Chinese Journal of Experimental Ophthalmology 2013;31(12):1169-1172
The researches on ocular dominance have been continuing deeply last decade years since the concept was first described by means by Porta in 1593.Based on the neurophysiology and molecular biology,it is confirmed that the procedure of ocular dominance involves sublayer 4C of the visual cortex,N-Methyl-D-aspartate (NMDA)-receptor,a-calmodulin kinase Ⅱ (aCaMK Ⅱ),synapses and about candidate gens.Meanwhile, ocular dominance theory plays an important guiding role in optometry,laser-assisted in situ keratomileusis and cataract operation. In this review, the mechanism and clinical application of ocular dominance were summarized.

BACKGROUND:BioFlex system as a new pedicle screw fixation of dynamic stabilization device has less been reported concerning its biomechanics.
OBJECTIVE:To study the effect of BioFlex system fixation at different decompression ranges on disc stress at adjacent segments.
METHODS:Eight samples of fresh calf spines were used. Under physiologic axial loads (500, 900, 2 300 N), electronic universal testing machine was used to simulate the lumbar spine at three physiological states (standing, sitting and bending, standing on a portable 20 kg weight and bending). Progressive decompression modeling for each specimen and dividing into five groups:(1) complete status group;(2) complete status+BioFlex group;(3) partial laminectomy+BioFlex group;(4) 1/2 medial facetectomy+BioFlex group;(5) total facetectomy+BioFlex group. Strain gauges were used to record the stress of disc annulus. Electronic universal testing machine was used to record load-displacement curve and calculate stiffness.
RESULTS AND CONCLUSION:(1) The stress of the adjacent segment of the intervertebral disc increased with the expansion of the range of decompression. Compared with the complete status, stress obviously increased after BioFlex fixation, showing significant differences (P<0.05). The stress was significantly increased in the 1/2 medial facetectomy+BioFlex group compared with the partial laminectomy+BioFlex group (P<0.05). However, no significant difference was detected between the partial laminectomy+BioFlex group and complete status+BioFlex group, and between total facetectomy+BioFlex group and 1/2 medial facetectomy+BioFlex group (P>0.05). (2) Axial stiffness reduced with the expansion of the range of decompression. Compared with the complete status, axial stiffness noticeably increased after BioFlex fixation. The difference was not significant among four kinds of reconstruction structures. (3) These findings confirmed that after BioFlex fixation, with the expansion of the range of decompression, the stress of adjacent segments of intervertebral disc gradual y increased, but different ranges of decompression cannot affect the stiffness of reconstruction structure.

BACKGROUND:Lumbar spinal stenosis can severely affect the activities and functions of the lower limbs, and even cause paralysis. OBJECTIVE:To evaluate the clinical effect of intervertebral bone graft with internal fixation for the treatment of lumbar spinal stenosis. METHODS:The lumbar spinal stenosis patients treated with posterior decompression and intervertebral bone graft with internal fixation were fol owed-up. The effect of intervertebral bone graft with internal fixation for the treatment of lumbar spinal stenosis was evaluated through assessing the clinical symptoms and improvement of signs, recovery of neurological function, as wel as intervertebral bone graft and fusion and the occurrence of complications. The effects of posterior decompression and intervertebral bone graft with internal fixation and simple decompression for the treatment of lumbar spinal stenosis were compared, in order to further determine the treatment of intervertebral bone graft with internal fixation. RESUTLS AND CONCLUSION:The fol ow-up results of the lumbar spinal stenosis patients treated with posterior decompression and intervertebral bone graft with internal fixation showed that the symptoms of low back pain, partial limb sensory disturbances and intermittent claudication were improved significantly;the neurological recovery score could be up to 90%or above. Radiographic examination showed good intervertebral bone graft and fusion with less complication of nerves, blood vessels and dural injury. Comparative study showed that the excel ent and good rate of posterior decompression and intervertebral bone graft with internal fixation can reach to 95.2%, while the excel ent and good rate of simple decompression was 23.8%, indicating that the effect of posterior decompression and intervertebral bone graft with internal fixation was significantly better than that of simple decompression.

OBJECTIVE:To probe into the tendency, characteristics and correlation factors of the allopurinol-induced adverse drug reactions(ADR). METHODS: The literature related to allopurinol-induced ADR retrieved from CNKI (between 1991 and 2008) as well as the allopurinol-induced clinical ADR cases collected in our hospital were summarized, analyzed and compared. RESULTS: A total of 77 ADR cases(reported in 76 papers) met the diagnostic criteria of ADR specified by Ministry of Public Health were collected,of which,10 cases(12.99%) died. A total of 67 clinical cases were collected from our hospital,of which,2 cases(2.99%) died, showing a high mortality rate. Mucocutaneous lesion was the chief ADR type, accounting for more than 90%,and the ADR chiefly appeared as drug rash and febrile and multiple organs or systems were often involved in the ADR. CONCLUSION: It is important to enhance ADR monitoring in clinical use of allopurinol.

BACKGROUND: Preliminary experimental study found that the human amniotic mesenchymal stem cells (hAMSCs)transplantation can improve nerve injury symptoms of rats with cerebral infarction.OBJECTIVE: To observe the survival, colonization and differentiation of hAMSCs in the infarct area of cerebralinfarction rats.METHODS: Sixty Sprague-Dawley rats were randomly assigned into hAMSCs transplantation, model or shamoperation groups (n=20/group). Animal models of middle cerebral artery occlusion were produced in the model andtransplantation groups by Zea-Longa method. One day after modeling, rats in the hAMSCs transplantation groupwere given in situ transplantation of 10 μL of hAMSCs (2×106) into the damaged striatum and cortex, while those inthe model and sham operation group were given the same volume of PBS. Within 1 week after transplantation, ratneurological defects were assessed and changes in their body mass were continuously monitored. Two weeks aftertransplantation, TTC staining was used to observe cerebral infarct size, hematoxylin-eosin staining was used forpathological observation of brain tissues, and immunofluorescent staining was used to detect expression ofneuron-specific nuclear protein.RESULTS AND CONCLUSION: With time, weight loss was increased while neurologic deficit scores were graduallyreduced in the hAMSCs and model groups. Compared with the model group, the weight loss and neurologic deficitscores were lower in the hAMSCs group,; however, there was a significant difference in the neurologic deficit scoresbut not in the weight loss between the two groups. Additionally, the hAMSCs significantly reduced infarct size, attenuated pathologic injury, and decreased the number of inflammatory cells. Immunofluorescence stainingshowed that the hAMSCs were observed at 1 week after transplantation under inverted luorescence microscope, and gradually differentiated into nerve cells at 2 weeks after transplantation. In conclusion, transplanted hAMSCsmay migrate to and survive in the cerebral infarct region, and differentiate into nerve cells in situ in rats with cerebralinfarction.
