On recently years,the tuberculosis shows the characteristic of higher incidence、 infection、drug-resistance、and Morbidity。However,the emphasis and difficult is preventing and controling of multidrug resistant tuberculosis.Its nosocomial and societal causes of occuring and prevailing were analyzed and control measures from seven facets were expounded in this article.Those include prevention knowledge widely,strengthen management and supervision of floating population,improve ability of diagnosis and therepy,develop combinasion treatment of traditional chinese medicine and western medicine and other assistant treatment, prevent community and nosocomial infection,perfect the drug-resistant surveillance system,carry out the study of new drug 、measure and treatment to anti-tuberculosis actively.
2."Postauricular ""revolving door"" flap in one stage reconstruction of concha defect"

Objective To study the technique of using the postauricular revolving door island flap in one-stage reconstruction of concha defect and its efficacy.Methods Fifteen cases of concha de fect were repaired by the postauricular revolving door island flap in one stage reconstruction from 2000-2010.The maximum size of defect was 2.7 cm× 3.0 cm,and the minimum was 1.0 cm×1.3 cm.Results All the island flaps were survived postoperatively,and satisfactory in auricular appearances of all patients by 6 months to 5 years follow-up.Conclusions The revolving door island flaps with abundant blood supply are transferred to reach the auricular defect,which match normal auricular tissue in contour,color,and texture. This flap on the retroauricular area is relatively concealed,and it therefore can be one of the best procedures for repair of the auricular defects,especially concha.
3.Short-time effect of selective head cooling on neonatal hypoxic-ischemic encephalopathy

Objective To assess short-time effect of selective head cooling on neonatal hypoxic-ischemic encephalopathy induced by perinatal asphyxia. Methods Twenty-three infants of moderate/severe hypoxic ischemic encephalopathy (HIE) were divided into treatment group ( 14 cases) and control group (9 cases). The head hypothermia in the treatment group [rectal temperature ( 34. 0 ~ 35.0) ℃, nasopharyngeal temperature (34. 0 ± 0. 5 ) ℃]was induced by circulating water cooling cap for up to 72 h. Control group were treated routinely. All newborns were monitored and analyzed for blood pressure, heart rate, neurological function. Neonatal behavioral neurological assessment (NBNA) was conducted for assessment of neurobehavioral development on day 28 after birth. Results Treatment group showed slower hear rate as nasopharyngeal temperature decreased and there was significant difference as compared with the control group ( P < 0.05 ). Apnea and arrhythmia were not found in either group. As compared with the control group, treatment group showed better results in seizure incidence, recovery of neonatal reflex( embracing reflection, sucking reflex and grasp reflex), and improvement of limb muscle tension. The NBNA score on day 28 was much higher in hypothermia group(35.00 ± 1.41 ) than that of control group(30. 67 ± 1.58) and there was significant difference (P < 0.05). Conclusion Selective head cooling is safe for newborns with moderate/severe HIE if the nasopharyngeal temperature maintains at (34 :± 0. 5 ) ℃, rectal temperature at 34 ~ 35 ℃. Selective head cooling can effectively improve the short-term nervous symptoms and the neurobehavioral score,but the long-term efficacy remains to be further studied.

The methylation inactivation of 14-3-3σ is widely recognized as one of the mechanisms of tumorigenesis,and be associated with the metastasis of NPC.
5.Combined detection of CYFRA21-1 and β2-MG for differentiation of malignant pleural effusions

Objective To elucidate the significance of CYFRA21-1 and β2-microglobulin expression in pleural fluid and develop diagnostic application for malignant pleural effusion differentiation. Methods Detected expression of CYFRA21-1 by RIA analysis and β2-MG by immunoturbidimetry assay (ITA), in 40 cases of benign pleural effusion and 40 cases of malignant pleural effusion. Results The mean level of CYFRA21-1 in malignant pleural effusions was (78.80±24.90)μg/L higher than that in benign pleural effusions (26.20±15.60)μg/L (P<0.05) and β2-MG in benign pleural effusions was (6.11±0.87)mg/L, significantly higher than that in malignant pleural effusions which was (3.12±0.91)mg/L respectively (P<0.05). Conclusion Evaluation of CYFRA21-1 and β2-MG in chest fluid can improve the differential diagnosis of malignant pleural effusion from benign pleural effusion and significantly increase the sensitivity of diagnosis.
6.Progress in research of polyamidoamine dendrimers as gene carriers

