1.The correlation between chronic hepatic diseases and small intestinal inflammation

Objective To investigate the correlation between chronic hepatic diseases and small intestinal inflammation.Methods Patients who received capsule endoscopy in The First Affiliated Hospital of Guangdong Pharmaceutical University were divided into groups of liver cirrhosis,non-alcoholic fatty liver disease(NAFLD),chronic hepatitis and non-hepatic disease according to clinic data from August 2011 to August 2015.The severity of small intestinal mucosal inflammation was graded according to Lewis Scoring system and incidence of small intestinal lesions in different groups and Lewis scores were compared.The liver function was also graded with liver noninvasive scoring systems.Then the correlation between liver function damage and small intestinal lesions was investigated.Results A total of 338 cases were enrolled in the study,including 25 cases of liver cirrhosis,47 cases of NAFLD,20 cases of chronic hepaitis and 246 cases of non-hepatic disease.There were 22 (88.0%),36 (76.6%),12 (60.0%) and 78 (31.7%) cases with lesions in small intestine in the four group respectively with significant differences(P<0.001).Rate of small intestinal villi edema was significantly higher in liver cirrhosis group,NAFLD group,chronic hepatitis group than that in non-hepatic disease group(all P<0.017).Small intestinal villi edema was found mainly in the upper and one third of middle parts in small intestine (P =0.033).Lewis scores of liver cirrhosis group (190.80±228.42)and NAFLD group(125.38± 191.31) were higher than those of non-hepatic disease group (42.91±97.69,P=0.021,P =0.034).Forns score,FIB-4 score,NAFLD-FS score and Child-Pugh score were positively correlated with Lewis score (correlation coefficient:0.247,0.244,0.223,0.284respectively,all P<0.001).Conclusion Chronic hepatic diseases such as liver cirrhosis,NAFLD,chronic hepatitis might be risk factors for small intestinal mucosal inflammation,and the severity of chronic hepatic diseases may be positively correlated with that of small intestinal mucosal lesions.
2.Male pseudohermaphroditism due to 17β-hydroxysteroid dehydrogenase 3 deficiency

Objective To investigate the clinical and genetic characteristics in a patient with 17β-hydroxy-steroid dehydrogenase (17β-HSD) 3 deficiency, regarding its pathophysiology and pathogenesis. Methods Clinical features and laboratory data were analyzed in a pedigree of 17β-HSD3 deficiency. Blood samples from the patient and his parents were collected. HSD17B3 gene was screened for mutations by PCR and subclone sequencing. Results The patient presented with pubertal virilization and gynecomastia. The physical examination showed female external genitalia and testes in inguinal canals. The chromosome karyotype was 46, XY. Serum FSH, LH, dehydroepiandrosterone sulfate, androstenedione and 17-OH-progesterone levels were raised, whereas plasma testosterone was lowered. Sequencing analysis revealed 4 nucleotide deletion (172-175del) of HSD17B3 gene. Conclusion Virilization and gynecomastia in puberty suggest the probability of 17β-HSD deficiency. It may be verified clinically by hCG-stimulating test and confirmed by gene diagnosis.
3.Proton magnetic resonance spectroscopy in chronic liver diseases related with hepatitis B

Objective To evaluate the application of 1H-MRS in chronic liver diseases related with hepatitis B, and to investigate the method of eliminating breathing influence to proton MR spectroscopy (1H-MRS) on liver. Methods Liver spectroscopy of 46 subjects were obtained by single-voxel PRESS sequence with respiratory gating with GE 1.5T scanner. The subjects were divided into 4 groups: the normal male group (10 cases), the normal female group (10 cases), the male hepatitis B cirrhosis group (14 cases) and the male liver cancer after hepatitis B group (12 cases) according to clinical data, MR manifestations and pathologic findings. Glutamine and glutamate complex (Glx) peak height served as a reference standard to judge the choline (Cho) peak height. The differences of Cho peak height of each group were compared. Results The peaks of Cho were lower than those of Glx in normal male group. The peaks of Cho were higher than those of Glx in 8 women of normal female group. The peaks of Cho were higher than those of Glx in 11 patients of the male hepatitis B cirrhosis group. The peaks of Cho were higher than those of Glx in 10 patients of the male liver cancer after hepatitis B group. The differences of Cho peak were statistically significant between normal male group and normal female group, the male hepatitis B cirrhosis group and the normal male group, the male liver cancer after hepatitis B group and the normal male group. Conclusion The peaks of Cho in normal males are lower than those of Glx. The peaks of Cho in the majority of normal females are higher than those of Glx. The peaks of Cho increase in male hepatitis B cirrhosis and liver cancer after hepatitis B. Respiratory gating can effectively eliminate the impact of breathing movement on proton MR spectroscopy of liver.

Objective To search the single nucleotide polymorphism (SNP) in exons of osteoprotegerin gene, and to analyse the relationship between SNP and bone mineral densities (BMD) in postmenopausal women. Methods Using PCR and direct sequencing to identify SNP and genotypes in 205 postmenopausal women. BMD at lumbar spine (L 2 4 ) and femoral neck (FN) were measured by dual energy X ray absorptiometry. Serum osteocalcin (BGP), osteoprotegerin (OPG), osteoprotegerin ligand (RANKL) and urinary N telopeptides of type Ⅰ collagen (NTx) were also measured. Results One SNP, G1181C, was found in exon 1 of OPG gene. The frequencies of G1181C genotypes in 205 postmenopausal women were 0.566, 0.346, and 0.088 for the genotypes GG, GC and CC respectively. [Summary] Hypercalcemia in pregnancy is a rare condition which brings considerable risks to mother and fetus. The most common cause is primary hyperparathyroidism(PHPT). The untypical symptoms and biochemical tests results add obstacles in the diagnosis of PHPT during pregnancy. The management is difficult, due to restrictions in choices of treatments and lack of clinical guidelines. Severity evaluation which takes consideration of calcium homeostasis during pregnancy is crucial for appropriate management. Parathyroidectomy during the second trimester is recommended for those with high serum calcium levels.
7.Inhibitory effects and mechanism of SAHA combined with Erlotinib on EGFR-TKI resistant lung cancer cell line H1975 cells

