1.Bronchoalveolar lavage for the treatment of neonatal pulmonary atelectasis under lung ultrasound monitoring in 32 cases

Objectves To investigate the effectiveness and safety of bronchoalveolar lavage for the treatment of neonatal pulmonary atelectasis under ultrasound monitoring.Methods Thirty-two neonates diagnosed with pulmonary atelectasis by lung ultrasound,and hospitalized in the Neonatal Intensive Care Unit of Bayi Children's Hospital,the General Hospital of the Chinese People's Liberation Army between July 2014 and June 2016,were included in this study.Bronchoalveolar lavage was performed in all the patients by injection of lavage fluid (0.9% NaC1 or 0.9% NaCl plus ambroxol hydrochloride and/or exogenous pulmonary surfactant) 1.5 to 3.0 ml via tracheal intubation.Lung ultrasonography was performed immediately after each lavage to reveal the status of lung recruitment.Repeated lavage one to three times made up of one course of treatment.The bronchoalveolar lavage could be performed for one to two courses daily according to the status of atelectasis recovery.Medical records were reviewed to analyze descriptively the effectiveness,side effects and complications of the bronchoalveolar lavage.Results Bronchoalveolar lavage was significantly effective in 25 patients (78.1%) with disappearance of pulmonary atelectasis after one course of treatment;effective in five cases (15.6%) with disappearance or reduction of atelectasis after two or three courses;with a total effectiveness rate of 93.8%(30/32).Bronchoalveolar lavage was ineffective in two cases (6.2%) with no remarkable change in atelectasis after three courses of treatment.Vital signs were stable in all the infants during the bronchoalveolar lavage,and no adverse effects and complications occurred.Conclusions Bronchoalveolar lavage is effective for the treatment of neonatal pulmonary atelectasis under ultrasound monitoring,and it is easy to operate and with no adverse effects and complications,and thus worth of clinical application.
2.Effects of extract of Ginkgo biloba on apoptosis of neurocytes and expression of inducible nitricoxide synthuse after spinal cord injury in rats

Objective To investigate the effect of extract of Ginkgo biloba (Egb) on apoptosis of nerve cells and its mechanism after spinal cord injury (SCI) in rats. Methods Forty eight adult SD rats weighing 200-230 g were divided equally and randomly into Egb group and normal saline (NS) group. After hemisectian of spinal cord at T9 vertebrae level, rats in Egb group were lavaged with 2 ml EGB (20 mg) daily and those in NS group with 2 ml NS daily. Tissue sections were collected and stained with Nissl's staining, myelin sheath staining, and inducible nitric oxide synthase (iNOS) immunohisto-chemistry as well as terminal deoxynueleotidyl transferase-mediated dUTP nick end lebeling (TUNEL) at days 1,7, 14 and 21 respectively to evaluate the injured spinal cord tissues after six rats from each group were sacrificed Results Nissl's staining manifested less swelling of the nerve cells near the injury epi-center ( rostral and caudal ), smaller cavity and demyelinated area and higher ratio of bilateral anterior horn neurons of transection side to normal side in Egb group, compared with NS group ( P <0.05). Ap-optotie index (AI) and expression of iNOS in NS group were higher than those in Egb group ( P <0.01 or P <0. 05). Furthermore, the rate of iNOS-positive cells was positively correlated with the AI (r = 0.729, P<0.01) after SCI. Conclusion Egb can prevent nerve cells from apoptosis after SCI in rats, as may be related with inhibition of expression of iNOS.
3.Remnant stomach-jejunal dual pathways reconstruction after laparoscope-assisted radical proximal gastrectomy

Objective To investigate the value of remnant stomach-jejunal dual pathways reconstruction after laparoscope-assisted radical proximal gastrectomy in the treatment of upper gastric cancer. Methods Twenty-five patients with upper gastric cancer underwent laparoscope-assisted radical proximal gastrectomy and the remnant distal stomach was preserved for side-to-side remnant stomach-jejunal anastomosis and end-to-side jejuno-jejunal anastomosis to reconstruct dual pathways. Results The mean operation time was (240±35) minutes, the mean number of lymph nodes dissected were 22±5, and all the incised margins were negative. No anastomotic leakage, obstruction or stenosis occurred. All patients received postoperative barium meal examination. A large amount of barium directly entered the jejunum, leaving a small amount of barium entered the jejunnum via the route of remnant stomach-duodenum, and was detained in the remnant stomach for 30-60 minutes. No esophageal reflux of barium was observed. All the patients were followed up for 4-18 months, no reflux esophagitis was detected and the short-term life quality was satisfactory. Conclusions Remnant stomach-jejunal dual pathways reconstruction prevents the reflux esophagitis and dumping syndrome, preserves the pathway of duodenum and promotes the life quality of patients.
4.Mechanisms for biocompatibility of chitosan: A new viewpoint