Polyamidoamine (PAMAM) dendrimers is considered a better genetic carrier for its ability to form with DNA a nano-sized polyelectrolyte complex of high stability and dissolubility. The mechanism of PAMAMmediated gene endocytosis has been fairly clear and the researches in vitro have confirmed that PAMAM had high transfection efficiency and low cytotoxicity. The researches in vivo have further confirmed that PAMAM had a potential application foreground in the field of gene therapy including in the prevention and treatment of ewing's sarcoma and pulmonary fibrosis, in heart transplantation, and in the preparation of falciparum malaria vaccine.
7.Tissue-engineered skin constructed by adipose-derived stem cell sheet

BACKGROUND:Wtih hyaluronic acid and col agen by cellautocrine as endogenous scaffolds, cellsheet technology can eliminate the disadvantages of exogenous scaffolds. Human adipose-derived stem cells combined with cellsheet technology used in the study of ful-thickness skin defects have become the current hot spot.
OBJECTIVE:To summarize the research progress in adipose-derived stem cellsheet.
METHODS:Articles related to adipose-derived stem cells, cellsheet technology and tissue-engineered skin were retrieved in VIP information and PubMed database from January 1993 to April 2014. The key words were“adipose-derived stem cell, wound healing, tissue-engineered skin, cellsheet”in English and Chinese, respectively. Total y 27 relevant articles were analyzed.
RESULTS AND CONCLUSION:Adipose-derived stem cells derived from discarded human adipose tissue are immmunocompatible and multi-potent, rendering them ideal for regenerative medicine applications. cellsheet technology seemingly provides several advantages over traditional tissue reconstruction with biodegradeable scaffolds. Adipose-derived stem cells combined with cellsheet technology can enhance tissue vascularization, promote wound healing, the secreted extracellular matrix provides the microenvironment for the celldifferentiation and growth, so it is expected to build a real three-dimensional tissue-engineered skin.
8.Relationship between executive function deficits and violent behaviors in male adolescents

Objective To explore the roles of executive function deficits in adolescent violent behaviors. MethodsAccording to the findings of general state questionnaire,modified overt aggression scale and interview,93 male juvenile delinquents were randomly divided into violent antisocial group(n=47) and nonviolent antisocial group(n=46).Besides,50 male adolescents in vocational schools were served as normal controls.All the participants were subjected to Wechsler adult intelligence scale(WAIS) test,Wisconsin card sorting test(WCST),Stroop color-word test(CWT) and Tower of Hanoi(TOH) test. Results There was no significant difference in IQ between violent antisocial group and nonviolent antisocial group(P0.05).In CWT,violent antisocial group and nonviolent antisocial group had significant difference with normal control group in number of correct responses of Stroop interference effects(SIE)(P0.05).In TOH,the total time spent in violent antisocial group and nonviolent antisocial group was significantly longer than normal control group(P0.05). Conclusion There exists executive function deficits in male adolescents with unlawful acts,and violent behaviors may have a closer relationship with executive function deficits.
9. Construction and identification of recombinant adenovirus containing wild-type human PKG I α gene

Objective: To clone human PKG I α gene and construct a recombinant adenovirus vector containing wild-type PKG I α. Methods: RT-PCR was used to amplify the full-length PKG gene from human pulmonary arterial smooth muscle. After T/A cloning, PKG I α cDNA was cloned into shuttle plasmid pAdTrack-CMV to construct pAdTrack-PKG I α. The plasmid was linearized by Pme I and transformed into BJ5183 E. coli, where the plasmid was recombined with pAdEasy-1 by homologous recombination, The recombinants were then transfected into Ad293 cells by Lipofectamine2000 for packaging the adenovirus; the recombinant adenovirus was traced by monitoring GFP expression under fluorescence microscope to determine the titer. Results: PKG I α was successfully amplified from human pulmonary arterial smooth muscle by RT-RCR. After 3 cycles of amplification, the titer of adenovirus containing wild-type PKG I α reached the indicated level. Conclusion: We have successfully constructed PKG I α gene and constructed the PKG I α recombinant adenovirus, which provides a foundation for the study of PKG Iα function and its role in hypoxia pulmonary vessel remodeling.
10.Effect of levosimendan on myocardial injury in patients with sepsis

