1.The levels of serum tHcy and Lp(a) and their relation with insulin resistance in patients with combined type 2 diabetes and CHD

Objective To investigate the levels of serum tHcy and Lp(a)and their association with insulin resistance in patients with type 2 diabetes and CHD. Methods T2DM patients were divided into two groups with CHD and without CHD in addition to normal control.The fasting serum levels of tHcy,Lp(a),FPG and FIns were detected, ISI were calculated,and the correlations among them were analyzed. Results The serum levels of tHcy,Lp(a),FIns,FPG of T2DM plus CHD were significantly higher than those of T2DM without CHD and controls(P<0.01~0.05).ISI was significantly lower in T2DM cases with CHD than those of T2DM cases without CHD and controls(P<0.01). ISI was negatively correlated with tHcy and Lp(a)in T2DM plus CHD group(P<0.05)but not correlated with those in T2DM case without CHD group(P>0.05). Conclusions Detections of the serum levels of tHcy and Lp(a)of T2DM play significant roles in knowing patient condition,guiding clinical treatment and earlier preventing and treating vascular complication of diabetes.
2.VITEK-32 and Microscan AS-4 in External Quality Assessment of Microbes: Their Application and Comparison

OBJECTIVE By using the VITEK32 and Microscan AS-4 microbiology identification system to identify the external quality control(QC) strains of microbes,to find out the difference and the coincidence degree with quality assessment result of the two systems. METHODS Thirty nine various strains of QC bacteria were isolated and cultured according to operation rule,and were identified by VITEK-32 and Microscan AS-4 and compared within them. RESULTS The coincidence rate of VITEK-32 and Microscan AS-4 to identify directly their species was 76.9% and 69.5% for 39 strains of QC bacteria,It was 89.7% and 76.9% after the serum or further tests,and the coincidence rate was 90.9% and 81.8% in 22 strains of Gram negatives,and 88.2% and 58.8% in 17 strains of Gram positives. CONCLUSIONS The detectable rate and coincidence rate of VITEK-32 are superior to those of Microscan AS-4,especially in the identification of Enterococcus and Streptococcus,the accuracy of Gram negatives is good,and of Gram positives is short,both them could not identify Salmonella and Shigella to their species.
3.Clinical psychological intervention on anxiety and depression in hemodialysis patients

Objective To explore the effects of clinical psychological intervention on anxiety and depression in hemodialysis patients .Methods Hemodialysis patients were randomly divided into two groups:psychological inter-vention group and control group ,25 patients in each group .In intervention group ,besides routine clinical treatment and nursing measures ,we analyzed the clinical information of the patients ,consulted the relevant documents ,combined the evidence and clinical experience ,and finally conducted the psychological intervention .Self-Rating Anxiety Scale and Self-Rating Depression Scale of these patients were analyzed .Results Before the intervention , the SAS and SDS scores of intervention group and control group were (57.3 ±1.5) versus (56.9 ±1.6) and (55.3 ±2.3) versus (56.1 ±3.5),respectively.The t values were 0.912(P>0.05) and 0.955(P>0.05),respectively.After interven-tion,the score before and after the trial between these two groups were significantly different (P<0.05).The SAS and SDS scores of intervention group and control group were (16.4 ±0.9) versus (10.1 ±1.3) and (14.3 ±2.1) ver-sus (9.7 ±1.5),respectively.Conclusion Clinical psychological intervention can effectively relieve anxiety and depression in hemodialysis patients .
4.Mechanism of breast cancer metastasis suppressor gene 1 inhibiting tumor metastasis

Breast cancer metastasis suppressor gene 1 (BRMS1) significantly reduces the invasion and metastasis of cancer cells.BRMS1 gene expression is decreased or deleted in the cells of various malignant tumors.BRMS1 gene can inhibit tumor cells invasion and metastasis by means of regulating gene transcription and protein translation by phosphoinositide signaling and nuclear factor-κB (NF-κB) signaling pathways,repairing intercellular communication and interacting with the mSin3-histone deacetylase (HDAC) complex,estrogen receptor and other proteins.BRMS1 gene may be a new target for the gene treatment of tumor metastasis.
5.Influence of insulin sensitivity on insulin resistance rats treated by puerarin

