1.Clinical analysis of silent paraganglioma and the literature review:21 cases

Objective To study the clinical characteristics of silent paraganglioma ( PGL) .Methods A total of 21 patients with silent PGL confirmed by surgical pathology and hospitalized in the First Affiliated Hospital of chongqing Medical University from Mar .2008 to Dec.2013 were enrolled in this study .The clinical manifesta-tions, hormone level, imaging findings, pathologic characteristic, treatment and follow-up data were reviewed and analyzed retrospectively .Results Among the 21 cases of silent PGL , 8 were male and 13 were female, with 47.05 years as the mean age(ranging from 26 to 67 years old).PGL tumors in silent group were mainly located in the right side, and left to right ratio was 2∶5.Silent PGL with tumor diameter <3 cm, 3-5 cm, >5 cm were 5, 7,and 9 cases respectively.Its constituent ratio was 23.8%, 33.3% and 42.9% respectively, significantly higher compared with that of symptomatic PGL (9.8%,43.9%,and 46.3%respectively).Silent PGL can be i-dentified from other adrenal incidentaloma based on enhanced CT value in each period .Biochemical examination or functional imaging further could facilitate the diagnosis of silent PGL .All the 21 cases were conducted with surgical treatment , and hypertensive crisis occurred to 4 cases during operation .Conclusions Silent PGL is not easy to be detected due to its non-specific clinical manifestations .The diagnosis needs the combined analysis on anatomical imaging characteristics , biochemical examination , functional imaging and pathological findings .Sur-gery is the most effective treatment .Postoperative pathological tips and long-term follow-up are both required for PGL, no matter benign or malignant .

3.Effects of aerobic exercise on the level of plasma homocysteine,total nitric oxide synthase and nitric oxide in rats fed a high methionine diet

Objective To observe the effects of aerobic exercise on the plasma content of homocysteine (Hcy),total nitric oxide synthase(T-NOS) and nitric oxide (NO) in rats fed a high methionine diet. Methods A total of 24 male Wistar rats were randomly divided into a normal diet(CR)group,a high methionine diet(MR) group and a high methionine diet plus swimming group(T+MR),and treated accordingly for 8 weeks.At the end of the treatment,all the animals were tested for plasma T-NOS,NO and homocysteine levels.Results Plasma homo-cysteine doubled,whereas T-NOS and NO levels in the MR group decreased significantly as compared with the CR group,suggesting that hyperhomocysteinemia was induced by the high methionine diet.Plasma homocysteine content decreased significantly in the T+CR group,but plasma T-NOS and NO increased significantly compared with the MR group.These indicators were not significantly different for the T+MR group from those of the CR group. Conclu-sion Adequate aerobic exercise can decrease the plasma level of homocysteine in rats fed a high methionine diet,and increase the plasma level of T-NOS and NO,helping prevent the development of hyperhomocysteinemia.
4.Effects of valproic acid on expression of SMN2 mRNA in neuron-like cells derived from patients with spinal muscular atrophy

Objective To study the effects of valproic acid (VPA) to the expression of SMN2 mRNA in neuron-like cells (NLCs) derived from patients with spinal muscular atrophy (SMA). Methods Polymerase chain reaction-restriction fragment length polymerphism (PCR-RFLP) was performed to select the patients with SMA. Mesenchymal stem cells (MSCs) derived from patients were induced into NLCs which were set as the model of neurons. The transcripts of SMN2 by reverse transcription-polymerase chain reaction (RT-PCR) combined with sequenceing were detected. Semi-quantitative RT-PCR analysis was performed to detect the changes of SMN2 mRNA expression between before and after the NLCs were treated by VPA. Results Two bands (266 bp and 212 bp) were found in the gel picture of RT-PCR, which were respectively the products of full length transcript (fl-SMN mRNA) and skipping exon 7 (SMN?7 mRNA). 6.The expressions of IL-33 and ST2 receptor in nervous system and their roles in pain

Interleukin-33 (IL-33),a novel member of IL-1 family,is a multifunctional cytokine.IL-33 can act as a transcriptional repressor.It can also bind to growth stimulation expressed gene 2 protein (ST2) to induce the activation of some inflammatory cells.IL-33 is extensively expressed in nervous system.Some studies showed that the levels of IL-33 and ST2 were significantly changed in pain.Some reports demonstrated that the IL-33/ST2 signaling pathway played critical roles in them.This present article will review the biological characteristics of IL-33/ST2 and their expressions in nervous system and significance of interventional pain related diseases.

