1.The longitudinal survey of social support and the quality of life of colorectal patients with permanent colostomy during postoperative rehabilitation stage

Objective To investigate the social support and the quality of life of colorectal patients with permanent colostomy,and to examine the stability and change of the social support and the quality of life during the post-operative recovery period.Methods A convenience sample of 30 colorectal patients with permanent colostomy were recruited initially.19 completed the follow-up interview 3 months later.Social Support Rating Scale (SSRS) and the Quality of Life Instrument for Cancer Patients(QLICP) were used to measure the social support and the quality of life.Results The dimensions of SSRS were on decline during 3 months after surgery.The scores of the physical and psychological domains of QLICP increased and the scores of the social,general and specific domains decreased but not significantly during the 3 months after surgery.The scores of specific domain were negatively correlated with the scores of utilization degree of social support,as well as the correlation of scores of specific domain and the total scores of social support.Conclusions The enterostomal therapist should pay attention to the change regulation and relationship between social support and life quality of patients with permanent colostomy,and provide them with physical,psychological,psychiatric and social support according to their demands in order to promote their recovery.
2.Experimental study of Ames test of Echinacea Herb P.E

Objective:To evaluate the potential mutagenicity of Echinacea Herb P.E via salmonella typhimurium Ames test. Methods:To test the Echinacea Herb P.E diffusion through Ames test. In the plate incorporation test of Salmonella Typhimurium,the average number of spontaneous revertants of TA97,TA98,TAl00,and TA102 at four concentrations was calculated after incubation at 37 ℃ for 48 hours. Results:Whether added S 9 or not,the number of induced revertants when compared to that of spontaneous revertants did not increase statistically significant. The result of Ames test was negative. Conclusion:Echinacea Herb P.E will not cause the increase of the back mutation in murine typhoid salmonella which shows that Echinacea Herb P.E will not cause gene mutation.
3.Clinical characteristics of elderly patients with hepatocellular carcinoma and prognosis following radical resection

Objective To investigate the clinical and pathological features of elderly patients with hepatocellular carcinoma (HCC)and the prognosis after radical resection.Methods From January 2013 to December 2014,98 elderly patients with primary liver cancer were enrolled in this study,with 120 non-elderly patients with primary liver cancer serving as the control group.Comparison was made concerning clinical and pathological characteristics,short term postoperative outcomes and long-term prognosis between the two groups.Results The average age of patients in the elderly group(68.4±3.7)was significantly higher than in the control group(53.6 ±5.3),and the difference was statistically significant(t=23.376,P<0.001).The positive rate of HBsAg in the elderly group was 38.8 %,higher than in the control group (70.0 %),and the difference was statistically significant (x2 =21.341,P<0.001).The incidence of liver failure in the elderly group was 4.1%,higher than in the control group (0.0 %),and the difference was statistically significant (xe =4.990,P =0.026).There was no significant difference in survival rate at 6 months,1 year and 2 years between two groups (x2 1.427,2.127,and 0.510,each P>0.05).There was no significant difference in recurrence rate between the two groups at 1 year and 2 years(x2 =0.205 and 0.706,each P>0.05).Conclusions Elderly patients with hepatocellular carcinoma present favorable clinical and pathological features and show similar short and long-term outcomes,compared with non-elderly patients.Radical resection is valuable in the treatment of hepatocellular carcinoma in elderly patients.
4.Effects of Methylmercury on Behavioral Teratogenesis of Rats

Objective To study the effect of methylmercury on the behavioral teratogenesis in filial r ats.Method Pregnant rats were injected intra-abdominally with 0 ,0.75,1.50,3.00 mg/kg methylmercury every other day,then early physiological fun ctions and behavioral functions of their filial rats were observed. Results The results indicated that when impregnated female ra ts were heavily exposed to methylmercury, early physiological development and be havioral development of their filial rats were retarded.Conclusion The methylmercury likely has the effect on behavioral teratogenesis of rats. 5.Expression of the partial protein encoded by mutated COL4A5 gene and analysis of the structure by circular dichroism

