1.Psychological intervention combined with hydrotalcite in treatment of gastroesophageal reflux disease clinical research

Objective To study the psychological intervention combined with hydrotalcite in treatment of gastroesophageal reflux disease clinical effect.Methods 100 patients with gastroesophageal reflux in our hospital from February 2015 to October 2016 were selected as the research object in this study.They were randomly divided into the control group and the experimental group, each group was divided into two groups.The control group of hydrotalcite in treatment of chewing, oral.The patients in the experimental group received psychological intervention on the basis of hydrotalcite treatment, strengthen the communication with patients, patients understand the inner confusion, eliminate the patient anxiety and discomfort, improve the treatment compliance of patients.Comparative analysis of the experimental group and control group of patients with treatment effect.Results After treatment, 50 patients in the experimental group, the total effective number of cases of clinical symptoms was improved in all cases, the effective rate of treatment was 92%, and the number of effective cases of electronic gastroscope mucosal repair was 47 cases, the effective rate was.In the control group of 50 cases, the total effective number of cases of clinical symptoms was improved in 40 cases, the effective rate of treatment was 80%, and the number of effective cases of electronic gastroscope mucosal repair was 41 cases, the effective rate was about 82%.The effective rate of the experimental group was significantly higher than that of the control group(P<0.05).Conclusion Psychological intervention combined with hydrotalcite in treatment of gastroesophageal reflux disease has good clinical effect, can improve the therapeutic effect in a certain extent, with further clinical promotion and application significance.
2.Occult neck matastasis and treatment of early oral tongue squamous carcinomas

Purpose:To study occult neck metastasis and the effects of salvage treatment. Methods:18 patients with stage Ⅰ or Ⅱ oral tongue squamous carcinomas had not received previous treatment. The tongue primary was excised via the transoral route and the neck was observed closely during follow-up.Results:The incidence of occult neck matastasis was 44% (7/16). The salvage rate after appearance of nodal matastasis was 27%(3/7).Conclusions:In oral tongue cancers, elective neck treatment should be considered regardless of a small primary and negative neck examination because of the high incidence of occult nodal matastasis and the poor outcome after salvage treatment.
3.COMPARATIVE ANALYSIS OF TONGUE SQUAMOUS CELL CARCINOMA BETWEEN YOUNG AND OLD PATIENTS

Objective Tongue cancer shows an increasing frequency in young individuals. There is controversy concerning the clinical course and outcome for oral tongue cancer in young patients.Methods A retrospective review of 23 patients under 40 years of age with squamous cell carcinoma of tongue was performed. These patients were matched to an older patients group. The 5-year disease-free survival, rates of local, regional and distant recurrence were determined for both groups. Results The 5-year disease-free survival for the young patients was 68 % versus 74 % in the older group (P>0.05). 7/23 (30 %) of young patients recurred locally versus 3/23 (14 %) of the older patients (P>0.05). 6/23 (26 %) of young patients recurred regionally versus 4/23 (18 %) patients in the older group (P>0.05). The metastatic rates were similar in both groups.Conclusion In this series, young patients with sguamous cell carcinoma of the oral tongue had a higher locolregional recurrence rate than did older patients. This dose not mean that there is a survival difference.
4.Relationship between ANXA1 and recurrence and metastasis of malignant tumors

Malignant neoplasm recurrence is the most important prognostic factor for patients.Looking for tar-get gene is an important part of cancer research.Annexin Ⅰ (ANXA1 )is to be discovered in the seventies century, composition by 13 calcium -phospholipid binding proteins annexin superfamily members of the first molecules. ANXA1 expressed in a variety of malignant tumors,ANXA1 expression levels were significantly reduced or absent in most tumor tissues.The study found that changed ANXA1 expression in tumor cells may have a causal relationship with the malignant phenotype of tumor cells.This paper aims to investigate the relationship between annexin Ⅰ and malignant neoplasm metastasis.
5.Design improvement and application of transfer trolley for collection and distribution in central sterile supply department

Objective To modify the existed transfer trolley for collection and distribution so as to enhance cleaning and disinfection.Methods The existed transfer trolley was added with an intake pipe,a spray nozzle,a drilled divider,a drainage pipe and etc for perfusion rinse,and equipped with auxiliary mechanisms such as a protective support and preserver.Omnidirectional cleaning and disinfection of the trolley were executed by the intake pipe,nozzle and high-pressure irrigation machine.Results The trolley prevented the disinfection solution from spitting,ensured the safety of the staff and environment,and had the qualified rate of disinfection being 100%.Conclusion The modified transfer trolley gains advantages in environmental protection,energy saving and practicability,and thus is worthy promoting clinically.
6.Association between helicobacter pylori infection and dyslipidemia in different gender population

