1.Analysis of problems existing in using process of micro-perfusion pump by low birth weight infants and the improvement measures

Objective To make a concrete analysis of micro-perfusion pump and compatible syringes used in neonatal intensive care unit (NICU) for low birth weight infants,and to find solutions to the defects in application.Methods 20 ml sterile syringes were set different infusion rates (1ml/h,5ml/h,6ml/h,10ml/h),different infusion time (5min,30min,lh).Then,the card slot on the micro-perfusion pump where the end of 20ml sterile syringe was placed was regulated.The regulated syringes were also set different infusion rates (5ml/h,6ml/h,l0ml/h) and time as before.The error accuracy was calculated.Results The results showed that at the end stage of 20ml sterile syringes pumping,there was always 0.5~1.5 ml residual liquid which could not be pumped out,but perfusion pump displayed injection was completed.Before regulation,the error accuracy between different infusion rates of 20ml sterile syringes was 6.00%,3.80%,2.33%,1.30% respectively.However,after improvement,the regulated syringes could completely pump all liquid out,and the error accuracy between different infusion rates and different infusion time was also superior to pre-improvement.Conclusions Improved micro-infusion pump and sterile solution prefilled syringe have no drug residual,and display high accuracy and small error accuracy.
2.Similarity Evaluation of Dissolution Profiles of Self-development and Original Preparation of Solifenacin Suc-cinate Tablet

OBJECTIVE:To explore the similarity of dissolution profiles of self-development and original preparation of Solife-nacin succinate tablet,and provide reference for the prescription and process screening of the former one and the quality similarity evaluation of the latter one. METHODS:The paddle method was adopted with rotational speed of 50 r/min,using water,pH1.2 hy-drochloric acid solution,pH4.0 acetate buffer solution and pH6.8 phosphate buffer solution as dissolution media,HPLC was used to determine the cumulative dissolution of main components of self-development and original preparation of Solifenacin succinate tablet at different time points,dissolution profile was drew,then f2 was used to evaluate its similarity. RESULTS:In the 4 dissolu-tion media,the f2 of both self-development and original preparation of Solifenacin succinate tablet was higher than 50,which indi-cated that the dissolution profiles showed similarity. CONCLUSIONS:The established HPLC is suitable for the dissolution determi-nation of Solifenacin succinate tablet;the dissolution profiles of the self-development and original preparations are basically simi-lar,which indicates the prescription and technology of self-development preparation are feasible.
3.The effects of robot-assisted lower-limb rehabilitation plus psychological intervention on post-stroke depression

Objective To observe the effects of a lower-limb rehabilitation robot plus psychological intervention on depression and lower limb motor function of the stroke patients.Methods Sixty stroke patients were randomly divided into a treatment group (n =30) and a control group (n =30).The patients of control group were treated with routine rehabilitation and psychological intervention,while the treatment group were treated with lowerlimb rehabilitation robot,in addition to the interventions of the control group,for 10-20 minutes daily,6 days per week for 8 weeks.Both groups were assessed with Hamilton Depression Scale (HAMD),Simplified Fugl-Meyer assessment (FMA) and Functional Ambulation Category (FAC) before and at the end of 2,4 and 8 weeks of treatment.Results There was no significant difference between the 2 groups in all the assessment parameters before and after 2 weeks of treatment.At the end of 4 weeks of treatment,both groups improved significantly with regard to the scores of HAMD,FMA and FAC classification,and the treatment group improved to a significantly greater extent than the control group (P < 0.05).At the end of 8 weeks of treatment the scores of HAMD,FMA and FAC classification were further improved significantly in treatment groups when compared with values obtained at the end of 4 weeks after treatment,but the scores of the control group was not significantly improved at the end of 8 weeks when compared with those at the end of 4 weeks of treatment.Conclusion Lower-limb rehabilitation robot plus psychological intervention can significantly help relieve the post-stroke depression and improve lower motor function of stroke patients.
4.The effects of cycling on the lower limb motor function and walking ability of hemiplegic stroke patients

