Expression of major histocompatibility complex class 1(MHC-1)and thyroid specific transcription factor-1(TTF-1)in thyroid follicular epithelia of Graves'disease and Hashimoto's thyroiditis measured by immunohistochemistry technique were all significantly increased as compared with those in normal controls,suggesting that TTF-1 and MHC-1 seem to contribute to the development of autoimmune thyroid diseases.

Aim The changes of the levels of blood sugar and plasma insulin in obese woman pa-tients with insulin resistance and the effect of insulin intravenous infusion during oper-ation were observed. Methods 10 patients of normal weight(contrast group, group A)and 20 patients of obesity women(ASA grade Ⅰ- Ⅱ) undergoing selective surgerywere investigated. The patients of obesity were randomly divided into two groups(group B and C, respectively). Groups A and B liquids without glucose, and in group Ctransfuse liquids with glucose and insulin(4g glucose add 1U insulin) were transfusedduring operation. The levels of blood sugar and plasma insulin were measured beforeand after anesthesia, during operation(30min) and after operation respectively. ResultsThere was an insulin resistance phenomenon in patients with obesity. The levels ofblood sugar and plasma insulin in patients with obesity were remarkably higher thanthose in patients of normal weight. The levels of blood sugar and plasma insulin wereextremely raised after anesthesia and during operation. The levles of blood sugar wasinhibited as insulin was infused during operation, but no hypoglycemic reaction wasfound. Conclusion There are insulin resistance phenomenon in patients with obesity.Insulin infused during operation can prevent blood sugar rise and minimize glucosemetabolism turbulence.

Objective To set up the standard of normal prostate volume based on MRI measurement.Methods 30 healthy male adults without any urogenital symptoms were included in this study.The subjects were divided into four groups according to their ages.The length,width and height of the prostate and central gland were measured by MRI.The volume,weight and the rate of central gland to prostate proportion were calculated and compared in various age groups.Results For normal prostate:the average width was(3.02?0.47)cm,the length was(4.19?0.52)cm,the height was(3.24?0.34)cm,the volume was(22.01?7.48)cm 3,and the weight was(23.11?7.86)g.There were significant differences in the value of length,the width,the volume and the weight in various age groups(P=0.006,0.001,0.014 respectively);For the central gland:the width was(2.01?0.41)cm,the length was(2.06?0.34)cm,the height was(2.01?0.32)cm,the volume was(4.46?1.74)cm 3,the weight was(4.68?1.82)g,and the ratio of central gland volume to whole prostate volume was 20.4%.The volume and weight of central gland was significantly different in various age groups.There were significant differences between the contrast-enhancement of central gland and peripheral zone(P=0.0001).The central gland was obviously enhanced at vein phase.Conclusion The volume of the prostate and the central gland are positive correlated with the age.With the ages increased,the prostate and the central gland volume are increased.

Objective To study the diagnosis and stage of benign prostatic hyperplasia(BPH)with quantitative MRI measurement.Methods 25 cases of BPH proved by pathology were undergone MRI examinations.The length,width and height of the prostate and central gland were measured on MR images.The volume,weight and the central gland to prostate proportion were calculated and compared in various age groups.Results The width,length,height,volume and weight of prostate in BPH were larger than those in normal prostate.The central gland to prostate proportion in BPH was 46.1%,which was much larger than that of normal prostate(20.4%).The width,length,height,volume and weight of prostate and central gland were significantly different in various hyperplasia degree groups(P

AIMTo search for potential anti-atherosclerosis drugs with vascular relaxation activity, a series of agonists of endothelial targets were designed and synthesized.

METHODSCoupling N-methyl-1,2, 3,6-tetrahydrapyridine ring system with 3,4-dibenzenesulfonyl-1,2,5-oxadiazole-2-oxide through esterification or amidation, a series of arecoline derivatives containing NO donors were designed and synthesised.

RESULTSA novel series of compounds structurally related to arecoline have been prepared, the proposed structures of eighteen new compounds were established by IR, 1H NMR, MS spectroscopy and elemental analysis. The effects of the target compounds on the vasodilation activity were tested in the isolated preparation of mice thoratic aorta.

CONCLUSIONThis preliminary pharmacological tests showed that the candidates have good vasodilation activities and were worthy to be intensively studied.

