1.Quantitative and qualitative research in the classification of nursing faculty in a certain third-class and first-grade hospital in Shanghai

Objective To classify the nursing faculty in a certain third-class and first-grade hospi-tal.Methods We carried out 2 times of expert consultations about the classification of 14 nursing man-agement faculty in a certain third-class and first-grade hospital by Delphi method.Besides we had further interviews with 5 nursing experts and 12 nurses by means of qualitative interview.Results We got weight of rationality and harmony degree of expert attitude with a authoritative coefficient of 0.7867.The results of qualitative interview included 4 thematic concep4 understanding degree,meaning,basis of classification system.Conclusion Scientific and operational classification system for clinical nurses could set basis for reasonable settlement of nurses resource and design of nurses'career.
2.Acute lumbar sprain treated with massage combined with acupuncture at different distal acupoints: a randomized controlled trial.

OBJECTIVETo observe treatment efficacy of acupuncture at different distal acupoints for acute lumbar sprain after massage.
METHODSOne hundred and fifty patients with acute lumbar sprain were randomly divided into a Yaotongdian (Extra) group, a Houxi (SI 3) group, a Weizhong (BL 40) group, a Chengshan (BL 57) group and a Shuigou (GV 26) group, total 5 groups, 30 cases in each one. After the same massage treatment and based on groups divided, different distal acupoints above-mentioned were selected to be acupunctured. Visual analogue scale (VAS) scores, lumbar activity and treatment efficacy of patients in 5 groups were evaluated.
RESULTSWith acupuncture at distal acupoints after massage,VAS scores of patients in 5 groups were decreased compared with those after massage treatment (all P<0.05); lumbar activity was all obviously improved (all P<0.01); and cured and markedly effective rates were all increased in comparison with those after massage (all P<0.01). But among 5 groups the differences of VAS scores, lumbar activity and cured and markedly effective rates were not statistically significant(all P>0.05).
CONCLUSIONBased on massage treatment, acupuncture at distal acupoints could further improve the treatment efficacy for acute lumbar sprain, but there is no obvious effective difference among every distal acupoint. As long as choosing acupoints with lower pain threshold and stimulating enough, good efficacy could be acquired for acute lumbar sprain.

3.Study of Protective Apparel on Hydroxyl Ammonium Nitrate(HAN) Propellant

Objective To study a protective apparel with protective suit and protective mask in order to protect the operators using new HAN propellant in the course of storage, transportation and filling-up. Methods According to the character of it, polyurethane-coated (PU)polyester fabric was applied in the main part of protective suit and mask, and then anti-fog technology to deal with PU as window material. Results The performance test and probation evaluation of the protective apparel were met relevant standards of the technical specifications. Conclusion Through reasonable structure designing, the defects in the protective apparel were overcome such as bad permeability and large load. The protective apparel was characteristized by portable, comfortable, practical and so on. Furthermore, it enriches the protective system for the liquid propellant.

5.Lipid profiles of patients on continuous ambulatory peritoneal dialysis

Objectives To investigate the serum lipid profiles in patients with continuous ambulatory peritoneal dialysis(CAPD).Methods Serum lipid levels were cross-sectional and prospective investigated in patients with CAPD.Results Compared with those in normal controls,serum TG and apoB increased while HDL,HDL 2,apo A1/apoB and LCAT activity decreased significantly in CAPD patients.TC,LDL-ch and Lp(a) also increased in CAPD patients.Serum Lp(a),LDL-ch,apoB and apoE increased significantly following the CAPD therapy.Conclusions Hyperlipidemia is quite common in CAPD patients and serum lipid levels may change with the time on CAPD.
6.Delphi technique application in the evaluation of access to ophthalmic surgery

Objective To establish an evaluation index system for access to ophthalmic surgery so as to standardize the access. Method Collect 14 ophthalmologists' recommendations on access criteria for ophthalmic surgery in two rounds of questionnaire survey by Delphi method. Results The return rate of rounds of consultation questionnaire was 93.33%, and the average expert authority coefficient was 0.78.Nine ophthalmic surgery access indicators were established. The coordination of expert coefficient was 0.521 after two consultation. It had significant difference ( P<0.01 ). Conclusion The ophthalmic surgery access indicator system is highly credible and allows quantitative assessment of ophthalmic surgery from various aspects. It is more demanding for ophthalmologist, and gets more involvement of patients in the evaluation of ophthalmic surgery access. It could contribute to scientific and rational decision-making on access for ophthalmic surgery.
7.Application of fresh amniotic membrane transplantation combined therapeutic soft corneal contact lens in pterygium