T mutation resulting in p.G1015V from an X linked Alport patient, and that of corresponding cDNA from a control were expressed in E.coli. The recombinant and mutant polypeptide was a fragment of COL4A5 , containing 158 amino acid residues with a glycine to valine substitution mutation in it. The secondary structure of the two recombinant proteins was analyzed using circular dichroism(CD) spectroscopy. Results: CD spectra of the control exhibited a negative peak near 200 nm whereas that of the patient exhibited a negative peak near 220 nm. The magnitude of the negative peak of the patient decreased as compared with that of the control. Furthermore, secondary structure of the control polypeptide was mainly composed of ? sheet and random coil without ? helix, whereas that of the patient presented 12.9% ? helix. Conclusion: Not only local structure of the substitution site but also folding kinetics of the entire ?5 chain may be changed due to Gly→Val substitution in Alport syndrome. We speculate that the abnormally folded polypeptide chain may not be assembled into the triple helix and the network of type Ⅳ collagen, or may be assembled into loosen triple helix then degraded easily, resulting in the pathognomonic ultrastructural changes of the glomerular basement membrane.
6.Effect of Gamma Aminobutyric Acid B Receptor on Brain Damage Induced by Recurrent Febrile Seizures

Objective To explore the effect of ? - aminobutyric acid B receptor(GABABR)on brain damage induced by recurrent febrile seizures (FS). Methods Rats were randomly divided into four groups: control group (37. 0 ℃ water, n = 8), FS group (45.2 ℃ water,n=8), FS + baclofen group (45.2 ℃ water,77 = 8), FS + phaclofen group (45. 2 ℃ water,n=8). FS in rats were induced for ten times in a bath of warm water, once every 2 days. The intensity, latency and duration of the seizure in rats were recorded. The expression of c - fos gene and Fos protein were examined by in situ hybridization and immunohistochemistry, respectively. Results Compared with those of FS group, the seizure latency gradually prolonged, and the seizure duration was shortened in FS + baclofen group. In FS+ phaclofen group, the seizure latency was shorter and the seizure duration was longer than those of FS group. The seizure intensity was lessened in FS + baclofen group while aggravated in FS + phaclofen group compared with that of FS group. The expression of c - fos gene and Fos protein increased significantly after recurrent FS. Baclofen down regulated the expression of c -fos gene and Fos protein, while phaclofen enhanced the expression of them. Conclusion The study by using the agonist and the inhibitor of GABABR showed that GABABR might play a crucial role in the development of FS- induced brain damage.
7.Species distribution of pathogens and prognostic factors of catheter-related bloodstream infections in Intensive Care Unit

Objective To investigate the incidence and the species distribution of catheter‐related bloodstream infection(CRBSI) in the intensive care unit(ICU) at our hospital ,and analyze the risk factors for CRBSI .Methods The hospitalized patients microbi‐ologically diagnosed as CRBSl were included in this study from January 2012 to June 2013 .Data were collected retrospectively and analyzed by software SPSS 19 .0 .Results There were 67 patients were diagnosed as nosocomial CRBSI of 987 cases ,in which 24 cases (35 .8% )died in the hospital .Eighty one strains were detected from 67 cases of CRBSI ,including 42 Gram‐positive(G+ ) bac‐teria(51 .9% ) ,36 Gram‐negative(G-)bacteria (44 .4% ) ,and 3 fungi(3 .7% ) .Staphylococcus epidermidis was predominant patho‐genic G+ bacteria ,and Acinetobacter baumannii was predominant G - bacteria .With multiple logistic regressions ,age≥65 ,high A‐PACHEⅡ score and polymicrobial CRBSI were independent predictors of worse outcome .Conclusion Within the latest 18 months , the prevalence of pathogens infection are Staphylococcus epidermidis and Acinetobacter baumannii in CRBSI in ICU .Advanced age , disease severity and polymicrobial CRBSI should be regarded as significant independent risk factor of the CRBSI patients in ICU for mortality .
8.A comparison of hepatitis B virus reactivation rates between liver resection and radiofrequency ablation for patients with tumor ≤5 cm

