1.Analysis on the diagnosis and report of malaria cases in China during 2005-2008

Objective To identify the problem in the diagnosis and report of different types of malaria in China and provide evidence for making malaria control and elimination strategy and measurement. Methods All data are obtained from Disease Reporting Information System of China CDC, and the percentages, diagnosis methods and onset-diagnosis duration are described and compared among different malaria types and various report institutions. Results The overall number of reported cases declined by 71.77% during 2005-2008, with a relatively high percent for type-unconfirmed malaria. The percentages of laboratory diagnosis for falciparum malaria and vivax malaria were 91.44% and 71.14%, respectively. 28.22% of vivax malaria were diagnosed by using clinical diagnosis method, and 22.45% of type-unconfirmed malaria by using laboratory diagnosis method. 37.54% of falciparum malaria and 71.79% of vivax malaria were reported by hospitals in villages and towns, and 33.41% of falciparum malaria by general hospitals. The onset-diagnosis durations for falciparum malaria, vivax malaria and type-unconfirmed malaria were 72 h, 96 h and 72 h, respectively. Conclusions The laboratory diagnosis level is not good enough, though it is the main method for malaria diagnosis in China. The ability of laboratory diagnosis for malaria should be further strengthened in gross root medical institutions to reach the goal of malaria elimination.
2.Inducing an animal model of avascular necrosis of the femoral head through exercise

Objective To establish an immature rabbit model of avascular necrosis of the femoral head in-duced by exercise. Methods Ten male, immature New Zealand white rabbits were subjected to large range, in- tense passive movement and concentric impingement on their right hips for 4 weeks. The left hips were used as self-controls. Then X-ray and magnetic resonance imaging, gross anatomical observation and histological examination were used to evaluate avascular necrosis of the femoral head. Result After 4 weeks, avascular necrosis of the femoral head was successfully replicated. Increased bone density, decreased osteoepiphysis height and indistinct bone trabec-ula were found in X-rays of the right hips. In MRI images obvious joint hydrops could be detected in all right hips, and schistic low signal areas in the femoral head could be seen in TIWI and T2WI images. Thin bone trabeculae of low density, with irregular and broken structures, were also found in H-E sections. Conclusion An immature rab-bit model of avascular necrosis of femoral head can be successfully induced through large range, intense, passive movement and concentric impingement.
3.Analysis of epidemiologic features of visceral leishmaniasis in different age groups in China from 2005 to 2012

Objective To analyze the epidemiologic features of visceral leishmaniasis in different age groups in China from 2005 to 2012,and to provide a scientific basis for prevention and treatment of the disease.Methods The visceral leishmaniasis cases were obtained from China disease epidemic monitoring information system from 2005 to 2012.All the cases were divided into subgroups by demographic characteristics:infant (<1 year old),childcare (≥1 and <4 years old),child (≥4 and <15 years old),adult (≥15 and <65 years old) and senior adult (above 65 years old).The epidemiologic features,such as gender,season of disease onset and area distribution of visceral leishmaniasis were analyzed.Results The total reported visceral leishmaniasis from 2005 to 2012 were 2 979,among which cases in infant,childcare,child,adult and senior adult were 24.9%,21.7%,20.0%,32.4% and 1.0%,respectively.Infant cases were most reported in April; childcare cases in December which began to increase since September; child cases in October and adult and senior adult in March.The reported cases were mainly from Xinjiang,Gansu and Sichuan areas.However,age characteristics differed among regions,which were mainly infants,both childcare and children,and adults,respectively in this three areas.The duration from onset to diagnosis was shortest in infant group (11 d) and longer in adult group (15 d).Conclusions The number of visceral leishmaniasis cases in different age groups peaks in different months.Regional distribution is not even.Cases are most concentrated in Xinjiang,Gansu and Sichuan areas.Infants predominate in Xinjiang,childcare and children in Gansu and adults in Sichuan.
4.Effect of Touqing capsule associated with acupuncture at Fengchi on cervical spondylosis

objective To evaluate the clinical effect of touqing capsule associated with Fengchi on the vertebral artery type of cervical spondylosis(CSA)in the attack stage.Methods A total of 100 patients with CSA, according to the doctor order, were randomly divided into a control group and a treatment group respectively, with 50 patients in each. The control group was treated with acupuncture therapy, the treatment group was additionally treated with Touqingcapsule on the basis of the control group, and both groups were treated for 20 days. The hemodynamics was detected by transcranial doppler, the change of TCM syndrome integral was observed before and after the treatment, and the clinical efficacy was evaluated.Results After treatment, the treatment group dizziness (1.10 ± 0.33 vs. 2.01 ± 0.26,t=14.909), neck shoulder ache (0.87 ± 0.16 vs. 1.92 ± 0.69,t=10.266), numbness (0.95 ± 0.14)vs. 1.87 ± 0.16,t=29.844), the total integral (2.16 ± 0.75 vs.3.89 ± 1.14,t=8.756) integral were significantly lower than the control group(P<0.01).Treatment group RVA (40.03 ± 5.89 cm/svs. 34.26 ± 6.26 cm/s,t=4.628), the LVA (42.14 ± 5.42 cm/svs.33.89 ± 6.08 cm/s,t=6.985) rise and PI (0.79 ± 0.14 vs.1.04 ± 0.19,t=7.312) reduction were significantly better than those of the control group (P<0.01). The total effective rate in the treatment group was significantly better than the control group (91.7%(44/48)vs. 74.5%(35/47),χ2=3.863, P=3.863).Conclusion Touqing Capsule associated with Fengchi is effective for CSA. Its functional mechanism is correlated with improvement of haemodynamics of vertebral-basilar artery of the CSA patients.
5.Research progress of TCM in treating radiation enteritis based on intestinal microecology