Objective To investigate the effects of exercising on a cycle ergometer on lower limb motor function and the walking ability of hemiplegic stroke patients.MethodsSixty hemiplegic stroke patients were randomly divided into a treatment group and a control group with 30 in each. Both groups were treated with routine rehabilitation, but the treatment group also undertook cycle ergometer training. The training lasted for 10-20 min/d, 6 d/week for 6 weeks. All patients were assessed with the Fugl-Meyer lower limb assessment ( FMA), functional ambulation category (FAC) classification and the modified Barthel index (MBI) pretreatment and 6 weeks post treatment.ResultsBefore intervention there was no significant difference between the two groups in any of the assessments. Six weeks post treatment, the average FMA, FCA classification and MBI results in the treatment group were significantly better than those pretreatment and better than those in the control group.ConclusionsCycling exercise combined with routine rehabilitation training can distinctly improve the motor function of the lower limbs and the walking ability of hemiplegic stroke patients.
5.The study of the effect of the different heat preservation methods preventing craniocerebral surgery patients with hypothermia

Objective To explore the effect of prevention intraoperative hypothermia of different heat preservation measures during craniotomy cerebroma resection surgery in the department of neurosurgery. Methods According to the inclusion criteria, a total of 90 patients with craniocerebral surgery were recruited and randomly divided into A, B, C groups with random number table method, and 30 patients in each group. And the three groups were respectively given different heat preservation measures of regular cover, regular cover with infusion heating, regular cover with force-air warming. Meanwhile the rectal temperatures of just entered into the operating room, immediately after anesthesia induction and every 30 minutes after induction of anesthesia, as well as the HR, BP, the amount of infusion and flushing liquor, operation time, incidence of chills during operation, the condition of surgical site infection and hospitalization days were recorded and analyzed statistically by means of the software of SPSS 13.0. Results Rectal temperatures of the three groups were no statistically significant difference (P>0.05) ,when the patients just were moved into the operating room, immediately after anesthesia induction and 0.5 h、1 h after anesthesia induction. The rectal temperatures of the three groups had statistically significant difference (P<0.05) from the 1.5h after anesthesia induction to the end of the surgery.The incidence rates of introperative hypothermia and chill of group C were 26.7%(8/30)and10.0% (3/30),lower than group A and group B:60.0%(18/30), 33.3%(10/30)and 53.3%(16/30), 13.3%(4/30), meanwhile the statistically significant difference was found in these three groups (χ2=7.500, P=0.024;χ2=6.237, P=0.044).The volume of intraoperative blood loss and transfusion were 200(100, 300), 45(0, 60) ml, which lower than group A and group B:300(200, 525), 215(0, 400) ml and 250(150, 500), 120(0, 200) ml, and statistically significant difference was also found in these three groups(H=7.612, P=0.022;H=6.194, P=0.045). Conclusions Regular cover with force-air warming can effectively reduce the incidence of intraoperative hypothermia and chills of craniotomy cerebroma resection patients in the department of neurosurgery and reduce intraoperative bleeding and transfusion volume, as well as ensure the effect of surgical treatment. So it′s worth popularizing in clinic.
6.Application of Dexamethasone Reduces Sensorineural High Frequency Hearing Loss after Mastoidectomy

Objective To investigate the protective effect of dexamethasone on hearing loss induced by canalwall-down mastoidectomy.Methods A total of 76 patients (76 ears) were randomized to dexamethasone group and non dexamethasone group with 38 patients in each group.For dexamethasone group,gelfoam soaked with dexamethasone (5 mg/ml) was topically applied to the round window niche at the end of the surgery,and dexamethasone was administrated intravenously at the dosage of 5 mg/50 kg immediately after the surgery.While for non-dexamethasone group,dexamctbasone was not used.The pure-tone audiograms were performed before mastoidectomy and 90 days after the surgery.Absolute bearing change was defined as the difference in hearing thresholds in decibels before and after the surgery.Results The changes of bone conduction thresholds in dexamethasone group were 8.3± 3.9 dB at 6 kHz,11.3±5.2 dB at 8 kHz,and 10.1±7.2 dB for HPTA (4-6-8 kHz high tone average).As in non-dexamethasone group,the changes of bone conduction thresholds were 13.7±4.6 dB at 6 kHz,25.1±5.4 dB at 8 kHz,19.3±9.7 dB for HPTA.There were significant differences in the changes of bone conduction thresholds between dexamethasone and non dexamethasone groups at frequencies of 6 kHz (P=0.039),8 kHz (P=0.007) and HPTA (P=0.009).Conclusion The results demonstrated that application of dexamethasone significantly reduced sensorineural high frequencies (6 kHz and 8 kHz) hearing loss after canal-wall down mastoidectomy,thus the use of dexamethasone is recommended.