Aim To investigate the effect of puerarin(Pue)on the experimental insulin resistance(IR)and the mechanism of Pue-treated metabolic syndrome(MS).Methods IR rat model was induced by i.v.small dosage streptozotocin(STZ)and high fatty diet.The effect of Pue on insulin sensitivity was studied by reformed hyperinsulinism euglycemia clamp technique(HEPC)on IR rats.Results The basic blood glucose(BBG),basic blood plasma insulin(BINS)and stable basic blood plasma insulin(SINS)of IR rats induced by high-fat feed were higher than those of control group,and those of the group treated by Pue were observably lower than those of IR model group.In HECT text,the average rate of glucose injection of IR model group was lower than that of control group from 60 min to 120 min,but the rate of high dose and middle dose of Pue group was significantly higher than that of IR model group.Conclusion The results suggest that Pue can improve the insulin sensitivity,and reduce basic blood glucose and blood plasma insulin of the experimental insulin resistance rats.
6.Analysis of the follow-up results concerning pregnancy, delivery and infants after assisted reproductive technique with GnRH-a for luteal support

Objective To investigate the results of follow-up visits of pregnancy course, delivery and infants of women who got clinically pregnant by assisted reproductive technique after gonadotropin-releasing hormone agonist (GnRH-a) added for luteal support, and to analyse the influence of adding GnRH-a in luteal support on the safety of mother and infant. Methods A retrospective analysis was carried out on the medical record from 215 patients who got clinically pregnant after luteal phase long regimen fresh-cycle transfer was operated. According to the differences in luteal support methods, the patients were assigned to Group A (124 patients, progesterone+dydrogesterone group), Group B (91 patients, GnRH-a added group). The patients′ pregnancy course, delivery time, and the growth and development of infants within 1-2 years were followed up. Results (1)There was no obvious difference between Group A and Group B in terms of the abortion ratio during the early pregnancy (8.1%, 12.1%), the rate of abortion villous deformity (50.0%, 9.1%), the rate of heterotopic pregnancy (10.5%, 5.5%) and rate of twin pregnancy (19.4%, 28.6%;all P>0.05).(2)Compared to group A, during the middle and late pregnancy of single or twin pregnancy in Group B , there was no obvious difference in the rate of fetal chromosomal abnormality, organ malformation incidence, late abortion rate and stillbirth rate (all P>0.05).(3)As to childbirth, in the case of twin pregnancy, there was a higher rate of premature delivery (60.0%, 39.1%;P=0.041), as well as rate of lower birth weight of newborn (56.0%, 34.8%; P=0.037) in group B.(4)The statistics on general growth and development as well as infantile common diseases within 2 years after birth indicated that there was no obvious difference between the two groups in single birth and twin birth subgroup (all P>0.05). Conclusion On the basis of controlling of implanted embryos and reducing the occurrence of twins, GnRH-a luteal support maybe relatively safe and effective.
7.Peptidoglycan modulation of chemokine production in normal human epidermal kerafinocytes and the role of Toll-like receptor 2 in this process

Objective To investigate the effect ofpeptidoglycan from Staphylococcus aureus on the release of several chemokines including intedeukin 8 (IL-8), regulated upon activation, normal T cell expressed and secreted (RANTES), macrophage-derived chemokine (MDC) by normal human epidermal keratinocytes (KCs) and the role of Toll-like receptor 2 (TLR2) in this process. Methods KCs were derived from the foreskin of a healthy boy and propagated. After 2 - 4 passages, KCs were collected and treated with various concentrations (3, 10, 30 and 100 mg/L) of peptidoglycan for 24 hours or with peptidoglycan of 100 mg/L for varying durations (3, 6, 12, 36 hours). A fi'action of KCs were pretreated with functional grade purified anti-TLR2 monoclonal antibody before the treatment with peptidoglycan of 100 mg/L. After additional 12-hour culture following the treatment, enzyme linked immunosorbent assay was used to detect the level of IL-8, RANTES and MDC in culture supernatants of KCs. Results KCs spontaneously released IL-8 and RANTES. Peptidoglycan increased the production of IL-8 but decreased that of RANTES by KCs. The levels of IL-8 were 209.96 ± 10.31 ng/L, 250.28 ± 9.52 ng/L, 285.11 ± 10.28 ng/L, 359.40 ± 6.93 ng/L in KCs treated with peptidoglycan of 3, 10, 30, 100 mg/L, respectively, compared to 135.41 ± 14.37 ng/L in untreated KCs (all P < 0.05). On the contrary, a significant decrement was seen in the secretion of RANTES by KCs treated with peptidoglycan of 10, 30, 100 mg/L compared with untreated KCs (110.72 ± 8.51 ng/L, 90.50 ±2.45 ng/L, 49.89 ± 13.74 ng/L vs 149.94 ± 18.71 ng/L, all P < 0.05). The monoclonal antibody to TLR-2 could markedly suppress the promotion of IL-8 production by peptidoglycan, but had no obvious influence on the inhibition of RANTES production by peptidoglycan. MDC could not be detected in the culture super-natants of KCs with or without peptidoglycan stimulation. Conclusion Peptidoglycan could inhibit RANTES secretion but induce IL-8 production by KCs likely via TLR2.
8.The design and clinical application of a new type of disposable heparin free dialysis tube

