1.The clinical study of minimally invasive peritoneal lavage treating the perforation of the upper part of the gastrointestinal tract

Objective To discuss the application of minimally invasive peritoneal lavage in treating the perforation of the upper part of the gastrointestinal tract.Methods The clinical data of 120 patients suffered with upper digestive tract perforation treated from 2005 to 2012 were reviewed retrospectively.86 cases received peritoneal lavage as therapy group,in which 26 cases received saline lavage,33 cases received antibiotic lavage,27 cases received dermacyn lavage.34 cases without peritoneal lavage as control group.The improved clinical indicators and postoperative complications were analyzed in two groups.Results The time of improved clinical indicators,peritonitis releasing,stomach resuming,food taken and being in hospital in therapy group were shorter than those of control group.The incidence of intraabdominal abscess of receiving salin lavage group and control group was the same,the intestinal adhesion rate in receiving peritoneal lavage group and the saline lavage group had no obvious difference,the incidence of intraabdominal abscess and intestinal adhesion in dermacyn lavage group was significantly lower than those of other groups.Conclusion Minimally invasive peritoneal lavage is a treatment of upper digestive tract perforation method,intestinal function recovers rapidly,abdominal complications are few,the effect of dermacyn peritoneal lavage on treatment of peforated ulcer of upper digestive tract is better than antibiotic lavage,which is suitable for clinical application.
2.Relation between myocardial injury and left ventricular function in rhe umatic mitral diseases

Objective To study the relation between myocardia l changes and left ventricular function in preoperative patients with combined m itral diseases provide theoretical basis for clinical therapy and evaluate the effects of treatment. Methods A total of patients with combined mitral lesions were selected randomly for study. The cases were divided into tw o groups according to the degree of ultrastructural damage of the myocardium bef ore aortic occlusion in mitral valve replacement (Flameny cla ssification). The preoperative pulmonary artery catheterization(Swan-Ganz cathe ter), color Doppler echocardiography, roentgenologic measurement of heart size, preoperative clinical data and postoperative follow-up data were collected for analysis. Results The results showed that myocardial lesion in mitral disease had intimate correlation with the cardiac enlargement, especial c orrelated with left ventricular enlargement and cardiac pumping. Conclus ion Results suggest that the myocardial lesions in mitral valvular dise ases mainly involved the process of rheumatism and reflected in the disturbance of left ventricular function. Operation should be performed before the appeare nce of non-reversible myocardium injury in patients with rheumatic mitral va lvul ar diseases.
3.Analysis of Diagnosis and Treatment on Lower Gastrointestinal Bleeding Caused by Small Intestinal Vascular Malformation(A Report of 12 Cases)

Objective To summarize the experience of diagnosis and treatment of lower gastrointestinal bleeding caused by small intestinal vascular malformation .Methods 12 patients with lower gastrointetinal bleeding caused by small intestinal vascular malformation were retrospectively reviewed.Results ⑴Repeated intermittent hematochezia were the most common cases in our date of lower gastrointestinal bleeding caused by small intestinal vascular malformation.⑵The most common course of disease was 2~3 years.⑶Bleeding was halted after operation in 100% cases and no one relapse.Conclusions ⑴Active bleeding period should be evaluated firstly by angiography and prepare to emergency operation.⑵By treatment combined with enteroscopy during operation could help to diagnose.⑶Surgical operation is the most effective treatment for bleeding of small intestinal vascular malformation.
4.Study on the application skills and safety of breast conserving surgery in radical mastectomy

