Objective To investigate the effect of the inhalation of nitrous oxide premixed with oxygen (50%∶50%) for analgesia labor on maternal and fetus. Methods A total of 100 cases of pregnant women were provided with nitrous oxide premixed with oxygen (50%∶50%) (control group); Another 100 cases were provided only with oxygen (comparison group). Recording duration of the Labor, way of delivery, bleeding volume, Apgars score, blood gas analysis to maternal radius artery and fetal umbilical blood among all patients. Results The effect for analgesia labor of the premixed gas was much better than that of control group, but there were no significant differences in time of labor, bleeding volume, Apgars score between the two groups. Conclusions The inhalation of nitrous oxide premixed with oxygen (50%∶50%) for analgesia labor benefits pregnant women because of keeping them being a good mental and physical condition. The inhalation of nitrous oxide for analgesia labor is a safe, effective and easy method.

[Objective] To probe into the psychological health status of middle-aged intellectuals and its influencing factors. [Methods]A cluster sampling of 1 474 middle-aged intellectuals was measured by the SCL-90 table and was analyzed by the Reliability Analysis, Principal Component Analysis and Analysis of Variance. [ Results ] The measurement table is highly reliable (Cronbach a = 0.949) and the Principle Component Analysis has main factors (A -1.97) and cumulative contribution rate is 80% . [Conclusion]Marital status, income and gender are the main influencing factors of psychological healthy status in middle-aged intellectuals.

Objective To study the expression and its significance of Platelet-derived growth factor(PDGF) and Collagen type Ⅰ (Col- Ⅰ) mRNA in hepatic tissue in mice exposed to arsenic in drinking water. Methods Fifty mice were divided into control group, sodium arsenite group(iAs3+ group, 300 mg/L) and sodium arsenate group (iAs5+ group, 300mg/L) at random. The mice were sacrificed after 10 months for liver function test (ALT, AST and GLB examined by automatic biochemical analyzer) and pathologic examinations. Masson staining and immunohistochemical SABC methods were used. Through the result of Masson staining fibrosis semi-quantitative analysis of the fibrosis number was carried out in the per unit area. Using immuonhistochemical SABC methods, the expression of PDGF and Col- Ⅰ was detected. Results ①Serum ALT was statistically significant among control group [(36.67±3.51) U/L], iAs3+ group[(61.46±13.85) U/L] and iAs5+ group[(43.31±4.21) U/L, F= 6.56, P < 0.05], especially between the control and iAs3+ group (P < 0.05) ;Serum AST had a statistical significance among control group [(135.00 ± 20.42)U/L], iA3+ group [(510.86 ± 59.01)U/L] and iAs5+ group[(258.93 ± 22.40) U/L, F= 83.36, P < 0.05] ;Serum GLB had no statistical significance among control group [(20.86 ± 0.61)g/L], iAs3+ group [(26.94± 3.73)g/L] and iAs5+ group[(24.59 ± 5.27)g/L, F = 2.80, P < 0.05]. ②Masson staining revealed notable liver cell necrosis, regeneration, infiltration of inflammatory cells in portal area. Masson staining Semi-quantitative results showed significantly increasing size of fibrosis in model group. Compared with control group(0.069 ± 0.013) and iAs5+ group(0.143 ± 0.122), fiber hyperplasia of iAs3+ group(0.192 ± 0.108) had statistical significance (F = 1.91, P < 0.05). ③Immunohistochemistry results showed higher expression of PDGF in the portal area, sepetal and preisinus cells in model group. The heavy expression of Col- Ⅰ distributed in the surrounding of blood vessels and bile duet and along portal wall extensting to the liver in model group. Semi-quantitative results showed that PDGF among control group(202.79 ± 7.46), iAs3+ group(174.38 ± 7.71) and iAs5+ group(177.64 ± 7.81) had a statistical significance(F= 102.91, P < 0.05);Col-Ⅰ had a statistical significance among control group(200.11 ± 7.46), iAs3+ group (176.47±10.20) and iAs5+ group (177.378 ±7.948, F = 78.51, P < 0.05). Conclusions Exposed in oral drinking arsenic, mice can develop hepatic injury and hepatic fibrosis. The PDGF and Col-Ⅰ play significant effects on the process of hepatic fibrosis caused by oral drinking arsenic exposed mice.