Objective To investigate the value of liver resection and radiofrequency ablation (RFA) on reactivation rates of hepatitis B virus (HBV) in hepatocellular cancer (HCC) patients with tumor ≤5 cm and determine influential factors.Methods Retrospective analysis was performed in clinical data of hepatocellular cancer patients with tumor ≤5 cm from Shengli Oilfield Centeral Hospital between July 2007 and March 2012,289 cases were assigned to liver resection group (n =157) and RFA group (n =132).Logistic regression was used to evaluate relative factors associated with HBV reactivation for univariate and multivariate analyses.The stratified x2 test was utilized to assess clinical outcomes in HBV reactivation subgroup with or without antiviral treatment.CD3 +,CD4+,CD8 +,CD4+/CD8 + and NK cell proportions were comparatively analysized.Results (1) The univariate and multivariate logistic regression analyses showed antiviral therapy,Child-Pugh grade,vascular invasion and treatment (liver resection or RFA) were significant risk factors of HBV reactivation (P < 0.05).(2) HBV reactivation was lower in patients who received antiviral therapy than those who did not (10/109 vs 33/180,x2 =4.497,P < 0.05).The viral reactivation rate for the liver resection group was higher than the RFA group in patients who did not receive antiviral therapy (24/98 vs 9/82,x2 =5.446,P < 0.05),but the difference was not significant in patients who received antiviral therapy (6/59 vs 4/50,x2 =0.153,P > 0.05).(3) The proportions of CD3 +,CD4 +,CD4 +/CD8 + and NK cell after treatment for 7 days decreased in various degrees for both the liver resection and RFA groups regardless of patients with or without antiviral therapy (P < O.05).For the patients without antiviral therapy,the cell proportions in 7 days after treatment were significantly higher in the RFA group than that in the liver resection group (P < 0.05),but the difference was not significant in patients with antiviral therapy (P > 0.05).Conclusions For the patients with tumor ≤5 cm,the proportions of immunological cells decreased in liver resection when compared with RFA.Preoperative antiviral therapy may have partial response in immunological suppression,and lowered the incidence of HBV reactivation.
9.Mutation of DKC1 Gene in a Family of Dyskeratosis Congenita

Objective To identify the mutation of DKC1 gene and its inheritance in a pedigree with dyskeratosis congenita (DKC). Methods The mutation was detected by polymerase chain reaction(PCR)and DNA sequencing, and restriction endonuclease digestion was performed to confirm the mutation. Results A transition mutation of C to T (1058C-T) in DKC1 gene was found in the proband and his brother. This mutation results in an amino acid change from alanine to valine (A353V) in dyskerin protein. The proband′s mother and sister were carriers of this mutation gene with no phenotype of DKC. Conclusion This pedigree is an X-linked form of DKC with 1058C-T mutation in DKC1 gene.
10.Logistic analysis of impact factors of treatment decision: a study on 147 patients with traumatic ruptured spleen

Objective To investigate the current status of treatment on traumatic ruptured spleen in a regional medical center in Tianjin,China.Method Logistic analysis of 16 impact ffactors of treatment decision on 147 patients with traumatic ruptured spleen.Results The type of trauma,pulse rate at admission,result of abdominal puncture and CT grade were related to treatment decision making.Splenectomy was carried out in 89 patients (60.54%) which accounted for 89.90% of all surgical procedures.Splenectomy was carried out in 50.85% of patients with CT grade 1 or 2 (or 85.71% of grade 1 and 2 surgery),and 100% of CT grade 3 or 4.Spleen preservation surgery was carried out in 7 patients with CT grade 1 or 2.Conclusion Splenectomy was carried out in most cases in this study.It is necessary to investigate the treatment of splenic trauma in regional medical centers,and to develop more reliable surgical techniques to carry out more spleen-preservation surgery ffor CT grade 1 or 2 ruptured spleen.
