1.The revised TNM staging system and surgical strategies for non-small cell lung cancer

The International Association for the Study of Lung Cancer (IASLC) has presented the details of the IASLC/International Union Against Cancer (UICC)/American Joint Committee on Cancer (AJCC) Revised Staging Classification for Lung Cancer. The IASLC is the largest world-wide professional organization solely dedicated to reduce the worldwide burden of lung cancer. The IASLC recognizes that the staging classification will be most valuable and accurate if it is based on the evaluation of outcomes of large numbers of cases carefully collected and analyzed in an extensive worldwide database. The analyses of the T, N and M descriptors as well as the stage groupings were performed in 67,725 non-small cell lung cancer (NSCLC)patients. Survival was the primary outcome, measured from the date of diagnosis or date of protocol registration for clinical staging, or the date of surgery for pathologic staging. The remarkable efforts of the IASLC Staging Committee have resulted in an evidence-based, validated and robust revision of the international staging system for NSCLC. This landmark contribution will improve our care of patients and lays a strong foundation for future refinements based on an expanding knowledge of lung cancer behavior and biology. This review outlines the changes in the tumor, node, metastasis (TNM) descriptors and stage groupings anticipated in the official new stage classification system for NSCLC.
2.Surgical progress on early stage non-small call lung cancer

Surgical resection (usually lobectomy) is considered the treatment option for individuals with stage Ⅰ and Ⅱ non-small cell lung cancer. The surgical treatment of stage Ⅰ and Ⅱ non-small cell lung cancer (NSCLC) continues to evolve in the areas of intraoperative lymph node staging (specifically the issue of lymph node dissection vs sampling), the role of sublobular resections instead of lobectomy for treatment of smaller tumors (especially peripheral carcinoma ≤2 cm in diameter), and the use of video-assisted techniques to perform anatomic lobectomy. Video-assisted thoracic surgery (VATS) lobectomy provides a minimally invasive approach for the management of early-stage lung cancer. Questions about the safety of VATS lobectomy and its adequacy as a cancer operation compared with open thoraeotomy have hindered its universal acceptance among thoracic surgeons. Evidence suggests that VATS lobectomy can be safely performed and is an adequate cancer operation for early-stage NSCLC. Recently, robots have been introduced into surgical procedures in an attempt to facilitate surgical performance. However, adequately powered well-balanced studies comparing VATS with open thoracotomy for lobectomy are lacking in the literature.

Objective: To investigate the expression of Cyclin G mRNA in leukemia patients and its clinical significance. Methods: RT-PCR was used to analyze the expression of Cyclin G mRNA in the mononuclear cells of 129 leukemia patients and 10 healthy controls. Results: (1) The expression of Cyclin G in acute leukemia (AL) and chronic leukemia (CL) patients was significantly higher than that in healthy controls (both P 50?109/L in AL and AML groups were significantly higher than those with WBC ≤ 50?109/L. Objective To investigate the preparation process of cold -dampness medicine iron stick and the curative effect of cold -dampness medicine iron stick,thus to provide reference for the clinical treatment of cold -dampness lumbago.Methods Using science and technology of preparation and the thermal effect of cold -dampness medicine iron stick was observed.1 20 cases with cold -dampness lumbago of dampness type with low back pain, according to the order of treatment were divided into the observation group and control group.63 cases of the treatment group were given cold -dampness medicine iron stick affixed to the surface of the skin,57 cases of the control group were given kanlisha affixed to the surface of the skin.The treatment effect after one course of treatment was observed. Results Cold -dampness medicine iron stick heat effect index of heat time was 1 2min,to thermal time was 1 0.8h, the highest temperature was 56.5℃,thermal equilibrium curve was gentle and lasting.The efficacy of the two groups of drugs within the observation time effect was obvious.The total effective rate of the observation group was 90.5%(57 /63),which was higher than 75.4%(43 /57)of the control group,the difference was statistically significant (χ2 =4.87,P <0.05).During the treatment,local skin blisters,rash,itching and other low temperature burns and other allergic reaction were not found.Conclusion Cold -dampness medicine iron stick process is stable and controllable in quality and heat balance curve slow long -lasting,determine the efficacy,innovative therapies,it is easily accepted by patients and worthy to be popularized in clinical practice.
5.Current status of the application of PDT in the treatment of lymphomas

