1.A comparative study on diagnostic value of endoscopic retrograde cholangiopancreatographyand MR cholangiopancreatography in malignant biliary obstraction

Objective Comparing the diagnostic values for the malignant obstruction diseases of biliary tract between endoscopic retrograde cholangiopancreatography(ERCP)and MR cholangiopancreatography (MRCP). Methods Parallel analysis for the ERCP and MRCP data of 142 malignancies confirmed by surgery and/or ERCP among 383 biliary tract patients was retrospectively carried out. Results MRCP and ERCP were performed with exploration of 137 cases,showing the total conformity of 96.4%; together with localization conformities of 92.3% and 87.3%,qualitative conformities of 78.5% and 89.8% respectively. Conclusions In diagnosing the malignant obstructing disease of biliary tract,MRCP has the advantages in convenience,safety and low false negative,but with higher false positive than ERCP; however,if combined with ERCP can get higher positive conformity.(J Intervent Radiol,2007,16: 673-675)
2.Acupoint injection of astragalus membranaceus injection on patients with tumor leukopenia after radiotherapy and chemotherapy and its influence on cellular immune function

Objective To observe the effect of Zusanli acupoint injection of Astragalus Membranaceus Injection on patients with tumor leukopenia after radiotherapy and chemotherapy and its influence on cellular immune function.Methods According to clinical case of random table,60 cases of malignant tumor patients in Hubei province Xiangyang city hospital from March 2010 to August 2011 were randomly divided into two groups of 30 patients in each group.The treatment group was treated with Zusanli acupoint injection of Astragalus Injection therapy,while the control group was taken Leucogen,vitaminB4,batyl alcohol orally.Venous blood and peripheral white blood cell were taken in both groups before and after treatment.Direct immunofluorescence staining and flow cytometry were used for the detection of peripheral blood T lymphocyte total number and its subsets.Results ① clinical efficacy:Total effectiveness was 77.5%in the treatment group,and 52.5% the control group,the difference was statistically significant between the two groups (x2=8.027,P<0.05).Comparison of the white blood cell count and T lymphocyte subsets of peripheral blood:the white blood cell count was (3.56~4.21) × 109/L in the treatment group,and (2.89~3.39) × 109/in the control group,there was significant difference between two groups (t=2.435,P<0.05); there was significant elevation of CD3 +,CD4+,CD4+/CD8 + [(62± 1.63) %,(43 ± 0.80) %,(1.48 ± 0.23) in the treatment group after the treatment comparing with (48± 1.25)%,(31 ± 0.74)%,(0.93± 0.12) before the treatment (P<0.05 or 0.01),while there was no such changes in the control group.Conclusion Zusanli acupoint injection of Astragalus Injection may modulate the cellular immune function of patients with cancer.It is effective in the treatment of leukopenia after radiotherapy and chemotherapy.
3.Competitive exclusion against Salmonella gallinarum of Salmonella enteritidis infected chickens.

To evaluate the degree of competitive exclusion against Salmonella gallinarum(S. gallinarum) of Salmonella enteritidis(S. enteritidis) infected chickens, fifty-six, 4-week old Hyline layer suspected of S. enteritidis infection were challenged with S. gallinarum. All chickens were tested for S. enteritidis isolation using cloacal swabs and serum plate agglutination test using S. enteritidis Ag. before challenge and classified into four groups(SE isolated, SE nonisolated, SE seropositive and SE seronegative). None of the SE isolated and the SE seropositive groups died after challenge and the average weight gains were 245.5g and 254.6g, respectively. But in the SE nonisolated and the SE seronegative groups, mortality was 18.2% and 20.6% and the average weight gains were 150.1g and 111.2g. The incidence of reisolation of S. gallinarum of the SE isolated and the SE seropositive groups were 41.7% and 47.6% from liver, 33.3% and 47.6% from spleen and 8.3% and 14.3% from cecum, respectively, and the SE nonisolated and the SE seronegative group were 63.6% and 64.7% from liver, 84.1% and 88.2% from spleen and 47.7% and 52.9% from cecum. The serological response of the SE isolated and the SE seropositive groups hardly changed from 75.0 and 81.8% before challenge to 75.0 and 85.7% after. But, the other two groups were found to be significantly higher after challenge and increased from 0 and 18.2% to 100%. Consequently, S. enteritidis preinfected chickens were found to be significant different in terms of mortality, weight gain, reisolation of S. gallinarum and serological response compared to noninfected chickens. Moreover, our study shows that S. enteritidis infected chickens appear strong competitive exclusion against the colonization of S. gallinarum.