Objective To establish the quantitative detection of capsule associated protein 10 (CAP10)and virulence-associated DEAD-box RNA helicase 1(VAD1)genes in Cryptococcus neoformans (CN) and compare the diagnostic values of single gene test and combined gene test in CN meningitis.MethodsTwenty-three CN meningitis patients with fungal culture or ink staining or CN antigen detection positive in cerebrospinal fluid (CSF) were recruited and patients with craniocerebral trauma were recruited as controls.Standard plasmids were constructed using standard CN strain.Real time fluorescent quantitative polymerase chain reaction (RT-FQ-PCR) was established to detect the mRNA expressions of CAP10 and VAD1 genes in the CSF of patients with CN meningitis,which were compared with the results of CSF ink staining,fungal culture and antigen detection.The diagnostic values of single gene test and combined gene test were compared by chi square test.Results Among 23 CN meningitis patients,22 (95.6％) were CAP10 mRNA positive detected by RT-FQ-PCR,which was significantly higher than both ink staining (16/23,69.6％,x2 =4.167,P＜0.05) and fungal culture (15/23,65.2％,x2=5.143,P＜0.05),respectively; but not significant different from antigen detection (21/23,91.3％,x2=0.500,P＞0.05).There were also no statistical significant differences between combined detection of CAP 10 + VAD1 and CAP 10 or VAD1 single gene test (P＞0.05).ConclusionRT-FQ-PCR detection is successfully established using virulence genes as target,which is superior to the conventional methods.

Objective To compare the diagnostic value of sound velocity(SV) and real-time ultrasonic elastography in differentiating benign or malignant of breast lesions.Methods 75 patients with 99 lesions were examined with the zone speed index technique and real-time ultrasonic elastography respectively.Then the SVs of the lesions were calculated and elastography of the lesions were scored with 5-scoring method.The ROC curves were constructed with histology as the golden standard.Results There was significant difference between the SVs of benign and malignant breast lesions(P =0.0001).1561 m/s was the best cufoff point.And the areas under the curves (AUC) of SV and real-time ultrasonic elastography were 0.842 and 0.968,respectively.There was significant difference between the two methods of AUC (P =0.023).The sensitivity,specificity and accuracy for SV were 81.5％,91.7％,88.9％,respectively.There was no significant difference between sensitivity,specificity and accuracy of the two methods(P =1.0,P =0.125,P =0.146).Conclusions Both SV method and real-time ultrasonic elastography are helpful in the evaluation of benign or malignant of breast lesions.

Objective To investigate the prevalence rate and related factors of excessive daytime sleepiness (EDS) in the elderly. Methods Epworth Sleep Scales (ESS) was used to make a randomized questionnaire among the retired elderly. Results 1 000 questionnaires were released and totally 768 qualified answers were returned. The mean score of ESS was 4.22?0.10 and the score ≥8 was used as an abmormal value. The prevlence rate of EDS in the elderly group was 10.9%. Regression analysis was done based on ESS score as dependent variable, and age, sex, duration of sleep, difficulty in falling asleep, early wakeup, daytime sleepiness, habitual snoring and loud unstable snoring as independent variables. Objective To study the effects of Compound Di Gui Capsule (CDGC) on the levels of glucose and lipid in diabetic rats.Methods Rats models with diabetes mellitus were induced by intraperitoneal injection of streptozotocin (50mg?kg-1).Then the diabetic rats were divided into different groups at random.CDGC groups were given CDGC in the dosages of 2.4,1.2,0.6 g?kg-1?d-1 respectively by gastric gavage for 90 d.Levels of whole blood glucose,glycosylated hemoglobin,total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C),nitric oxide (NO),total antioxidation capacity (T-AOC) were determined.The insulin level and the pathological changes of pancreatic tissues were also observed.Results The glucose level of diabetic rat was decreased 2h after administration.After administration of CDGC for 90d,the glycosylated hemoglobin,TG,LDL-C and NO were markedly decreased,islet ?cells were protected and restored,and the insulin level and T-AOC were increased.Conclusion CDGC can regulate the metabolism of glucose and lipid in diabetic rats,and can protect and restore the pancreatic islet cells.

