1.Literature Review of Allopurinol and Analysis of Allopurinol-induced Adverse Reactions

OBJECTIVE:To probe into the tendency,characteristics and correlation factors of the allopurinol-induced adverse drug reactions(ADR). METHODS: The literature related to allopurinol-induced ADR retrieved from CNKI (between 1991 and 2008) as well as the allopurinol-induced clinical ADR cases collected in our hospital were summarized,analyzed and compared. RESULTS: A total of 77 ADR cases(reported in 76 papers) met the diagnostic criteria of ADR specified by Ministry of Public Health were collected,of which,10 cases(12.99%) died. A total of 67 clinical cases were collected from our hospital,of which,2 cases(2.99%) died,showing a high mortality rate. Mucocutaneous lesion was the chief ADR type,accounting for more than 90%,and the ADR chiefly appeared as drug rash and febrile and multiple organs or systems were often involved in the ADR. CONCLUSION: It is important to enhance ADR monitoring in clinical use of allopurinol.
2.Evaluation of Clinical Effect of Rabeprazole Combined with Teprenone Capsules in Treatment of Gastric Ulcer by Marking Targeting Biopsy and Leptin

Objective To evaluate clinical effect of rabeprazole combined with teprenone capsules in treatment of gastric ulcer by marking targeting biopsy and leptin.Methods A total of 118 patients with active gastric ulcer confirmed by endoscopy were randomly divided into two groups.Patients in the treatment group (n=60) were given rabeprazole 10 mg,bid,and teprenone capsules 50 mg,tid.Patients in the control group (n=58) were given rabeprazole 10 mg,bid.Both groups were treated continuously for 56 days.Before and after treatment,2 groups were labeled with biopsy,the clinical efficacy and the healing rate of two groups were recorded,the quality of healing and the expression of leptin were compared.The level of leptin was tested after treatment.Results After 10 days,the difference of clinical curative effect was not statistically significant (P>0.05).After 56 days,the difference of clinical curative effect was statistically significant (P<0.05);ulcer healing rate (93.33%)in treatment group was higher than that of control group (72.41%);ulcer healing quality (93.33%) in treatment group was higher than that of control group (58.62%);leptin level of treatment group was lower than that of the control group;gastric ulcer recurrence rate (3.8%) in treatment group was lower than that of the control group (24.0%) (all P<0.05).Conclusion Rabeprazole combined with teprenone in the treatment of gastric ulcer is better than rabeprazole.Marking targeting biopsy and leptin can be used to evaluate the healing quality of gastric ulcer more accurately,which can be an evaluation index of the quality of gastric ulcer healing and used as an indicator of the quality of gastric ulcer healing.
3.Effects of Tongmai Decoction on PLT, PAgR, D-D and TEG in elderly patients with intertrochanteric fracture in perioperative period

AIM To investigate the effects of Tongmai Decoction on PLT,PAgR,D-D and TEG in elderly patients with intertrochanteric fracture in perioperative period.METHODS One hundred and fifteen recruited patient subjects were randomly divided into two groups.Fifty-seven cases in the control group were treated with Low Molecular Weight Heparins Calcium Injection,and 58 cases in the treatment group were intervened with both Low Molecular Weight Heparins Calcium Injection and Tongmai Decoction.The preoperative and postoperative dynamic changes of PLT,PAgR,D-D and TEG (R value,K value,alpha angle,MA),and adverse drug reactions in the two groups were detected 1,3,7 d prior to,and after the treatments,respectively.RESULTS No statistically significant difference between the two groups' PLT,PAgR and D-D contents were observed before the operation (P > 0.05).The levels of PLT,PAgR and D-D in both groups increased one day after the operation with similar change degree (P > 0.05).On the 3rd and 7th days after the surgery,the PLT,PAgR and D-D contents in the two groups began to drop,and the decline in the treatment group was more significant (P < 0.05).No significantly different TEG (R value,K value,alpha angle,MA) between the two groups was noticed before the operation (P > 0.05).Patients of both groups displayed their R value,K value decrease,and alpha Angle,MA increase (P > 0.05) one day after the operation with similar change degree.Their R value,K value increased,and alpha angle,MA decreased,and as well as the statistically significant change between the two groups were discovered on the 3rd and 7th postoperative days (P < 0.05).CONCLUSION Tongmai Decoction can effectively reduce blood hypercoagulability and prevent deep venous thrombosis in terms of PLT,PAgR,D-D and TEG (R value,K value,alpha angle,MA) improvement in elderly patients with intertrochanteric fracture in perioperative period.
4.An analysis on the influenza -related excess mortality in Ningbo City