Patients with radioactive enteritis generally have certain intestinal microecological imbalance. Traditional Chinese Medicine (TCM) has showed good advantage in regulating intestinal microbial flora. In clinical practice, patients are treated based on syndrome differentiation of heat toxin damaging collaterals, cold-heat mixed syndrome, spleen deficiency and dampness stagnation, spleen and kidney yang deficiency, yin deficiency and body fluid deficiency. The Baitouweng Decoction, Wumei Pill, Sijunzi Decoction are the common prescriptions. TCM can promote the balance of intestinal microecology and treat digestive diseases such as radioactive enteritis, by improving the abundance of intestinal flora, inhibiting the level of inflammatory cytokines, and playing the role of probiotics and immune regulation.
6.Minimal invasive surgery of pelvic fractures sustained in earthquake through an incision near the rectus abdominis muscle.

OBJECTIVETo assess the effects of minimal invasive surgery through an incision near the rectus abdominis muscle for treatment of pelvic fractures sustained in earthquake.
METHODSNine patients with pelvic fractures during Ludian earthquake (August 3, 2014) were treated with minimal invasive surgery through an incision near the rectus abdominis muscle followed by anterior ring fixation and reconstruction plate or posterior ring fixation with percutaneous sacroiliac lag screws. The operative time, incision length, blood loss volume, and pre- and postoperative pain levels were recorded. The surgical complications such as lateral femoral cutaneous nerve injury and hip adduction incapability were observed.
RESULTSThe minimal invasive surgery achieved satisfactory clinical effects and allowed convenient operation with better surgical exposure, shorter operative time, less blood loss and pain. The patients showed excellent fracture reduction and stable internal fixation without lateral femoral cutaneous nerve injury or hip adduction incapability.
CONCLUSIONSThe minimal invasive surgery through an incision near the rectus abdominis muscle is suitable for treatment of pelvic fractures with anteriorly interior fixation, especially in rescuing victims in the event of an earthquake where blood supply can be very limited.

According to the requirement of the national assessment for achieving the infection control criteria, 42 villages (among them,25 villages belonged to the first stratum, and 17 villages belonged to the second stratum) in 14 counties from 5 provinces, including Hunnan, Hubei, Jiangxi, Anhui and Yunnan, were selected as sampling villages for the assessment.The results from the field assessment showed that 154 out of 9 067 people were found infected with Sckistosoma japonicum, with an average infection rate of 1.7% ranged from 0.31 % to 4.10% , and only Yongping Village from Weishan County and Tenglong Village from Eryuan County were not found any case. A total of 46 out of 3 323 head of cattle were infected with S. japonicum, with an average infection rate of 1.38% ranged from 0.26% to 3.79% , and no any infected individual detected in Nanling County. No outbreak occurred in those sampling villages. Therefore, it is indicated that the five sampling provinces have reached the national criteria on infection control of schistosomiasis.

10.Plantar approach for Lisfranc injury: an anatomical study and preliminary clinical application

Objective:To explore the safety, efficacy and preliminary clinical application of the single plantar approach or in combination with the dorsalis pedis approach in the treatment of Lisfranc injury with poor dorsalis pedis soft tissue, metatarsal avulsion fracture or complicated multi-column lesions.Methods:(1) Six fresh cadaveric specimens of adult foot were collected and dissected through the plantar approach in order to determine the skin incision of the plantar approach and the safe area for plate-screw internal fixation, including start-stop points and courses of plantar nerves, blood vessels, tendons and ligaments, followed by plate-screw fixation on the specimens. (2) After feasibility of the plantar approach was confirmed by our anatomical study, it was used to treat the 3 patients who were admitted to Department of Orthopedics, The Third Hospital Affiliated to Southern Medical University between September 2020 and November 2021 for Lisfranc injury with severe necrosis due to dorsalis pedis skin contusion or metatarsal base avulsion fracture. They were 2 males and one female, with an average age of 51 years (from 34 to 68 years). The preliminary clinical efficacy was evaluated in terms of visual analogue scale (VAS), midfoot score of American Orthopaedic Foot and Ankle Surgeons (AOFAS), Maryland score, Kofoed score, fracture healing at the last follow-up and postoperative complications.Results:(1) Regarding the anatomical exposure range, the metatarsal side of the first metatarsal wedge joint was exposed medially and the metatarsal side of the third metatarsal wedge joint was exposed laterally; the peroneus longus tendon, Lisfranc plantar ligament and interosseous ligament were explored. X-ray films after the simulated operation showed satisfactory plate positions. (2) As for the preliminary clinical application, all patients were followed up for 6 to 14 months (mean, 11 months). At the last follow-up, the VAS score ranged from 0 to 1 (mean, 0.5), AOFAS score from 85 to 92 (mean, 89), Maryland score from 93 to 96 (mean, 95), and Kofoed score from 92 to 95 (mean, 94). There were no early complications such as fascial compartment syndrome, skin necrosis or infection. All fractures got united, with no complications like traumatic arthritis, muscle atrophy or screw loosening.Conclusion:Testified by the anatomical study, the plantar approach can be used to treat Lisfranc injury with poor dorsalis pedis soft tissue, metatarsal avulsion fracture or complicated multi-column lesions, leading to safe, effective and satisfactory clinical outcomes.
