1.Differentiation of marrow stromal cells into neurons: current progress

Bone marrow stromal cells(MSCs) have been shown to be extremely versatile.Recently,it is demonstrated that MSCs differentiate into neurons.Moreover,under the appropriate conditions,MSCs differentiate into special phynotype of neurons,such as 5-hydroxytryptamine-sensitive neurons,and dopaminergic neurons.The mechanism remains to be elucidated,and an overexpression of neuronal specific genes(such as Sox,Pax,Notch gene) may play an important role,especially RB/PBO gene.However,RB triggers cholinergic differentiation.Extrinsic factor such as concentration of cAMP plays certain role in differentiation.MSCs have also been shown to have certain therapeutic effect on neuronal degeneration disease.These cells have great potential because it is easy to isolate,culture,amplify,and no ethics neither immunologic rejection problem was concerned.
2.Comparison Study on the Associated Gene Expression in Thymus-dependent Lymphocyte of COPD Patients with Pulmonary Qi Deficiency Syndrome and Pulmonary Yin Deficiency Syndrome

Objective To study the associated gene expression in thymus-dependent lymopholyte of the COPD patients with pulmonary qi deficiency syndrome(PQDS)and pulmonary yin deficiency syndrome (PYDS)by gene chips.Methods Collect peripheral blood of patients with PQDS and PYDS(as experimental samples)and of nomal man(as control samples).By Ficoll method,peripheral blood lymphocyte was collected,then thymus-dependent lymphocyte were extracted and purified by flow cytometry.Total RNA were extracted by one-step technique and purified.Then,synthesis double strand cDNA template from total RNA,transcription of cRNA probe with biotin labeling,subsequently,cRNA sample were fragmented. The gene chip(Human Genome U133 Plus 2.0 Array,38500 genes)was hybridized and scanned.Then fluorescent signal value of gene expressing was obtained,and differential expression genes were sifted. Results There were 45 genes expressed differently among PQDS and normal man,including up-regulated 41 and down-regulated 4;32 genes expressed differently between PYDS and normal man,including up-regulated 19 and down-regulated 13;5 up-regulated genes expressed differently between PQDS and PYDS with normal man.Conclusion Gene chip can be applied to study gene expression profiles effectively and to screen PQDS and PYDS associated genes.
3.Risk factors for early death after abdominal damage control operation

Objective To report the early mortality and associated risk factors after damage control operation (DCO) in patients with severe abdominal trauma.Methods A total of 146 patients hospitalized from January to March in 2015 were retrospectively analyzed.Early death was defined as death occurring within 72 h of first surgery and before the definitive surgery.Based on the death definition, the subjects were divided into death group and survival group.The two groups were compared for gender, age, injury time, injury classification, vital signs, need for cardiopulmonary resuscitation, Glasgow coma score (GCS), injury severity score (ISS), hemoglobin, platelets, prothrombin time (PT), activated partial thromboplastin time (APTT), blood pH, base excess, operative time, and postoperative acute physiology and chronic health evaluation score Ⅱ (APACHE Ⅱ).Logistic regression analysis was applied to examine the predicators of early postoperative mortality.Results There were 118 patients (80.8%) in survival group and 28 patients (19.2%) in death group.Survival and death groups differed significantly with regard to the percent of elderly (6.8% vs.21.4%), percent of patients with multiple injury (62.7% vs.85.7%), body temperature [(36.1 ± 0.4) vs.(35.2 ± 0.8) ℃], percent of patients with greater ISS (31.4% vs.64.3%), PT [(12.1±1.5) vs.(13.9±1.2)s], bloodpH (7.25±0.04vs.7.08±0.11), base excess [(-8.9±2.8) vs.(-10.6±3.3)mmol/L], postoperative APACHE Ⅱ[(12.8 ± 1.8) vs.(17.5 ± 2.0) points] (P < 0.05).Logistic regression analysis identified the age (OR=1.512, 95% CI 1.112-4.157,P<0.05), ISS (OR =1.313,95% CI 1.134-5.442, P < 0.05), APACHE Ⅱ (OR =1.361,95 % CI 1.182-5.222, P < 0.05) as the independent risk factors for early mortality.Conclusion The patients underwent DCO for severe abdominal trauma has a high early mortality, which is closely associated with the age, injury severity and postoperative medical status.
4.Follow-up Study on Long-term Prognosis of Gastric Cancer Patients with Acute Disease

