1.The Application of Vltrasound-guided Percutaneous Umbilical Blood Sampling

Objective To evaluate the efficacy and safety of ultrasound-guided percutaneous umbilical blood sampling. Methods Eight hundred and seventy-two pregnancies with proper indications underwent ultrasound-guided percutaneous umbilical sampling. The data of each procedure were prospectively collected and subsequently analyzed for successful rate and pregnancy outcomes. Results Of 872 ultrasound-guided percutaneous umbilical sampling, 62.7% were done successfully at the first attempt, 36.0% were done successfully by two to four attempts. 31.4% fetuses had mild procedure-related complications and 0.3% fetuses had severe complications. Conclusion We should improve the efficacy and safety of ultrasound-guided percutaneous umbilical sampling and make it a correct, safe and effective procedure for prenatal diagnosis.
2.Hemodynamic Changes of Renal Artery in Diabetic Children with Color Doppler Ultrasonography

Purpose To study the hemodynamic changes of kidney interlobar artery in children with type 1 diabetes, and to explore the clinical value of the color Doppler ultrasound in diabetic nephropathy during the incubation period. Materials and Methods The study was performed in 52 children with type 1 diabetes and in 51 age-matched healthy children as control group. The resistance index (RI) of interlobar artery was measured with color Doppler ultrasonography, and the clinical data and laboratory indicators were analyzed. Results The left kidney RI, right kidney RI and average RI were all significantly higher in children with diabetes than in age-matched healthy group (P<0.01). The mean RI were correlated positively with (HbA1c) (r=0.96, P<0.01) and diabetes duration (r=0.31, P<0.01), respectively. Fasting blood glucose, glycated hemoglobin, and glomerular filtration rate were significantly higher in children with diabetes than in age-matched healthy group (P<0.01). Conclusion The RI of interlobar artery increases in children with type 1 diabetes. Early changes of renal hemodynamics in children with type 1 diabetes are detectable with Doppler sonography, and have certain value in early diagnosis of diabetic nephropathy.
3.Therapeutic Effect Evaluation of Traditional Chinese Medical Manipulation for Congenital Muscular Torticollis in Infants with Real-time Tissue Elastography

Objective To evaluate the clinical curative effect of traditional Chinese medical massage for the treatment of congenital muscular torticollis in the infants with real-time tissue elastography technology, and to explore the clinical application value of real-time tissue elastography. Methods A retrospective analysis was carried out in 120 infants with clinical diagnosis of congenital muscular torticollis. All of the infants were treated with traditional Chinese medical massage once a day, 10 times for a course of treatment, and the treatment lasted 18 courses. The differences of image of the injured sternomastoid muscle were observed before and after treatment by real-time tissue elastography. Results Before massage therapy, the differences between the image scores of injured sternomastoid muscle and those of the healthy sternomastoid muscle were significant ( P<0.01), and the sensitivity of real-time tissue elastography for the diagnosis of congenital muscular torticollis was 93.3% . After massage therapy, the mass of injured sternomastoid muscle was reduced, and the myogenic torticollis was improved, the image scores of injured sternomastoid muscle were similar to those of the healthy sternomastoid muscle ( P>0.05) . The differences of the image scores of injured sternomastoid muscle were sternomastoid muscle ( P> 0.05) . The differences of the image scores of injured sternomastoid muscle were significant before and after therapy ( P<0.01) Conclusion Traditional Chinese medical massage has certain therapeutic effect for the treatment of congenital muscular torticollis in the infants, and real-time tissue elastography technology can reflect the changes of muscle tissue directly and noninvasively, which will provide objective basis for the clinical diagnosis and treatment of congenital muscular torticollis in the infants.
4.Evaluation of neonatal hypoxic-ischemic encephalopathy by ultrasound measurement of the hemodynamics in the central branches of the middle cerebral artery.

OBJECTIVETo evaluate the hemodynamic changes in the central branches of the middle cerebral artery in different stages of neonatal hypoxic-ischemic encephalopathy (HIE) and provide new evidence for clinical diagnosis of HIE.
METHODSFrom March, 2013 to July, 2013, a total of 136 newborn infants were diagnosed to have HIE in our center. We performed two-dimensional ultrasonography and color Doppler ultrasound for assessments of systolic velocity (Vs) and resistant index (RI) of the central branches of the middle cerebral artery. The data were compared with the results of a control group consisting of 251 normal full-term infants.
RESULTSInfants with mild HIE showed hyperechoic changes in the white matter around the ventricle, while in moderate and severe HIE, such hyperechoic changes were diffuse in both hemispheres with unclear echoes of the brain structures. Pulse Doppler assessments of hemodynamics of the central branches of the middle cerebral artery demonstrated a significant decrease in Vs and an increase in RI regardless of HIE severity (P<0.05). In addition, Vs and RI values in mild HIE infants differed significantly (P<0.05) from those in infants with moderate and severe HIE, who had comparable Vs and RI values (P>0.05).
CONCLUSIONTranscranial ultrasonography may provide dynamic information on cerebral blood flow in neonates and hemodynamic parameters of the central branches of the middle cerebral artery are valuable for clinical diagnosis and early intervention of HIE.

5.Ultrasonography and magnetic resonance imaging for diagnosis of neonatal agenesis of the corpus callosum: a comparative study.

OBJECTIVETo summarize the ultrasonography features of neonate corpus callosum agenesis for better diagnosis of this condition.
METHODSA total of 8563 neonates were screened by cerebral ultrasound in neonate care unit of our hospital from June 2010 to December 2012, and 37 cases of agenesis of the corpus callosum were identified. The diagnostic accuracy of ultrasonography and magnetic resonance imaging (MRI) for this condition was evaluated.
RESULTSThe sensitivity, specificity, and accuracy of ultrasound diagnosis for complete and incomplete absence of neonate corpus callosum were 100% and 90%, 90.9% and 94.1%, 94.6% and 91.9% in the 37 cases, respectively. The Kappa value of ultrasonography and MRI were 0.890 and 0.837, with a consistent rate of 91%.
CONCLUSIONUltrasonography and MRI show a high consistency in the diagnosis of neonatal agenesis of the corpus callosum.
