1.Design of health service resources management based on GIS

Objective To design a data management solution based on geographic information system for health service units to deal with health resources of subordinate medical institutions.Methods The whole solution was developed with layer logic function of data service, application and presentation. Storage, transformation and relationship of both spatial data and health service data were totally managed by relational database.Results The system was able to import, export, maintain, sum, query and analyze geography spatial information and health service information.Conclusion A new method is provided to improve capability of health support and health service management efficiency.
2.Validation of the EuroSCORE and the STS-PROM in adult patients undergoing aortic valve replacement

Objective The aim of the study was to analyze the predictive value of the European system for cardiac operative risk evaluation score (EuroSCORE) and the Society of Thoracic Surgeons predicted risk of mortality (STS-PROM) in -dult patients undergoing aortic valve replacement (AVR).Methods We carried out a retrospective statistical analysis on 521 adult patients undergoing AVR between 1999 and 2008 in Changhai hospital.Patients with concomitant coronary artery bypass grafting were also included.Excluded from this study were patients having surgery for congenital heart defects,aneurysm of thoracic aorta and atrial fibrillation.Operative mortality was defined as death before discharge from the hospital.The mortality risk calculation of EuroSCORE and STS-PROM for aortic valve procedures was performed by the online available EuroSCORE or STS score calculator.Based on the additive EuroSCORE risk calculation,patients were divided into low-risk,medium-risk and high-risk groups.The valuation of three different algorithms depended on the assessment of two features:calibration and discrimination.A comparison of observed and predicted mortality rates was also performed.Results A total of 521 patients were identified as having undergone aortic valve replacement.In-hospital mortality was 4% (21 cases) overall.The expected mortality for the additive,logistic EuroSCORE and the STS-PROM was 3.36%,2.82% and 1.25%,respectively.The observed to expected ratio was 1.2 for additive EuroSCORE,1.43 for logistic EuroSCORE and 3.23 for STS-PROM.The STS-PROM underpredicted observed mortality significantly ( P < 0.01 ) and showed poor calibration in predicting in-hospital mortality in the entire cohort,medium- and high-risk subgroups.The logistic EuroSCORE underpredicted observed mortality in the mediumrisk subgroup ( P < 0.05 ).EuroSCORE underpredicted in-hospital mortality in the high-risk subgroup with the observed-expected mortality rate of 1.84 for additive EuroSCORE and 1.46 for logistic EuroSCORE.The EuroSCORE in three subgroups showed poor discrimination in predicting mortality as well as the STS-PROM did in the medium- and high-risk subgroups ( ROC < 0.7).Conclusion Both the EuroSCORE and the STS-PROM give an imprecise prediction for individual operative risk in patients undergoing aortic valve replacement in our study.These algorithms seem unsuitable to identify a high-risk patient population undergoing isolated AVR.It is necessary to construct a risk stratification model for valve surgery according to the profiles of Chinese patients.
3.Study on the Resistance Mechanisms of Carbapenem-Resistant Klebsiella Oxytoca

Objective To investigate the resistance mechanism of Carbapenem-Resistant Klebsiella oxytoca.Methods Car-bapenem-Resistant Klebsiellaoxytoca were collected from Fujian Medical University Union Hospital.The modified hodge test (MHT)was used for carbapenemase phenotype screening.The minimum inhibit concentration(MIC)was detected using agar dilution method for 1 7 drugs.PCR and DNA sequencing were used to detect commonβ-Lactamase genes and carbapene-mases genes.Conj ugation experiments demonstrated the transferability of the carbapenem-resistant determinants.Results 5 Carbapenem-Resistant Klebsiella oxytoca of 4 isolates were positive detected by MHT.Minimum inhibit concentration was detected by using agar dilution method for 17 drugs.More than 80% isolates were resistance to nine drugs.2 isolates conju-gated successfully of 5 Carbapenem-Resistant Klebsiella oxytoca Isolates.There were 2 isolates included carbapenemases gene (1 isolates were only IMP producers,1 isolate contained the IMP and KPC),3 isolates produce ESBLs gene.Conclution The due to CRE strains isolated from Fujian Medical University Union Hospital may be metallo-enzyme carbapenemase and KPC gene.And the isolate that produce two Carbapenem-Resistant gene had been found in this hospital.
4.Needs of Rehabilitations in Persons with Mental Retardation in Beijing

