With the aging of the global population, osteoporosis (OP) is becoming a major public health concern worldwide. Currently, the commonly used anti-osteoporosis drugs in clinical practice have limited application due to many side effects. Therefore, developing more effective and safer strategies for the prevention and treatment of OP has become a research focus in this field. In recent years, the clinical efficacy and advantages of traditional Chinese medicine (TCM) in treating OP have been gradually recognized. With the deepening pharmacological research on TCM for OP prevention and treatment, it is found that the active ingredients of Scutellaria baicalensis can promote bone formation or inhibit bone resorption by regulating signaling pathways, including Wnt/β-catenin, osteoprotegerin (OB)/receptor activator of nuclear factor-κB ligand (RANKL)/RANK (OPG/RANKL/RANK), and bone morphogenetic protein 2 (BMP-2)/Smad, mitogen-activated protein kinase (MAPK), and mammalian target of rapamycin (mTOR). However, existing research on active ingredients of S. baicalensis for OP treatment is scattered, making it difficult for scholars to gain a systematic understanding of its research and application. This review summarized the literature on the active ingredients of S. baicalensis in OP treatment worldwide, clarified their mechanisms of action, and explored some issues, providing references for the integration of TCM in OP prevention and treatment.

4. Down-regulation of METTL5 inhibits proliferation, migration and invasion of triple-negative breast cancer cells through Wnt/6-catenin signaling pathway

Aim To investigate the role and potential mechanism of methyltransferase-like 5 (METTL5) in triple-negative breast cancer (TNBC) . Methods The expression of METTL5 in TNBC tumor tissues and cell lines was detected by immunohistochemistry and Western blot. After shRNA targeting METTL5 (shRNAMETTL5) was transfected into TNBC cells, cell proliferation, migration and invasion were detected by CCK-8, colony formation, wound healing and Transwell assays, respectively. Western blot was used to detect the expression of Wnt/p-catenin signaling-related key proteins. A xenograft tumor model was constructed to verify the effect of METTL5 knockdown on the growth of TNBC cells and Wnt/p-catenin signaling activity in vivo. Results The expression of METTL5 was up-regulated in TNBC tumor tissues and cell lines (P < 0. 01) . Knockdown of METTL5 significantly inhibited the proliferation, migration and invasion of TNBC cells and reduced the expression of Wnt/p-catenin signaling molecules (3-catenin, cyclin Dl, matrix metalloproteinase (MMP) -2 and MMP-7 (all P < 0. 01) . Knockdown of METTL5 reduced tumor growth and Wnt/pcatenin signaling activity in vivo. Conclusions Knockdown of METTL5 can inhibit the proliferation, migration and invasion of TNBC cells, which may be related to the inhibition of Wnt/p-catenin signaling pathway.
5.Deconstructing the Application of Danggui Beimu Kushen Pill in Tumor from the Pathogenesis of Phlegm-stasis-toxin

[Objective]To explore the clinical application of Danggui Beimu Kushen Pill in tumor from the pathogenesis of phlegm-stasis-toxin.[Methods]Based on the relevant discussion of Lingshu,the important pathogenesis of tumor occurrence and development was discussed.Meanwhile,the mechanism of formulation of Danggui Beimu Kushen Pill was deconstructed on the basis of the pathogenesis of phlegm,stasis and poison,and related clinical cases were listed.[Results]The pathogenesis of ma-lignant tumor was"no phlegm stasis,no shape"and"no toxic evil,no malignancy".The formulation principle of Danggui Beimu Kushen Pill was the method of eliminating phlegm and removing stasis and detoxifying.Two cases were examplifed,the patient in case 1 was prostate cancer after adverse urination,because the patient had the body of phlegm stasis for a long time,after the operation,the collaterals were more damaged with phlegm stasis stagnation and adverse gasification,so Danggui Beimu Kushen Pill was used to remove phlegm and stasis,added with diuretic products for cure;In case 2,the patient had the whole urine blood due to the rupture of the advanced tumor lesion of kidney cancer,because the patient had phlegm and poison glued together,the pulse was warm,resulting in kidney collateral injury and forced blood overflow,so Danggui Beimu Kushen Pill and Xijiao Dihuang Soup were used to clear heat and free strangury,and cool the blood to stop bleeding.[Conclusion]De-constructing Danggui Beimu Kushen Pill from the pathogenesis of"phlegm-stasis-toxin"can better understand the decoction and apply it in tumor clinic.
6.An optimal medicinal and edible Chinese herbal formula attenuates particulate matter-induced lung injury through its anti-oxidative, anti-inflammatory and anti-apoptosis activities.