4.Type III Hyperlipoproteinemia.

We report a case of type III hyperlipoproteinemia which is called a broad-beta disease. A 53 year old female patient visited our clinic for the evaluation of multiple yellowish papules on extremities and eyelids. The patient showed various types of xanthoma includiiig eruptive, tuberous, tendinous xanthomas and xanthelasma palpebrarum, xanthoma striatum palmare. The blood chemistry revealed a marked elevstion of cholesterol and triglycerides and agarose gel electrophoresis showed a single peak at prebeta and beta portion without separation. On histopathologic studies, typical foam cells were showen.

5.Splitted fractions and unoverlapping analysis of chemical constituents of Poria cocos.

With the combined applications of steam distillation, water extraction and alcohol precipitation, liquid-liquid extraction and column chromatography over macroporous resin, a splitted-fractions method of the chemical constituents of Poria cocos was established. The unoverlapping property of the fractions of P. cocos was qualitatively analysed by using principal component analysis and cluster analysis. With angle cosine, squared euclidean distance and the overlapping analysis of peak area of crude herbs, the unoverlapping property of the fractions of P. cocos was half-quantitatively analysed. The chemical components of P. cocos was divided into the fractions of polysaccharide, petroleum ether, ethyl acetate, alcohol eluate from macroporous resin and water eluate from macroporous resin. Non similarity degree among each chemical fraction was above 80% and main chemical components were identified. The established method for splitting fractions of P. cocos has good stability and repeatability and all chemical components in P. cocos could be completely divided into six fractions. It is the first time that the author half-quantitatively analyse the unoverlapping property of the chemical fractions of P. cocos.

6.Combination of bone cement filling and plate internal fixation with limb salvage is used for metastatic malignant bone tumors☆

BACKGROUND:Currently, limb salvage therapy has become the standard treatment of malignant bone tumors way, but improper treatments wil result in tumor recurrence, secondary infection, internal fixation or prosthesis loosening.
OBJECTIVE:Based on the traditional surgical principle for metastatic bone tumors of the limbs, this study designed a user-friendly, individualized, simplistic pal iative treatment regimen from the actual conditions of patients to observe the reasonability, clinical efficacy and prognosis of bone cement fil ing combined with internal fixation in the treatment of metastatic malignant bone tumors.
METHODS:Thirty-one patients with metastatic malignant bone tumors who required salvage treatment were screened from the Department of Orthopedics, the 421 Hospital of Chinese PLA, and their clinical data were retrospectively analyzed. Al the 31 patients were divided into two groups:tumor removal+internal fixation group (non-chemoradiotherapy group, n=11) treated with bone cement fil ing plus plate internal fixation (pal iative
treatment);tumor removal+internal fixation+chemoradiotherapy group (chemoradiotherapy group, n=20), treated with radiotherapy before internal fixation plus plate internal fixation with limb salvage. The fol ow-up period was 4-38 months, averagely 18 months.
RESULTS AND CONCLUSION:The fol ow-up results showed that in the non-chemoradiotherapy group, al the 11 patients survived, who could live independently and have good motor functions;in the chemoradiotherapy
group, 17 of the 20 patients survived and the rest three patients died of tumor metastasis, their poor conditions and complications at 9 and 13 months after internal fixation. In patients undergoing tumor removal+plate internal fixation with limb salvage, the integrated scores for nerve and motor functions were increased by more than
level 1. These findings confirm that a simple pal iative therapy of bone cement fil ing and internal fixation without chemoradiotherapy is better for metastatic malignant bone tumors patients who require limb salvage.
7.Analysis of the monitoring results of death causes of permanent residents in Hainan, 2014-2020

