1.Preliminary elaboration on emergent properties of traditional Chinese medicine.

Plant and animal derived secondary metabolites are the result of initiative and adaptability of natural evolution due to its adaptive stress responses. Based on countless attempts, rational thinking and thousands of years of clinical practice by ancient Chinese, the medicines were endowed with advantages for the treatment of diseases and keeping health balance through multiple components combination instead of single components, featured by a complex system with emergent properties. The emergence of traditional Chinese medicine is because of the integration of various components and its complex interactions. How to obtain the new multicomponent entities with the biological equivalent effect is an important and fundamental work for TCM-based new drug research and development. Currently, recognition of TCM emergence and development of related technical methods needs strengthened, and the understanding and research of TCM require a systematic integration of the holistic and reductive methods.

3.Prognostic analysis of 55 neonates with severe asphyxia

Objective:
To analyze the prognosis of neonates with severe asphyxia, so as to provide insights into improvements of prognosis among neonates with severe asphyxia.
Methods:
Neonates with severe asphyxia born in Hangzhou Women's Hospital from 2016 to 2021 were recruited, and neonates' birth weight, gender, Apgar score, arterial blood pH value within 30 minutes after birth and maternal delivery data were collected. Outpatient follow-up was performed among neonates one year after birth, and the prognosis was evaluated with the Gesell Developmental Scale. Death or development quotient (DQ) of <75 was defined poor prognosis, and the clinical characteristics were compared between the good prognosis group and the poor prognosis group.
Results:
A total of 55 neonates with severe asphyxia were enrolled, including 26 boys and 29 girls, with a median birth weight of 2 845.00 g (interquartile range, 1 948.00 g). There were 52 lying-in women (3 women had twins), with a mean age of (30.56±4.08) years, including 35 women with dystocia (67.31%) and 37 primiparas (71.15%). Of 55 neonates with severe asphyxia, there were 40 neonates with good prognosis (72.73%) and 15 neonates with poor prognosis (27.27%), including 13 deaths and 2 cases with DQ of <75. The incidence of abnormal fetal position was higher in the good prognosis group than in the poor prognosis group (36.84% vs. 0%; P<0.05), and the incidence of uterine torsion or uterine rupture was higher in the poor prognosis group than in the good prognosis group (21.43% vs. 0%; P<0.05). The median Apgar score at 5 min after birth and arterial blood pH within 30 min after birth were lower in the poor prognosis group [3.00 (interquartile range, 4.00) points and (6.93±0.23)] than in the good prognosis group [6.00 (interquartile range, 4.00) points and (7.23±0.15)] (P<0.05).
Conclusion
Maternal uterine torsion or uterine rupture, low Apgar score at 5 min after birth, and low arterial blood pH within 30 min after birth may aggravate the poor prognosis among neonates with severe asphyxia.
4.Establishment of a modified model of adriamycin nephrosis in rats

Objective To study the adriamycin nephrosis model in different development pathological stages. Methods The rat adriamycin nephrosis model was established by injecting adriamycin into tail-vein twice every two weeks to detect blood and urina biochemical indicators and to observe the pathological changes of the renal tissues. Results The model showed serious edema,proteinuria,hypoproteinemia,and hyperlipidemia. Podocytes were swollen and mesangial cells developed mild hyperplasia at the end of the fourth week. The nephric tubule atrophied at the end of the eighth week accompanied with adhesion between glomeruli and Bowman's capsule. Glomeruli sclerosis of mild or medium degree was observed at the end of the twelfth week with obvious lymphocyte infiltration in the renal interstitium as well as the formation of collagen fibers. Conclusion The adriamycin nephrosis model was successfully developed by injecting adriamycin 4 mg/kg into tail-vein twice every two weeks. The acute model is similar to human minimal change disease,and the chronic model is similar to human focal segmental glomerulosclerosis.

6.Improved anatomic M-mode echocardiography evaluation of regional short-axis myocardial function of left ventricle in coronary heart disease

Objective To evaluate the movement function of the left ventricular short-axis with parameters (velocity, acceleration and relatively force) measured with improved anatomical M-mode ultrasound. Methods Sixteen patients with myocardial ischemia coronary heart disease who had taken percutaneous transluminal coronary intervention (PCI) were enrolled. All patients were divided into two groups: group A, single major vascular stenosis (n=8);group B, more major vascular stenosis (n=8). M-mode curves of short-axis sections at mitral valve level, papillary level and apical level were recorded, then the peak myocardial velocities, acceleration and force of the systolic phase, early-diastole phase and end-diastole phase were measured at the post processing station. Results The variety ranges of average velocity and acceleration of the three levels during systolic phase were higher than that during diastole phase. The changing rate of velocity and acceleration in group B was higher than that in group A. Conclusion The new parameters that are measured and calculated with anatomical M-mode ultrasound can reflect the movement functions of coronary heart disease patients taken PCI.
7.Synthesis and in vitro antibacterial activity of 7-(4-alkoxyimino-3-methyl-3-methylaminopiperidin-1-yl)quinolones.