OBJECTIVE:
Identifying novel strategies to prevent particulate matter (PM)-induced lung injury is crucial for the reduction of the morbidity of chronic respiratory diseases. The combined intervention represented by herbal formulae for simultaneously targeting multiple pathological processes can provide a more beneficial effect than the single intervention. The aim of this paper is therefore to design a safe and effective medicinal and edible Chinese herbs (MECHs) formula against PM-induced lung injury.
METHODS:
PM-induced oxidative stress, inflammatory response and apoptosis A549 cell model were used to screen anti-oxidant, anti-inflammatory and anti-apoptotic MECHs, respectively. A network pharmacology method was utilized to rationally design a novel herbal formula. Ultra performance liquid chromatography-mass spectrometer was utilized to assess the quality control of MECHs formula. The excretion of magnetic iron oxide nanospheres of the MECHs formula was estimated in zebrafish. The MECH formula against PM-induced lung injury was investigated with mice experiments.
RESULTS:
Five selected herbs were rationally designed to form a new MECH formula, including Citri Exocarpium Rubrum (Juhong), Lablab Semen Album (Baibiandou), Atractylodis Macrocephalae Rhizoma (Baizhu), Mori Folium (Sangye) and Polygonati Odorati Rhizoma (Yuzhu). The formula effectively promoted the magnetic iron oxide nanospheres excretion in zebrafish. The mid/high dose formula significantly prevented PM-induced lung damage in mice by enhancing the activity of SOD and GSH-Px, reducing the MDA and ROS level and attenuating the upregulation of pro-inflammatory cytokine (IL-6, IL-8, IL-1β and TNF-α), down regulating the protein expression of NF-κB, STAT3 and Caspase-3.
CONCLUSION
Our findings suggest that the effective MECHs formula will become a novel strategy for preventing PM-induced lung injury and provide a paradigm for the development of functional foods using MECHs.
7.Establishment of risk prediction nomograph model for sepsis related acute respiratory distress syndrome.