Objective To examine influenza -related excess mortality in Ningbo City from 2010—2014.Methods Data related to death and virology were collected from surveillance system of Ningbo Center for Disease Control and Prevention from 2010 to 2014.Rate differential model was used for data analysis to calculate excess mortality caused by five different cause of death.Results The duration of influenza prevalence was about 37 weeks from 2010 to 2014.The prevalent influenza strains from 2010—2013 were type B,H1NI and H3N2,and the prevalent influenza strains in 2014 were H1NI and H3N2 .From 2010 to 2014,for all cause of death (AC),the average influence -related excess mortality per week was 2.54 per 100 000 in Ningbo City.In five influence seasons,the average influence -related excess mortality for AC were 12.66 /100 000,15.19 /100 000,27.86 /100 000,12.66 /100 000 and 25.32 /100 000,respectively.The average influence -related excess mortality for five cause of death (PI,RC,IHD,COPD,AC)were 1.68 /100 000,12.83 /100 000,1.34 /100 000,0.84 /100 000 and 18.74 /100 000.During 5 monitoring years,annual excess mortality of≥65 group were more higher than that of total population,and annual excess mortality of 0 -4 group were lower than that of total population.Conclusion Influenza was prevalent mainly in winter and spring.The excess mortality resulted from influenza was high.Vaccinating people older than 65 and chronic cardiovascular patients could reduce the risk.
5.Effects of sodium hyaluronate on growth and adhesion of two human colorectal cancer cell lines in vitro.

OBJECTIVETo investigate the effects of sodium hyaluronate on the growth and adhesion of colorectal cancer cells.
METHODSHuman colorectal cancer cell lines SW620 and Colo205 were treated with sodium hyaluronate (25 -2,500 microg/ml), and cancer cell proliferation was measured by MTT assay in vitro. Flow-cytometric analysis was applied to detect expression of CD44 on SW620 and Colo205 cells.
RESULTIn vitro sodium hyaluronate enhanced proliferation of Colo205 cells, but it had no appreciable effect on SW620 growth under the same doses, Meantime, CD44 expression on cancer cells decreased compared with controls.
CONCLUSIONIn vitro sodium hyaluronate has different effects on growth of different colorectal cancer cell lines, but can inhibit CD44 expression of colorectal cancer cells and influence their ability of adhesion.

6.Effect of 3D-printed nano-β-tricalcium phosphate in repairing seawater-soaked tibial defects in rabbits

Objective:To explore the repairing effects of 3D-printed nano-β-tricalcium phosphate (β-TCP) scaffolds loaded with vancomycin and bone morphogenetic protein-2 (BMP-2) for seawater -soaked tibial bone defects in rabbits. Methods:A total of 27 male New Zealand White rabbits were assigned to the normal group using a random number table method, with each group consisting of 9 rabbits. The rabbit tibial bone defect model was created using the osteotomy surgical method. Eight hours after operation, the wounds in the control group and seawater group were immersed in seawater for 2 hours, and those in the normal group were not immersed. After an observation period of 5-7 days, no significant redness or purulent discharge was observed in the wound appearance, then debridement was performed followed by corresponding implantations: the control group with gelatin sponges loaded with vancomycin and BMP-2, and the other two groups with 3D-printed nano-β-TCP scaffolds loaded with vancomycin and BMP-2. After filling the bone defects with the respective materials, all groups underwent layer-by-layer suturing of the wound, followed by disinfection with iodine and injection of gentamicin to prevent infection. The affected limbs were then immobilized using a plaster cast. The affected limbs were imaged using anteroposterior X-ray at 4, 8 and 16 weeks after operation, and the repair effects were evaluated using the Lane-Sandhu X-ray scoring system. At 16 weeks after operation, the bone defect tissues were collected for HE staining to observe bone tissue growth.Results:At 4 weeks after operation, the Lane-Sandhu X-ray score in the control group was significantly lower than that in the normal group [(2.8±1.1)points vs. (1.1±0.9)points] ( P<0.05), and that in the seawater group [(2.2±1.0)points] was not significantly different from those in the other two groups (all P>0.05). At 8 weeks after operation, the seawater group [(6.1±0.9)points] and the control group [(2.8±1.0)points] exhibited lower Lane-Sandhu X-ray score compared to the normal group [(8.2±1.0)points] (all P<0.05), and the seawater group showed a higher score compared to the control group ( P<0.05). At 16 weeks after operation, the control group [(3.8±1.0)points] exhibited a lower Lane-Sandhu X-ray score compared to the normal group [(10.0±1.3)points] and the seawater group [(9.3±1.2)points] (all P<0.05), while no significant difference was noted between the latter two ( P>0.05). At 16 weeks after operation, histological observations revealed varying degrees of bone tissue formation in three groups, with the normal group showing the best bone defect repair effect, followed by the seawater group. Conclusion:The 3D-printed nano-β-TCP scaffolds loaded with vancomycin and BMP-2 are effective for the treatment of seawater -soaked bone defects, which can promote bone tissue repair.
7.Development of a GeXP based multiplex RT-PCR assay for simultaneous differentiation of nine human hand food mouth disease pathogens.

