1.SELENIUM LOAD TESTS IN THE CHILDREN FROM KASHIN-BECK DISEASE AFFECTED AREAS

Se load tests were carried out by administering 150?g Se as 0.1% sodium selenite 0. 5ml in 5 groups of male children (7~13 years) including 2 groups from different Kashin-Beck disease affected areas, 2 groups from non-endemic areas and another from an endemic area supplementing Se-fortilled salt for 4 years. The results showed that Se urinary excretion amounts, 6,12 and 24 hour accumulated rates and 24 hour net excretion amounts as well as rates after Se administration were similar between 2 endemic groups, but all far lower than those of 2 non-endemic groups; that urinary Se excretion peak in endemic groups delayed as comparied with non-endemic groups and that the above—mentioned parameters in endemic group supplementing Se restored to the levels of non-endemic groups. All those findings suggested that the children from Kashin-Beck disease affected areas might be in a Se deficiency status and Se supplementation to the endemic children consuming Se fortified salt significantly improve their Se deficiency status.

3.Construction of pRluc-hNTSR1-pcDNA3.1 eukaryotic expression vectors and its expression in isolated cells

Objective To construct expression vectors that Renilla reniformis (Rluc) fused with neurotensin type 1 receptor (NTSR1),and to investigate the interaction between NTSR1 and other receptors,as well as intercellular signal transduction mechanism mediated by neurotensinl-R.Methods The human NTSR1 gene was amplified by PCR using the plasmid pcDNA3.1-hNTSR1 as template.The PCR product was digested,ligased with the plasmid pRluc and then be transformed into the competent cell Top10.The construct was identified by DNA sequencing.The recombinant plasmid was transiently transfected into human embryonic kidney 293 ( HEK293 )cells,and the expression of pRluc-hNTSR1-pcDNA3.1 was detected by confocal microscopy and Western blot.Results The fragment of 1257 bp was amplified by PCR,and the DNA sequences were identical with the gene in GenBank ( NM_002531 ).Western blot showed a band about 90kDa.Confocal microscopy showed that NTSR1 was expressed on the plasma membrane.Conclusion The pRluc-hNTSR1-pcDNA3.1 eukaryotic expression vector is successfully constructed,and the expression vector can be used to investigate the interaction between NTSR1 and other receptors,as well as intercellular signal transduction mechanism mediated by neurotensinl-R,which will provide new target for drug development.
4.The experience in treatment of thirty-two cases of brachial artery trauma associated with humeral super-middle fracture

Objective To explore the treatment methods for brachial artery trauma associated with humeral super-middle fracture.Methods Thirty-two patients of brachial artery trauma associated with humeral super-middle fracture were treated in our department from March 2001 to April 2005 with in tramedallary expanded self-locking nail (IESN) combined with autotransplantation of great saphenous vein to repair brachial artery.Results All patients were followed up for 12 to 18 months,13.6 months on the average.No stenosis occurred at the anastomosed veins according to color Doppler ultrasound.All fractures achieyed clinical healing and the limbs survived well.Conclusion The IESN combined with autotransplantation of great saphenous vein to repair brachial artery is an effective method to treat brachial artery traumaassociated with humeral super-middle fracture,having the advantage of reasonable operation procedure.stabile immobilization and satisfactory functional recovery.
5.Aescine in treatment of lumbar intervertebral disc prolapse

AIM:To observe the therapeutic effect of aescine in the treatment of lumbar intervertebral disc prolapse(LIDP).METHODS:One hundred and seventy-six patients with LIDP were divided into 2 groups:aescine group of 80 patients[M56,F24;age(43±s17)a,course of disease(7.4±2.3) mo] received conventional therapy plus aescine 10-15 mg in 5 % glucose or 0.9 % sodium chloride injection 250 mL,iv,drip,bid,for 7-10 d.Control group of 96 patients[M57,F39;age(42±16)a,course of disease(7±3)mo]only received conventional therapy,including towing,massage,acupuncture,anodyne.Therapeutic effects of both groups were compared after 4 wk.RESULTS:The total effective rate of aescine and control group was 95 % and 81 %,respectively(P<0.05),while in those course of disease less than 0.5 a the effective rate was 97 % and 85 %,respectively(P<0.05).No obvious adverse reactions occoured. CONCLUSION:Aescine is safe and effective for the treatment of LIDP.The shorter the disease course is,the better the therapeutic effects are.
6.Fluoroscopically guided percutaneous internal fixation with the Universal Spine System for thoracolumbar fractures

