1.Investigation on Public Welfare of Public Hospital Based on Patients' Perspective

Objectives:To understand the patients' cognition and satisfaction with public welfare in public hospitals.Methods:A questionnaire survey was conducted among 600 patients in Beijing municipal public hospitals and the data was statistically analyzed by SPSS 22.0.Results:Patients' understanding of public welfare was not ideal,the satisfaction was quite well,and the satisfaction of the patients in the experimental hospitals was more satisfactory than those in non-experimental hospitals.The patients have urgent needs on rational drug use,mutual recognition of medical results,smooth channels of patient rights,reduced medical expense and shortened waiting time.Conclusions:It should strengthen the publicity and governance of the public welfare of public hospitals to enhance the patients' understanding,breakthrough the urgent needs of public welfare to improve patients' satisfaction,and intensify the policy linkage and explore diversified implement forms of public welfare to improve service level.
2.COMPARATIVE STUDY OF p~(53) GENE TRANSCRIPTS IN THYROID TUMOR AND ADJA-CENT TISSUES OF THYROID TUMOR

To determine whether p53 gene transcripts were altered in thyroid tumor and adjacent tissues of thyroid tumor, total RNA from 12 tumor specimens (9 carcinomas and 3 adenomas) and 4 adjacent tissues of thyroid tumor was exmined by Northern blot hybridization and reverse transcription——polymerase chain reaction. It was found that p53 gene transcripts were present in all tumors and the adjacent tissues of tumor and were significantly more obvious in the samples of carcinomas as compared with those of adenomas and in the samples of tumor as compared with those of the adjacent tissues of tumor. These data suggest that excessive p53 gene expressions are involved in thyroid carcinogenesis.
4.Prescription rules of Chinese herbal medicines in treatment of gastric cancer.

Analysis of the nature, tastes, channel distributions and effects of the frequently used herbal medicines in the prescriptions involved in the clinical literatures about treatment of gastric cancer published from 1988 to 2007 was made in the paper. The literatures were categorized into three types: 1) treatment of middle- and late-stage gastric cancer; 2) prevention and treatment of the recurrence and metastasis after operation; 3) Chinese herbal medicines combined with chemotherapy for enhancing efficacy and reducing toxicity. The most frequently used herbal medicines in the three literature types were qi-invigorating herbs, such as Atractylodes, Astragalus, Codonopsis, Glycyrrhiza and Ginseng, etc. The herbal medicines for promoting urination to subside swelling such as tuckahoe and Semen Coicis, etc were used more frequently than the herbal medicines for regulating qi such as dried orange peel and putchuck, etc, as well as for clearing away heat to remove toxin such as spreading hedyotis herb, Herba Scutellariae Barbatae, yangtao actinidia root, and Rhizoma Paridis, etc. From another angle, the most frequently used herbal medicines for the treatment of gatric cancer were those cold, warm and neutral in nature, sweet, bitter and pungent in taste, and distributed to spleen and liver channels.
5.Clinical Analysis of the Heart Rate Variability in Patients with Old Myocardial Infarction

Objective To investigate the effect of old myocardial infarction on the heart rate variability.Methods Totally 86 patients with old myocardial infarction and 78 healthy people were enrolled in the study.Twenty-four-hour heart rate variability in the time domains including NN,SDNN,SDANN,RMSSD,and PNN50 and the frequency domains including VLF,LF,HF,and LF/HF were analyzed.Results Both the time domain and frequency domain indexes of the heart rate variability in patients with old myocardial infarction significantly decreased than those in the healthy peolple(P 0.05).Conclusion There might be autonomic nervous system dysfunction in the patients with old myocardial infarction.

8.Warm water infusion versus air insufflation in the application of difficult colonoscopy

Objective To explore the effects of warm water insufflation applied in difficult colonoscopy on its success rate, insertion time, operation difficulty level and tolerance of patients. Method 200 cases of the difficult defecation of non-narcotic colonoscopy, IBS and always has a history of pelvic/abdominal surgery patients, were randomly divided into warm water infusion group (experimental group) and traditional method of air insufflation group (control group), then compare the differences of the success rate of the two groups of patients, the insertion time and degree of tolerance in patients. Results All the patients completed the investigation in this study, neither bowel perforation nor other complications occurred. Experimental group success rate of the whole colon examination, the insertion time, the operator satisfaction and check again wishes were significantly higher than that of control group (P < 0.05). In abdominal pressure and utilization, abdominal pain scoring position transformation, the former was significantly lower than the latter (P < 0.05). There is no significant difference (P > 0.05) in Polyp detection rate. Conclusion Warm water insufflation colonoscopy can effectively improve the tolerance and accelerating the colonoscopy in patients with difficult insertion process, improve work efficiency, and is an ideal method, which is worth of promoting.
9.Determination and Consistency Analysis of Particle Size Distribution of Budesonide Nasal Spray

OBJECTIVE:To establish the method for the determination of particle size distribution of Budesonide nasal spray, and to analyze the consistency of particle size distribution of spray samples. METHODS:Water was used as dispersant for mixing and dispersing(1800 r/min). The particle sizes [d(0.1),d(0.5),d(0.9)] corresponded to accumulative particle size of 10%,50%and 90%were used as characteristic value. The distribution of granularity was determined by laser scattering method. The consisten-cy of particle size distribution of samples from 2 manufacturers (A,B) were analyzed among different batches or same batch of same manufacturer by SAS 9.3 statistical software. RESULTS:The mean values of d(0.1),d(0.5) and d(0.9) were 3.96 μm, 29.58 μm and 67.10 μm in manufacturer A. The mean values of d(0.1),d(0.5)and d(0.9)were 2.00 μm,7.53 μm and 28.51 μm in manufacturer B. By analysis,there was great difference in particle size of samples from 2 manufacturers. The particle size of the samples from manufacturer A were larger than that of manufacturer B. The consistency among different batches from manufacturer B was better,and the consistency among same batch were all good from 2 manufacturers. CONCLUSIONS:The established meth-od is suitable for particle size distribution of Budesonide nasal spray and the consistency analysis of particle size distribution.
10.Distribution and drug sensitivity of pathogenic bacteria in the rehabilitation ward

Objective To investigate the bacterial infection status,etiology distribution and drug resistance of neurological rehabilitation hospitalized patients,to guide the clinical rational use of drugs,to provide a scientific basis for the development of effective prevention and control measures.Methods From January 201 1 to December 201 4,the clinical data of 334 hospitalized patients in department of neurological rehabilitation of our hospital were ret-rospectively analyzed.The pathogenic culture results were analyzed.Results There were 334 strains of pathogenic bacteria,83.53% of gram negative bacteria,47 gram positive bacteria(1 4.07%),8 fungi(2.39%).The top three infected gram negative bacterias were Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa.The main Gram positive bacteria was enterococcus.The detective gram negative bacterias were mainly sensitive to piperacillin penicillin/tazobactam and cefoperazone /sulbactam and imipenem,meropenem,cefepime,amikacin.The resistance rate of Escherichia coli to gentamicin,piperacillin and levofloxacin was more than 50%.The resistance rate of Kleb-siella pneumoniae to gentamicin and piperacillin was more than 50%.Conclusion The infection rate of urinary tract and respiratory tract is higher in patients with neurological rehabilitation ward,and the infection rate is higher,and the main part of gram negative bacilli,should be aimed at high risk patients and key links,strengthen the implementation of infection prevention and control measures,rational use of antimicrobial agents.