OBJECTIVE:In recent years, chitosan has been widely used as tissue engineering scaffolds. In this paper we reviewed the research progress in chitosan biocompatibility and gave a hypothesison possible mechanism of interactions between cells and chitosan. A model system to test this hypothesis was also discussed. DATA SOURCES: Literatures about chitosan biocompatibility were retrieved with computer in Medline, Pubmed and Elsevier from January 1998 to December 2006 with the key words of."chitosan, biocompatibility, surface charge, cell adhesion" in English.STUDY SELECTION: Literatures about chitosan biocompatibility and interactions between chitosan and cells, especially the influence of chitosan charges on cell attachment, were included, whereas repeated experiments were excluded.DATA EXTRACTION: Totally 374 literatures were collected. Among which, 30 were admitted and reviewed.DATA SYNTHESIS: Many mammalian cells can adhere, spread and proliferate on chitosan materials. It is widely accepted that the biocompatibility of chitosan is due to the electrostatic attractive force between positively charged amino groups on chitosan chains and negatively charged cell membranes. However, the pKa value of chitosan amino groups is 6.2-6.8 and the positive charge of chitosan chains is largely decreased under physiological condition as a result of amino groups unprotonation. Thus whether the chitosan's biocompatibility is due to its positive charge remains doubtful and needs further study.CONCLUSION: Based on prior studies, we hypothesize that the positive charge of amino groups on chitosan chains might not be the major factor in biocompatibility of chitosan material. Agarose/chitosan blending hydrogels is supposed to be an appropriate model system to test this hypothesis.
5.Role of emergency and staged hepatectomy in peritoneal metastasis associated with spontaneous rupture of resectable hepatocellular carcinoma patients

Objective To evaluate the role of emergency and staged hepatectomy in peritoneal metastasis associated with ruptured hemorrhage of resectable hepatocellular carcinoma (HCC) patients,and investigate the impact of surgery timing-selecting on peritoneal metastasis of postoperative HCC patients.Methods A retrospective analysis was conduct on the pooled data from 38 HCC patients with spontaneously ruptured hemorrhage treated in our hospital from August 2011 to January 2016.These patients were divided into emergency group who underwent hepatectomy within 24 hours at admission,and staged group who underwent the procedure one week after admission.Perioperative events,overall survival (OS) and disease-freesurvival (DFS) rates,incidence of recurrent and metastatic disease were compared between the two groups.Results The perioperative blood loss and transfusion were much more in emergency group than staged group (both P < 0.05).Nevertheless,the incidence of postoperative mortality was not significantly different (6.0% vs 0%,P > 0.05).The median survival was 22.5 months in emergency group versus 14.2 months in staged group.The 6-month,1-year,3-year OS rates in emergency group were 88.2%,82.4% and 30.3% respectively,and 6-month,1-year,3-year DFS rates were 81.3%,54.7% and 27.3%.The 1-year OS and 6-month DFS rates were higher than those of staged group (both P < 0.05).The incidence of peritoneal metastasis in staged group was higher than that in emergency group,but it was not significantly different (38.1% vs 29.4%,P > 0.05).Univariate and multivariate analysis indicated that tumor diameter ≥ 10 cm and AFP > 10 000 μg/L were the risk factors for peritoneal metastasis after hepatectomy for HCC patients with spontaneously ruptured hemorrhage.Conclusions Emergency hepatectomy would warrant a better short-term prognosis compared with staged hepatectomy for the HCC patients with spontaneously ruptured hemorrhage.Staged hepatectomy would not raise the possibility of postoperative peritoneal metastasis.The predictors of tumor diameter ≥ 10 cm and AFP > 10 000 μg/L were risk factors for peritoneal metastasis after hepatectomy for spontaneously ruptured HCC patients.
6.A Novel Animal Model for Anterior Cruciate Ligament Reconstruction Using Oval Tunnels

Objective To study the general shape of anterior cruciate ligament (ACL) insertion in rabbits and establish an animal model of ACL reconstruction using oval tunnels.Methods Eighteen mature white New Zealand rabbits were used in this study.Eight of them were used for anatomy study and the other 10 were for building an animal model.After removal of the medial femoral condyle and other soft tissues around ACL,the morphology of the ACL insertion was examined and the diameter of ACL insertions was measured using a caliper.An oval-tunnel dilator (1.6 mm×2.5 mm) was designed to make an oval-tunnel in the right knee of the rabbits while a round tunnel was drilled using a 2 mm diameter Kirschner wire in the left knee of the rabbits.Their hamstring tendon grafts were harvested as grafts for both sides and the compatibility between the bone tunnel and graft was examined for both groups.Right after the surgery,the knees of both sides were given the three-dimensional CT scan.Results The shape of ACL insertion of rabbits was oval.In the femur side,the average major and minor diameter of the ACL insertion was 5.28 ± 0.83 mm and 2.61 ± 0.33 mm respectively.In the tibial side,the major and the minor diameter of the ACL insertion was 5.33 ± 0.40 mm and 2.68 ±0.11 mm.The bone tunnel was compatible with the graft in both groups.In the oval tunnel ACL reconstruction group,the cross sectional area of the femoral bone tunnel was 3.18 ± 0.09 mm2 and the cross sectional area of the tibial bone tunnel was 3.26 ± 0.15 mm2.In the round tunnel ACL reconstruction group,the corresponding measurements were 3.13 ± 0.10 mm2 and 3.11 ± 0.11 mm2 respectively.There was no significant difference between the two groups.Conclusion The shape of ACL insertion in rabbits is oval.Using the self-made oval tunnel dilator we have successfully built an oval tunnel ACL reconstruction animal model with a good compatibility between the bone tunnel and graft.This lays the foundation for further research in the future.
7.Laparnscopic repair for adult inguinal hernia in 512 cases

