1.Clinical Research of Reinforcing Kidney Decoction for Metabolic Syndrome of Phlegm-Blood Stasis Due to Kidney Deficiency

Objective To study the clinical efficacy of Reinforcing Kidney Decoction for the treatment of metabolic syndrome diagnosed as traditional Chinese medicine (TCM) syndrome of phlegm-blood stasis due to kidney deficiency. Method 42 recipients who volunteered to accept the TCM therapy were up to the inclusion criteria and finally fulfilled this study. Of all the cases, there were 26 males and 16 females, their old years ranged from 39 to 60, which were divided into obesity type diabetics group and metabolic syndrome group, both of which were treated with oral Reinforcing Kidney Decoction combined with western medicine therapy, with a course of 4 weeks. The changes of clinical symptoms, body weight index (BMI), fasting blood glucose (FBG), 2 h post prandial glucose (2hFBG), total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL-C) levels, and serum uric acid were analyzed. Results Therapeutic effect in two groups ranked as significant, general and null effect, 14/5/2 vs 8/11/2, respectively. Dizziness and heaviness feeling of limbs have been improved. 2.Analysis on risk factors acute kidney injury complicated by acute gastrointestinal hemorrhage

Objective Few prospective data are currently available on acute gastrointestinal hemorrhage (AGIH) as a complication in acute kidney injury (AKI).The aim of the present study was to find out clinical characteristics,incidence,etiology,risk factors,and outcome of AGIH in patients with AKI.Methods We performed a prospective study on an inceptione cohprt of 512 patients admitted for AKI in our hospital.Data on clinical risk factors for bleeding,frequency of occurrence of AGIH,in-hospital mortality were collected,and independent predictors of AGIH were identified.Results A total of 53 patients had AGIH as a complication of AKI,and 45 were upper AGIH.Fifteen patients had clinically severe bleeding.Independent baseline predictors of AGIH were severity of illness,cardiac failure,mechanical ventilation,low platelet count,chronic hepatic disease,liever cirrhosis,severe AKI.Inhospital mortality was 52.8% in patients with AGIH,and 22.2% in the other patients.AGIH was significantly associated with an increase in hospital mortality.Conclusions AGIH are frequent complications of AKI.In this clinical condition,AGIH is more often due to upper gastrointestinal bleeding and is associated with a significantly increased risk of death.Both renal and extrarenal risk factors are related to the occurrence of AGIH.
3.Application of digital technology in the lateral arm perforator flap for reconstruction of soft tissue defect of hand

To investigate the effectiveness of the digital technology in repairing hand soft tissue defect with the lateral arm perforator flap. Methods From January, 2015 to February, 2018, 15 cases of hand soft tissue defect were treated with free lateral arm perforator flaps transferring. Injury site: 7 cases on the back of the hand, 4 cases on the palm, and 4 cases on the mouth of the tiger. All of them had tendon exposure. The size of defect was 2.0 cm×1.5 cm to 14.0 cm×5.0 cm. CT angiography was conducted and analyzed before operation, and the donor sites and preferred perforators were selected.Then the CTA data was imported into the Mimics 17.0 software to recon-struct the three dimensional structure of the perforators, bone, and skin. The flap was obtained on the basis of preop-erative design during operation.The size of flaps varied from 3.0 cm×2.0 cm to 15.0 cm×6.0 cm.The survival status of the flap, the appearance and function of the hand were followed-up and analyzed after operation. Results All the flaps survived. The wounds and incisions at donor sites healed primarily. The patients were followed-up for 6-12 (mean, 8) months. The hand function recovered satisfactorily, and the sensation was S2-S3 at 6 months after operation. According to the functional assessment criteria of upper limb formulated by the Hand Surgery Branch of Chinese Medical Association, the results were excellent in 4 cases, good in 9 cases, and fair in 2 cases. The linear scars were observed at the donor sites. Conclusion The lateral arm perforator flap has a stable vessel pedicle, good texture and sensation. So it is a good method to repair soft tissue defects of hand. The preoperative individualized design of the lateral arm perforator flap can realize through CTA digital technology and Mimics 17.0 software. It can reduce the operation risk.
