1.Clinical observation on the treatment of anemia of hypothyroidism with complex melanteritum pill

Objective To observe the curative effect of complex melanteritum pill in treating anemia of hypothyroidism.Methods A total of 50 patients were randomly recrutied into a control group(20 patients)and a treatment group(30 patients).The treatment group was treated by Levothyroxine Sodium Tablets,50 μg qd,and complex melanteritum pill,nine pills/time,three times daily;while the control group was treated by levothyroxine sodium tablets,50 μg qd.Both groups were treated for 8 weeks,without blood transfusion,adding iron and folic acid and vitamin B12,etc;Results The basic cure rate and the total effective rate in the treatment group was 50.0%and 80.0%respectively;the basic cure rate and the total effective rate in the control group was 30.0%.and 50.0%respectively.There was a significant difference between the two groups by comparison(P<0.05).Conclusion Complex melanteritum pill was effective in treating hypothyroidism with few side effects.
2.Clinical analysis of three internal fixation devices for intertrochanteric fractures of femur

Objective To compare the clinical effects of 3i nternal fixation devices,130?comp res-sion-plate,Richards screw-plate system and Gamma locking nail,in tre atment of intertrochanteric fractu res of femur in order to provide evidence fo r clinical choice.Methods The 3different internal fixation devices were statistically compared in terms of t heir surgery time,volume of operative bleeding,hospitalization time,postop-erative complications,as well as excellent and good rate of function in t reatment of intertrochanteric frac tures of femur.Results Compared with the other two treatmen t groups,the Gamma locking nail grou p presented the best therapeutic effects.Its posto perative complications were significantly lower and its excellent and f ine rate of function was significantly higher t han those of the other two groups.Conclusion Gamma locking nails are a kind of effective internal fixation device for the treatment of intertro chanteric fractures in elderly patients,because it has better biomechanical properties which facilitate fracture healing and recovery of hip joint function.[
3.Effect of Family Rehabilitation on Growth and Development of Children with Hypoxic Ischemic Encephalopathy

Abstract: Objective To explore the effect of family rehabilitation on growth and development of children with hypoxic ischemic encephalopathy (HIE). Methods 80 children with moderate HIE were divided into control group (n=40) and observation group (n=40). The control group received routine intervention and nursing, and the observation group accepted family rehabilitation in addition. Mental development index (MDI) and psychomoter developmental index (PDI) were compared when they were 3 and 6 months old with intellectual development testing, and their development quotients (DQ) were compared at the age of 1 year. Results The MDI and PDI scores were higher (P<0.01) in the observation group than in the control group when they were 3 and 6 months old. The DQ were higher (P<0.05) in the observation group than in the control group when they were 1 year old. Conclusion Family rehabilitation can promote the development of intelligence in children with HIE.
4.Drug susceptibility test for 16 Mycobacteria tuberculosis isolates from patients with cutaneous tuberculosis

Objective To test the drug susceptibility of 16 Mycobactena tuberculosis isolates from patients with cutaneous tuberculosis,and to provide a basis for the treatment of this entity.Methods Sixteen strains of mycobacterium were isolated from patients with cutaneous tuberculosis.and they were consistently identified as M.tuberculosis by biochemistry and molecular biology.Absolute concentration method was used to test the susceptibility of these isolates to isoniazid.streptomycin.rifampicin and ethambutol.For two strains resistant to streptomycin,PCR and sequencing were performed to analyse the mutation of rpsL gene.Results Out ofthe 16 strains of M tuberculosis.2 were resistant to streptomycin but sensitive to isoniazid,rifampicin and ethambutol,and 14 were sensitive to isoniazid,streptomycin,rifampicin and ethambutol.Sequencing of the rpsL gene revealed a mutation of AAG to AGG at codon 43 in one streptomycinresistant strain and a substitution of CGC bv CAC at codon 54 in another resistant strain.Conclusions In past five years,the resistance ratio of M tuberculosis was low in patients with cutaneous tuberculosis,and streptomycin resistance predominated in these strains.
5.Influences of the network medical health management on the coping styles, hope level and quality of life in patients with primary kidney cancer surgery

Objective To explore the influences of the network medical health management on the coping styles , hope level and quality of life in patients with primary kidney cancer surgery .Methods The patients of control group received normal health management and the participants included 77 kidney cancer patients who were laboratory diagnosed during March 2011 and September 2012 .The patients of intervention group received the network medical health management based on the normal health management and the participants included 79 kidney cancer patients who were laboratory diagnosed during March 2013 and September 2014 .The coping styles , hope level and quality of life before and after the intervention were compared between the two groups .Results Before the intervention , the coping styles and the hope level had no statistical differences between the two groups (P>0.05).After the intervention for the coping styles , the intervention group had higher scores of the positive coping style (25.44 ±2.61) and lower scores of the negative coping style (9.76 ±1.89) and presented statistical differences compared with that of the control group ( t=-10.468, 7.572;P <0.01).For the hope level, there was no statistical significance between two groups before intervention (P >0.05); however, after intervention, the intervention group had higher scores of positive attitudes (14.5 ±2.6), positive behaviors (15.1 ±2.4) and close contact with people (15.6 ±2.1), and presented statistical differences with that of the control group (t=4.965, 5.569, 4.965; P<0.05).For the quality of life, there were no statistical differences for the dimensions of PF , RF and DY (P>0.05) but the intervention group performed better on other 12 dimensions′scores ( P <0.05 ); the quality of life in the intervention group was better than that of control group .Conclusions The network medical health management should be further applied to the clinical practice , because it can significantly improve the coping styles , hope level and quality of life among the patient with primary kidney cancer surgery .
6.Application of PBL health education in primary renal cell carcinoma surgical patients