Objective To study the skills and safety of breast conserving surgery in radical mastectomy.Methods A total of one hundred and seventy-eight patients with early breast cancer were randomly divided into two groups: 93 cases in the breast conserving surgery group and 85 cases in the radical group,according to the surgical method.The patients in the breast conserving surgery group received stageⅠbreast conserving surgery after modified radical mastectomy for breast cancer,while the patients in the radical group adopted modified radical mastectomy.Then the two groups were compared in terms of postoperative complications,breast appearance evaluation,follow-up on quality of life and tumor recurrence.Results There were not statistically significant differences in postoperative complication rate between the two groups(30.11% vs.23.53%, χ2=0.976,P=0.323);the rate of excellent breast appearance in the conserving surgery group was 86.02%,significantly higher than that of the radical group(38.82%,χ2=42.675,P<0.001);the scores of physiological status,psychologic status,social function and environmental circumstance in the conserving surgery group were significantly higher than those of the radical group,the differences were statistically significant(P<0.05);the difference on tumor recurrence rate of the two groups was not statistically significant(χ2=0.009,P>0.05).Conclusion After radical mastectomy,stageⅠbreast conserving surgery could help patients maintain their breast appearance and improve their life quality,without any increase in the rate of postoperative complications and long-term local morbidity.It is a safe and reliable surgical method.
6.Cephalometric analysis of Class Ⅱ division 1 malocclusion after bite-opening treatment with fixed inclined bite plate

Objective: To evaluate the effects of fixed inclined bite plate on jaw bone and dentition in juveniles with Class Ⅱ division 1 malocclusion.Methods: Eighteen patients(10 male and 8 female aged 11.5~13.8 years old) were treated with fixed inclined bite plate for bite-opening, twelve patients(6 male and 6 female) were involved as the control. Cephalograms were taken before and after the treatment.Results: After treatment ANB was significantly reduced and mandibular development was improved; AFH and PFH were significantly increased; the first molar in mandible was siginificantly elongated.Conclusion: Fixed inclined bite plate is effective in the improvement of sagittal skeletal relationship and in the orthodontic treatment in the juveniles with Class Ⅱ division 1 malocclusion.
7.Chemical constituents from root of Polygala fallax

Objective To investigate the chemical constituents of Polygala fallax.Methods The chemical constituents were isolated and purified via silica gel and Sephadex LH-20 column chromatography as well as recrystallization.Their structures were determined on the basis of spectral analysis and physicochemical properties.Results Six compounds were isolated and identified as 3-O?-D-glucopyranosyl senegenic acid(Ⅰ),tenuifolin(Ⅱ),presenegenin(Ⅲ),1,7-dimethoxy-2,3-methylenedioxy xanthone(Ⅳ),sinapinic acid(Ⅴ),ferulic acid(Ⅵ).Conclusion Compounds Ⅲ—Ⅵ are isolated from P.fallax for the first time.Compound Ⅰ is a new triterpenoid saponin,named fallaxsaponin A.
8.The initial study on mechanism in postpone skeletal muscle aging process of D-galactose rats by movement training and soy polypeptide supplement.

OBJECTIVETo observe the effect of the postpone in skeletal muscle aging process of D-galactose rats by weight training and soy polypeptide supplement in 6 weeks, and discuss the initial mechanism.
METHODSixty male SD rats (three month old)were randomly assigned: 6 week control (C6,) and 6 week model (M6) 6 for each group, 12 week model (M12), big load (B12), small load (S12), peptide (P12), peptide + big load (PB12) and peptide + small load group (PS12) 8 for each group, eight fourteen month rats were taken in the natural aging group. The rats were killed by the end of 6th week and 12th week, tested the indicators.
RESULTCompare with group C6, the indicators in group M6 showed aging in different levels; Compare with group M12, weight training or soy polypeptide supplement in all intervention groups could increase the content of skeletal muscle superoxide dismutase (SOD), SOD/MDA, the serum growth hormone(GH), insulin-like growth factor-1 (IGF-I)and skeletal muscle IGF-I mRNA, decreased the malondialdehyde (MDA) content of skeletal muscle, and they had notable interaction.
CONCLUSIONRat skeletal muscle aging model can be copied successfully by D-galactose hypodermic, and go on with 6-week weight training or soy polypeptide supplement, they can postpone the skeletal muscle aging process of D-galactose rats, and the two interference way united can have more obvious effect. Its preliminary mechanism may be related to the reduction of skeletal muscle oxidative stress and lipid peroxidation, the correction of hormones and related factors metabolic disorders, the elevation of skeletal muscle IGF-I mRNA expression and so on.