OBJECTIVE:
To explore the risk factors of acute respiratory distress syndrome (ARDS) in patients with sepsis and to construct a risk nomogram model.
METHODS:
The clinical data of 234 sepsis patients admitted to the intensive care unit (ICU) of Tianjin Hospital from January 2019 to May 2022 were retrospectively analyzed. The patients were divided into non-ARDS group (156 cases) and ARDS group (78 cases) according to the presence or absence of ARDS. The gender, age, hypertension, diabetes, coronary heart disease, smoking history, history of alcoholism, temperature, respiratory rate (RR), mean arterial pressure (MAP), pulmonary infection, white blood cell count (WBC), hemoglobin (Hb), platelet count (PLT), prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB), D-dimer, oxygenation index (PaO2/FiO2), lactic acid (Lac), procalcitonin (PCT), brain natriuretic peptide (BNP), albumin (ALB), blood urea nitrogen (BUN), serum creatinine (SCr), acute physiology and chronic health evaluation II (APACHE II), sequential organ failure assessment (SOFA) were compared between the two groups. Univariate and multivariate Logistic regression were used to analyze the risk factors of sepsis related ARDS. Based on the screened independent risk factors, a nomogram prediction model was constructed, and Bootstrap method was used for internal verification. The receiver operator characteristic curve (ROC curve) was drawn, and the area under the ROC curve (AUC) was calculated to verify the prediction and accuracy of the model.
RESULTS:
There were no significant differences in gender, age, hypertension, diabetes, coronary heart disease, smoking history, alcoholism history, temperature, WBC, Hb, PLT, PT, APTT, FIB, PCT, BNP and SCr between the two groups. There were significant differences in RR, MAP, pulmonary infection, D-dimer, PaO2/FiO2, Lac, ALB, BUN, APACHE II score and SOFA score (all P < 0.05). Multivariate Logistic regression analysis showed that increased RR, low MAP, pulmonary infection, high Lac and high APACHE II score were independent risk factors for sepsis related ARDS [RR: odds ratio (OR) = 1.167, 95% confidence interval (95%CI) was 1.019-1.336; MAP: OR = 0.962, 95%CI was 0.932-0.994; pulmonary infection: OR = 0.428, 95%CI was 0.189-0.966; Lac: OR = 1.684, 95%CI was 1.036-2.735; APACHE II score: OR = 1.577, 95%CI was 1.202-2.067; all P < 0.05]. Based on the above independent risk factors, a risk nomograph model was established to predict sepsis related ARDS (accuracy was 81.62%, sensitivity was 66.67%, specificity was 89.10%). The predicted values were basically consistent with the measured values, and the AUC was 0.866 (95%CI was 0.819-0.914).
CONCLUSIONS
Increased RR, low MAP, pulmonary infection, high Lac and high APACHE II score are independent risk factors for sepsis related ARDS. Establishment of a risk nomograph model based on these factors may guide to predict the risk of ARDS in sepsis patients.

8.Serum immunoglobulin concentration and the risk of type 2 diabetes mellitus in adults in Tianjin City: a prospective cohort study.

Objective: To evaluate the association between immunoglobulin concentration and the risk of type 2 diabetes mellitus (T2DM) in adults in Tianjin City. Methods: Based on the Tianjin Chronic Low-grade Systemic Inflammation and Health (TCLSIHealth) cohort from January 2010 to December 2018, subjects who had completed the measurement of baseline immunoglobulin concentration and blood glucose concentration and not been diagnosed with any type of diabetes at baseline were selected in this study. The collected data included the concentration of serum immunoglobulin (IgG, IgM, IgA and IgE), fasting blood glucose and other potential confounders. The subjects were divided into four groups from Q1 to Q4 according to the quartiles of baseline immunoglobulin concentration. The multivariable Cox regression model was used to assess the association between the baseline immunoglobulin concentration and T2DM. Results: A total of 6 315 subjects aged (50.1±10.0) years were included. About 390 subjects were newly diagnosed with T2DM during the follow-up period. The incidence rate was 16.8/1 000 person-years. After adjusting for age, sex, waist circumference, smoking status, drinking status, eosinophil ratio, metabolic syndrome, first-or second-degree family history, and reciprocal adjusting for other immunoglobulin concentrations, compared to the lowest quartile concentration group Q1, subjects in group Q4 with the highest quartile of IgG concentration showed a lower risk of T2DM (HR=0.71, 95%CI: 0.52-0.97), and subjects in group Q4 with the highest quartile of IgM concentration also had a decreased risk of T2DM (HR=0.66, 95%CI: 0.47-0.91). Subjects in group Q4 with the highest quartile of IgA concentration had an increased risk of T2DM (HR=1.56, 95%CI: 1.18-2.07). The risk of T2DM decreased with the increase of serum IgG and IgM concentrations (Ptrend=0.018, Ptrend=0.010) and increased with the increase of serum IgA concentrations (Ptrend<0.001). No association was found between the concentration of IgE and T2DM risk (HR=0.99, 95%CI: 0.74-1.31, Ptrend=0.891). Conclusion: The concentration of IgG and IgM is negatively associated with the risk of T2DM, and the concentration of IgA is positively associated with the risk of T2DM in Tianjin City. The concentrations of IgG, IgM and IgA could be a predictor of hyperglycemia and T2DM.