A multiplex RT-PCR assay based on GeXP system was developed in order to detect simultaneously human enterovirus 71 (EV71) and coxsackievirus A16 (CVA16) and other coxsackieviruses (CVA4, 5, 9 and 10, CVB1, 3 and 5). Enterovirus detection was performed with a mixture of 12 pairs of oligonucleotide primers including one pair of published primers for amplifying all known pan-enterovirus genomes and eleven primer pairs specific for detection of the VP1 genes of EV71, C A16, CVA4, CVA5, CVA9, CVA10, CVB1, CVB3 and CVB5, respectively. The specificity of multiplex RT-PCR system was examined using enterovirus cell cultures and positive strains identified previously from hand-foot-and-mouth disease (HFMD) patients. Serial dilution of titrated EV71 and C A16 cell cultures and in vitro transcripted RNA of enterovirus VP1 regions were used to detect the sensitivity of the multiplex RT-PCR system. The limit of detection for this multiplex RT-PCR system was 10(0.5) TCID50/microL for EV71 and C A16 cell cultures and 1000 copies for in vitro transcripted RNA of nine viruses per assay. This multiplex RT-PCR assay is a rapid, sensitive and specific assay for the diagnosis of common enterovirus infection in cases of HFMD outbreak and is also potentially useful for molecular epidemiological investigation.

8.Impact of the origin of sinus node artery on recurrence after pulmonary vein isolation in patients with paroxysmal atrial fibrillation.

BACKGROUNDMajor atrial coronary arteries, including the sinus node artery (SNA), were commonly found in the areas involved in atrial fibrillation (AF) ablation and could cause difficulties in achieving linear block at the left atrial (LA) roof. The SNA is a major atrial coronary artery of the atrial coronary circulation. This study aimed to determine impact of the origin of SNA on recurrence of AF after pulmonary vein isolation (PVI) in patients with paroxysmal AF.
METHODSSeventy-eight patients underwent coronary angiography for suspected coronary heart disease, followed by catheter ablation for paroxysmal AF. According to the origin of SNA from angiographic findings, they were divided into right SNA group (SNA originating from the right coronary artery) and left SNA group (SNA originating from the left circumflex artery). Guided by an electroanatomic mapping system, circumferential pulmonary vein ablation (CPVA) was performed in both groups and PVI was the procedural endpoint. All patients were followed up at 1, 3, 6, 9 and 12 months post-ablation. Recurrence was defined as any episode of atrial tachyarrhythmias (ATAs), including AF, atrial flutter or atrial tachycardia, that lasted longer than 30 seconds after a blanking period of 3 months.
RESULTSThe SNA originated from the right coronary artery in 34 patients (43.6%) and the left circumflex artery in 44 patients (56.4%). Freedom from AF and antiarrhythmic drugs (AADs) at 1 year was 67.9% (53/78) for all patients. After 1 year follow-up, 79.4% (27/34) in right SNA group and 59.1% (26/44) in left SNA group (P = 0.042) were in sinus rhythm. On multivariate analysis, left atrium size (HR = 1.451, 95%CI: 1.240 - 1.697, P < 0.001) and a left SNA (HR = 6.22, 95%CI: 2.01 - 19.25, P = 0.002) were the independent predictors of AF recurrence.
CONCLUSIONSThe left SNA is more frequent in the patients with paroxysmal AF. After one year follow-up, the presence of a left SNA was identified as an independent predictor of AF recurrence after CPVA in paroxysmal AF.

9.Extending the CONSORT Statement to moxibustion.

The STandards for Reporting Interventions in Clinical Trials Of Moxibustion (STRICTOM), in the form of a checklist and descriptions of checklist items, were designed to improve reporting of moxibustion trials, and thereby facilitating their interpretation and replication. The STRICTOM checklist included 7 items and 16 sub-items. These set out reporting guidelines for the moxibustion rationale, details of moxibustion, treatment regimen, other components of treatment, treatment provider background, control and comparator interventions, and precaution measures. In addition, there were descriptions of each item and examples of good reporting. It is intended that the STRICTOM can be used in conjunction with the main CONSORT Statement, extensions for nonpharmacologic treatment and pragmatic trials, and thereby raise the quality of reporting of clinical trials of moxibustion. Further comments will be solicited from the experts of the CONSORT Group, the STRICTA Group, acupuncture and moxibustion societies, and clinical trial authors for optimizing the STRICTOM.

10.Analysis on infection risk in secondary close contacts of COVID-19 patients.

Objective: To study the infection rate of secondary close contacts of COVID-19 patients, and assess the infection risk in the contacts. Methods: COVID-19 patients' close contacts (with a clear exposure time to index case) with negative nucleic acid test results and secondary close contacts were surveyed in continuous isolation and medical observation in this prospective study. The dynamic nucleic acid test results of the close contacts and secondary contacts of COVID-19 patients were collected to assess their risk of infection. Results: A total of 4 533 close contacts were surveyed, in whom 14 were confirmed as COVID-19 patients with overall secondary attack rate of 0.31%, and 4 201 secondary contacts were tracked, in whom no subsequent infections occurred. Conclusion: Close contacts of COVID-19 patients entered in centralized isolation for medical observation with negative nucleic acid tese results,the secondary close contacts of COVID-19 patients have no risk of infection.
