1.Effect of Bortezomib combined with chemotherapeutic medicine on hepatic and colonic cancer cell lines

Objective To explore the influence of arsenous acid on hepatic and colonic cancer cell lines BEL-7402 and HT-29 .The effect of bortezomib combined with chemotherapeutic medicine and the optimal combination plan of them were also investigated .Methods We detected cell proliferation by MTT to get the inhi-bition rate of two cell lines and further to determine IC 50 ( inhibitory concentration50%) of 5-Fu,oxaliplatin or arsenous acid;preincubated cancer cells for 2 h、4 h or 8 h by bortezomib .Chemotheraputic medicine were com-bined respectively .The cell proliferation and apoptosis were analysed by MTT ,TUNEL and Annexin V -PI.Re-sult s The 24 h inhibition rate of arsenous acid to BEL -7402 and HT-29 was:0.59 ±0.09、0.71 ±0.12 re-spectively;the IC50 of 5 -Fu,Oxaliplatin or arsenous acid to BEL -7402 was:5.33 ±0.07 mg/L,28.73 ± 0.72 mg/L,25.93 ±4.05 mg/L, while to HT -29 was:7.33 ±1.13 mg/L、53.94 ±1.23 mg/L,21.93 ± 2.05 mg/L.Both inhibition rate and apoptosis rate were enhanced when chemotheraputic medicine was combined with bortezomib .Co nclusion Arsenous acid can inhibit the growth of cancer cells obviously;the susceptibility of each chemotherapy medicine can be strengthened significantly when it is combined with bortezomib ,P<0.05.
2.Cleaning and Disinfection for Digestive Endoscope:A Discussion about Continuing Improvement of Surveillance Management

OBJECTIVE To approach the method for continuing improvement of quality control in cleaning and disinfection.METHODS Looked for the problems in the process of cleaning and disinfection of endoscope,and the reasons wereanalyzed.The specific measures for continuing improvement of quality control in cleaning and disinfection of endoscope were formulated and fulfilled.The examining norms of nursing quality were modified,and the surveillance management of the quality control and the cleaning and disinfection of endoscope were enhanced.RESULTS For continuing improvement of quality control in cleaning and disinfection of endoscope,the total quality of nursing service must be enhanced.CONCLUSIONS For one year practice,we found that continuing improvement of quality control in cleaning and disinfection fo endoscope could enhance total quality of nursing care,and ensure medical treatment safety while decreasing onset of hospital infection.

4.Evaluation of MRI enhancement in different tissues of prostate using pharmacokinetic model

Objective To study the effect of MRI on the enhancement of normal prostate, benign prostatic hyperplasia and prostate cancer by pharmacokinetic model. Methods A total of 80 patients with prostate cancer diagnosed by pathological examination in Lishui Central Hospital from February 2015 to August 2016 were selected as the subjects in this study. 1.5T superconducting magnetic resonance imaging was used, the serial needle was FMPSPGR for enhanced scan of the prostate of the patient, interlayer was 2 mm and the thickness was 7 mm, continuous scanning was performed 35 times, after the elbow vein indwelling catheter, the Gd-DTPA was injected rapidly at the beginning of the scan according to the 0.1 mmol/kg dose. By scanning data, inspection data transfer and workstation for patients with scanning results were analyzed using two room pharmacokinetic model calculation of ROI, the patients of normal prostate peripheral zone, benign prostatic hyperplasia and prostate cancer index, maximum enhancement of contrast agent distribution index, exchange rate, contrast absorption amplitude. Results After a series of calculation and analysis, the maximum enhancement index of prostate cancer, the distribution index of contrast agent, the contrast agent exchange rate and the contrast agent absorption index of prostate cancer were the highest. The maximum enhancement index, contrast agent distribution index, contrast agent exchange rate and contrast agent absorption index were significantly lower than those of prostate cancer, The difference was statistically significant (P<0.05). The indexes of normal prostate were significantly lower than those of benign prostatic hyperplasia (BPH) and prostate cancer (P<0.05). Conclusion The application of drug evaluation pharmacokinetic model of prostate MRI in different tissues to enhancing effect on patients with prostate tumor and non tumor tissue blood supply characteristics more accurately describe and differentiate, to improve the accuracy of identification and diagnosis of prostate in clinical treatment in a certain extent, with further clinical promotion and application significance.
5.Ginsenoside-Rg1 protects human dermal fibroblasts against 8-methoxypsoralen plus ultraviolet A-induced photoaging through p53-dependent signaling pathways

