1.Histologic changes of arteries after hydraulic dilation

Objective To study the histologic changes of arteries dilation under different hydraulic pressure. Methods Right carotid arteries were dilated by using hydraulic method with different pressures (40, 80, 120 kPa) normal saline. The endothelial cells, internal elastic membrane (IEM), smooth muscle (SM) and adventitia of the arteries were studied under microscope. The arterial calibre, thickness of tunica intima and smooth muscle were measured under TJTY-300 automatic medical photograph analyser. Results 1) The calibre of the dilated arteries increased. 2) When the pressure was 40 kPa, there were mild injuries of endothelial cells, internal elastic membrane and smooth muscle; all the injuries recovered to normal 1 week later. 3) The degree of injury of the individual layers of the arteries gradually aggravated as the pressure raised up to over 80 kPa. The repair also required long periods of time and was found imperfect. The proliferation of fibres of connective tissues took place one week later. Conclusion 1) Blood flow could be increased after hydraulic dilation of the artery, and the strength of washing away the small thrombosis over the site of anastomosis could be increased. 2) For safety, the hydraulic pressure of arterial dilation should be better kept within 40 kPa.
2.Theoretical study and clinical practice of platelet-rich plasma in improving bone repair

OBJECTIVE: To review the development and mechanism of platelet-rich plasma (PRP) and the unsolved problems so as to provide reference for the clinical application of PRP.DATA SOURCES: Articles on effects of platelet-rich plasma on bone repair are searched from Medline between January 1995 and June 2005 on computer. The key words were platelet rich plasma, bone, and repair.Meanwhile, the same search was conducted to determine the correlated articles during January 1998 to June 2005 from Wanfang database with key words of platelet-rich plasma, bone and repair in Chinese.STUDY SELECTION: Literatures at home and abroad on the PRP and bone repair were chosen; Non-randomized controlled literatures were included.DATA EXTRACTION: Totally 40 out of 49 reports related to PRP and bone repair met the criteria. 9 reports were excluded due to the repeated same research. The rest 40 reports were sorted out and conducted literature review.DATA SYNTHESIS: Platelet-rich plasma was originally used in clinic to repair mandibular defect in 1998, by adding PRP to grafts with a radiographic maturation rate 1.62 to 2.16 times that of grafts without PRP. Up to now, PRP has been used in many medical areas to accelerate tissue healing due to its advantages of safety, simple, low-cost. But some problems still remain to be studied and solved.CONCLUSION: PRP includes many sorts of growth factors and has been proved to be beneficial to the maturation of both bone tissue and soft tissues. PRP is autologous and can be produced easily and safely from autologous blood, without the concerns of transmissions and immunological rejection of various diseases.
3.Hospital Infection of Inpatients with Indwelling Catheter:Objective Monitoring and Analysis

OBJECTIVE To investigate the incidence of hospital infection in patients with indwelling catheter and its risk factors, the inpatients were monitored and the measures were taken to prevent the patients from infection. METHODS Monitoring 617 patients with indwelling catheter, retrospective survey and analysis study were available , including a plan to detect microbe, to study the medical history and to examine the patients. RESULTS Eighty nine patients with 106 cases-times of urinary tract infection happened, and its incidence rate was 14.42 percent with the highest rate in all the hospital infection cases. The main risk factors of urinary tract infection included inserting catheter, unclean operation, delayed removing of catheter and unreasonable application of antibiotic . CONCLUSIONS Controlling the indication, shortening the time of indwelling catheter, keeping urinary aseptic manipulation and rational application of antibiotic are effective measurement for the patients with indwelling catheter.
4.Treatment of hypertrophic nonunion by using Ilizarov circular fixator

Objective To evalute the effect of Ilizarov technique in the treatment of hypertrophic nonunion.Methods Form June 2008 to December 2010,12 patients with hypertrophic nonunion were treated with Ilizarov technique,including 10 males and 2 females with an average age of 46.5 years.The pathology sites of nonunion were kept as closed as possible without any bone graft during operation.As to patients who had ever been treated with plate or intramedullary nail,the hardware should be removed by minimal invasive approach.These procedures aimed to keep the vascularity of nonunion site intact.Ilizarov apparatus were preoperatively constructed.Distal segment and proximal segment of nonunion were mounted respectively with two external circle using the smooth wires and half pins.The two-circle stabilizing one segment was nominated with transosseous modules.Distal module and proximal one was connected with a pair of axial hinges.The pathology sites were gradually distracted from the seventh day postoperatively,0.25 mm/d.Accompanying with deformity correction,limb length discrepancy (LLD) also were restored simultaneously.Then,all the screws and nuts in the apparatus should be tightened,which was favourable to the callus consolidation.Results All 12 cases of nonunion healed without any bone graft.The fixator wearing time lasted 6-12 months,with an average of 8 months.Correction of deformity and LLD were achieved.The average lengthening was 3.0 cm (range,2.0-5.5 cm),the average correction angle was 23° (range,10°-30°).After 6-18 months follow-up,all the patients restored satisfactory function.Conclusion Hypertrophic nonunion can be treated successfully with Ilizarov technique.The key of successful callus distraction is strictly identifying the indications.
5.Combination therapy with miR34a and doxorubicin synergistically inhibits Dox-resistant breast cancer progression

Resistance to breast cancer (BCa) chemotherapy severely hampers the patient's prognosis. MicroRNAs provide a potential therapeutic prospect for BCa. In this study, the reversal function of microRNA34a (miR34a) on doxorubicin (Dox) resistance of BCa and the possible mechanism was investigated. We found that the relative level of miR34a was significantly decreased in Dox-resistant breast cancer cell MCF-7 (MCF-7/A) compared with Dox-sensitive MCF-7 cells. Transfection with miR34a significantly suppressed the invasion, migration, adhesion of MCF-7/A cells without inhibiting their growth obviously. The combination of miR34a and Dox could significantly inhibit the proliferation, migration, invasion and induce the apoptosis of MCF-7/A cells. The synergistic effect of this combination on resistant MCF-7/A cells has no obvious relation with the expressions of classical drug-resistant proteins P-GP, MRP and GST-