9.Clinical efficacy of humidified high flow versus conventional nasal cannula oxygen inhalation on hypoxemia after complex ventral hernia surgery in elderly patients

Objective:To investigate the clinical efficacy of humidified high flow nasal cannula oxygen inhalation (HFNC) versus conventional nasal cannula oxygen inhalation on hypoxemia after complex ventral hernia surgery in elderly patients.Methods:The retrospective cohort study was conducted. The clinical data of 80 elderly patients with hypoxemia after complex ventral hernia surgery who were admitted to Huadong Hospital Affiliated to Fudan University from January 2021 to June 2022 were collected. There were 44 males and 36 females, aged (74±7)years. Of the 80 patients, 40 cases undergoing HFNC were allocated into HFNC group, and 40 cases undergoing conventional nasal cannula oxygen inhalation were allocated into conventional group, respectively. Observation indicators: (1) postoperative blood gas analysis; (2) postoperative complications. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was conducted using the independent sample t test. Measurement data with skewed distribution were represented as M( Q1, Q3), and comparison between groups was conducted using the rank sum test. Count data were represented as absolute numbers or percentages, and comparison between groups was conducted using the chi-square test or Fisher exact probability. Repeated measurement data were analyzed using the repeated ANOVA. Results:(1) Postoperative blood gas analysis. Before surgery and at 12 hours, 24 hours, 3 days after surgery, the partial pressure of blood oxygen was (13.5±2.3)kPa, (13.4±3.2)kPa, (13.8±2.3)kPa, (13.7±2.0)kPa for the HFNC group, and (12.7±2.1)kPa, (12.9±2.4)kPa, (12.3±2.5)kPa, (13.9±2.1) kPa for the conventional group. The partial pressure of carbon dioxide was (5.6±0.7)kPa, (5.0±0.6)kPa, (4.7±0.6)kPa, (4.9±0.6)kPa for the HFNC group, and (5.6±0.6)kPa, (4.4±0.8)kPa, (5.0±4.8)kPa, (5.1±1.1)kPa for the conventional group. The saturation of blood oxygen was 97.8%±2.2%, 98.1%±2.1%, 98.9%±1.8%, 99.2%±2.0% for the HFNC group, and 97.8%±3.1%, 97.8%±2.1%, 99.0%±1.5%, 98.8%±2.0% for the conventional group. The oxygenation index was 259±28, 300±45, 352±46, 353±57 for the HFNC group, and 262±29, 297±54, 304±63, 345±53 for the conventional group, respectively. There was a significant difference in the interven-tion effect of partial pressure of blood oxygen between the two groups ( Fgroup=4.09, P<0.05) and no significant difference in the time effect or interaction effect ( Ftime=2.37, Finteraction=1.71, P>0.05). There were significant differences in the time effect and interaction effect of partial pressure of carbon dioxide between the two groups ( Ftime=7.23, Finteraction=13.21, P<0.05) and no significant difference in the intervention effect ( Fgroup=1.02, P>0.05). There was a significant difference in the time effect of saturation of blood oxygen between the two groups ( Ftime=5.54, P<0.05) and no significant difference in the intervention effect or interaction effect ( Fgroup=1.78, Finteraction=0.46, P>0.05). There were signifi-cant differences in the intervention effect, time effect, interaction effect of oxygenation index between the two groups ( Fgroup=8.21, Ftime=42.07, Finteraction=3.49, P<0.05). (2) Postoperative complications. The time in intensive care unit and cases with pulmonary infection were 3(3,3)days and 3 for the HFNC group, versus 6(5,7)days and 10 for the conventional group, showing significant differences between the two groups ( Z=27.50, χ2=4.50, P<0.05). Cases with atelectasis and endotrachead intubation were 0 and 1 for the HFNC group, versus 4 and 7 for the conventional group, showing no significant difference between the two groups ( P>0.05). There was no death in either group. Conclusion:Humidified high flow oxygen inhalation has certain advantages over conventional nasal cannula oxygen inhalation in improving partial pressure of blood oxygen and oxygenation index after complex ventral hernia surgery in elderly patients.