Aim To investigate the roles of p53-dependent signaling pathways in the process of ginsenoside Rg1 protection against 8-methoxypsoralen(8-MOP) and subsequent ultraviolet A(UVA) irradiation induced photoaging model in human dermal fibroblasts(HDFs).Methods Photoaging model was established by 8-MOP/UVA in skin HDFs.Flow cytometry, enzyme cytochemistry, immunofluorescence and Western blot were employed.Results Pretreatment with ginsenoside Rg1 could significantly reduce the amount of UVA-generated 8-oxo-dG and relieve the photoaging representation.Compared with 8-MOP/UVA treatment group, pretreatment with Ginsenoside Rg1 could decrease the expression of SA β-galatosides(SA-β-Gal), and down-regulate the level of senescence associated proteins(p16 and p21).Conclusions Ginsenoside Rg1 has prominent dose-dependent antagonism on senescence of skin HDFs induced by 8-MOP/UVA, and its mechanism may be due to its antioxidation which reduces the production of photoproducts to protect telomere against abnormal shortening.
6.Influence of N-acetylcysteine on expressions of nuclear factor-kappa B in local cerebral ischemia-reperfusion

BACKGROUND: Nuclear factor-kappa B (NF-κB) is an important transcription factor, which can promote the transcription of many target genes after activated.OBJECTIVE: To investigate the expressions of NF-κB in local cerebral ischemia-reperfusion and the influence of the pretreatment of the N-acetylcysteine.DESIGN: Randomized grouping experiment with animals as subjects.SETTING: Department of Neurology, the Second Clinical Medical College, Harbin Medical University.MATERIALS: The experiment was finished in the Animal Experimental Center and Laboratory of Pathology of Harbin Medical University. Ninetynine male healthy Wister rats were randomly divided into 3 groups: Sham-operated group(n=l 1), saline control group(n=44), N-acetylcysteine group(n=44).METHODS: Rat models of cerebral ischemia were made with the method of thread blocking improved by Longa et al in rats of the three groups. A nylon line with a smooth spherical captular end of 0.26 mm in diameter made by heating was inserted through the cut of crotch of the common carotid artery. The prepared line for common carotid artery was tied tightly and the arteriole clamp of internal carotid artery was unclamped. The nylon line entered the common carotid artery and the inserted length of the saline control group and the N-acetylcysteine group from the crotch of the internal and external carotid artery was calculated about (18.5±0.5)mm in order to obstruct the blood supply of the middle cerebral artery. The inserted depth in sham-operated group was less than 15 mm and the blood supply of the middle cerebral artery was kept normal. Intraperitoneal injection of N-acetylcysteine was given with 150 mg/kg at 30 minutes before ischemia in N-acetylcysteine group and injection of normal saline was given with equal volume at 30 minutes before ischemia in saline control group.Eleven rats each time in saline control group and N-acetylcysteine group were killed by cutting off heads at the time points of ischemia 6, 24 hours,and reperfusion 1 hour after ischemia 6, 24 hours. The express of NF-κB of brain tissue was observed with immunchistochemical method. Percentage of cerebral infarction of rats in each group was determined by dyeing of tetrachloro red tetrazoline. Apoptosis of brain tissue cells was detected with terminal deoxynucleotide transferase-mediated dUTP nick-end labeling (TUNEL).MAIN OUTCOME MESURES: Percentage of cerebral infart volume of rats in each group, the activity of combination of the NF-κB and apoptosis of cells.RESULTS: Ninety-nine animals attended the experiment, all of them entered the final analysis. ① Percentages of infarct volume at 1 hour of ischemia and 6, 24 hours of reperfusion in N-acetylcysteine group were (8.39±2.54)%, (24.54±6.02)% respectively, and that of corresponding saline control group were (15.50±4.18)%,(32.22±3.99)%. The focus of infartion with ischemia for 24 hours in each group was increased as compared with that for 6 hours and the infarct volume in group with N-acetylcysteine was obviously decreased as compared with that in saline control group (P < 0.01). ② NF-κB p56 transfered from the kytoplasm to the nucelus after the ischemia and reperfusion. The rates of p56 masculine cells in N-acetylcysteine group of ischemia for 6 and 24 hours were (0.462±0.022)%, (0.452±0.015)% respectively, the express of which was decreased as compared with that in saline control group [(0.563±0.028 )%,(0.554±0.013)%] (P < 0.01 ). ③ Cells of apoptosis pretreated with N-acetylcysteine were obviously decreased as compared with that pretreated with normal saline.CONCLUSION: Focal cerebral ischemia and reperfusion can activate NF-κB p65, which participate in the damage of cerebral ischemia and reperfusion. NF-κB can inhibit the express of p65, and relieve the nerve injury and so have the effct of protection for brain.
7.Study of Bilingual teaching in Neurology for Seven-year Medical Master

