1.Discussion of the preservation method of novopen needle after repeated uses

Objective To investigate the safety of novopen needle after repeated uses in order to save the expenses of patients. Methods 60 diabetic patients undergoing insulin therapy were random-ized into the common temperature group and the refrigerated storage group. The novopens were stored at common temperature in a clean and dry place, while the refrigerated storage group was stored in cold clos-et of a refrigerator. Oropharyngeal swab bacterial culture was used to compare the incidence of infection in two groups on the first, third, fifth, and seventh days. Results There was no bacterial growth in either storage condition. Conclusions The novopen needle can be repeatedly used in clinic work.But they must be used individually. Storage at common temperature or in a refrigerater can reach the same effect.
2.Effects of penehyclidine hydrochloride injection combined with safflower injection on TLRs / NF-κB signaling pathway and T cell subsets in mononuclear cells of patients with organophosphorus pesticide poisoning

Objective To investigate the effects of penehyclidine hydrochloride injection combined with safflower injection on TLRs/NF-κB signaling pathway and T cell subsets in mononuclear cells of patients with organophosphorus pesticide poisoning. Methods 80 cases of organophosphate poisoning patients from June 2014 to June 2016 in onglu first people's hospital were selected,randomly divided into control group and observation group, the control group was treated with penehyclidine hydrochloride,observation group were treated with Honghua injection based on the treatment of control group. The effects were compared between two groups of TLRs/NF-κB in peripheral blood mononuclear cells pathway and T cell subsets in peripheral blood, and compared two groups of patients with cholinesterase activity. Results On the third,fifth and 7th day of treatment,cholinesterase activity in both groups was higher than that before treatment,the activity of cholinesterase in the observation group was significantly higher than that in the control group (P<0.05).After treatment,compared with before treatment,the levels of TLR2,TLR4 and NF-κB in the two groups were significantly decreased,and TLR2,TLR4,NF-κB levels in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05),Th17,Th17/Treg levels in the observation group were significantly lower,and lower than the control group,Treg content was significantly higher,and higher than the control group,the difference was statistically significant (P<0.05). Conclusion Penehyclidine hydrochloride injection combined with safflower injection can help to reduce Th17,Th17/Treg levels,increased Treg cell content,regulate the inflammatory response, and promote recovery of cholinesterase activity.
3.Effect of nonenzymatically glucosylated low density lipoprotein on in vitro cultured macrophages and monocytes

We observed the catabolic characterestics of in vitro prepared nonenzymatically glucosylated low density lipoprotein (glcLDL) in mouse peritoneal macrophages (M?) and human monocyte line (U_(937); and found that glcLDL did not stimulate the increase degradation of glcLDL significantly nor did the accumulation of glcLDL cholesterol in mouse peritoneal M?. However the degradation of glcLDL by U_(937) cells was positively proportional to the concentration of glcLDL. The pre-treatment of U_(937) cells with 12-O-tetradecanoylphorbol 13-acetate (TPA) increased the degradation of glcLDL and was also positively proportional to the concentration of glcLDL. The relationship between these phenomena and the accelerated developement of atherosclerosis in diabetes mellitus was discussed.
4.Endoscopic histoacryl injection combined with ligation in the treatment of severe esophageal and gastric varices

BACKGROUND:As a commonly used tissue adhesive in China, histoacryl can produce transient polymerization and rapidly solidify biological tissue, and it has good prospect in treatment of esophageal and gastric varicose bleeding. OBJECTIVE: To explore the short- and long-term clinical efficacy of endoscopic histoacryl injection combined with ligation for the treatment of esophageal and gastric varices. METHODS: Ninety patients with esophageal and gastric varices were treated by endoscopic adhesive injection and dense ligation. There were 64 males and 26 females, aged 22-75 years. At 2-4 weeks post-treatment, patients were re-checked under an endoscope. The short-term efficacy and adverse reactions, such as the hemostasis rate, varicose vein, varicose vein diameter, red color sign and portal hypertension gastropathy were observed. Al patients were folowed up for 3-39 months after varicose vein was eradicated, to monitor the middle- and long-term clinical efficacy, including varicose vein, tissue adhesive discharging and local proliferation, rebleeding, recurrence and complications. RESULTS AND CONCLUSION: (1) Short-term efficacy: The number of varicose veins significantly reduced, red color signs reduced, and portal hypertension gastropathy improved (P< 0.01), but three cases appeared early rebleeding. The adhesives began to discharge from 2 weeks to 3 months post-treatment, no case appeared esophageal stenosis, perforation, severe infection, heterotopic embolism and other injection-related complication. (2) Middle- and long-term efficacy: varicose vein recurred in nine cases and five cases developed long-term rebleeding. Experimental findings indicate that, endoscopic histoacryl injection combined with ligation is effective to treat severe esophageal varices or (and) bleeding, with high success rate and good hemostasis effect, and it also prevents long-term rebleeding and recurrence.
5.An Immunohistochemical and Histochemical Study on AutonomicInnervation of the Gall Bladder and Bile Ducts in Cats