Objective:To investigate the effect of levosimendan on myocardial injury in patients with sepsis.Methods:Eighty-two patients with sepsis complicated by myocardial injury who received treatment in Yinzhou Second Hospital from June 2015 to September 2017 were included in this study. They were randomly assigned to receive either dobutamine treatment (control group, n = 41) or levosimendan treatment (study group, n = 41) based on conventional basic treatment. Before and after treatment, serum levels of heart-type fatty acid-binding protein (H-FABP), cardiac troponin I (cTnI), N-terminal pro brain natriuretic peptide (NT-proBNP), left ventricular ejection fraction (LEVF), Acute Physiology, Age, and Chronic Health Evaluation II (APACHE II) score, intensive care unit (ICU) stay, and 28-day mortality were compared between the control and study groups. Results:Before treatment, there were no significant differences in serum levels of H-FABP, cTnI, and NT-proBNP as well as LVEF and APACHE II score between the control and study groups (all P > 0.05). At 6 hours after treatment, serum levels of H-FABP, cTnI, and NT-proBNP in the control and study groups [(26.22 ± 7.22) μg/L vs. (39.93 ± 9.85) μg/L, (25.97 ± 6.93) μg/L vs. (34.86 ± 8.55) μg/L, (0.004 ± 0.002) μg/L vs. (1.580 ± 0.360) μg/L, (0.003 ± 0.003) μg/L vs. (0.760 ± 0.210) μg/L, (1 561.73 ± 633.70) ng/L vs. (2 570.06 ± 747.95) ng/L, (1 602.28 ± 681.45) ng/L vs. (2 225.53 ± 585.14) ng/L] were significantly increased compared with before treatment ( t = 7.188, 5.172, 28.031, 23.079, 6.586, 4.443, all P < 0.05). After treatment, serum levels of H-FABP, cTnI, and NT-proBNP in the study group were significantly lower than those in the control group ( t = 2.489, 12.598, 2.323, all P < 0.05). In each group, serum level of H-FABP at 72 hours after treatment was significantly lower than that at 6 hours after treatment [(39.93 ± 9.85) μg/L vs. (6.28 ± 1.07) μg/L, (34.86 ± 8.55) μg/L vs. (5.82 ± 1.88) μg/L], serum levels of cTnI and NT-proBNP at 72 hours after treatment were significantly increased compared with those at 6 hours after treatment [(1.58 ± 0.36) μg/L vs. (2.72 ± 0.55) μg/L, (0.76 ± 0.21) μg/L vs. (1.78 ± 0.49) μg/L, (2 570.06 ± 747.95 ) ng/L vs. (3 623.27 ± 1 105.28) ng/L, (2 225.53 ± 585.14) ng/L vs. (3 128.08 ± 1 098.07) ng/L, t = 11.105, 12.251, 5.053, 4.645, all P < 0.05). At 72 hours after treatment, serum levels of cTnI and NT-proBNP levels in the control group were significantly higher than those in the study group ( t = 8.171, 2.035, both P < 0.05). At 72 hours after treatment, there was no significant difference in serum H-FABP level between the control and study groups ( P > 0.05). At 72 hours after treatment, APACHE II score in each group was significantly decreased and LVEF in each group was significantly increased compared with before treatment ( t = 7.718, 11.380, 9.049, 9.501, all P < 0.05). The change in APACHE II score at 72 hours after treatment relative to before treatment in the study group was more obvious than that in the control group ( t = 2.583, P < 0.05). At 72 hours after treatment, there were no significant differences in LVEF, ICU stay and 28-day mortality between the control and study groups (all P > 0.05). Conclusion:Levosimendan can reduce the serum levels of H-FABP, cTnI and NT-proBNP as well as APACHE II score in patients with sepsis, increase serum level of LVEF, and alleviate myocardial injury.
