1.Recent developments of drug eluting stent coatings.

Drug eluting stents (DESs) have revolutionized the interventional cardiology over the past decade since the first DES became commercially available in Europe in 2002. Compared to bare metal stents that are deployed to keep the vessel open by mechanical force, DESs have an additional function of reducing restenosis by the action of the drug on the target site. Coatings on the stent surface which ensure the maximum delivery of therapeutic agents to the target site with minimal systematic toxicity, also play an important role in adjusting the drug release profile. Coating material and technology not only affect the surface biocompatibility and the integrity maintenance during the implanting process, but also decide the way of drug delivering and transmitting from the coating. This paper reviews the basic principles of DES coating design, the categories of DES coatings, the commonly used clinical DES coatings and their efficiency in reducing restenosis, and finally provides the future perspectives for DES coatings.
2.Hollow periodic mesoporous organosilicas nanoparticles for efficient ultrasound-based imaging and ultrasound-mediated therapy

Objective To Study the application of ultrasound mediated drug-loading ultrasound contrast agents in auxiliary cancer chemotherapy.Methods Carrying paclitaxel (PTX) hollow periodic mesoporous organosilicas(HPMOs) were prepared by Structural differences selective etching method.The drug release kinetics of ultrasound mediated in vitro were evaluated at different time points of ultrasound radiation to PTX-HPMOs.The cell survival were evaluated by CCK8 experiment.Results ① The drug loading of PTX-HPMOs was 91 mg/g.②The achieved HMONs showed the high stability and excellent contrast enhancement for ultrasound imaging both in vitro.HPMOs showed the same good effect of contrastenhanced ultrasound imaging as SonoVue.③Drug-loaded HMONs exhibit ultrasound-responsive drug release performance.The ultrasonic stimulation can increase the release of 18 % of PTX each time.④ After ultrasonic stimulation,PTX-HPMOs showed good antitumor effect in vitro.Conclusions The new type of drug loading ultrasound contrast agents show good ultrasonic imaging and can help the cancer chemotherapy at the same time.

4.Analysis of risk factors for diabetic nephropathy in patients with type-2 diabetes mellitus

Objective To explore the risk factors of diabetic nephropathy.Methods According to the excretion rate of proteinuria,90 patients were divided into 3 groups:normal diabetic proteinuria group (DM),diabetic micro-proteinuria group (DN1),and clinical diabetic proteinuria group (DN2).We compared patients'ages,diabetic course,cholesterol,triglyceride,glycosylated hemoglobin,high density lipoprotein cholesterol (HDL),low density lipoprotein cholesterol (LDL),serum p-selectin,serum C-reactive protein,urinary monocyte chemotactic protein,and proteinuria excretion rate.Logistic regression analysis was used to analyze the relation between DN and various factors.Results Differences among these groups were statistically significant in type 2 diabetic course,HDL,LDL,p-selectin,C-reactive protein,glycosylated hemoglobin,and urinary monocyte chemotactic protein (P < 0.05).Logistic regression analysis showed that diabetic course,LDL,C-reactive protein,p-selectin,and urinary monocyte chemotactic protein were independent risk factor (OR values were 2.238,1.062,6.723,1.166,and 1.046).Conclusions Occurrence and severity of DN had relationship with course of diabetes,microvascular lesions,and inflammatory reaction.Emphasis on monitoring and evaluation of the DN-related factors would contribute to the prevention and treatment of DN.
5.Changes of plasma fibrinolytic factors in pulmonary thromboembolism rats after rce-combinant prourokinase treatment

