Objective To investigate the effects of peroxisome proliferator activated receptor gamma (PPARγ) on malignant arrhythmia in myocardial of ischemia/reperfusion(I/R) rats. Methods Twenty-four SD rats were randomly divided into four groups:Sham group, I/R group, rosiglitazone (ROS) group and PPARγ inhibitor GW9662 (GW) group. The myocardial I/R injury was induced by ligation of the left anterior descending coronary artery, with ischemia for 30 min and reperfusion for 2 h. The whole process limb Ⅱ lead electrocardiogram was applied to observe the frequency of malignant arrhythmia and record the corrected changes of QT(QTc) interval. RT-PCR was used to detect the expression of PPARγ mRNA. Results There were 5 cases of malignant arrhythmia in ROS group, 2 in I/R and 1 in GW group, 0 was found in Sham group. There was a prolongation of the QTc interval in ROS group than the other groups after ischemic stage (P<0.05). Compared with I/R group and ROS group, the QTc intervals were shorten in ischemia 30 min and reperfusion process in GW group (P<0.05). Compared with sham group, the expression of PPARγ mRNA was significantly increased in other three groups (P<0.05). The expression level of PPARγ mRNA was the highest in ROS group. The expression level of PPARγ mRNA was reduced in GW group compared with that of I/R group and ROS group (P<0.05). Conclusion Over expression of PPARγ may lead to the occurrence of malignant arrhythmia in myocardial ischemia/reperfusion rats.

Objective To investigate the effects of PPAR-gamma expression on reperfusion arrhythmias in different precon ditioning myocardial ischemia/reperfusion(I/R) animal model. Methods Thirty-two SD rats were randomly divided into 4 groups(n =8):rosiglitazone+ I/R group (ROS group),GW9662 + I/R group(GW group),I/R group and sham group (Sham group). The I/ R animal model was constructed by ligation of the left anterior descending coronary artery, with ischemia for 30 min and reperfusion for 120 min.The dynamic limb Ⅱ lead electrocardiogram monitoring was performed;PPAR-gamma mRNA was detected by fluorescent quantitative PCR and the change of PPAR-gamma protein expression was detected by Western blot. Results The increasing range of QRS wave width detected before operation,at 30 min of ischemia,at 1,2 h of reperfusion from large to small were in turn the ROS group,I/R group,GW group and Sham group;the reperfusion arrhythmia score in the ROS group was significantly higher than that in the other groups(P<0.05),while the GW group was relatively reduced. The expression level of PPAR-gamma mRNA in the ROS group detected by fluorescent quantitative PCR was significantly up-regulated(P<0.05),while which in the GW group was down-regulated compared with the I/R group and ROS group(P<0.05).The expression of PPAR-gamma protein was similar to that of PPAR-gamma mRNA.Conclusion Up-regulation of myocardial PPAR-gamma expression may increase the occurrence of reperfusion arrhythmias in myocardial I/R animal model.

Objective:To analyze the application value of health education based on knowledge, attitude and practice (KAP) in patients before gastrointestinal endoscopy in the Department of Gastroenterology. Methods:Three hundred and fifty patients underwent gastrointestinal endoscopy in the Department of Gastroenterology of Jingqiao Health Center of Nanjing Lishui District from January 2019 to October 2020 were selected and divided into the control group ( n=175) and the observation group ( n=175) according to the random number table method. Patients in the control group chose regular health education, and patients in the observation group adopted health education in the KAP model. The gastrointestinal cleanliness before the examination, the Self-rating Anxiety Scale (SAS) score before and after the examination, the awareness rate of medical knowledge, the examination compliance rate, the diet compliance rate and the nursing satisfaction rate were compared between the two groups of patients. Results:Patients of gastrointestinal cleanliness level 1, 2, 3, and 4 in the observation group accounted for 83.43% (146/175), 13.14% (23/175), 3.43% (6/175) and 0, respectively, and those in the control group were 49.71% (87/175), 35.43% (62/175), 9.14% (17/175) and 4.57% (8/175) ( Z value was 6.743, P<0.001). The SAS scores of the observation group after the examination was 42.30±4.77, and 50.17±5.12 in the control group. There was significant difference between the two groups after the examination ( t value was 14.878, P<0.05). The medical-related knowledge awareness rate, the inspection compliance rate, and the diet compliance rate of the observation group were 100.00%(175/175), 98.29% (172/175) and 97.71%(171/175), and those rates of the control group were 90.29%(158/175), 83.43% (146/175), and 79.43%(139/175), respectively ( χ2 value was 17.867, 23.250, 28.903, all P<0.05). Conclusions:The knowledge-attitude-practice model-based health education can reduce the preoperative anxiety of patients undergoing gastrointestinal endoscopy, and improve their compliance and satisfaction with nursing.

Objective: :To study the effect of silencing DKK1 (Dickkopf1) gene on the proliferation, cell cycle and apoptosis of gastric cancer AGS cells and the action mechanism. Methods: :The DKK1-shRNA vector was constructed and transfected into AGS cells. The stably transfected cell lines were screened. The total protein and RNAof the transfected cells were extracted and the mRNAand protein expressions of DKK1 were detected by qPCR and WB, respectively. The experiment was divided into blank control group (Control), negative control group (shNC) and DKK1 silence group (DKK1-shRNA). CCK8 assay was used to detect the proliferation ofAGS cells of each group cultured for 0, 24, 48, 72, 96, 120 and 144 h, and flow cytometry was used to analyze the cell cycle and apoptosis in each group. The relationship between DKK1 and clinicopathological features of gastric cancer was analyzed after searching HPA database. Results:The gastric cancer AGS cells with stable DKK1 gene knockdown was successfully established, and it was confirmed that the mRNA and proteinexpressions of DKK1 in DKK1-shRNA group decreased by 72% and 47%, respectively, compared to shNC group (all P<0.05). The cell proliferation curve showed that, the cell proliferation in DKKl-shRNAgroup significantly decreased after 72 hour of culture compared with that in control and shNC groups (P<0.05). The cell number of S phase decreased from 32.06% to 25.87%, while the number of G2/M phase increased from 8.49% to 21.26% compared with shNC group (all P<0.05). The number of apoptotic cells also statistically increased from 10.34% to 20.65% (all P<0.05). The data of HPAdatabase showed that DKK1 mRNAlevel in gastric cancer tissues was significantly higher than that in normal tissues, and the high expression of DKK1 mRNAwas negatively correlat
ed with the survival rate of gastric cancer patients. Conclusion: : Silencing DKK1 gene can inhibit the proliferation of gastric cancer cells, arrest cells in G2/M phase and promote cell apoptosis. DKK1 plays a pro-carcinogenic effect in gastric cancer.