Objective To investigate the needs of rehabilitations in persons with mental retardation in Beijing.Methods 738 persons with mental retardation surveyed in the Second China National Sample Survey on Disability were involved,their rehabilitation condition and requisition were evaluated.Results 63.69% of them needed rehabilitation in community and family,73.31% needed rehabilitation training,their needs on medical service were met,needs on rehabilitation training were above provided service,needs on assistance articles and utensils were low,and needs on employment were high.Conclusion The case management system for persons with mental retardation should be built to meet their rehabilitation requisitions on different age and grade with various approaches.
5.Research status on the determination of occlusal vertical dimension of edentulous jaws

It is the key step of the occlusal reconstruction to determine the correct occlusal vertical dimension for edentulous patients. Although there are several methods in clinical practice, there is no ideal way. This paper summarizes the following methods: inter-occlusal distance, closest speaking space method, pre-extraction record, face proportion division method and facial appearance observation, cephalometric analysis, electromyography, swallowing, bite force determination and self feeling method. In this paper, the current research status and the feasibility of each method are discussed.
6.Research progress of three-dimensional finite element analysis in unilateral maxillary defect restorations

Unilateral maxillary defects are common clinical maxillofacial deformities. Because of their large area and the complexity of the maxillary structure, the distribution of pressure from dental prostheses and on the sustentacular tissue is usually uneven, which often results in pain or ulceration of the soft tissue and agomphiasis during the therapeutic process. Recently, the finite element method has been used to guide prosthesis design and implantation. This method is conducive to the restoration and stability of the dental prosthesis and the protection of the remaining tissue, which improves restoration quality and patient satisfaction. This paper summarizes the establishment of a three-dimensional finite element model of unilateral maxillary defects and its application in repairing unilateral maxillary defects with traditional prostheses, implant-supported prostheses and surgical flap transplantation combined with prostheses.

8.Active hexose correlated compound potentiates the antitumor effects of low-dose 5-fluorouracil through modulation of immune function in hepatoma 22 tumor-bearing mice.

BACKGROUND/OBJECTIVES: A variety of immunomodulators can improve the efficacy of low-dose chemotherapeutics. Active hexose correlated compound (AHCC), a mushroom mycelia extract, has been shown to be a strong immunomodulator. Whether AHCC could enhance the antitumor effect of low-dose 5-fluorouracil (5-FU) via regulation of host immunity is unknown. MATERIALS/METHODS: In the current study Hepatoma 22 (H22) tumor-bearing mice were treated with PBS, 5-FU (10 mg.kg-1.d-1, i.p), or AHCC (360 mg.kg-1.d-1, i.g) plus 5-FU, respectively, for 5 d. CD3+, CD4+, CD8+, and NK in peripheral blood were detected by flow cytometry. ALT, AST, BUN, and Cr levels were measured by biochemical assay. IL-2 and TNFalpha in serum were measured using the RIA kit and apoptosis of tumor was detected by TUNEL staining. Bax, Bcl-2, and TS protein levels were measured by immunohistochemical staining and mRNA level was evaluated by RT-PCR. RESULTS: Diet consumption and body weight showed that AHCC had no apparent toxicity. AHCC could reverse liver injury and myelosuppression induced by 5-FU (P < 0.05). Compared to mice treated with 5-FU, mice treated with AHCC plus 5-FU had higher thymus index, percentages of CD3+, CD4+, and NK cells (P < 0.01), and ratio of CD4+/CD8+ (P < 0.01) in peripheral blood. Radioimmunoassay showed that mice treated with AHCC plus 5-FU had the highest serum levels of IL-2 and TNFalpha compared with the vehicle group and 5-FU group. More importantly, the combination of AHCC and 5-FU produced a more potent antitumor effect (P < 0.05) and caused more severe apoptosis in tumor tissue (P < 0.05) compared with the 5-FU group. In addition, the combination of AHCC and 5-FU further up-regulated the expression of Bcl-2 associated X protein (Bax) (P < 0.01), while it down-regulated the expression of B cell lymphoma 2 (Bcl-2) (P < 0.01). CONCLUSIONS: These results support the claim that AHCC might be beneficial for cancer patients receiving chemotherapy.