Objective To assess the association between helicobacter pylori (HP) infection and dyslipidemia in different gender population.Methods We conducted cross-sectional analysis using data of 1921 cases of demographic characteristics,anthropometry,life style,lipid profile,etc.,from the subjects who received health examination from January 2010 to June 2012 in Department of Geriatrics,Tongji Hospital.Diagnosis of HP infection was achieved by using 14C-Urea Breath Test (14C-UBT).The participants were divided into HP infection positive group and HP infection negative group by 14C-UBT.Results In female subjects,the levels of total cholesterol/high-density lipoprotein cholesterol (TC/HDL-c) were higher in HP positive group than HP negative group (P < 0.05),but there was no obvious difference between the levels of low density lipoprotein cholesterol (LDL-c) in HP positive group and HP negative group.However,in the male subjects,the levels of LDL-c and TC/HDL-c were significantly increased in HP positive group than HP negative group (P < 0.05).HP positive group had a greater risk for high TC/HDL-c both in female and male subjects.In female subjects,the risk for high TC/HDL-c in HP positive group was 1.90 times of that in HP negative group (95% CI,1.06 ~ 3.38).In male subjects,the risk for high TC/HDLc in HP positive group was 1.56 times of that in HP negative group (95% CI,1.21 ~ 2.00).But only in male subjects,the risk for high LDL-c in HP positive group was 2.33 times of that in HP negative group (95 % CI,1.34 ~ 4.06).Conclusions We observed that HP infection was probability associated with dyslipidemia.

Objective To detect the relationship between the molecular defects and their phenotypes in children with growth hormone insensitivity syndrome (GHIS).Methods 21 patients defined as GHIS were enrolled in the study.4 candidate genes (GHR,IGFALS,JAK2,and STAT5B) were analyzed by genomic DNA sequence screening and clinical relevance analysis.Results The statistical descriptions of the patients were showed as an average height standard deviation (SDS)-4.33 ± 1.91 (-9.17 to-2.21),average serum peak values of GH (22.67 ±20.98) tg/L (11.33 to 104.21 μg/L),basal serum insulin-like growth factor-Ⅰ SDS-2.65 ± 0.53 (-3.57 to -1.79),insulin-like growth factor-binding protein 3 SDS-1.77 ± 1.64 (-4.13 to 0.96).Bone age of backward difference (chronological age-bone age) (43.10 ± 19.54) months (6 to 82 months).One of two children with severe growth failure and mid-face hypoplasia was found to a homozygote for G to A gene mutation in the intron 6 splice donor consensus sequences (IVS6 ds+ 1 G-A) in the GHR gene,causing its functional defect.3 cases with mild dwarf were found gene variations as novel finding:c.1097T>C c.1098C>T p.V366A pathogenic variant,c.1229C>T p.S410L and nt1843707 A→G of 5' UTR region in the IGFALS gene.JAK2 and STAT5b genes mutations were not found.Conclusion Molecular pathology of GHIS is considered as involving the defects of GHR and its signal pathway.The mutation of intron 6 splice donor sequences in GHR gene has been reported which affect the function of GHR.The 3 novel type base variants in IGFALS gene,causing non severe dwarfism,might be suspected with pathogenic roles of GHIS.
8.The clinicopathologic characteristics of desmoplastic fibroblastoma in neck and nape

OBJECTIVE To describe the clinical, CT or MRI, pathological characteristics of desmoplastic fibroblastoma for helping the diagnosis and treatment. METHODS The clinical data of 4 cases with desmoplastic fibroblastoma were retrospectively analyzed. RESULTS The tumors of all 4 cases were found in the compartment of muscles, and can be removed by conservative excision. The 4 cases were followed up for 38, 7, 6, 3 months respectively, and no local recurrences were found. CONCLUSION Desmoplastic fibroblastoma is a benign fibroblastic neoplasm with distinctive clinical, CT or MRI imaging and pathological characteristics. Optimal management is conservative excision with functional preservation. Needle aspiration cytology and immunohistochemistry are of non-diagnostic value.
9.Application effect of high quality nursing mode in patients with type 2 di-abetic osteoporosis

Objective To study the effect of using high quality nursing mode in patients with type 2 diabetic osteo-porosis. Methods From January 1, 2012 to January 1, 2014 in our hospital 120 cases of type 2 diabetic osteoporosis were randomly divided into the study group and the control group, the control group were given routine care, patients in study group were given high quality nursing mode. Results The study group patients in nursing after the body mass in-dex, self rating anxiety scale, self rating depression scale were significantly better than the control group (P<0.05); the difference of the study group patients after care at the time of hospitalization and the control group had statistical sig-nificance (P<0.05). Conclusion Type 2 diabetic osteoporosis patients with high quality nursing, application effect is remarkable, can effectively improve the patients with anxiety, depression and other negative emotions, shorten the hos-pitalization time, improve the quality of life of patients, it is worth widely used in clinical nursing.