Objective To investigate the effects of application of problem-based learning (PBL) education in primary renal cell carcinoma surgical patients.Methods A total of 1 00 renal cell carcinoma patients who received surgical treatments from July 201 3 to January 201 5 in Cancer Hospital Affiliated to Zhengzhou University were randomly divided into control group (50 cases)and observation group (50 cases). The patients in control group received routine health education and the patients in observation group received PBL health education.The psychological status,satisfaction with nursing and occurrence of postoperative complications were compared in two groups when admitted and discharged from hospital.Results The difference of psychological status in two groups was not significant when admitted hospital (P >0.05),while it is better in observation group when discharged from hospital (P <0.05).In addition,the degree of satisfaction was higher in observation group (P <0.05).Furthermore,the occurrence of complications was lower in observation group (P <0.05).Conclusions Health education based on PBL can arouse patients′enthusiasm in actively learning disease-related knowledge, improve their psychological status, decrease the occurrence of postoperative complications and increase patients′satisfaction.
7.Efficiency and safety of endoscopic therapy for early esophageal cancer and precancerous lesions with length more than 5 cm

Objective To compare the effectiveness and safety of endoscopic submucosal dissection ( ESD) with endoscopic piecemeal mucosal resection ( EPMR) for early esophageal cancer and precancerous lesions with length more than 5 cm. Methods A retrospective analysis was performed on data of 85 patients diagnosed as early esophageal cancer and precancerous lesions with length more than 5 cm in Fujian Medical Association of Early Esophageal Carcinoma from January 2012 to July 2017. The patients were divided into ESD group (52 cases) and EPMR group (33 cases), and the effectiveness and safety between the two groups were compared. Results There was no significant difference on the complete resection rate between the two groups[86. 5% (45/52) VS 87. 9% (29/33), P>0. 05]. The operative time (58. 53±30. 50 min VS 32. 06±9. 12 min), postoperative fasting time (4. 18±1. 30 d VS 3. 67±0. 96 d), postoperative hospital-stay time (7. 45±2. 44 d VS 6. 54±1. 73 d), and postoperative antibiotics using time (3. 48±2. 33 d VS 1. 96±2. 20 d) in ESD group were higher than those in EPMR group (all P<0. 05). There were no significant difference in the rate of intraoperative complication and short-term postoperative complication, such as fever, chest pain, and postoperative bleeding, between the two groups ( all P>0. 05 ) . But the postoperative stricture rate of ESD group was higher than that of EPMR group[23. 1% (12/52) VS 6. 1%(2/33), P<0. 05]. During the follow-up of 3-63 months, 5 cases recurred in ESD group and 1 case in EPMR group, with no significant difference ( P>0. 05). Conclusion ESD and EPMR have equivalent efficacy and safety on the treatment of early esophageal cancer and precancerous lesion. EPMR has a shorter operative time, lower rate of post-operative stricture, and is easier to master.
8.Clinical features of 19 severe cases of COVID-19 in Beijing

