1.Effects of Bifid Triple Viable Capsules Combined with Dahuang Chishao Decoction on Endotoxemia and Intestinal Mucosal Barrier Function of Patients with Hepatocirrhosis

OBJECTIVE:To discuss effects of Bifid triple viable capsules combined with Dahuang chishao decoction on endo-toxemia and intestinal mucosal barrier function of patients with hepatocirrhosis. METHODS:90 patients with hepatocirrhosis were randomly divided into traditional Chinese and western medicine group and western medicine group with 45 cases in each group. Both groups were given symptomatic treatment,such as liver protection and diuresis,lowering portal pressure and nutrition sup-port. Based on this,western medicine group was given Bifid triple viable capsules 0.42 g,tid,po;traditional Chinese and western medicine group was additionally given Dahuang chishao decoction orally,one dose a day,on the basis of western medicine group. Both 2 groups were treated for 6 weeks. The changes of liver function index,serum endotoxin(ET),procalcitonin(PCT)and di-amine oxidase (DAO) levels of patients were observed and recorded in 2 groups before and after treatment. RESULTS:After 6 weeks treatment,serum ALT,AST and TBIL,ET,PCT and DAO levels of patients in 2 groups declined significantly,compared with before treatment,with statistical significance(P<0.05 or P<0.01);the decrease of traditional Chinese and western medicine group was more significant than western medicine group,with statistical significance(P<0.05). CONCLUSIONS:Bifid triple via-ble capsules combined with Dahuang chishao decoction can reduce ALT,AST and TBIL levels in patients with hepatocirrhosis,pro-tect liver function;reduce serum ET,PCT and DAO levels,release endotoxemia,and protect and remedy intestinal mucosal barri-er function.
2.Effect of PaCO_2 Modulating During Operation on Post-Operative Cognitive Function of Patients Undergoing OPCAB

Objective To observe the effect of PaCO_2 modulating during operation on post-operative cognitive function of patients undergoing off-pump coronary artery(OPCAB). Methods Thirty patients undergoing OPCAB were randomly divided into traditional group (G_ 1 , n=15) and modulated group (G_ 2 , n=15). During operation, PaCO_ 2 in G_ 1 maintained 35 mmHg to 39 mmHg with relatively fixed ventilation parameters setting, and PaCO_ 2 in G_ 2 ranged from 40 mmHg to 45 mmHg by adjusting ventilation parameters. Continuous cardiac output index (CCI), SvO_ 2 , regional cerebral O_ 2 saturation (rSO_ 2 ) and PaCO_ 2 were recorded before distal anastomosis(T_ 1 ), at 5 min of the first distal vessel anastomosis(T_ 2 ), second distal vessel anastomosis(T_ 3 ) and third distal vessel anastomosis(T_ 4 ), and 20 min after the completion of coronary artery anastomoses. HDS-R and ADL were used to examine the patients' cognitive function. Results There were no significant differences in pre- and post-operative HDS-R and pre-operative ADL scores between groups. 3.Effects of different doses of colloid on systemic hemodynamics during general anesthesia induction

Objective To explore the relationship between administration of different doses of colloid before general anesthesia induction and general anesthesia-induced hypotension. Methods Fifty patients for selective gastrointestinal operations were randomly divided into 5 groups according to the volume of colloid administered 30 min before general anesthesia induction: 0 mL/kg group (control group), 4 mL/kg group, 8 mL/kg group, 12 mL/kg group and 15 mL/kg group. To replace the deficit of hypovolemia before operation, crystalloid was given to each patient. Anesthesia was induced with propofol 25 min after infusion. Hemodynamic parameters were compared before and after anesthesia induction among groups. Results The changes in diastolic blood pressure and mean arterial blood pressure were related to colloid supplementation volume (r=-0.657, P<0.01). There were significant differences between control group and 8 mL/kg group, 12 mL/kg group and 15 mL/kg group (P<0.05), while there was no significant difference among 8 mL/kg group, 12 mL/kg group and 15 mL/kg group. Conclusion Administration of colloid before general anesthesia induction attenuates the severity of general anesthesia-induced hypotension, especially when the dose of colloid is over 8 mL/kg.
4.Application value of MDCT double period enhanced scanning with low-tension water enteroclysis pres-entations for colon cancer preoperative staging

Objective This paper is mainly to discuss accuracy and clinical application value of MDCT double-period enhanced scanning with low -tension water enteroclysis for colon cancer preoperative TNM stag-ing.Methods Sixty-two colon cancer patients with complete images and pathological data were selected in our hospital from January 2012 to May 2013 .We retrospectively analyzed CT image changes of the tumor location ,the extent of tumor invasion,the surrounding fat space,lymph node metastasis and distant metastasis.We compared them with postoperative pathology to prove the accuracy of MDCT double -period enhanced scanning with low -tension water enteroclysis.Results The results showed that its accuracy rate reached to 90.32%(56/62)in co-lon cancer preoperative Stage T,80.64%(50/62)in Stage N,and 100%(62/62)in Stage M respectively.Con-clusions MDCT double-period enhanced scanning with low -tension water enteroclysis can accurately display the site of colon cancer and determine the scope of tumor invasion ,lymph node metastasis and distant metastasis , and give more precise diagnosis of colon cancer and preoperative staging assessments .In conclusion , it can be used as the preferable method of preoperative examination in the colon cancer .

In order to further improve the innovative behavior of nurses by applying the empowerment theory management mode, it is helpful for nursing managers to train nursing talents by using empowerment theory.

Since the biological characteristics of esophageal gastric junction (EGJ) cancer are different from those of gastric cancer and esophageal cancer, the choice of initial treatment is particularly important. This article introduces a case of locally advanced EGJ can-cer with single metastasis factor treated with preoperative radiochemotherapy in the Beijing Cancer Hospital. Through the multidisci-plinary team, we aim to achieve a better prognosis for this patient and propose new treatment practices for EGJ cancer.
8.Clinical investigation on the alteration of 99Tcm-MIBI washout rate in ischemic myocardium

Objective To investigate the changes of early and delayed washout rates of 99Tcm-methoxyisobutylisonitrile (MIBI) in ischemic heart disease (IHD), and to explore the value of 99Tcm-MIBI SPECT in evaluating impairment of ischemic myocardial cells. Methods Patients diagnosed of IHD with three-vessel stenosis ( ≥50% ) without myocardial infarction based on angiography (CAG) underwent 99Tcm-MIBI static planar and gated SPECT imaging. The early (90 min after the intravenous injection) and delayed (4 h after the intravenous injection) washout rates of 99Tcm-MIBI and left ventricular ejection fraction (LVEF) of IHD patients and normal subjects were compared using t-test. Linear correlation analysis was performed between the early, delayed washout rates and LVEF measured by gated SPECT. Results Statistically significant lower early washout rate of 99Tcm-MIBI was observed in IHD group than control group: (13.44 ± 2.87 )%vs ( 17.32 ± 4.92) %, t = 2.384, P < 0.05, but higher delayed washout rate of 99Tcm-MIBI was observed in IHD group than control group: (19.24 ±4.71)% vs (15.23 ±3.81)%, t= -2.246, P<0.05. LVEF in IHD group was significantly lower than that in control group: (55.71 ±7.97)% vs (67.75 ±5.43)%, t =-4.418, P <0.01. There were no correlations between the early/delayed washout rates and LVEF, respectively in IHD patients (r = -0.212, P > 0.05; r =0.352, P > 0.05, respectively). Conclusion 99Tcm-MIBI washout rate may reflect myocardial cell impairment due to IHD.