Studies have found that obesity is not only closely related with the occurrence and development of breast cancer,but also can significantly increase the risk of breast cancer recurrence and death,especially the occurrence of postmenopausal estrogen receptor positive breast cancer.Therefore,it is of great importance to understand the influence of obesity on the prognosis of breast cancer.The intervene of diet and lifestyle,metformin and other drags,and other obesity targeted therapies have provided direction for future research on these influence.
8.The effect of hyperbaric oxygen therapy combined with repetitive transcranial magnetic stimulation on patients with cognitive dysfunction after cerebral infarction

Objective To observe the effect of hyperbaric oxygen (HBO) therapy combined with repeated transcranial magnetic stimulation (rTMS) on patients with cognitive dysfunction after cerebral infarction.Methods A total of 81 patients with cognitive dysfunction after cerebral infarction were randomly divided into a control group,an HBO group and a combined treatment group,each of 27.In addition to basic medication and traditional rehabilitation therapy,the HBO group was also treated with hyperbaric oxygen,while the combined treatment group received both HBO and rTMS.The mini-mental state examination (MMSE) and the Montreal cognitive assessment (MoCA) were administered before and after the four weeks of treatment.Results The average MMSE scores of the HBO and combined treatment groups were both significantly higher than that of the control group,with the former significantly higher than the latter.Moreover,the average MoCA score of the HBO group was also significantly higher than that of the combined treatment group,and both were significantly higher than that of the control group.Conclusion Hyperbaric oxygen therapy can more effectively improve the cognitive function of patients with cognitive dysfunction after the cerebral infarction when it is combined with repeated transcranial magnetic stimulation.
9.Imaging features of oblique vaginal septum syndrome and its diagnostic value

Objective To investigate the imaging findings of oblique vaginal septum syndrome and its diagnostic value.Methods Clinical and radiographic data of 13 patients with oblique vaginal septum syndrome were analyzed retrospectively and relevant literatures were reviewed.All 13 patients underwent ultrasonography examination preoperatively,and 8 of them underwent MRI examination,and 3 patients underwent CT examination as well.The position of the oblique septum,the size of the cavity in the rear of oblique septum, the positional relationship between the cavity in the rear of oblique septum and the cervix,and the concomitant changes of the uterine appendages and urinary system were analyzed.Results The ultrasonography and MRI examinations all manifested as uterus didelphys,cervix duplex,double vagina and obstructed hemivagina,and oblique septum originated from the middle of cervix duplex and attached to one side of the vaginal wall,shielding the ipsilateral cervix;oblique vaginal septum was on the right side in 4 cases and on the left side in 9 cases;the average volume of the cavity in the rear of oblique septum in 6 cases of type Ⅰ was 255 mL,and the average volume in 6 cases of type Ⅱ was 74 mL,and the volume in 1 case of type Ⅲ was 56 mL;2 cases were combined with ipsilateral hematosalpinx and 1 case was combined with a chocolate cyst of ipsilateral ovary;12 cases were accompanied with ipsilateral renal agenesis and 1 case was accompanied with ipsilateral renal hypoplasia.CT examinations of 3 cases all presented as uterus didelphys,a cystic hypodensity lesion under unilateral cervix and ipsilateral renal agenesis.Conclusion Imaging findings of oblique vaginal septum syndrome are quite characteristic. Ultrasonography,CT and MRI examinations have certain value for accurate diagnosis of the disease.

Sepsis-associated acute kidney injury (SA-AKI) is defined as the presence of acute kidney injury (AKI) in the context of sepsis. In the setting of genetic susceptibility, sepsis can lead to SA-AKI through various mechanisms. Based on differences in pathophysiological mechanisms, SA-AKI is categorized into different "endotypes" and manifests as distinct "subtypes". The combination of biomarkers and predictive models has the potential to early identify high-risk AKI patients and elucidate SA-AKI "endotypes". Volume resuscitation and blood purification are optimized strategies for SA-AKI treatment. Furthermore, clinical research on SA-AKI in children is promising.