Objective To investigate the inhibition and possible mechanism of SAHA alone and combined with Er-lotinib on EGFR-TKI resistant lung cancer cell line H1975 cells. Methods The inhibitory effects of SAHA, Erlo-tinib and combined effect on cells were evaluated by CCK-8, colony formation assay and Transwell invasion assay. Western blot analysis was used to analyze the expression of PI3K, AKT, p-AKT, mTOR and p-mTOR protein in cells. Results SAHA and Erlotinib showed growth inhibition effects on H1975 cells with a dose-dependent man-nerand displayed synergistic inhibition effect. The combination of the drugs was significantly stronger than the single drug in the formation and invasion inhibition of tumor cells. The combined group had a strong inhibitory effect on the PI3K/AKT/mTOR signaling pathway. Conclusion SAHA combined with Erlotinib has synergistic inhibitory effects on the growth, cloning and invasion of EGFR-TKI lung cancer cell line H1975, which may be associated with inhibitory effects on the PI3K/AKT/mTOR signaling pathway.
8.Application progress of enhanced recovery after surgery in postanesthesia care unit

The strategy of enhanced recovery after surgery (ERAS) runs through the whole perioperative period, which requires to reduce the traumatic stress and psychological stress caused by disease, surgery and anesthesia, reduce postoperative complications, accelerate the early postoperative rehabilitation and improve the long-term prognosis. The recovery period of anesthesia is a special period with high incidence of multiple complications, and its severity may endanger the lives of patients. It is a great challenge for the medical staff in the post anesthesia recovery room to quickly and safely recover the patients from the anesthesia state to the physiological state close to the pre anesthesia state, so as to reduce the complications in the post anesthesia recovery room, shorten the residence time, and achieve safe and comfortable recovery. Therefore, this article reviews the clinical progress of ERAS strategy in postanesthesia care unit.
9.Effect of LINC00894 gene on proliferation and metastasis of gastric cancer cells via miR-205-5p/ZEB1 axis

Objective To investigate the effects of long non-coding RNA 00894(LINC00894)gene on prolifera-tion and metastasis of human gastric cancer cells,and to verify the regulatory relationship of LINC00894,miR-205-5p and ZEB1 in gastric cancer.Methods The expression level of LINC00894 in gastric cancer cell lines,normal gastric lines,clinical gastric cancer and normal gastric tissue samples were determined by RT-qPCR.Through fol-low-up,the relationship between the expression level of LINC00894 and the prognosis of gastric cancer patients was explored.LINC00894 knockdown cell lines and overexpression cell lines were constructed,and the knockdown and overexpression efficiency was detected by RT-qPCR.Cell proliferation and metastatic capacity were determined by CCK 8,clone formation and Transwell assays.Dual-luciferase reporter assays,RT-qPCR assays and Western blot assays were used to examine the targeted regulatory relationships of LINC00894,miR-205-5p and ZEB1.Results The expression of LINC00894 gene in gastric cancer tissues or cells was significantly higher than that in normal gas-tric tissues or cells,moreover,gastric cancer patients with high LINC00894 gene expression had a worse prognosis.The knockdown of LINC00894 inhibited the viability,clonogenesis,migration and invasion ability of gastric cancer cells,and conversely,the overexpression of LINC00894 obtained the opposite results.LINC00894 promoted ZEB1 expression by targeted downregulation of miR-205-5p expression.LINC00894 promoted the expression of ZEB1 by targeting miR-205-5p and down-regulating its expression.Conclusion LINC00894 serves as an oncogene in gastric cancer and may promote proliferation and metastasis of gastric cancer cells via regulating miR-205-5p/ZEB1 axis.
10.Application of China consensus on the protocol of early gastric cancer screening in Guangdong province

Objective:To assess the application value of China consensus on the protocol of early gastric cancer screening in Guangdong province.Methods:A new quantitative scoring system was used in Cantonese residents who underwent early gastric cancer screening from March 2018 to March 2019. According to the scores of initial screening, patients were divided into high-risk, medium-risk and low-risk groups. The detection rates of early gastric cancer, precancerous diseases and precancerous lesions under gastroscopy in each group were compared. Chi-square test was performed for statistical analysis.Results:A total of 545 individuals were selected for gastroscopy, in which 32 cases were classified into high-risk group, 184 into medium-risk group and 329 into low-risk group. The results of gastroscopy examination showed that high-risk group had the highest detection rate of early gastric cancer (12.5%), followed by medium-risk group (1.1%) and low-risk group (0) ( χ2=41.85, P<0.01); the detection rates of precancerous diseases exhibited a similar pattern: high-risk group (60.9%) > medium-risk group (52.4%) > low-risk group (34.3%) ( χ2=18.00, P<0.01). The detection rates of precancerous lesions were 17.9%, 8.8% and 8.8%, respectively, with no significant difference ( χ2=2.58, P=0.28). In terms of the positive rate of endoscopy, high-risk group (71.9%) showed the highest positive rate, followed by medium-risk group (57.1%) and low-risk group (40.1%) ( χ2=21.54, P<0.01). Conclusion:China consensus on the protocol of early gastric cancer screeing is of application value for the screening of early gastric cancer and precancerous lesions in the populations at risk of gastric cancer in Guangdong province.
