic and thorough investigation and a multi-disciplinary approach are vital components of the treatment plan. Bronchoscopy in the hands of a skilled practitioner is still a diagnostic and therapeutic mainstay. (Author)

2.Study on forcast model of local goiter in Bac Giang province

A study on 13 communes of 8 districts in B¾c Giang province and results of epidemiological investigation of goiter at 12 communes has shown that the level of iodine and calcium in the soil can be used to forcast the local goiter in B¾c Giang province. The simple, rapid and inexpensive to contribute to prevent, control and eliminate the local goiter in B¾c Giang province.

3.On occasion of 3 cases of the feminized testicle

The classification, diagnosis and management of the feminized testicle were implemented in the institute of mother and infant protection and care during 1998-1999 showed that the feminized testicle only found after the puberty age with the reasons of the primary menostasis or abnormality of the external genitalia. The clinical condition of 3 patients were typical. The hormone replacement therapy was used after the removal of testicle and reconstruction of the external genitalia.

4.Investigation of 265 neonatal tetanus cases from 1994 to 1996

The analysis showed that 207 mothers (78% of the cases) had never received tetanus toxiod. 96% (254) of the cases did not meet the criteria set for "protection at birth", 73.8% of the mothers did not seek antenatal care. The rate of delivery at home was 71%. The rate of delivery by traditional birth attendants (TBA) was 50%.

5.A case of chediac Higashi syndrome

The patients is 6 years old girl, who was diagnosied Chediac-Higashi syndrome phase acceleration according to the following criteria:(1) Photophobia, partial oculocutaneous albinism, intermittent high febrile fine light light hair, hepatosplenomegaly (2) family past history: Consanguineous marriage (3) Giant granules within the bone marrow and peripheral blood granule leukocyte. The patient was responded to well with antibiotic, high dose C vitamin and corticoid. The diagnosis, treatment and Etiology of this sydrome are also discussed.

6.A study on some difficulties of inpatients in Gia Luong district hospital, Bac Ninh province

Subjects: Patients without health insurance were discharged in Gia Luong district hospital, Bac Ninh province in 4/1998; 81,08% population of Gia Luong district have no health insurance 0,8% population received the human health insurance. 383 inpatients without health insurance (58,8%) and 41,2% inpatient with health insurance. The average duration of treatment was 7,4 days, less than this of inpatient with health insurance (8,5 days).Total cost/patient/round of treatment was 375.000 VND. This cost was higher 2,7 times than average income. 40,5% inpatients had to borrow. 1/4 (25%) patients want to early discharged during uncompleted free-disease. The rate of poor population received hospital bursar reduction or was very low because of the complicated procedure.

7.Remarks on 97 cases of cancer of rectum

On 97 cases treated in the Military Hospital No108, the remarks are as follows: 1) Rectal cancers occurred mainly in the age ranged 40-60 years old, men to women ratio is 1.85. 2) Clinic symptoms include constipation, diarrhoea with blood and pus occurred alternatively, tenesmus… 3) Most of tumors located at the lower rectal ampulla; adenoepithelioma accounted for 93.2%. 4) Rectal resection with sphincter conservation performed on 4.1%; Mile's operation on 59.8%; and palliative operation on 38.1% of cases; bleeding complication happened on 4.1% of cases and the most common post operative complication was perineum infection 26.8%. 5) Mortality 6.2%. 6) More than 5 years survival 52.3%

8.A case report of Von-hippel-Lidau disease

The authors report a case of Von hippel Lindau disease in a 20 years old patients with presenting signs: diminution of visual acuity, intracramial hypertension with a left cerebollar syndrome and existence of an retinal angioma on the right side and papilla oedema on the left side. A scanning examination revealed a cystic tumor with a solid little mass. Angiography showed a capillary nodus coressponding to the mass lesion, with feeding arteries coming from the anterio superior cerebellar artery. Removal of the cyst and the tumor was successful. Pathological examination: Hemangi oblastoma. Negative ultrasound examination of the kidney, pancreas.

9.A bleeding neck mass: An unusual case of neurofibroma

OBJECTIVES: 1. To present an unusual case of neurofibroma of the neck that does not meet the criteria for neurofibromatosis. 2. To describe the role of surgery in the treatment and reconstruction of a cosmetically unacceptable, disfiguring lesion. 3. To emphasize the importance of early detection and possibility of malignant transformation in neurofibroma. 4. To present the criteria in the diagnosis of neurofibromatosis. DESIGN: Case Report SETTING: Tertiary Hospital PATIENT: A 36 years old male with a huge bleeding neck mass. RESULT: Repeated incision biopsy of the lateral neck mass revealed neurofibroma. CT scan of the mass showed that it was superficial to the sternocleidomastoid muscle and was not adherent to the major vessels of the neck. A wide excision of the mass was done and the defect was reconstructed with a pectoralis major flap. CONCLUSION: Acase of neurofibroma that does not meet the criteria for von Recklinghausen's disease is presented. This may represent a distinct form of neurofibromatosis. In this kind of tumor, surgery remains the treatment of choice for resectable lesions. Early detection of the tumor is imperative for its successful treatment. (Author)

10.Bone marrow depression and hepatitis B: report of a case in the Ho Chi Minh center for Hematology and Blood Transfusion

A 76 year old male was admitted to hospital due to high- fever and pallor. Physical examination revealed fever, anemia, petechiane, yellowish sclera. Hemogram showed pancytopenia: WBC: 1.8 x 109/l (SN: 0.11; L 0.86; M 0.77x109/l); Hct: 26.4%; Hb: 8.7 g/dl; PLT: 63x109/l. Bone marrow aspiration and bone marrow biopsy showed hypocellularity (10-20%). Elevation of liver enzymes (SGOT/SGPT: 351/371 UI/I), hyperbilirubine (D/T: 1.8/2.1mg/dl), HbsAg (+). A diagnosis of aplastic anemia/hepatitis syndrome was made. The patient was given supportive treatment with blood transfusion, antibiotics, nutrition, polyvitamine. Two months later, the patient completely recovered from aplastic anemia and hepatitis.