Objective To study the feasibility of fluoroscopically guided percutaneous internal fixation with the Universal Spine System(USS) in the treatment of thoracolumbar fractures.Methods A total of 8 patients with thoracolumbar fractures without the need of decompression of the spinal canal underwent posterior percutaneous pedicle screw fixation by using the USS from October 2002 to May 2004.A fixation rod was placed under the vertebrae muscle to complete the reduction and fixation.Results The operation time was 120~240 min(mean,140 min),and the intraoperative blood loss was 20~90 ml(mean,40 ml).The Cobb's angle decreased from 21.5??4.3? preoperatively to 2.7??1.5? postoperatively(t=17.541,P=0.001).The collapse of vertebra was 18.5%?4.1% preoperatively and 2.4%?1.0% postoperatively(t=16.504,P=0.001).All the patients got out of bed 3 weeks after operation with the assistance of girdle brace.Effects of treatment were evaluated according to the Frankel grade at the third postoperative week: 2 patients recovered from grade C to D,1 patient from D to E,1 patient remained in D, and 4 kept in E. Conclusions Percutaneous internal fixation by using the USS in the treatment of thoracolumbar fractures without the need of decompression of the spinal canal offers advantages of minimal invasion and quick recovery,though it has some technical difficulties and disadvantages of long-time exposure under X-ray.

8.Impacts of the lump-sum prepayment practice of medical insurance and the countermeasures

The paper analyzed the impact of the trial for "lump-sum prepayment practice of medical insurance" on tertiary hospitals in Shanghai initiated by Shanghai Medical Insurance Bureau since 2009. Based on the analysis, the authors recommended that the hospitals should adapt to the changes by raising quality of care, improving cost accounting, informationizing and refining medical insurance information management, as well as controlling expenses on a rational basis. These approaches will help them adapt to and promote the ongoing health reform in China.
9.Role of G-protein-coupled receptor kinases in cell migration and signal transduction

G-protein-coupled receptor kinases (GRKs) are a family of serine/threonine protein kinases. The investigators pay much attention to the roles of GRKs in the signal transduction through G-protein-coupled receptors (GPCRs) with arrestin ever since a long time ago. Due to the physiological and pathological observations with the methods of deletion or overexpression, GRKs are considered as new drug targets. The kinases play a role in the pathogenesis of hypertension and cell migration through GPCRs and Hedgehog signaling pathways. As the development of research techniques, especially bioluminescence resonance energy transfer (BRET) and fluorescence resonance energy transfer (FRET), the special mechanism of GRKs for GPCRs is more evident. In this review, we discuss the recent achievement in the roles of GRKs signaling and the related newest research techniques.
10.Mutant prevention concentrations study of linezolid and vancomycin against methicillin-resistant staphylococcus aureus isolates

AIM:To compare the mutant prevention concentration(MPC)of linezolid and vancomycin against methicillin resistant staphylococcus aureus(MRSA)and study the correlation between minimal inhibitory concentration(MIC)and MPC.METHODS:MICs and MPCs of two drugs against 35 MRSA clinical stains were determined by agar plates dilution method.The correlations between MIC and MPC were determined by linear regression.The ability to restrict the resistance was evaluated according the pharmacokinetics of two drugs.RESULTS:The MPCs of two drugs against MRSA were 16 and 8 ?g/mL and the MPC/MIC was 8.MPCs and MICs correlated poorly(R2 were 0.32 and 0.008,respectively).According to pharmacokinetics of two drugs,the concentration of linezolid was inside the MSW(mutant selective window)for the entire dosage interval,while the concentration of vancomycin exceeded the MPC for the most dosage interval.CONCLUSION:The capacity of vancomycin for restricting the selection of MRSA resistant mutants is stronger than that of linezolid.There is low correlation between MPC and MIC.