Abstract: Objective To understand the general situation of death causes of permanent residents in Hainan Province from 2014 to 2020, analyze the causes of death, and provide scientific basis for the formulation of policies related to healthy Hainan. Methods The Hainan Provincial Population Death Information Registration and Management System was used to report data, and the death information of permanent residents in Hainan Province from 2014 to 2020 was obtained. Excel 2010 and SPSS 26.0 software were applied to perform statistical analysis related to the indicators including crude mortality rate, standardized mortality rate, ranking of causes of death, and composition ratio; the standardized mortality rate was calculated based on the data of the 2010 National Census Data. Results From 2014 to 2020, the average annual resident population of Hainan Province was 9 175 300, and average annual resident population at each of the eight monitoringp oints is 297 100, anaverage of 83 878 cumulative deaths were reported, the annual total mortality rate was 504.09/100 000, the standard mortality rate was 618.69/100 000. The standardized mortality rates of male and female were 806.18/100 000 and 444.36/100 000 respectively (P<0.01). The mortality rate of all age groups showed that the crude mortality rate of 0~<1 years old group showed a decreasing trend with the passage of time, and the crude mortality rate was significantly higher than that of other age groups. From the age of 20, with the increase of age, the overall crude mortality rate of residents keeps increasing. The crude death rate increases rapidly after the age of 65, and peaks especially after the age of 85. The leading causes of death were circulatory diseases, tumors and respiratory diseases. From 2014 to 2020, the mortality rate of circulatory diseases and tumors, the two main causes of death, was more than 100/100 000. Conclusion Chronic non-communicable diseases are still the first cause of death among permanent residents in Hainan Province. Targeted intervention and treatment of chronic non-communicable diseases is beneficial to reduce their mortality.
8.Is There Any Indicator to Predict the Difficulty of PTBD?.

PURPOSE: A retrospective study was performed to find any indicator to predict the difficulty of percutaneous transhepatic biliary drainage(PTBD) before the procedure and to evaluate correlationship between the chronological change of fluoroscopic time and the operator's experience. MATERIALS AND METHODS: The materials included the consecutive 138 cases of PTBD of 120 patients with obstructive jaundice. Under the presumption that the difficulty of procedure would be reflected on fluoroscopic time, the correlation between fluoroscopic time and eight factors(i. e. ;sex, age, disease, approach, degree of IHD dilatation, total bilirubin, alkaline phosphatase, and operator) were analysed. Also chronological change of fluoroscopic time in three operators, who performed more than 25 procedures, were evaluated. RESULTS: Statistically significant factors were operator (P=0.000), sex (P:0.028), approach(P=0.048), and total billrubin (P=0.021). Good correlation between the chronological change of fluoroscopic time and the operator's experience was found. CONCLUSION: The significant factors could be used as an indicator to predict the difficulty before PTBD procedure. The chronological change of fluoroscopic time could be used as an indirect estimate in the stabilization of the operator fechnigue.

9.The Effect of Red Pepper (Capasicum Annum L.) on Growth, Calcium Absorption, Serum Vitamin A Content and Histology of Albino Rats.

Twenty-four ma1e a1bino rats were chosen at random and divided into two dietary groups: one for control (diet 1) and the other for an experimental diet (diet 2). Diet II consisted of the basal diet supplemented with red pepper powder in increasing amounts from 1.25%, 1.8% and 2.5% to 3.6%. With a red pepper diet at the 2.5% level, there was a significantly lower efficiency of food utilization. (p=0.05) The apparent digestlbility of calcium with red pepper concentrations from 2.5 to 35% was about 13% greater than in tile control diet. Thus red pepper appears to affect the solubility of calcium in the diet. Serum alpha-Carotene and Vitamin A showed a tendency to increase slightly with a red pepper diet. No histological changes in the gastric mucosa cells were observed in rats fed a red pepper diet. Further investigation is required to define the effects of red pepper in different species of animals.

10.A Case of Scrub Typhus Complicated with Acute Renal Failure Meningoencephalitis and Hepatitis.

Scrub typhus is a zoonosis transmitted by a chigger of trombiculid mite and manifested with fever, skin rash, myalgia, and hepatitis etc. The renal involvement of the disease is prese-ted with transient microscopic hematuria and/or proteinuria. However acute renal failure or meningoencephalitis is very rare. We report a case of scrub typhus manifested with acute renal failure, meningoencephalitis, and hepatitis. The patient was treated with doxycycline and continuous arteriovenous hemofiltration (CAVH) and recovered successfully.
