OBJECTIVE To study the risk factors of pulmonary infection and prognosis in cerebral apoplectic patients.METHODS A retrospective survey on 497 cerebral apoplectic patients was analyzed to investigate the rate of pulmonary infection,the type and the severity of cerebral apoplexy,the underlying disease,the age,and the prognosis.RESULTS The rate of infection highly increased in the older patients with the severe diseases and many riskfactors.CONCLUSIONS Controlling the rate of pulmonary infection can help us to succeed in treating cerebral apoplexy.
2.Lung Protection Due to Leukocyte-removed Priming Fluid Peri-cardiopulmonary Bypass in Infants

Objective To investigate the impact of leukocyte-removed priming fluid on lung function peri-CPB in infants. Methods Selected 60 infants of less than 1-year-old, with ventricular septal defect (VSD), were randomly assigned to the experimental group and the control group, the experimental group took blood leukocyte-removed priming fluid, and the control group took banked blood-derived priming fluid. Indicators of OI, PaO2/PAO2, A-aDO2 and RI were recorded at the corresponding period of cardiopulmonary bypass (pre-CPB, 2h, 6h, 12h, 24h and 48h post-CPB). Esophageal squamous cell carcinoma(ESCC) is a common malignancy in china,and TNM staging is the standard staging system for ESCC.However,in clinical practice,patients' prognosis is not always consistent with the staging.This phenomenon may be attributed from the fact that the current limitation and imperfect of the TNM staging system,and besides,another possible reason might be that some prognostic factors for ESCC other than TNM descriptors may exist.Among all the prognostic factors for ESCC,molecular biomarkers received widely concern.We searched the literatures and identified 12 molecular biomarkers that received the most concern and validated them in our single surgeon team.The results showed that only P21,COX-2 and E-cadherin were significant prognostic factors for ESCC in this series.Therefore,in the current study,we reviewed the literature regarding the prognostic significance of the above mentioned three biomarkers for clinical reference.

Objective To investigate the applications of antibacterial agents to outpatients in primary hos -pitals in Hefei City of Anhui Province,and to provide reference for rational use of antibacterial agents.Methods In 2011, fourty-five primary hospitals in Hefei City were selected randomly ,including urban community health service centers (Group A) and township hospitals(Group B),and thirty or fourty outpatient prescriptions were analyzed monthly . Results In Group A, the percentage and intensity of antimicrobial usage , the proportion of the combination and injectable formulation were ( 45.36 ±20.02 )%, ( 89.73 ±25.50 ) DDDs · ( 100 cases ) -1 · d-1 , 13.34%, 23.16%,respectively,and the data in Group B were (61.36 ±17.18)%,(108.46 ±32.27)DDDs· (100 cases) -1 · d-1,29.13%,46.39%,respectively,which the former were significantly lower than the latter.Conclusion In primary hospitals,the applications of antibacterial agents to outpatiants are not rataional ,including high percentages of usage and unreasonable selection of species ,and more supervision and training need to be given to the medical staff , especially in township hospitals .
6.Comparison of Effects on Intraoperative Cerebral Metabolism between Total Intravenous Anesthesia and Intravenous Inhalation Anesthesia in Patients in Neurosurgery

Objective To compare the effect on intraoperative cerebral metabolism between the propofol combined with remifentanil infusion in total intravenous anesthesia and the desflurane combined with remifentanil intravenous inhalation anesthesia in patients in neurosurgery.Methods Thirty-four patients were randomly divided into the propofol combined with remifentanil group (group A,n=16) and the desflurane combined with remifentanil group (group B,n =18).The B-ultrasound under the guide of retrograde catheterization through right internal jugular vein and artery was prepared after the patients entered the operation room.Atropine,propofol,fentanyl,rocuronium were used in the induction of anesthesia.The mechanical ventilation was applied after conventional trachea cannula.Once patients were anesthetized steadily,jugular bulb venous oxygen saturation (SjvO2),jugular bulb venous oxygen partial pressure (PjvO2),Jugular Bulb venous hemoglogin (Hbv),jugular bulb venous lactate (Ljv),jugular bulb venous gluxose (Gv),arterial oxygen saturation (SaO2),arterial oxygen pressure (PaO2),arterin (Hba),arterial blood lactate (La),arterial blood gluxose (Ga),arterial blood oxygen content (CaO2),jugular bulb venous oxygen content (CjvO2),arteriovenous oxygen difference (AVDO2),cerebral extraction of oxygen (CEO2),cerebral lactate production rate (CLP) and cerebral glucose uptake rate (CGU) at different time [before anesthesia induction (T1),1 hours after the start of the operation (T2),2 hours after the start of the operation (T3),half an hour after the operation]were collected.Results (1) The value of SjvO2,PjvO2,CjvO2 and CaO2 in group B was significantly higher than that in group A (P<0.001);The value of AVDO2 and CEO2 in group B was lower than that in group A (P<0.05);(2) The value of Gv and Ga in group B was higher than that in group A (P<0.05);There were no significant differences about CGU in two groups (P>0.05);(3) The value of CLP in group B was lower than that in group A (P<0.05);there were no significant differences about Ljv and La in two groups (P>0.05);(4) Compared with the value at moment of T1 between group A and group B,the value ofPaO2,SaO2,PjvO2 and SjvO2 were increased with time (P<0.05),the value of CaO2,CjvO2,AVDO2 and CEO2 showed a downward trend (P <0.05).Conclusion (1) Both total intravenous anesthesia and intravenous inhalation anesthesia can reduce the cerebral oxygen metabolism;(2) For the cerebral protection of neurosurgery operation,it seems that the effect of intravenous inhalation anesthesia is more stronger than total intravenous anesthesia.
7.Simultaneous determination of contents of five major bioactive components contained in Polygoni Cuspidati Rhizoma et Radix by microwave extraction-ultra performance liquid chromatography.