Objective To observe the changes in fibrinolytic factors in rats with pulmonary thromboembolism (PTE) after recombinant prourokinase ( rPro-UK) treatment and its significance .Methods PTE was induced in male Sprague-Dawley (SD) rats by injecting heated 125iodine-labeled fibrinogen(Fib) autologous thromboemboli into external jugular veins.Twenty-eight rats were randomly assigned into following groups (7 rats each):①healthy control group;②PTE 5 d group,the rats in which were sacrificed at 5 d after the PTE model was made; ③ PTE3d receiving rPro-UK thrombolytic treament groups including multibolus treatment sub group ( rPro-UK was given in 1 mg/kg on the post-PTE third day followed by 2 consecutive days of a lower dose 0.25 mg/kg and rats were sacrificed 2 h after the last injection at the same time as PTE5d group) and single bolus treatment sub group ( rPro-UK was given in 1 mg/kg on the post-PTE third day followed by 2 consecutive days of 0.5 ml saline and rats were sacrificad at the same time as the former group ).The rats were quickly sacrificad at the fixed time through carotid bleeding and plasma samples were reserved for analysis of uroki -nase-type plasminogen activator (u-PA), urokinase-type plasminogen activator receptor (u-PAR), fibrinogen (Fib) andα2-antiplasmin (α2-AP) .Results ①Plasma concentrations of u-PA and u-PAR were increased were significantly in rPro-UK multibolus treatment sub group than in PTE 5 d group(Pu-PA <0.05,Pu-PAR <0.01)and rPro-UK single bolus treatment sub group(Pu-PA <0.01,Pu-PAR <0.05),correlated with the thrombolysis rate in rPro-UK multibolus treatment sub group (ru-PA =0.766,P<0.05;ru-PAR=0.785,P<0.05).② No difference of plasma Fib and α2-AP was seen between Pro-UK treatment groups and PTE 5 d group(P >0.05).Conclusion ① Plasma levels of endogenous u-PA and u-PAR are increased at different time points after PTE and are further enhanced after Pro-UK treatment, which promotes endogenous fibrinolysis and thrombus lysis .This is probably related to increased synthesis and secretion of endothelial cells which may be a key thrombolytic mechanism of Pro-UK.②Absence of systemic activation of the fibrinolytic system in Pro-UK multibo-lus treatment sub group means that the regimen is feasible and Pro-UK is fibrin specific .
6.Non-invasive closed placement of nasojejunal feeding tube during Ivor-Lewis esophagectomy for esophageal carcinoma

Objectives:To improve the surgical procedures and investigate the feasibility of the closed placement of nasojejunal tube during Ivor-Lewis esophagectomy. Methods:From January 2010 to December 2013, 85 patients (72 males and 13 females) with esophageal or gastric cardiac carcinoma underwent Ivor-Lewis esophagectomy in our department. Briefly, the general surgical proce-dures were performed as follows:1) stomach mobilization and enlargement of esophageal hiatus and pyloric sphincter digital fracture via laparotomy; 2) tubular stomach reconstruction, esophageal carcinoma resection, and intra-thoracic esophagogatrostomy via right posterolateral thoracotomy;and 3) forward closed placement of feeding tube through the nostrils and jejunum of patients under the guid-ance of a surgeon, who palpates the pylorus through the hiatus with the use of fingers. Results:No operative death or feeding tube-asso-ciated adverse event was observed. Among the 85 patients who have undergone Ivor-Lewis esophagectomy, feeding tube placement in-to the jejunum during surgery failed in 33 cases. The success rate of nasojejunal feeding tube placement was 61.2%(52/85). Twelve pa-tients with successful tube placement did not receive enteral feeding for several reasons and were thereby transferred to parenteral group. Significant differences were observed in terms of the nutritional cost and proportion between enteral feeding and parenteral groups (?1,469 ± 741 vs.?3,223 ± 917, P<0.001;3.4%vs. 7.2%, P<0.001). No differences in postoperative hospital stay and morbidi-ty were observed between the two groups (P>0.05). Conclusion:The novel forward closed placement of nasojejunal feeding tube dur-ing Ivor-Lewis esophagectomy provides a non-invasive, feasible, simple, and economical method for postoperative nutritional support. Surgeons could perform this novel technique successfully in practice.
7.Colorimetric Determination of Isocarbophos Based on Enhanced Peroxidase-like Activity of Hemin

A simple colorimetric method for detection of isocarbophos was developed based on the enhanced peroxidase-like activity of hemin. Hemin could catalyze the oxidation of peroxidase substrate 3,3′,5,5′-tetramethylbenzidine (TMB) by H2O2, which made TMB to lose one electron and caused reaction solution color changing from initial transparent to blue-green. Adding isocarbophos improved hemin affinity to substrates, which further enhanced peroxidase-like activity of hemin and made TMB to lose two electrons. The color of TMB solution was further changed from blue-green to yellow, and the degree of color change was proportional to the concentration of isocarbophos. Under the optimal conditions, the present analytical method for isocarbophos detection had a dynamic range from 2 μg/L to 100 μg/L with a detection limit of 1.2 μg/L (3σ). The selectivity assay demonstrated that other organophosphorus pesticides exhibited negligible interferences for isocarbophos detection. The application of the proposed method in the practical samples showed that the mean recovery of isocarbophos was in the range of 94.4%-113.0%, thus it could be widely applied to rapidly and sensitively detect isocarbophos in the agricultural products.
8.The 4 year follow-up investigation of cognitive function in patients after acute stroke