To explore new agents of quinolone derivatives with high antibacterial activity, 7-(4-alkoxyimino-3-methyl-3-methylaminopiperidin-1-yl)quinolones were designed and synthesized, and their activity against gram-positive and gram-negative strains was tested in vitro. Sixteen target compounds were obtained. Their structures were established by 1H NMR, HRMS and X-ray crystallographic analysis. Compounds 14k and 14m-14o show good antibacterial activity against the tested five gram-positive strains and five gram-negative strains (MIC: 0.25-16 micromg x mL(-1)), of which the most active compound 14o is 8-fold more potent than levofloxacin against S. pneumoniae (MIC: 4 microg x mL(-1)), and comparable to levofloxacin against S. aureus, S. epidermidis, E. faecalis and E. coli (MIC: 0.25-1 microg x mL(-1)), but generally less potent than gemifloxacin.
8.Preparation and in vitro Drug Release of Betahistine Dihydrochloride Sustained-release Matrix Tablets

Objective: To prepare betahistine dihydrochloride sustained-release matrix tablets. Methods: The tablets were pre-pared with water soluble HPMC matrix, and the release behaviors were investigated by single factor study. The formula and preparation procedures were optimized by orthogonal design. Results:The optimal technology was as follows:using 60% HPMC K15M as the ma-trix material, calcium hydrogen phosphate as the filler, 10% PVP in 90% alcohol as the bonding agent;wet granulation compression technique was used to prepare the tablets with the tablet weight of 500mg. The in vitro drug release fits a Higuchi equation and the drug can be sustained-released within 12 h. Conclusion:The preparation technology is simple and the tablets have sustainol release behav-ior.
9.Effect of electroacupuncture on activity of extracellular signal-regulated kinase 1/2 in spinal dorsal horn during morphine tolerance in rats

Objective To investigate the effect of electroacupuncture (EA) on the activity of extracellular signal-regulated kinase 1/2 (ERK1/2) in the spinal dorsal horn during morphine tolerance in rats.Methods Twenty-five healthy male Sprague-Dawley rats in which intrathecal (IT) catheters were successfully implanted with-out complications were randomly divided into 5 groups (n =5 each):group normal saline (NS) 10 μl IT twice a day × 7 days; group M morphine 10μg (10 μl) IT twice a day × 7 days; group MO morphine 10 μg + missense oligonucleotide (ODN) 10μg (10 μl) twice a day × 7 days; group AO morphine 10 μg + antisense ODN 10 μg (10μl) twice a day×7 days; group EA morphine 10 μg (10μl) twice a day×7 days + EA (frequency 2 Hz,wave length 1 ms,intensity 3 mA).EA of Yanglingquan and Zusanli lasting for 30 min was performed after first IT administration of morphine once a day for 7 days.Mechanical pain threshold (MPT) was measured before IT administration and at day 2,4,6 of IT administration and 1 day after the end of IT administration.The animals were sacrificed after the last MPT measurement,and L3-5 segments of the spinal cord were isolated to detect the expression of ERK1/2 and phosphor-ERK1/2 (p-ERK1/2) in the spinal dorsal horn by Western blot analysis.Results Morphine tolerance developed in groups M,MO and AO,and was lightest in group EA.Compared with group NS,the expression of p-ERK1/2 was significantly up-regulated in groups M and MO,the expression of ERK1/2 was down-regulated in group AO (P < 0.05),and no significant change was found in the indexes mentioned above in group EA (P > 0.05).Compared with groups M and MO,the expression of p-ERK1/2 and ERK1/2 was significantly down-regulated in group AO,and the expression of p-ERK1/2 was down-regulated in group EA (P <0.05).Compared with group AO,the expression of p-ERK1/2 and ERK1/2 was significantly up-regulated in group EA (P < 0.05).Conclusion EA can inhibit chronic morphine tolerance-induced increased activity of ERK1/2 in the spinal dorsal horn and alleviate the development of morphine tolerance in rats.