Objective To evaluate anatomic hepatectomy (AH) vs lithotomy of liver parenchyma (LLP) for regional hepatolithiasis.Methods From January 2012 to June 2015,fifty-nine cases underwent LLP and 47 received AH.Clinical end-points included residual stones,infection of the raw surface of liver remnant,time to restoration of liver function,bile leakage,recurrent stones,morbidity and mortality.Results The time of liver function restoration was not statistically different between the two groups (2.96 d vs.2.82 d,P ＞0.05).Patients in AH group suffered from less residual stones (2 cases vs.13 cases,P ＜0.01),lower infection rate of the raw surface of liver remnant (1 case vs.21 cases,P ＜ 0.01),and less bile leakage rate (0 case vs.7 cases,P ＜ 0.05).With a median follow-up of 31 months (range 3-48 mon).AH group suffered from less recurrent hepatolithiasis (0 case vs.12 cases,P ＜ 0.01).There was no mortality in both groups.Conclusions Anatomic hepatectomy is a safe and effective treatment for hepatolithiasis,with a high stone clearance rate and low surgical complications.

Objective To analyze the value of color Doppler ultrasound in the diagnosis of lower extremity arterial disease in patients with type 2 diabetes mellitus.Methods A total of 800 patients with type 2 diabetes mellitus underwent color Doppler ultrasonography to examine anterior tibial artery (ATA),dorsalis pedis artery (DPA) and posterior tibial artery (PTA).Ultrasonic findings including vascular diameter,stenosis ratio and hemodynamics of lower extremity arterial disease were analyzed retrospectively.Results ATA and DPA had more plaques and stenosis than PTA.There was no statistical difference of vascular diameter,stenosis ratio and hemodynamics between left and right lower extremity artery in patients with type 2 diabetes mellitus including diabetic foot.Conclusion Color Doppler ultrasound is a useful method in the diagnosis of lower extremity arterial disease in patients with type 2 diabetes mellitus,providing information of stenosis ratio and hemodynamics of lower extremity artery,so as contributing to the clinical therapy of this disease.

Objective To evaluate the value of non invasive examinations in the diagnosis of vascular disease of lower extremity. Methods Segmental systemic pressure (SEG), pulse volume recorder (PVR) and continuous wave (CW) were adopted to examination of 36 patients with arterosclerosis obliterans of lower limbs, and the foundings were compared with that of MRA and operation . Results In all 36 cases, non invasive examinations detected abdominal aorta and common iliac artery with femoral artery occlusion in 12 cases ,femoropoplitea artery occlusion in 28 cases, popliteal artery and the distal artery occlusion in 16 case; MRA together with operation confirmed abdominal aorta and common iliac artery with femoral artery occlusion in 11 cases, femoropopliteal artery occlusion in 32 cases and popliteal artery and the distal artery occlusion in 13 cases. The accurent diagnosis rate of non invasive examination was 85.7%. Conclusions Although PVR has not yet been used to replace the arteriography to directly observe the local situation of arterial lesions , it can preliminarily determine the obstruction sites and severe degree of the occluded arteries. PVR has convenient and reliability in the diagnosis,evaluation of the therapeutic effect and the postoperative follow up of blood vessel diseases,it is worthy to be widely applied in the clinical practice.