Objective To investigate the 4-year follow up of cognitive function outcomes and characteristics in patients after stroke.Methods Sixty three cases according with the diagnostic standard of acute unifocal subcortical stroke were consecutively collected in neurological ward from December 2009 to November 2010.They were followed up for average four years.Forty one out of them completed the neuropsychology tests identical to the baseline,which covered the general cognition function,attention,execution,memory,language,spatial,etc.According to the standard of clinical diagnosis,cognition function is divided into five degrees,including normal,VCI-ND,mild VaD,moderate VaD,and severe VaD.The improved group had 13 cases whose cognition function was improved by one or more ranks.The progressive group had 12 cases whose cognition function progressed by one or more ranks.The stable group had 16 cases whose cognition function remained the same as the baseline.Results According to qualitative analysis on the baseline versus 4-year follow-up outcome,in 13 improved cases,8 were VCI-ND and 5 were mild VaD.In 16 stable cases,11 were normal,4 were VCI-ND and 1 was mild VaD.In 12 progressive cases,3 were normal (change to mild VaD after follow-up),5 were VCI-ND (change to mild and moderate VaD after follow-up) and 4 were mild VaD (change to moderate VaD after 4-year follow-up).In the comparison of baseline cognition function among the improved,progressive and stable group,there was only one significantly different score (the right number of SCWT-A) in the improved and progressive group.The cognition function of improved group had significant differences in CFT-copy,right number of SCWT-C and the time of TMT-B before versus after follow-up.The cognition function of progressive group had significant differences in AVLT-Delay Recall and CFT-Recall.Conclusion Long-term cognitive function outcome after stroke is heterogenetic.The location of cognitive impairment or progression is not the same model for different cognitive outcome group.
9.Expression of matrix metalloproteinase-2,9 and it′s tissue inhibitor-1,2 in endometrial carcinoma

Objective To study the expression of matrix metalloproteinase(MMP)-2,9 and tissue inhibitor of metalloproteinase(TIMP)-1,2 protein in human endometrial carcinoma tissue and its relation to the invasion and metastasis of endometrial carcinoma. Methods Immunocytochemistry and zymography techniques were used to measure the MMP-2,MMP-9,TIMP-1,TIMP-2 protein levels and activities in endometrial carcinoma tissue of 37 patients and control group composed of 7 normal postmenstrual endometrial samples. Results The MMP-2,MMP-9,TIMP-1 and TIMP-2 proteins mainly expressed in endometrial carcinoma cells, glandular cells and endothelial cells. The strongly positive expression proportions of MMP-2,9 and TIMP-1 proteins in grade Ⅲ carcinoma cells were respectively 73%, 20% and 67%, which were higher than those in gradeⅡ (13%, 0, 27%) and gradeⅠ (0) ones ( P
10.Quantitative analysis of plaque neovascularization with contrast-enhanced ultrasound in rabbit model : a pilot study

Objective To varify contrast-enhanced ultrasound (CEUS) quantitative analysis plaque enhancement is able to assess the intraplaque neovascularization.Methods The abdominal aortas were examined with CEUS in 9 atherosclerosis rabbits established by feeding high-fat diet.Sonographic examinations were performed with ultrasound instruments equipped with low mechanical index contrast imaging software.Contrast agent SonoVue was used with a dose of 0.2 ml,The enhanced intensity (EI) with the plaque and the ratio of EI within the plaque to that in the lumen of the carotid artery was calculated with QLab software,Each aorta specimen was stained with F8 (von willebrand factor) for mierovessel.The quantitative parameters EI and ratio were compared with the immunohistochemical study results.Results The mean value of EI and ratio were (3.91 ± 0.87)dB and 0.30 ± 0.06,respectively.Five specimens showed positive F8 staining and 4 showed negative.EI and ratio were significantly greater in positive group than in negative group [(4.47 ± 0.60) dB vs (3.22 ± 0.62) dB,0.34 ± 0.05 vs 0.25 ± 0.05].CEUS quantitative parameters EI ( r =0.779,P =0.013) and ratio ( r =0.693,P =0.019) were correlated to microvessels marked with F8,respectively.Conclusions It is histologically validated that CEUS quantifies the neovascularization in atherosclerosis plaques.CEUS provides a window for plaque risk stratification.