To evaluate the clinical effect of therapeutic soft corneal contact lens in combination with fresh amniotic membrane transplantation in the treatment of pterygium and its clinical value.
●METHODS: This study was a retrospective analysis of 200 pterygium excision patients (200 cases, 220 eyes) in hospital from January 2010-June 2011. The patients were divided into two groups. Group 1 (100 cases, 110 eyes) were patients treated with fresh amniotic membrane transplantation and therapeutic soft corneal contact lens while group 2 ( control group, 100 cases, 110 eyes) did not use corneal contact lens. The clinical outcomes in these two groups were compared in a 6 - 18mo postoperative follow-up observation.
●RESULTS: ln control group using corneal contact lens the epithelium repair time ranged from 2 - 7d with an average time of 3. 8d; in group 1 the epithelial repair time ranged from 1 - 5d with an average time of 2. 5d. The patients were followed up for 6 - 18mo, 9 recurrent eyes were found in control group, the recurrence rate was 8. 2%; 5 recurrent eyes were found in experimental group the recurreat rate was 4. 5%. There were significant differences in the 2 groups comparing treatment outcomes (P< 0. 05). Early stimulative symptoms after surgery can be reduced by wearing contact lenses.
●CONCLUSlON:The therapeutic effect of fresh amniotic membrane transplantation after pterygium excision is enhanced by the therapeutic application of corneal contact lens. The epithelial repair time is shortened, the recurrence rate is reduced after pterygium excision and stimulative symptoms including photophobia, tearing, foreign body sensation and ophthalmodynia caused by corneal epithelial defect can be relieved.

9.Rlationship between serum levels of uric acid and prognosis of infection in the critically ill

Objective To explore the clinical implications of the initial levels of serum uric acid (SUA) after admission in the critically ill patients with infection,and to investigate the relationship between SUA and prognosis of infection.Methods From January 2003 to April 2010,471 patients with infection were admitted to the ICU of Huashan Hospital,Fudan University,Shanghai.Data,including serum uric acid (SUA),serum creatinine,blood urea nitrogen (BUN) and other relevant laborartory results within 24hours after admission,were retrospectively analyzed.The distribution of uric acid was described.The t test was used to evaluate the relation between SUA and preexisting disorders.Patients with different level of SUA were classfied for further analysis.x2 test was used to examine the difference in the prognosis of infection.Results The mean initial level of SUA within 24 hours after admission was 0.232 ± 0.131 mmol/L and the median 0.199 mmol/L. Remarkable difference in initial levels of SUA were observed in patients with preexisting hypertension (t=-3.084,P=0.002),diabetes mellitus (t=-2.487,P=0.013),cerebral infarction (t=-3.061,P=0.002),renal insufficiency (t=-4.547,P<0.01),central nervous system infection (t =5.096,P < 0.01 ) and trauma (t =2.875,P =0.004).Serum uric acid was linearly correlated with serum creatinine and blood urea nitrogen (F =159.470 and 165.059,respectively; P <0.01).No statistical correlation was found between initial levels of SUA and prognosis of infection (x2=60.892,P=0.100).Conclusions There is no direct correlation between the initial levels of SUA afteradmission and the prognosis of infection in the critically ill patients.
10.Nerve growth factor promotes endogenous growth factor releasing from burn wounds

BACKGROUND: Studies have confirmed that nerve growth factor can promote wound tissue to release al kinds of endogenous growth factors and growth factor receptors, which play a positive regulatory role. The nerve growth factor can promote cel proliferation and accelerate wound healing, thus making the wound healing developed from the passive waiting healing to active control healing. OBJECTIVE: To investigate the effects of local application of nerve growth factor on the expressions of transforming growth factor β1 and basic fibroblast growth factor in rat burn wounds. METHODS: Twenty-four adult Sprague Dawley rats were used in the study, and Ⅱ degree deep burn wound was made on the back of rats. Then, these rats were randomly divided into four groups. After burn wound debridement, the wounds were covered with gauzes containing 1, 2.5 and 5 μg/mL nerve growth factor solution and normal saline respectively. At 3, 5, 9 and 14 days after treatment, the wound healing time and percentage of residual wound were observed. Then, wound tissues were cut for histological examination, in order to detect the expressions of transforming growth factor β1 and basic fibroblast growth factor in wounds, as wel as the cel ular DNA cycle. RESULTS AND CONCLUSION: The wound healing time in the treatment groups was shorter than that in the control group, especial y in 5 ug/mL nerve growth factor treatment group (P < 0.01), and the percentage of residual wound in the treatment groups was less than that in the control group. The histological examination showed the number of nucleated cells in the superficial dermis of the treatment groups was significant increased when compared with that in the control group; the expressions of transforming growth factor β1 and basic fibroblast growth factor in the treatment groups at different time points were stronger than those in the control group, and the expressions at 5 and 9 days were stronger than those at 3 and 14 days; percentage of cells in S phase of the treatment groups was significantly increased, especial y in 5 mg/L nerve growth factor group (P < 0.01). The results indicate that local application of nerve growth factor can accelerate wound healing by increasing the expressions of transforming growth factor β1 and basic fibroblast growth factor, stimulating mitosis and promoting proliferation.