OBJECTIVETo establish a microwave extraction and UPLC method for simultaneous determination of polydatin, resveratrol, anthraglycoside B, emodin and physicion contained in Polygoni Cuspidati Rhizoma et Radix, in order to provide scientific basis for improving quality standards of Polygoni Cuspidati Rhizoma et Radix.
METHODThe test solutions were prepared in a MDS-8 closed microwave system at 160 degrees C with methanol as the solvent. The UPLC analysis was performed in a Waters Acquity UPLC system. A BEH C18 column (2. 1 mm x 100 mm, 1.7 microm) was adopted for gradient elution with acetonitrile and water as the mobile phase. The temperature of column was 30 degrees C, and the detection wavelength was 226 nm.
RESULTThe five active components can be completely extracted in 10 minutes and separated completely in 12 minutes according to UPLC analysis, with a good linearity (r > or = 0. 999 6) within the linear ranges. The average recovery rate was 97.00%-103.7% with RSD < or = 2. 2%. Despite a large difference in content among tested components from Polygoni Cuspidati Rhizoma et Radix, the total content of the five major constituents was relatiely stable (3.683 3%-7.1031%).
CONCLUSIONThe microwave extraction-ultra performance liquid chromatography method in simultaneous determination for contents of five major bioactive components contained in polygoni cuspidati rhizoma et radix is so rapid and highly reproducible that it can be used for quality control and assessment of Polygoni Cuspidati Rhizoma et Radix.

8.Effects of improved TCM hot ironing technology on the gastrointestinal reaction in patients with mammary cancer

Objective To evaluate the effects of gastrointestinal reaction through the breast cancer chemotherapy period with improved traditional Chinese medicine ( TCM) hot ironing technology. Methods We randomly divided 80 cases of breast cancer in the chemotherapy period into control group and treatment group (40 cases in each group ). The patients of control group used Granisetron Hydrochloride Injection 3 mg, bid, for three days, while the patients of treatment group used improved TCM hot ironing technology on Zhongwan for 30 mins, qd for 3 consecutive days. The nausea, vomiting, anorexia and its degree changing were compared at chemotherapy day, 1d, 2d and 3d after chemotherapy between two groups. Results The proportion of nausea, vomiting and anorexia degree in treatment group from the day of chemotherapy to the 3 days after chemotherapy were significantly super to those of control group (P < 0. 05). Conclusions Improved TCM hot ironing technology can improve gastrointestinal reaction through chemotherapy period and worthy for clinical promotion.
9.Construction of lncRNA4.9 interfering lentivirus vector

Objective To construct an interfering lentivirus vector of long non-coding RNA 4.9 (lncRNA4.9) transcribed by human cytomegalovirus (HCMV).Methods Three interfering sequences targeting lncRNA 4.9 were designed and synthesized.The shuttle plasmid GV248 and the target interfering sequence were combined and constructed.The recombinant plasmid was co-transfected with the skeleton plasmids pHelper1.0 and pHelper2.0 to 293T cells.Viral particles were collected and copies were determined.The interfering lentivirus vector was transfected into THP-1 cells to observe the fluorescence expression,and the interfering efficiency was detected by real-time RT-PCR.Results Three groups of lentivirus interference vectors (LV1,LV2,LV3) were constructed,LV2 and LV3 can interfere with the expression of lncRNA4.9,and the interference efficiency of LV2 group was the highest.Conclusions The interfering lentivirus vector of lncRNA 4.9 was successfully constructed,which laid a foundation for further study on the function of lncRNA 4.9.
10.Cholesteryl ester transfer protein levels and gene deficiency in Chinese patients with cardio-cerebrovascular diseases.

OBJECTIVETo detect cholesteryl ester transfer protein (CETP) levels, frequencies of CETP D442G and I 14A mutations and characteristics of abnormal lipids in patients with cardio-cerebro vascular diseases.
METHODSNinety-four myocardial infarction (MI) patients, 110 stroke patients and 335 healthy controls were selected. The CETP concentration was determined using ELISA. The CETP activity was measured using a substrate of (14)C-radiolabeled discoidal bilayer particles. The CETP gene mutations were detected by PCR-RFLP.
RESULTSThe CETP concentrations in the MI and stroke group, were higher than those in the controls. The gene mutation frequencies of D442G in the MI, stroke and control group were 3.5%, 3.6% and 5%, respectively, and the frequencies of I 14A were 1.05%, 0.91% and 1%, respectively. One case of D442G homozygote was detected in the healthy group. The frequency of two CETP gene mutations showed no significant difference among the patients and controls. The CETP concentration and activity in subjects with CETP mutations were one-third of those in the control group. The level of HDL-C, apo-A1 increased in the mutation subjects, while the TG level decreased.
CONCLUSIONSThe CETP level increased significantly in patients with cardio-cerebrovascular diseases. The carriers of CETP deficiency had CETP and lipid abnormalities.
