1.Observation of clinical effect of budesonide combined with ambroxol hydrochloride on the treatment of infant bronchopneu-monia through oxygen atomizing inhalation

Objective To observe the clinical effect of budesonide combined with ambroxol hydrochloride on the treatment of infant bronchopneumonia through oxygen atomizing inhalation .Methods One hundred and twenty -two cases of infant patients suf-fered with bronchopneumonia were randomly divided into two groups , control group (60 cases), the patients accept conventional treat-ment such as antibiotics, oxygen inhalation and anti-cough;treatment group (62 cases), patients were given budesonide combined with ambroxol hydrochloride through oxygen atomizing inhalation , beside conventional treatment .Main clinical signs and pulmonary symp-toms disappear time and treatment efficacy in two groups were observed and analyzed .Results The total effectiveness was obviously higher in treatment group(91.9%) than that of control group(81.7%) ( P <0.01).Wheezing disappearance time, wheezing sounds disappear time , crackles disappear time and cough disappear time in treatment group were significantly shorter than that in the control group ( P <0.01 ) .There were no adverse reactions on infant patients in both groups .Conclusions The effect of budesonide com-bined with ambroxol hydrochloride on the treatment of infant bronchopneumonia through oxygen atomizing inhalation was significant , and it is worthy of promotion in the clinic .
2.Endoscopic treatment of gastroesophageal variceal hemorrhage combined with hepatic carcinoma

Objective To evaluate the safety and efficacy of endoscopic treatment in patients with primary hepatic carcinoma and gastroesophageal variceal hemorrhage. Methods Thirty cases of hepatic carcinoma with gastroesophageal variceal hemorrhage diagnosed with gastroscopy between January 2008 and June 2015 in Chinese PLA General Hospital and Hainan branch were included. Patients′ clinical data, complications and therapeutic effects of endoscopy were retrospectively analyzed. Results All patients underwent endoscopy successfully. No recurrent bleeding was found in any case after treatment. Eight cases of varicosis nearly disappeared. Six?month follow?up showed three cases of gastrointestinal bleeding, one case of death of bleeding, and no recurrence hemorrhage in 27 others. Conclusion Endoscopic treatment could safely and effectively control the primary liver cancer combined with gastroesophageal variceal hemorrhage, hence may provide opportunity for further treatment of liver cancer.
3.Comparative study of neonatal rat models of hypoxia-ischemia induced white matter injury of brain

Objective To investigate the difference among neonatal rat models of hypoxia-ischemia induced white matter injury of brain by three different methods and to provide theory basis on choosing the proper animal model for deep study on white matter injury of premature infants.Methods Three different hypoxia-ischemia methods were applied to induce brain white matter injury model of Wistar rats of postnatal 3rd day which were left carotid artery ligation followed by 6% oxygen for 30 minutes and for 4 hours respectively,and bilateral carotid artery ligation followed by 8% oxygen for 30 minutes.Compared brain pathological results of rats in different groups,changes in white matter of brain by glial fibrillary acidic protein (GFAP) and myelin basic protein (MBP) immunostaining,and changes in time of opening eyes and eyes appearance.Results Left carotid artery ligation followed by 6% oxygen for 30 minutes resulted in sparse and vague periventricular white matter without necrotic lesions,and the degree of GFAP intensity [(5 021.63 ± 358.92) OD] increase and MBP intensity [(18 488.63 ± 1 822.62) OD] decrease were lowest (P < 0.000 1),as well as the delay of opening time of the left eye.Left carotid artery ligation followed by 6% oxygen for 4 hours resulted in necrotic lesions of periventricular white matter,with the degree of GFAP intensity [(6 069.13 ± 458.61) OD] and MBP intensity [(15 003.38 ± 1 559.11) OD],and also the delay of opening time of the left eye.Bilateral carotid artery ligation followed by 8% oxygen for 30 minutes caused cystic necrosis of bilateral periventricular white matters,and the degree of GFAP intensity [(6 194.50 ±432.69) OD] increase and MBP intensity [(10 119.35 ± 735.16) OD] decrease of the left side were highest(P < 0.000 1),as well as the delay of opening time and cataract in both eyes.Conclusion Left carotid artery ligation followed by 6% oxygen for 30 minutes is more suitable for investigating mild brain white matter injury.Left carotid artery ligation followed by 6% oxygen for 4 hours and bilateral carotid artery ligation followed by 8% oxygen for 30 minutes are more proper for the investigation of severe periventricular leukemacia cases.
4.Identification of a Bacillus sp. Strain with Fibrinolytic Activity and Primary Study on Fermentation Process