We investigated the students of a seven-year medical master in the bilingual teaching program in Neurology with a questionnaire.We found that the bilingual teaching program has the following problems:the students don't have a solid foundation of medical English terminology and the content of the English teaching was higher than the ability of the students.So we think that we need to impress upon the students the importance of the program.We can also raise the overall quality of this program through an improvement of teaching methods,regulation of the proportion of English content in all courses and suitable midterm and final examinations.These alterations will effectively improve the bilingual teaching program.
9.The safety of endoscopic ligation of gastric and colic full-thickness wall with detachable nylon loops

Objective To evaluate the safety of endoscopic ligation of gastric and colic full-thickness wall with nylon loops.Methods Endoscopic ligation of full gastric and colic wall with detachable nylon loops, assisted by laparotomy, were performed in 4 dogs.Ligation healing was assessed on 3, 7, 10and 17 days after the procedure.Results All animals recovered after the procedure, without any clinical signs of infection or hemorrhage.Postoperative examination did not reveal gastric or colonic perforation, or local abscess.Loop ligature showed ulcer-like lesion.Histopathology revealed muscularis propria disruption, granulation tissue bridging the site of ligation and formation of scar tissue.Regenerative mucosa coyered the surface of the ligature completely 17 days after the operation.Conclusion Endoscopic ligation of gastric and colic full-thickness wall with detachable nylon loops is safe.Its application can be extended to other fields.
10.Clinical application of modified peroral endoscopic myotomy to treatment of achalasia(with video)

Objective To compare the difference between modified peroral endoscopic myotomy (Liu?POEM) and conventional POEM for achalasia. Methods Thirty achalasia patients treated with Liu?POEM and 30 with conventional POEM were enrolled. A retrospective study was performed to compare the conventional POEM and Liu?POEM procedures by evaluating total operation time, postoperative complications and symptoms( Eckardt score) . Results The average total operation time of Liu?POEM was 27?13 ±11?42 min and the average myotomy time was 13?20±5?09 min. There was no pneumomediastinum, subcutaneous emphysema or fever. The average total operation time of conventional POEM was 51?22 ± 25?63 min. The average myotomy time was 11?18±7?61 min. There were three cases(10%) of subcutaneous emphysema but recovered after two days without any special treatment. One patient who underwent conventional POEM had fever( the highest temperature was 37?6℃) and his temperature subsided to normal after physical cooling in one day. Postoperative Eckardt scores of patients were all less than 3. After postoperative follow?up of 3 to 12 months, no complications occurred in any patient. Conclusion Liu?POEM is a modified approach to treat achalasia, advantageous over conventional POEM in more simplified operation procedure, shorter operation time and less invasiveness.