9.The value of myeloid-derived suppressor cell and T lymphocyte subsets in the diagnosis and treatment of active pulmonary tuberculosis

Objective:To investigate the levels and correlation between myeloid-derived suppressor cell (MDSC) and T lymphocyte subsets in peripheral blood of patients with active pulmonary tuberculosis.Methods:A total of 38 patients with active pulmonary tuberculosis in Nanjing Second Hospital from February 2019 to June 2020 were selected as the tuberculosis group, and 23 healthy outpatient physical examination patients were selected as the healthy control group during the same period. The levels of MDSC, clinically related indicators, inflammatory cytokines and lymphocyte subsets were compared between each group, and the correlation between MDSC and lymphocyte subsets was analyzed. Meanwhile, the levels of MDSC and lymphocyte subsets before and after treatment were compared.Results:The MDSC and CRP in tuberculosis group were higher than those in healthy group: (16.41 ± 2.13)% vs. (1.82 ± 0.54)%, (25.42 ± 10.56) mg/L vs. (5.82 ± 1.39) mg/L ( P<0.05). Serum inflammatory cytokines tumor necrosis factor (TNF)-α, interleukin (IL)-6, IL-1β, IL-10 and interferon (IFN)-γ in tuberculosis group were significantly higher than those in healthy control group ( P<0.05). T lymphocyte subsets CD 3+ T cell, CD 4+ T cell, CD 8+ T cell and CD 16/56+ nature killer (NK) cell in tuberculosis group were significantly lower than those in healthy control group ( P<0.05), while the number of CD 19+ B cell was not statistically significant ( P>0.05). Correlation analysis showed that MDSC was negatively correlated with T lymphocyte subsets CD 3+ T cell ( r = -0.73, P<0.001), CD 4+ T cell ( r = -0.68, P<0.001) and CD 8+ T cell ( r = -0.53, P = 0.001), but had no significant correlation with CD 16/56+ NK cell ( r = -0.10, P = 0.561). CD 3+ T cell, CD 4+ T cell, CD 8+ T cell and CD 16/56+ NK cell were significantly different in peripheral blood MDSC before and after treatment ( P<0.05). Conclusions:MDSC, CD 3+ T cell, CD 4+ T cell, CD 8+ T cell and CD 16/56+ NK cell have a guiding role in the diagnosis and evaluation of the curative effect of active pulmonary tuberculosis, with high value in clinical application.
10.Comparative study on the shade matching performance of dental students under D55 and D65 light sources conditions.

OBJECTIVE:
This study aimed to compare the effect of D55 and D65 light sources on the visual colorimetry performance of dental students by using a homemade light-source shelf.
METHODS:
Two Vitapan 3D-Master shade guides were randomly selected. One set was used as shade guides. Ten commonly used shade tabs of 2L2.5, 2M2, 2R2.5, 3M2, 3R2.5, 3L1.5, 3R1.5, 3L2.5, 4R1.5, and 4L1.5 were selected from the other set with covered value marks and numbered from 1 to 10. After the colorimetric training, 49 undergraduate dental students were randomly divided into two groups. Each student randomly selected two of the 10 shade tabs, and the colors were subsequently matched under D65 and D55 light sources from a distance of approximately 40 cm. The average color difference (ΔE) between the color selected by each participant and the actual color of shade tabs was calculated. Paired t test was used for statistical analysis.
RESULTS:
The ΔE values between the color selected by each participant and the actual color of the shade tabs under the D55 light source varied from 0 to 6.540. The average value was 2.501. The ΔE values between the color selected by each participant and the actual color of the shade tabs under the D65 light source varied from 0 to 6.610. The average value was 2.530. No statistically significant difference was observed between the results under the two light sources (P=0.921).
CONCLUSIONS
Both D55 and D65 daylight lamps can be used for daily dental colorimetry. These two different color temperatures showed no significant difference.
