1.Analysis of risk factors of cerebral infarction in young adults and its correlation with TOAST types

Objective To explore the risk factors of youth cerebral infarction and its correlation with clinical TOAST types.Methods 82 young patients with acute cerebral infarction(aged from 18 to 45 years old)were select-ed.The risk factors for youth cerebral infarction patients,and the proportion of TOAST subtype and related risk factors were analyzed.Results Risk factors for youth cerebral infarction were as follows:hyperlipidemia (χ2 =48.703,P <0.05),hypertension (χ2 =40.829,P <0.05),carotid sclerosis (χ2 =46.217,P <0.05),hyperhomocysteinemia (χ2 =40.255,P <0.05),smoking history (χ2 =7.853,P <0.05),diabetes (χ2 =18.256,P <0.05)and family history (χ2 =5.944,P <0.05),heart disease (χ2 =5.754,P <0.05).The proportion of their TOAST subtypes were as following:small artery occlusion lacunar(SAD)(39.0%),large artery atherosclerosis(LAA)(24.4%),stroke of other undetemined etiology(SUE)(17.0%),and acute stroke of other detemined etiology(SOE)(12.2%),cardio-embolism(CE)type(7.3%).Major risk factors for LAA subtype included hyperlipidemia,hypertension and carotid atherosclerosis;Major risk factor for SUE subtype was hyperhomocysteinemia;Major risk factors for SOE included hypertension,diabetes.Major risk factor for CE subtype was heart disease.Conclusion The highest proportion of TOAST types in youth cerebral infarction group is small artery occlusion lacunar.Major risk factors for this group of youth cerebral infarction are as follows:hypertension,carotid atherosclerosis,heart disease,diabetes,hyperhomocys-teinemia,obesity,smoking and family history,and these risk factors should be actively intervened.

Traditional Chinese herbal medicines have been used for treating various ailments since medicine be- gan.The belief that natural products are much safer than synthetic drugs has led to dramatic growth of herbal medi- cine usage.Like synthetic drugs,herbal preparations also have potential to cause adverse effects and drug interac- tions.In this paper,pharmacokinetic studies on herb- drug interactions are reviewed and discussed in order to promote usage of herbs rationally and safely.

4.Study on chronotropic response in coronary disease with percutaneous coronary intervention

Objecitve To investigate the changes of chronotropic response before and after percutaneous coronary intervention ( PCI)in patients with coronary disease .Methods A total of 339 patients with coronary disease was included in this study .All sub-jects underwent treadmill exercise test and coronary angiogram , and some patients underwent PCI if necessary .The parameters of chro-notropic response were recorded and analyzed , including ratio of the highest to predicted heart rates ( rHR) ,chronotropic response in-dex ( CRI) , and heart rate reserve ( HRR) .After coronary angiogram , the score of gensin was recorded and analyzed .Results There was significant difference in the parameter of CRI between unstable angina pectoris and silent myocardial ischemia groups ( P <0.05 ) , CRI were 0.80 ±0.11 and 0.89 ±0.07 , respectively .After coronary angiogram and PCI , there were significant differences in the parameters of rHR, CRI, and HRR between pre-therapy and post-treatment ( t =2.440, 1.977, 2.529, all P <0.05).A nega-tive correlation was found between the parameters of rHR , CRI, and HRR and the score of Gensin ( r =-0.686 , -0.673 , and-0.672, all P <0.05).The significant difference in rHR, CRI, and HRR existed between the groups of <20 and >40 ( t =2.567, 2.223, 2.062, all P <0.05).Conclusions Parameters (rHR, CRI, and HRR) had important clinical values in evaluating the changes of chronotropic response before and after PCI in patients with coronary disease with a negative correlation with the score of Gensin.
5.Content Determination of Zolpidem Tartrate in Microsamples of Rat Plasma by HPLC-fluorescence Method

OBJECTIVE:To establish a method for the content determination of zolpidem tartrate in microsamples of rat plas-ma. METHODS:Rats were given zolpidem tartrate solution 3 mg/kg intragastrically,and 0.2 ml blood sample were collected and isolated. 50 μl plasma was precipitated by methanol,and the supernatant was determined by HPLC-fluorescence combined with ex-ternal method. Agilent HC-C18 column was used with mobile phase consisted of 0.03 mol/L KH2PO4 solution(containing 0.2% tri-ethylamine)-methanol (33∶67,V/V) at flow rate of 1.0 ml/min. The excitation and emission wavelengths were 254 nm and 390 nm,respectively. The sample size was 20 μl. RESULTS:The linear ranges of zolpidem tartrate in plasma was 2-200 μg/L(r=0.999 7),and the limit of quantification was 2 μg/L. The method recoveries of zolpidem were (96.96 ± 1.35)%-(105.0 ± 5.36)%(RSD=2.20%-4.88%,n=5),and extraction recoveries were (79.72 ± 0.01)%-(80.77 ± 0.02)%(RSD=1.34%-3.90%,n=5). The intra-day and inter-day RSDs were 1.40%-5.10% and 3.22%-9.25%(n=5),respectively. CONCLUSIONS:The method is simple,sensitive and suitable for the content determination of zolpidem tartrate in microsamples of plasma.