This paper is to report the autonomic innervation of the gall bladder and extrahepatic bile ducts studied on whole mount stretch preparations and cryostat sections with acetylcholinesterase staining, glyoxylic acid induced fluorescence histochemical method, and PAP immunohistochemical method.The autonomic nerves of the gall bladder and extrahepatic bile dudts in cats consist of two plexuses, composed of cholinergic, adrenergic and peptidergic nerves, and situated in the subserosa and submucosa respectively. They are known as subserosal and submucosal ganglionated plexuses. They distribute in the region from the fundus of the bladder, to the neck and the cystic duct, through the common bile duct downward, finally to the duodenal papilla.The adrenergic nerves mainly run along the, blood vessels, distributing in the serosa and lamina propria of the mucosa of the bladder, and participating in the formation of the subepithelial and perivasctilar plexuses. The adrenergic nerves are most densely distributed in the cystic duct and mosl sparsely in the fundus. This finding is in good agreement with the tissue concentration of noradrenaline(NA) determined with biochemical asssay, which is also the highest in the tissue of the cystic duct. The difference of NA concentration between the cystic duct tissue and other biliary tissues was statistically Gignificant(P0.05).Immunohistochemical study revealed that there are 4 kinds of immunoactivc peptidergic nerves, i. e. nerves with vasoactive intestinal polypeptide (VIP), substance P(SP), L-enkephalin (L-ENK), and somatostatin (SOM) respectively. The 4 kinds of nerves distribute extensively in all the tissue layers of the ex-trahepatic biliary tract and participate in the formation of all the nerve plexuses. Among them, VIP nerves are the richest, then the SP and L-ENK nerves, and the SOM nerves the sparsest. In general, peptidergic nerves are densely distributed in the sphincter of Oddi.Finally the physiological significance of the peptidergic nerves was discussed.
6.Investigation and Feasibility Analysis of Pharmacy Trusteeship in Some Shanghai Medical Institutions

Objective:To explore the feasibility of pharmacy trusteeship in Shanghai medical institutions to provide useful informa-tion for the reform of medicine without added profit and pharmacy transition to clinical pharmacy. Methods: On-site investigation of seven hospitals and one famous drug retail company in Shanghai was performed combined with the communication with relative person-nel and non-random sampling questionnaire survey on the staff of pharmacy. Statwing software was used in the statistical analysis of the data. Results: A total of 93 valid questionnaires in the study were taken back. Although one hospital has achieved great progress in pharmacy trusteeship, most of respondents had negative attitudes and the common thought was that pharmacy trusteeship did more harm than good to the development of clinical pharmacy at present. Conclusion: As a consequence of drug zero interest policy and clinical pharmacy development, pharmacy trusteeship should be explored and attempted. The duties and responsibilities of all parts in pharma-cy trusteeship should be definite in law in order to standardize drug retail market and protect the interests of patient.

Objective To discuss the etiological factor,diagnosis and treatment of gastroduodenal perforation.Methods The clinical datas of 198 cases in our hospital were submitted to a retrospective study.Results There were 44 cases of gastric ulcer,146 cases of gastric ulcer and 8 cases of gastric cancer,while 192 were cured,6 died.196 cases accepted operation(188 with gastroduodenal neoplasty and 8 with subtotal gastrectomy),while 2 were treated by conservative therapy.Conclusion Peptic ulcer is the principal etiological factor of gastroduodenal perforation,and inject air into gastroinotestinal tract with a gastric tube can increase the diagnostic rate.The neoplasty still remain to be a modus operandi of gastroduodenal perforation.
9.The effect observation of carvedilol and metoprolol on MMP-9 changes and left ventricular remodeling in patients with hypertension heart disease

Objective To study the curative effect of MMP-9 changes on left ventricular hypertrophy patients with hypertension heart disease by carvedilol and metoprolol.Methods 168 cases were divided into two groups.The control group(84 cases)applied metoprolol.The observation group applied carvedilol.The changes of IVST,LVPWT, LVMI and LVGI,expression of MMP-9 were observed in two groups after treatment.Results The values of IVST, LVPWT,LVMI and LVGI,expression of MMP-9 were decreased after treatment.But the decreased values of IVST [(1.72 ±0.35 )mm vs (0.36 ±0.11 )mm,t =5.75,P <0.05 ],LVPWT[(0.66 ±0.25 )mm vs (0.24 ± 0.18)mm,t =4.32,P <0.05],LVMI[(16.23 ±3.35)g/m2 vs (7.44 ±2.24)g/m2 ,t =9.86,P <0.05 ],LVGI [(0.52 ±0.12)g/m2 vs (0.40 ±0.11)g/m2 ,t =4.24,P <0.05],MMP-9[(76.23 ±20.87)vs(42.55 ±10.87),t =6.54,P <0.05]were higher in the observation group than in the control group.Conclusion The treatment of carvedilol is satisfied,can improve ventricular remodeling,decreased the expression of MMP-9,and is worthy of being recommended in left ventricular hypertrophy patients with hypertension heart disease.
10.Clinical Observation on 30 Cases of Acute Osteoarthritis of Knee Joint Treated by Sishen Jian

Objective To observe the clinical effect of herbal decoction Sishen Jian on acute osteoarthritis of knee joint.Methods The 60 osteoarthritis cases in the attack stage were randomized into treatment group(30 cases)and control group(30 cases).The treatment group was given the decoction Sishen Jian and Diclofenac Sodium Sustained Release Capsules and the control group was given the capsules only.Two weeks made one course of treatment.Before and after treatment,the symptoms such as pain and morning stiff- ness,the signs including joint swelling and tenderness,and the erythrocyte sedimentation rate(ESR)and C reactive protein were ob- served for comparison.Results The total effective rate of the treatment group and control group was 96.66% and 53.26% respec- tively.The difference of two groups was significant(P