Objective To evaluate the efficacy,safety and side effects of 0.5% povidone-iodine in intravesical treatment of interstitial cystitis (IC). Methods Thirty-three cases of IC were divided randomly into group A (18 cases) and group B (15 cases). Group A was treated by intravesical instillation with 50 ml of 0.5% povidone-iodine which was kept in the bladder for 2 hours,once a day for 2 weeks. Group B was treated in the same way,but with 1:5000 furacilin instead. The symptoms,results of cystoscope inspect and adverse effects were investigated. Objective To evaluate the image quality of 64-multi detector computed tomography (MDCT)and the clinical accuracy in detecting coronary artery lesions.Methods One hundred and five patients were studied by MDCT.The results were compared with invasive coronary angiography(ICA). Patients were excluded for atrial fibrillation,but not for high heart rate,coronary calcification,or obesity. MDCT was analyzed with regard to image quality and presence of coronary artery lesions.Results The data evaluation of the image quality was based on a total of 1365 segments(13 coronary segments for each patient),of which 1144 segments were considered to have diagnostic image quality,but 221 segments (16.2%)could not be sufficiently evaluated because of severe calcifications(153 segments)and motion artifacts(68 segments).The median calcium score[Agatston score equivalent(ASE)]was 154(range 0—1983).87 of the 105 patients had an ASE of less than 1,000[median 105(range 0—994)],and 18 patients had an ASE greater than 1000[median 1477(range 1115—1983)].For detecting lesions with 50% or greater narrowing(without any exclusion criteria),the overall sensitivity,specificity,positive predictive value,and negative predictive value were 85.7%,97.9%,93.0%,and 95.5%,respectively. When limiting the number of patients to those with a calcium score of less than 1000 ASE,the threshold- corrected sensitivity for lesions with 50% or greater narrowing was 96.0%;specificity,98.9%;positive predictive value,95.3%;and negative predictive value,99.0%.Conclusion Our results indicate high quantitative and qualitative diagnostic accuracy of 64-slice MSCT in comparison to QCA in a broad spectrum of patients.

Objective To evaluate the clinical significance of cytokeratin19(CK19) mRNA in the peripheral blood of patients with non-small cell lung cancer (NSCLC). Methods The expression of CK19 mRNA was detected by reverse transcription polymerase chain reaction (RT-PCR) in the peripheral blood from 98 NSCLC patients (NSCLC group), 20 benign pulmonary lesion patients (benign pulmonary lesion group) and 20 healthy adults(control group). Results The positive rate of CK19 mRNA in peripheral blood of NSCLC group, benign pulmonary lesion group and control group was 77.55 % (76/98), 3.57% (1/28),0 (0/20) respectively,and there was significant difference among three groups ( x2 = 73.680,P ＜ 0.01 ). The positive rate of CK19 mRNA in different pathological types of NSCLC had no significant difference (x2 =0.414, P ＞ 0.05 ), but had significant difference in different lymphy node metastasis status( x2 = 17.523, P ＜0.01 ) and different clinical stage ( x2 =13.453,P ＜ 0.01 ). Conclusions The higher expression of CK19mRNA in peripheral blood of NSCLC may relate to cancer metastasis. Detection of CK19 mRNA has significance in early prediction of its prognosis.

Objective To evaluate the current situation of two-way referral system by qualitative study.Methods The qualitative study was carried out in Department of General Internal Medicine of Peking Union Hospital and 4 clinics under Beijing Gaobeidian Community Health Center from June to December 2009.In-depth interview was conducted on 6 patients,who were referring from community health center to tertiary hospital and 16 general practitioners in the community.The method of thematic framework was used for data analysis.Results Only 1 of the 6 patients,who made the appointment,completed the two-way referral.From the view of the patients,the interrupt of the two-way referral was due to long-time waiting,inconvenient transportation to the hospital,availability of specialists,unsmooth process of referral and unsatisfied doctors.However,the community health service staff thought that difficulties of the two-way referral originated from the system,doctors,patients,and doctor-patient relationship.Sometimes patients distrusted doctors both in hospital and in community; communications between doctors or between doctors and patients were less efficient; patients had multiple choice for their primary care and they may go to any hospital freely bypass community-based referral system; patients had excessive expectation for medicine and doctors; some patients paid less attention to their own health,and so on.For improvement of the two-way referral system,the community health service staff proposed concrete measures in terms of relevant policy,the hospital,community health service,the doctors and patients; they emphasized the importance of upgrading their own communication ability and professional skills for primary care.Conclusions Many factors may influence successful implement of two-way referral,so it is necessary to improve the relevant policy and procedures and to upgrade medical staff in community.