6.Clinical research of liposome paclitaxel combined with nedaplatin in treatment of advanced non-small cell lung cancer

Objective To evaluate the clinical efficacy,overall survival and toxicities in the patients with advanced non-small cell lung cancer treated by liposome paclitaxel combined with nedaplatin as first-line treating.Methods 34 cases with advanced NSCLC were treated with liposome paclitaxel 150 mg/m2 on day 1 and nedaplatin 80 mg/m2 on day 1 by intravenous infusion,with 21 days as one cycle.The patients were treated with chemotherapy more than 2 cycles.Efficacy evaluation and adverse events were evaluated every 2 cycles.Results 34 patients were available for evaluation of efficacy and adverse events.The patients recieved 174 cycles of chemotherapy,and the median was 5.3 cycles.The objective response rate of liposome paclitaxel combined with nedaplatin was 32.3 %,the disease control rate was 67.6 %,the median overall survival was 9.5 months (95 % CI 6.2-10.7),1 years survival rate was 40.6 %.The main adverse events were hematological and gastrointestinal toxicities.Frequent grade Ⅲ-Ⅳ toxicities included neutropenia (41.7 %),anemia (17.6 %),thrombocytopenia (8.8 %),nausea and vomiting (8.8 %),diarrhea (5.8 %).Conclusions The regimen of liposome paclitaxel combined with nedaplatin is against advanced non-small cell lung cancer with high efficacy and acceptable toxicities,and it may be used as a new choice for treatment of advanced non-small cell lung cancer.
7.A prospective clinical research for surgical approach of cardiac

Objective To compare the results of surgical treatment of cardiac carcinoma through left thoracic approach and abdominal approach.Methods After finished cardiac carcinoma resection and gastroesophagus anastomosis in thorax through left thoracic approach,standard D2 regional lymph nodes dissection or/and combined pancreas and spleen resection were performed immediately through abdominal approach in 19 patients.Two surgical specimens were examined individually.Results 122 lymph nodes were founded in the abdominal surgical specimens,in which 23 were metastasis lymph nodes founded in 7 patients (7/19,37 %).Conclusion Left thoracotomy alone can no achieve complete resection for advanced gastric cardiac carcinoma.It should be treated through the combined abdominal and thoracic approach.
8.Correlation between white matter microstructural lesions by diffusion tensor imaging and clinical profiles in patients with temporal lobe epilepsy and hippocampal sclerosis

Objective Diffusion tensor imaging(DTI)was used to explore the characteristics of white matter (WM)microstruc-ture lesion in patients with temporal lobe epilepsy(TLE)and hippocampal sclerosis(HS).Then we investigated the influence of the age of epilepsy onset and the course on WM changes.Methods DTI data were obtained in 30 TLE patients with HS and 23 age and sex-matched healthy controls.Thirteen patients had left HS (LHS)and seventeen had right HS (RHS).Between-group compari-sons of fractional anisotropy (FA)were carried out by using track-based spatial statistics (TBSS).In addition,FA in regions of in-terest (ROI)where white matter lesions had statistically significant difference was correlated with the age of onset and the course. Results Subjects in the LHS group,as compared to healthy controls,demonstrated these clusters of reduced FA:bilateral corona radiata,the left genu of corpus callosum,the right body and splenium of corpus callosum,the right posterior thalamic radiation,ex-ternal capsule and superior longitudinal fasciculus.White the subjects in the RHS group,the clusters of reduced FA are involving bi-lateral corpus callosum,corona radiata,posterior thalamic radiation,cingulum,superior longitudinal fasciculus,and the right ante-rior limb of internal capsule,the right retrolenticular part of internal capsule and external capsule.In LHS group,the FA of the left genu of corpus callosum was negatively correlated with the course,and the FA of the right anterior corona radiata was positively cor-related with the age of onset.In RHS group,the FA of the bilateral superior corona radiata was positively correlated with the course.Conclusion DTI detects extensive changes in TLE with HS that are not confined to temporal lobe.The age of epilepsy onset in LHS was correlated with the changes of anterior corona radiata,while the course and the WM change were not definitely linked to each other.