7.Application of Ultrasound Guided Drainage Combined with Different Hardeners for Treatment of Simple Breast Cysts

Purpose To investigate the clinical effect of ultrasound guided drainage combined with different hardeners for simple breast cysts,and to further provide reference for individual treatment.Materials and Methods Clinical data of 117 cases breast simple cysts were analyzed retrospectively,of which 42 cases were routine ultrasound-guided puncture combined with fluid injection of hypertonic sugar (group A),40 cases were routine ultrasound guided drainage combined with ethanol injection therapy (group B),and 35 cases were routine ultrasound guided therapy alone (control group).The clinical efficacy and adverse reactions were compared among the three groups after treatment.Results The total effective rate of group A was 95.24%,group B was 97.50%,and control group was 82.86%.Total effective rate of group A and B had no significant difference (P>0.05),which were both higher than control group (P<0.05).The adverse reactions rate of group B was higher than group A and control group,which had statistical significance (P<0.05).Conclusion Ultrasound guided injection of hypertonic sugar or ethanol after drainage treatment for simple breast cysts can both obtain satisfactory clinical results,but adverse reactions of the former one are mild,it should be widely applied.
8.Efficacy and safety of conbercept for choroidal neovascularization secondary to chronic central serous chorioretinopathy

Objective:To evaluate the efficacy and safety of conbercept in patients with choroidal neovascularization secondary to chronic central serous chorioretinopathy (CSC-CNV).Methods:A retrospective case study was performed.The medical records of 13 patients (14 eyes) diagnosed as chronic CSC-CNV in Hangzhou Branch of Eye Hospital of Wenzhou Medical University from September 2015 to January 2018 were collected.All the study eyes received intravitreal injection of conbercept (0.05 ml/0.5 mg) under one intravitreal injection and pro re nata (PRN) treatment.The best corrected visual acuity (BCVA) and central macular thickness (CMT) before initial injection and 1 week, 1 month, 2, 3 and 6 months after initial injection were measured and analyzed.This study followed the Declaration of Helsinki and written informed consent was obtained from each patient before initial injection.The study protocol was approved by the Ethics Committee of Hangzhou Branch of Eye Hospital of Wenzhou Medical University (No.2019-029-K-28).Results:During the 6-month follow-up, the mean administration times was 1.93±0.83, and all the CNV secondary to CSC did not grow outside the retinal pigment epithelium layer.The BCVA values before initial injection and 1 week, 1 month, 2, 3 and 6 months after initial injection were 0.51±0.32, 0.43±0.34, 0.36±0.35, 0.31±0.28, 0.27±0.29 and 0.26±0.30, respectively, with a significant difference among different time points ( F=21.225, P<0.05). The BCVA values at each time point after initial injection were significantly better than that before initial injection (all at P<0.05). The CMT values before initial injection and 1 week and 1 month, 2, 3, 6 months after initial injection were (299.07±132.90), (216.50±70.94), (203.00±61.87), (234.29±95.70), (194.21±46.46) and (207.43±55.46) μm, respectively, and the difference was statistically significant among different time points ( F=3.768, P<0.05). The CMT values at each time point after initial injection were significantly better than that before initial injection (all at P<0.05). No severe treatment complications were observed during the follow-up period. Conclusions:Intravitreal injection of conbercept is safe and can effectively reduce the CMT and improve BCVA of chronic CSC-CNV patients in the short term.
9.Discussion on the management practice of focus drugs in Ningbo

This paper introduced the management of focus drugs in Ningbo, and evaluated the preliminary results by comparing the changes in terms of the income of these drugs before and after the measures taken. More measures should be taken by establishing a unified drug catalogue, strengthening the assessment, carrying prescription comment and periodic notification. The pharmacists can play a leading role in clinical rational drug use. These efforts can serve for scientific control of the unreasonable growth of medical expenses.
10.Serum levels of CXC chemokine ligand 10 and chemokine receptors in patients with type 1 diabetes mellitus

Serum levels of CXC chemokine ligand 10 (CXCL10) and chemokine receptor 3 (CXCR3) were determined in 50 patients with type 1 diabetes mellitus (T1 DM) and 30 normal control subjects by ELISA method.The results showed that serum levels of CXCL10 and CXCR3 in T1DM patients were significantly higher than those in normal subjects [(258.17 ± 39.12 vs 96.47 ± 26.91) ng/L,(851.87 ± 70.04 vs 441.82 ± 72.24) pg/ml,both P<0.05].Serum level of CXCL10 in patients dropped sequentially with durations of diabetes <6 weeks,≥6 weeks or <3 years,and ≥ 3 years,being statistically significant between groups (P<0.05).These results suggest that serum levels of CXC10 and CXCR3 may reflect the immune activity in T1DM.
