1.Association between metabolic syndrome and prostate volume
Chinese Journal of Geriatrics 2012;31(11):935-937
Objective To evaluate the relationship between parameters of MS and prostate volume. Methods A total of 843 men aged 30 to 49 years were enrolled from August 2008 to July 2011.Prostate volume by ultrasonography, serum prostate specific antigen, digital rectal examination, urinary analysis, and MS-related parameters were investigated. We evaluated the correlations of prostate volume with MS and MS components. Results Prostate volume was largerin the MS group (28.3±5.5 ml) than in the non-MS group (23.2±5.6 ml,P<0.05).The prostate volumes in subjects with abnormal levels of fasting plasma glucose (FPG) (25.4±5.5 ml),body mass index (BMI) (25.4±5.3 ml) and waist circumference (30.2±4.8 ml) were increased compared with those of subjects with normal parameters (23.6±5.5 ml,22.6±5.5 and 23.5±5.3 ml,respectively, all P<0.05).The liner regression analysis showed that the FPG,BMI,WC,systolic pressure and triglyceride had a positive correlation with the prostate volume(r=0.216,0.397,0.411,0.170,0.184,respectively; all P<0.05),but negatively related to HDL-C(r=-0.107,P<0.05).Conclusions The patients with abnormal FPG,BMI and waist circumference have larger prostate volume.

Objective To investigate the relationship between metabolic syndrome (MS) and early recurrence of benign prostatic hyperplasia (BPH) after prostatectomy in the elderly. Methods A total of 152 men aged 65 to 88 years with prostatectomy for more than 5 years were enrolled from August 2008 to March 2013.Blood pressure, body weight, body height, body mass index (BMI) and international prostate symptom score (IPSS) were detected.Levels of fasting blood glucose (FBG),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C),prostate specific antigen (PSA) and maximum urinary flow rate (Qmax) were determined. The correlation of MS-related parameters with prostate volume,IPSS and Qmax were evaluated. Results BPH recurrence rate was significantly higher in patients with MS than without MS (11/26 vs.27/126,x2=12.76,P<0.001).PSA level, prostate volume and IPSS were significantly higher or larger in the MS group than in the non-MS group [(1.96±0.82)μg/L vs.(1.81±0.90)μg/L,(28.26±5.50) ml vs.(22.38±4.00)ml,(11.12±3.18)vs.(7.11±2.37),F=28.654,44.41,38.56,respectively,P<0.001],while Qmax value was significantly lower in the MS group than in the non-MS group [(14.77±5.29) ml/s vs.(19.80±4.70) ml/s,F=59.48,P<0.001].The liner regression analysis showed that in postprostatectomy recurrence group,PV and IPSS had significantly positive correlations with levels of FBG and TG,and BMI (P<0.05),and had significantly positive correlations with BMI and FBG in non-recurrence group (P<0.05),while Qmax had a significantly negative correlation with BMI,systolic blood pressure and FBG in the two groups (P<0.05).Conclusions There is a significant relationship between MS and higher BPH recurrence in elderly patients.

To explore the effectiveness and safety of endovascular recanalization for chronic internal carotid artery occlusion patients under Hasan classification. To evaluate the guiding value of Hasan classification for endovascular recanalization for chronic internal carotid artery occlusion. To investigate the effect of successful endovascular recanalization on systolic blood pressure. Methods We retrospectively reviewed 30 consecutive chronic internal carotid artery occlusion patients.According to Hasan classification and DSA results of our hospital, the study subjects were divided into 4 types:A,B,C and D.After admission, the rate of recanalization, perioperative complication and followup outcomes were collected. Results Among the 30 CICAO patients,83.3% were male and 16.7% were female, with an average age of (58.5±8.6) years. The success rate of endovascular recanalization in 30 patients was 63.3%.According to Hasan classification, the success rate of endovascular recanalization in type A was 81.8%,Type B 75%,Type C 50%,Type D 0%;The perioperative complications was 23.3%,9.1% for type A and 37.5% for type B,Type C 37.5%,Type D 0%,no severe stroke or death occurred during perioperative period. No related complications occurred in all patients during followup. There was statistical significance in systolic blood pressure before and after interventional recanalization in successful group (P<0.01).There was no significant difference in preoperative and postoperative systolic blood pressure between the unsuccessful group of interventional recanalization (P>0.05).Conclusion Interventional recanalization for CICAO patients is safe and effective according to Hasan classification criteria. This classification can screen patients suitable for interventional recanalization, predict the success rate of surgery.Successful interventional recanalization may result in a decrease in postoperative systolic blood pressure.