An extracellar fibrinolytic strain was isolated from fermented shrimp paste. In addition to general physiological and biochemical properties, the strain was identified by 16S rDNA sequence and systematic analysis. The results showed that 16S rDNA sequence of the strain had high similarity with AY601723 and AB195282, suggesting that the strain is a subspecies of Bacillus sp. It was named as Bacillus sp. nov. SK006 by CCTCC. The medium composition and fermentation conditions for fibrinolytic enzyme production were also optimized in the research.

6.Effect of early goal directed therapy on tissue perfusion in patients with septic shock

BACKGROUND: This study aimed to observe the effect of early goal directed therapy (EGDT) on tissue perfusion, microcirculation and tissue oxygenation in patients with septic shock. METHODS: Patients with early septic shock (<24 hours) who had been admitted to the ICU of Zhongda Hospital Affiliated to Southeast University from September 2009 through May 2011 were enrolled (research time: 12 months), and they didn't meet the criteria of EGDT. Patients who had one of the following were excluded: stroke, brain injury, other types of shock, severe heart failure, acute myocardial infarction, age below 18 years, pregnancy, end-stage disease, cardiac arrest, extensive burns, oral bleeding, difficulty in opening the mouth, and the onset of septic shock beyond 24 hours. Patients treated with the standard protocol of EGDT were included. Transcutaneous pressure of oxygen and carbon dioxide (PtcO2, PtcCO2) were monitored and hemodynamic measurements were obtained. Side-stream dark field (SDF) imaging device was applied to obtain sublingual microcirculation. Hemodynamics, tissue oxygen, and sublingual microcirculation were compared before and after EGDT. If the variable meets the normal distribution, Student's t test was applied. Otherwise, Wilcoxon's rank-sum test was used. Correlation between variables was analyzed with Pearson's product-moment correlation coefficient method. RESULTS: Twenty patients were involved, but one patient wasn't analyzed because he didn't meet the EGDT criteria. PtcO2 and PtcCO2 were monitored in 19 patients, of whom sublingual microcirculation was obtained. After EGDT, PtcO2 increased from 62.7±24.0 mmHg to 78.0±30.9 mmHg (P<0.05) and tissue oxygenation index (PtcO2/FiO2) was 110.7±60.4 mmHg before EGDT and 141.6±78.2 mmHg after EGDT (P<0.05). The difference between PtcCO2 and PCO2 decreased significantly after EGDT (P<0.05). The density of perfused small vessels (PPV) and microcirculatory flow index of small vessels (MFI) tended to increase, but there were no significant differences between them (P>0.05). PtcO2, PtcO2/FiO2, and PtcCO2 were not linearly related to central venous saturation, lactate, oxygen delivery, and oxygen consumption (P>0.05). CONCLUSION: Peripheral perfusion was improved after EGDT in patients with septic shock, and it was not exactly reflected by the index of systemic perfusion.

Objective To understand the psychological health status of the middle and primary school students,and to provide some advice of their mental health care.Methods In 11 611 middle and primary school students in 18 schools in Maojian area of Shiyan city,the students,teachers and parents at 5 schods were investigated a randomely,targets were examined by Achenbach Child Behavior Checklist(CBCL),Pupil Rating Scale Revised Screening for Learning(PRS),Conner's Parent Symptom Questionnaire(PSQ)and Conner's Teacher Rating Scale(TRS)and self-established questionnaires for general information.Results The investigation demonstrated:89.29% of students possessed very promising dreams and sublime pursuits,and had correct outlook on life and values;10.71% of students had comparatively severe psychological problems.The major problems appeared to be indefinite purpose of study,shorted of mental endurance and severe impulsivetrend.Moreover,an-xiety,emotion instability and solitary trend caused indifference,sheepish ness and difficulty in their communication.Conclusion Schools,families and society pay close attention to students,mental health and help the youngsters develop in a healthily psychdogical way.
8.Study on in vitro release and percutaneous absorption of Huoxue Zhitong gel.