Background:Gastric cancer is a common gastrointestinal malignancy. The detection rate of early gastric cancer is still low in China,and some gastric cancer patients visit the hospital due to acute disease,such as gastric perforation. Aims:To investigate the influence of acute disease on long-term prognosis in gastric cancer patients. Methods:A total of 318 patients with gastric cancer from Jan. 2009 to Jan. 2015 at Shaoxing People’s Hospital were enrolled,and were divided into acute disease group and non-acute disease group. Clinicopathological characteristics were analyzed. Patients were followed up,and survival rate was compared between the two groups. Results:Fifty-three(16. 7% )patients had acute disease,and the remaining 265(83. 3% )patients were without acute disease. Compared with non-acute disease group, percentage of TNM Ⅳ stage was significantly increased(P < 0. 05),and percentage of radical surgery was significantly decreased in acute disease group(P < 0. 05). Kaplan-Meier assay showed that survival rate in acute disease group was significantly decreased when compared with non-acute disease group( P < 0. 05). After adjusting the TNM stage and surgical treatment,no significant difference in survival rate was found between the two groups. Conclusions:Gastric cancer patients with acute disease have lower survival rate. Acute disease may be not an independent prognostic factor, higher TNM stage and lower proportion of radical surgery are the main reasons for poor prognosis in gastric cancer patients with acute disease.
5.Clinical application of the covered self-expanding stent in the treatment of malignant esophageal stricture

Objective To investigate the value of covered self expending metallic stent in the treatment of malignant esophageal stricture. Methods Thirty two patients with dysphagia due to malignant esophageal obstructions, esophagobronchial and anastomotic stenoses were undergone inserting self expending metallic stents. The procedure was done under fluoroscopic guidance. Sixteen patients underwent radiotherapy and (or) chemotherap after the procedure. All patients underwent esophagography before and after the stent placement, and were followed up to death. 6.Preliminary analysis of differentially expressed protein profiles in two primary colonic tumor cell lines

Objective To analyze the differences in protein pro fi le between laterally spreading tumor (LST) cell line and SW480 cell line using t wo-dimensional electrophoresis (2-DE). Methods 2-DE was empl oyed to isolate the total proteins of the two cell lines.The gels were stained with silver, and the differential protein expressions were analyzed with Melanin e 3 software. Results The protein spots of the two cell lines w ere 1285?51(LST) and 1184?47(SW480) respectively,containing 96?7 differential protein spots with 50?6 expressed or obviously increased only in LST cell line and 47?5 in SW480 cell line. Conclusion The protein profiles between LST and SW480 cell lins are different and further analysis on the differ ent interested spots is required to acquire the biology-associated proteins spe cific LST.
7.The influence on adhesion between laterally spreading tumor cell and collagen Ⅰ mediated by membrane galectin-1 monoclonal antibody inhibition

Objective To observe the influence on adhesion between LST-R1 cell and collagen Ⅰ mediated by galectin-1 antibody inhibition. Methods Laser confocal scanning microscopy was employed to detect the expression of galectin-1 on LST-R1 cell membrane.LST-R1 cells inhibited by galectin-1 antibody or not were seeded in the 48-well plate coated with collagen Ⅰ (20?g/well), and the morphology of the adhesive LST-R1 cells was observed and the adhesive cells were counted. Results Laser confocal scanning microscopy showed the expression of galectin-1 on the membrane of LST-R1 cells.The binding rate between galectin-1 and its antibody was 38.2%?0.92%. In the galectin-1 antibody inhibiting group,more and more irregular angular adhesive cells appeared, and most of the adhesive cells were growing in singles, while the adhesive cells,mainly clustered, in the control group were round or roudish. 8.Clinical evaluation on the effect of paclitaxel in the treatment of advanced breast cancer

Objective To evaluate the efficacy and toxicity of chemotherapy containing paclitaxel in patients with advanced breast cancer.Methods Chemotherapy containing paclitaxel was used in 30 patients with advanced breast cancer.Paclitaxel was administered by iv infusion at a dose of 135*!mg/m 2 in 3-hour every 3~4 weeks for an average of 4 sessions. All patients received premedication of dexamethasone, diphenhydramine and cimetidine to prevent allergic reactions. Results The overall response rate was 43%(13/30),including complete remission(CR) in 3(10%) cases and partial remission(PR) in 10(33%). The average duration of remission was 1.6 months in CR cases and 5 months in PR cases respectively.The major toxicity associated with paclitaxel administration includes neutropenia,myalgia,arthralgia, numbness of hands and feet,alopecia and flushing of face. Conclusion Paclitaxel is an effective agent for treatment of advanced breast cancer and its side effects are tolerable.
9.Therapeutic Effect of Atomoxtine in Attention Deficit Hyperactivity Disorder with Comorbidity

Atomoxetine is a noradrenaline transmitter reuptake inhibitor which can selectively inhibit presynaptic noradrenaline transport protein,strengthen noradrenaline's resupination effect,and increases noradrenaline's activity released by synaptic cleft to improve the symptoms of the attention deficit hyperactivity disorder(ADHD)patients.ADHD patients often accompany with other psychological diseases like oppositional defiant disorder,anxiety disorder,which may hinder the diagnosis of ADHD.There is still no accepted effective medicine nowadays.Currently,research on the treatment of the ADHD is more and more popular.Good therapeutic effect of atomoxetine to treat ADHD has been found in resent studies.This article summarized the current management of using atomoxetine to treat ADHD.