Objective To summarize the experiences in laparoscopic inguinal hernia repairing for adult patients. Methods Clinical data of 512 hernia cases admitted in our center from March 2007 to Sep 2010 were retrospectively analyzed.There were 437 cases of single-sided hernia,including 281 indirect inguinal hernia,86 direct inguinal hernia,15 femoral hernia,16 combined inguinal hernia and 39 recurrent hernia.There were also 75 cases of double-sided inguinal hernia,including 3 recurrent hernia.There were 41 acute incarcerated hernia cases.The average postoperative follow up time was(29 ± 12) months. Results 507 cases underwent successful laparoscopic repair,and 5 cases were converted to open procedure.There were 238 TAPP and 269 TEP in laparoscopic operations.The average operative time for TAPP was (69 ±19) min,and (58 ±15) min for TEP.The average length of postoperative stay was (5.0 ± 1.5) days.The percentage of resuming normal activity after 2 weeks and 4 weeks were 95.7% (485/507) and 99.0%(502/507).The most common postoperative complications were seroma (9.7%,49/507),transient paresthesia (4.1%,21/507) and chronic pain (0.8%,4/507).The recurrence rate was 0.6% (3/507).Conclusions Laparoscopic repair of inguinal hernia has the advantage of less trauma,faster recovery,and lower recurrence rate.
8.Optimization theory and practical application of membrane science technology based on resource of traditional Chinese medicine residue.

Resource of traditional Chinese medicine residue is an inevitable choice to form new industries characterized of modem, environmental protection and intensive in the Chinese medicine industry. Based on the analysis of source and the main chemical composition of the herb residue, and for the advantages of membrane science and technology used in the pharmaceutical industry, especially membrane separation technology used in improvement technical reserves of traditional extraction and separation process in the pharmaceutical industry, it is proposed that membrane science and technology is one of the most important choices in technological design of traditional Chinese medicine resource industrialization. Traditional Chinese medicine residue is a very complex material system in composition and character, and scientific and effective "separation" process is the key areas of technology to re-use it. Integrated process can improve the productivity of the target product, enhance the purity of the product in the separation process, and solve many tasks which conventional separation is difficult to achieve. As integrated separation technology has the advantages of simplified process and reduced consumption, which are in line with the trend of the modern pharmaceutical industry, the membrane separation technology can provide a broad platform for integrated process, and membrane separation technology with its integrated technology have broad application prospects in achieving resource and industrialization process of traditional Chinese medicine residue. We discuss the principles, methods and applications practice of effective component resources in herb residue using membrane separation and integrated technology, describe the extraction, separation, concentration and purification application of membrane technology in traditional Chinese medicine residue, and systematically discourse suitability and feasibility of membrane technology in the process of traditional Chinese medicine resource industrialization in this paper.

9.The renaturation and purification of RGD-staphylokinase by gel filtration.

A recombinant RGD-Staphylokinase(RGD-Sak) with thrombolytic and anti-thrombolytic bifunction was expressed in E. coli. The expression product accumulates as inclusion bodies. In order to obtain active molecule, the RGD-Sak in the inclusion body should be denatured and then renatured. The renaturation of RGD-Sak was performed by gel filtration. Comparing with the traditional way of dilution renaturation, gel filtration way is better than the traditional one, since there are some advantages, such as simple processing, high recovery, low cost and higher purity after renaturation, After renaturation, RGD-Sak was purified by Q-Sepharose FF, and the purity was more than 95%. Analysis of CD spectra showed that the final product from the two renaturation ways have similar CD spectra. It was demonstrated that RGD-Sak molecules proceeded correct refolding through gel filtration or dilution renaturation process.

10.Advance in studies on effect of traditional Chinese (Tibetan) medicines in prevention and treatment of acute altitude sickness.

Acute altitude reaction is a stress response of organism to special altitude environmental factors such as hypoxia, low pressure, cold, dry and strong ultraviolet. As it is the most incident disease in high altitude areas, its prevention remains a problem to be solved. In China, the traditional Chinese (Tibetan) medicines have been recognized as an effective means of preventing and treating acute altitude sicknesses. Some single-recipe or compound traditional Chinese (Tibetan) medicines have been proved to be effective for acute altitude sicknesses. In this article, we will describe traditional Chinese (Tibetan) medicines of different types with efficacy in prevention and treatment of altitude sicknesses.