To evaluate in vitro release and transdermal behaviors of Huoxue Zhitong gel, modified Franz diffusion cell methods was applied to investigate in vitro transdermal absorption of Huoxue Zhitong gel and the content of paeonolan in receptor fluid composed of PEG400%-95% ethanol-water (l:3:6)were determined by HPLC. The results were processed and different equations were fitted. The release law were in accordance with Weibull equation and the fitting equation was In[-1/(1 - Q)] = -0.790 51nt - 1.7012 (r = 0.9809). In 8 hours, cumulative release of paeonol was 85. 18% and the release rate was 2.827 µg . cm-2 h-1. Transdermal actions were consistent with zero-level model fit and the fitting equation was Q(t) = 1.7579t + 0. 7213 (r = 0.9991). In 8 hours, cumulative transdermal rate and transmission rate of paeonol was 54. 85%, 1. 820 µg . cm-2 h-1. So the Huoxue Zhitong gel had a good release and transdermal properties.

9.Treatment of surgical brain injury with immune tolerance induced by intrathymic injection of brain antigen

Objective To determine the effect of induction of immune tolerance by intrathymic injection of myelin basic protein (MBP) for treatment of surgical brain injury (SBI) in rats.Methods Twenty-four male SD rats were divided into experimental group,control group and sham group with 8 rats each,according to the random number table.SBI model was established in experimental group and control group.Thymuses were exposed and injected with MBP (experimental group) or saline (control group) respectively.Sham group received craniotomy without durotomy and thymus paracentesis.Modified neurological severity score (MNSS),cerebral edema volume,levels of peripheral blood proinflammatory cytokine interleukin (IL)-2 and anti-inflammatory cytokine IL-4,and CD4 +/CD8 + T-cell ratio were measured at postoperative 1,3,7,14 and 21 d.Results MNSS between control and experimental groups differed significantly at each time point (P < 0.05).Compared to sham group,MNSS in experimental group was recovered the same value at 21 d (P > 0.05),but in control group remained high over the study (P < 0.05).Less cerebral edema was observed in control and experimental groups than in control group at each time point (P < 0.05),but all were lower than these in sham group (P < 0.05).Lower IL-2 concentrations and higher IL-4 concentrations were observed in experimental group than in control group at 3,7 and 14 d (P <0.05).Compared to sham group,IL-2 concentrations in experimental group and IL-4 concentrations in control group revealed no significant differences at each time point (P >0.05).CD4 +/CD8 + T-cell ratio between experimental group and control group revealed significant difference at 1,7 and 14 d (P <0.05).CD4 +/CD8 + T-cell ratio in experimental group and control group recovered to the same level in sham group at 7 and 21 d(P > 0.05).Conclusion Induction of immune tolerance through intrathymic injection of MBP can reduce neurological deficit and brain edema and facilitate the recovery of function from SBI.
10.Identification of serum biomarkers for rheumatoid arthritis using protein fingerprint

Objective To identify serum biomarkers for rheumatoid arthritis(RA)by protein finger- print pattern. Methods One hundred and forty-one serum samples of 90 RA patients, 20 systemic lupus ery- thematosus(SLE)patients, and 31 healthy individuals were randomly divided into training set(n=93, 60 RA patients, 13 SLE patients and 20 healthy individuals)and test set(n=48, 30 RA patients, 7 SLE patients and 11 healthy individuals). They were detected by surface enhanced laser desorption/ionization-time of flight- mass spectrometry(SELDI-TOF-MS). The protein fingerprint pattern obtained from SELDI-TOF was trained by a multi-layer artificial neural network(ANN)to establish a diagnostic model. Results The detective mod- el obtained by ANN was used to detect the 48 unknown serum samples. The sensitivity and specificity for RA detection was 90% and 90.9% respectively. Conclusion In comparison with traditional methods, SELDI- TOF-MS could identify new serum biomarkers in RA. Combined with ANN, it provides high sensitivity and specificity for RA diagnosis.