Objective To observe the effect of liver-soothing and spleen-strengthening,essence-tonifying and eyesight-improving herbal medicine for ametropic amblyopia(AA) in children.Methods AA children were randomized into 2 groups.The two groups received treatment of optometry and sight-enhancing device based on microcomputer.Meanwhile,group A(52 affected eyes in 29 patients) received oral use of Ximing Decoction with liver-soothing and spleen-strengthening,essence-tonifying and eyesight-improving actions,which is mainly composed of Radix Curcumae,Radix Pseudostellariae,Radix Panacis Quinquefolii,Fructus Aurantii Immaturus,Herba Agastaches,Flos Buddlejae,Nux Prinsepiae,Fructus Setariae Germinatus,Radix Ophiopogonis,Fructus Crataegi and Massa Medicata Fermentata.Group B(46 affected eyes in 28 patients) received oral use of levodopa additionally.Seven days constituted one treatment course.The treatment lasted 8 courses.Before and after treatment,corrected visual acuity(CVA) was observed.Results In group A,CVA of 13 eyes were improved,AA of 33 eyes cured,6 eyes ineffective,and the total effective rate was 88.46%.In group B,CVA of 10 eyes were improved,AA of 12 eyes cured,24 eyes ineffective,and the total effective rate was 47.83%.The effect in group A was superior to that in group B(P

Objective To compare the therapeutic effect of Microendoscopic Discectomy (MED) with love's method for the treatment of lumbar disk herniation. Methods Pre- and postoperative JOA score, the length of incision, blood lost, operative time existed weight of disk, body temperature variation and time on getting out of bed after operation were compared between the MED group and the love's method group randomized 35 cases respectively since December 1999. 9.Relationship between carotid sclerosis and lipoprotein(a) in patients with coronary artery disease

ObjectiveTo study the relationship between carotid sclerosis and lipoprotein (LP) (a) in patients with coronary artery disease. MethodsOne hundred and fifty-nine coronary artery disease patients with coronary arteriongraphy (CAG) positive were selected as the study group, and Gensini points were taken in these patients. Forty CAG normal patients was in control group, and all of them were examined with ultrasound, and serum levels of LP (a) were tested. According to ultrasound, study group were divided into 4 subgroups: normal intima group(24 cases), mild stenosis group(41 cases), moderate stenosis group (82 cases) and severe stenosis group ( 12 cases). ResultsThe rate of carotid sclerosis in study group [84.9% (135/159)] was significantly higher than that in control group [42.5% ( 17/40 )] (P < 0.01 ). Gensini points in moderate and severe stenosis group were significantly higher than those in normal intima and mild stenosis group (P < 0.05 or < 0.01 ), Gensini points in severe stenosis group were significantly higher than those in moderately group(P < 0.01 ), but there was no significant difference between mild stenosis group and normal intima group(P > 0.05 ). The level of LP(a) in study group[(35.27 + 23.38) mg/L] was significantly higher than that in control group[(23.13 ± 16.56) mg/L](P< 0.01 ). The level of LP(a) in mild stenosis group was significantly lower than that in moderate and severe stenosis group(P< 0.05 or < 0.01 ), and there was no significantly difference compared with that in normal intima group(P> 0.05 ). ConclusionsCarotid sclerosis ultrasonic testing helps to predict the risk degree of coronary artery disease. The level of LP(a ) is closely related with carotid sclerosis and has important value in judging the condition of patients with coronary artery disease.
10.Minimally invasive percutaneous nephrolithotomy: A report of 52 cases

Objective To investigate the effect of minimally invasive percutaneous nephrolithotomy(MPCNL) in the management of renal or ureteral calculi.Methods MPCNL was performed in 52 patients with renal or ureteral calculi under the localization of C-arm fluoroscopy and/or B-ultrasonography.Results Among the 52 patients,a conversion to open surgery was needed in 1 patient because of perforation of the renal pelvis.The stones were completely extracted on one session in 33 patients,and by a re-operation of MPCNL through the fistulization tube 1 week later in 15 patients.The total stone-free rate was 92.3%.The operation time was 1.5~4 h(mean, 2.5 h).No blood transfusion was needed.Conclusions Minimally invasive percutaneous nephrolithotomy is a safe and effective procedure,especially suitable for basic hospitals.