Objective To investigate the clinical effect of a new type of disposable heparin freedialysis tube in uremic patients without heparin dialysis. Methods A total of 110 patients withoutheparin dialysis were divided into control group and observation group by random digits table method with55 cases each. Based on the same pre dialysis and dialysis for heparin saline flushing of discontinuoussaline on the control group using the traditional dialysis pipe for hemodialysis without heparin, theobservation group used disposable non heparin dialysis pipe for hemodialysis without heparin. The numberof patients with venous catheter blockage, blood loss and completion of dialysis time in two groups wasrecorded. Results The number of patients with venous catheter blockage, blood loss and completion ofdialysis time was 6, 6, 47 cases in control group and 0, 0, 54 cases in observation group. The differences were statistically significant (χ2=4.407, 4.407, 4.356, P<0.05). Conclusions The new disposableheparin free dialysis tube can reduce the loss of blood and prolong the treatment time, and the operation issimple, safe and effective, and has good application value and application prospect.
9.Study on the changes of gonadotropin releasing hormone agonist in pinopodes

Objective To study the changes of gonadotropin releasing hormone agonist (GnRH-a) in pinopodes during luteal phase and to explore the possible mechanism of GnRH-a in luteal phase support of assisted reproductive technology (ART).Methods Totally 40 primary infertility women who were treated with ART due to male factors were enrolled,according to the order of the group they were randomly divided into experimental group and control group.On the 7th day after ovulation,the experimental group received a subcutaneous injection of 0.1 mg of GnRH-a,while the control group received a subcutaneous injection of placebo only (0.9% salinc 2 ml),3 days later they came to the clinic again.Serum estradiol and progesterone levels were measured before and after treatment in each group.Pinopodes were collected for electron microscopic examination.Levels of ER and PR were detected by western blot.Results (1) There was no significant difference between the experimental group and the control group in the estrogen level before and after the treatment (all P>0.05).The level of progesterone in the experimental group after treatment [(66.8± 14.9) nmol/L] was significantly higher than that before treatment(P<0.05);also significantly higher than the same period of the control group (P<0.05).(2) There was no significant difference in the expression of ER protein in the experimental group before and after treatment (P>0.05).The expression of PR in the experimental group after treatment was significantly lower than that before treatment (P<0.05);also lower than the same period of the control group (P<0.05).(3) Expression amount of pinopodes in the experimental group after treatment was significantly higher than that before treatment [65% (13/20) versus 25% (5/20),P< 0.05],and the development trend was more mature [the percentage of maturation:75% (15/20) versus 35% (7/ 20),P<0.05].Expression amount of pinopodes after treatment and the percentage of maturation in the experimental group were significantly higher than those in the same period of control group (P<0.05).Conclusion GnRH-a in luteal phase support may play a role through the corpus luteum,which may promote the secretion of progesterone,downregulation of PR expression,promote the growth of pinopodes,and improve the endometrial receptivity.
10.The effects of external treatment with honey for acute cellulitis on local symptoms and C-reactive protein

group was (1.20 ± 1.09),(1.09 ± 1.04), (0.86 ± 0.42), (1.89 ± 1.30) and that of the control group was (2.40 ± 1.87), (2.34 ± 1.71), (1.60 ± 1.33),(3.49 ± 1.72) respectively, which had statistical significance between the two groups (t=3.139-4.390, P<0.01). the Blood CRP values and CRP positive rate in the observation group, were (6.34±2.74) and 17.14%(6/35) respectively and which of the control group were (10.35±7.37) and 42.86%(15/35), the difference of the two groups was statistically significant (t=2.881, P<0.01 andχ2= 5.250, P < 0.05). Conclusions Topical treatment of acute cellulitis with honey can effectively improve the clinical symptoms, reduce the level of c-reactive protein, which is better than magnesium sulfate wet compress.