Objective:To understand the clinical characteristics of patients with Corona virus Disease 2019 (COVID-19) so as to further provide practical references for clinical practice through collecting and analyzing the clinical data of COVID-19 patients admitted to Beijing Yuoan Hospital.Methods:The clinical data were summarized and analyzed for COVID-19 classified as severe patients and critically ill patients between January 28, 2020 and February 12, 2020 in Beijing Youan Hospital, including their age, gender, course of disease, epidemiological history, previous history, clinical symptoms, laboratory tests, CT findings of lungs, treatment and clinical outcomes. Statistical processing was performed using SPSS software (version 22.0).Results:Nineteen patients were included in this study and divided into two groups by investigators, i. e. ten of the patients had severe type and 9 had the critically ill type of the disease. The average age of the patients was 75 years (range from 58 to 94 years), including 10 males and 9 females. Their course of diseases varied from 1 day to 14 days with a median of 5 days. Among these patients, 4 patients resided in Wuhan, 13 patients had been in close contact with confirmed patients (including family gatherings, during meals, living in the same room, etc.), and 2 patients had no relevant epidemiological history. Fourteen patients suffered from underlying chronic diseases (including 11 cases with hypertension, 4 cases with coronary heart disease, 4 cases with cardiac insufficiency, 3 cases of chronic obstructive pulmonary disease, 2 cases of previous anterior myocardial infarction, 3 cases of arrhythmia, 1 case of heart valve disorders, 2 cases of diabetes mellitus, 2 cases of chronic renal disease, etc.), and 4 patients had 3 or more underlying diseases at the same time. The first symptoms at onset of illness were fever in 18 cases, cough in 13 cases, and dyspnea in 8 cases. Seventeen patients had absolute lymphocyte count (ALC) lower than the normal range. Eleven patients experienced different degrees of hepatic injury, 10 patients had renal injury, 7 patients had abnormal myocardial enzymes to a varying degree. C-reactive protein was increased in 18 patients.12 patients received hormone therapy, 4 patients were treated with gamma globulin, 3 patients used non-invasive ventilator mechanical ventilation, and 7 patients used an invasive ventilator to assist ventilation. Moreover, 1 patient received continuous renal replacement therapy (CRRT), and 1 patient was treated with extracorporeal membrane oxygenation (ECMO). As of February 17, 2020, 10 patients had recovered or been discharged upon treatment, 4 patients were still in the intensive care unit (ICU), and 5 patients had died.Conclusions:The elderly and individuals who had underlying diseases were considered as independent predictors for developing severe and critical disease, and were at higher risk of increased mortality. The decrease of ALC and the increase of C-reactive protein levels in the patients, and the abnormality of liver function, renal function and myocardial enzyme spectrum to different degrees suggest the correlation with severe cases. Steroids and ventilator are effective in the treatment of severe cases.
9.Optimization of purification process and component analysis of alkaloids from Zanthoxylum bungeanum Maxim

Objective To optimize the process conditions and analyze the components of alkaloids from Zanthoxylum bungeanum Maxim(Z. bungeanum)using macroporous resin. Methods Combining single factor tests and orthogonal tests, the content of hydroxy-α-sanshool(HAS)and hydroxy-β-sanshool(HBS)were considered as indexes to determine the best process parameters. Ultra-performance liquid chromatography-quadrupole tandem time-of-flight mass spectrometry(UPLC-Q-TOF-MSE)was used to identify the structures of alkaloids. Results The optimal conditions were Mitsubishi HP-20 macroporous resin, the loading solution concentration was 0.2 g crude drug/ml, the ratio of crude drug to resin volume was 1 g∶2.5 ml, the diameter/height ratio of resin column was 1∶7, the dynamic adsorption flow rate was 4 times of bed volume(BV)per hour, and the adsorption time was 1 h. Impurities were removed by using 2 BV of 20% ethanol, 5 BV of 80% ethanol was used to elution, and the content of HAS and HBS was 4.71% and 1.02%, respectively. A total of 20 alkaloids were identified from Z. bungeanum. Conclusion This method was stable and feasible, obtaining high purity and various kinds of alkaloids, which could be used for the enrichment and purification of alkaloids from Z. bungeanum.
10.Distribution and seasonal fluctuation of visceral leishmaniasis vectors sandflie in Lüliang City of Shanxi Province in 2023

Objective To investigate the distribution and seasonal fluctuations of visceral leishmaniasis vectors sandflies in Lüliang City, Shanxi Province, so as to provide insights into assessment of the visceral leishmaniasis transmission risk and formulation of visceral leishmaniasis control measures. Methods A total of 12 natural villages were sampled from Shilou County, Lishi District, Lanxian County, Linxian County and Wenshui County in Lüliang City, Shanxi Province from June to September, 2023, and sandflies were captured using light traps from 7 breeding habitats, including farmers’ houses, sheep pens, cattle pens, chicken coops, pig pens, mule and horse pens, and loess-cave dwellings. Following morphological identification of the sandfly species, the distribution of sandflies and the seasonal fluctuations of the sandfly density were analyzed. In addition, the Leishmania was detected in sandflies using a real-time fluorescence quantitative PCR assay. Results A total of 2 831 sandflies were captured with 156 light traps in Lüliang City from June to September, 2023, including 2 638 female sandflies (93.18%) and 193 male sandflies (6.82%), and the average density was 16.91 sandflies/(light-night). The seasonal fluctuations of the sandfly density all appeared a unimodal distribution in all survey sites, and the sandfly density peaked in July and then declined rapidly. Among all types of breeding habitats, the greatest sandfly density was found in sheep pens [39.04 sandflies/(light-night)]. In addition, 4.08% (2/49) of the sandfly samples were tested positive for Leishmania nucleic acid as revealed by the real-time fluorescence quantitative PCR assay. Conclusions Sandflies were widely distributed in Lüliang City, Shanxi Province in 2023, and the peak of the sandfly density was observed in July, which had a visceral leishmaniasis transmission risk. Intensified surveillance of visceral leishmaniasis and sandfly vectors is required and targeted vector control is recommended.
