Objective To summarize the related factors and nursing countermeasures of complications after the laparoscopy-assisted surgery for congenital hypertrophic pyloric stenosis. Methods Clinical data about the occurrence of complications and nursing points were retrospectively analyzed in 106 cases of neonates and infants. Results Surgical complications occurred in 16 cases, including mucosal perforation in 2 cases, abdominal cavity hemorrhage in 1 case, puncture hole infection in 1 case, repeated postoperative vomiting caused by incomplete pyloric separation in 1 case, postoperative hypercapnia in 2 cases, subcutaneous emphysema in 1 case,8 cases of postoperative hypothermia. No operative deaths. No poking holes hernia and abdominal organ injury.By careful observation and nursing, all cases got rid of postoperative complucations.Within postoperative 3~7 days, they were recovered and discharged,no nursing complications occurred. Conclusions Early discovery of complications and timely nursing intervention are important guarantee for patients to pass through laparoscopic surgery smoothly and obtain swift recovery.

Objective To explore the effect of combined application of improved microencapsulation and intravenous injection of donor liver cells and spleen cells on pancreatic islet xenografts. Methods New born male pigs and SD rats were selected as donors and recipients respectively, and pig pancreatic islet cells were microencapsulated by static electricity improvement microencapsulation. Islet transplantation was performed after injection of donor liver cells and spleen cells via tail vein. The changes in blood glucose and serum C-peptide, the function-possessed days of islet xenografts and diabetes rats living period were observed. Results In combined application group, the function-possessed days of islet xenografts and diabetes rats living period were significantly prolonged (P

Objective To investigate the value of CARE Dose 4D technique in decreasing radiation dose and improving image quality of multi-slice spiral CT in adult chest scanning. Methods 100patients of chest CT scanning were equally divided into study group and control group randomly.CARE Dose 4D Technique was used in study group.Effective mAs value, volume CT dose index (CTDIvol) and dose length product (DLP) were displayed automatically in machine while chest scanning; those values and actual mAs value of every image were recorded respectively. The image quality at apex of lung, lower edge of aorta arch, middle area of left atrium and base of lung on every image of 400 images was judged and classified as three level (excellent, good, poor) by two deputy chief physicians with double blind method, the image noise at corresponding parts was measured. Results While setting 80 mAs for quality reference mAs,the effective mAs value in study group most decreased 44 mAs than control group with an average decrease of 9.60( 12.0% ) ,CTDIvol with 4.75 mGy with an average decrease of 0.95 mC y( 11.0% ) ,DLP 99.50% in study group, with 98.0% in control group. But it was higher at apex of lung and base of lung, lower at middle area of left atrium, and similar at lower edge of aorta arch in study group than contrast group. The image noise were lower at apex of lung and base of lung in study group than control group (t =6.299 and 2.332,all P < 0.05 ) ,higher at middle area of left atrium in study group than control group (t=3.078,P <0.05) and similar at lower edge of aorta arch in study group than control group (t =1.191,P >0.05).Conclusions CARE Dose 4D technique provides a function regulated mAs real-time on line,it not only raises utilization rate of radiation and decreases radiation dose, but also promises and increases image quality in chest CT scanning, and has some clinical significance.

Objective To investigate the clinical significance of serum citrulline levels in evaluating the remnant bowel absorptive function with enterostomy in the neonatal period by means of tandem mass spectrum so as to guide clinical treatment with parenteral nutrition(PN) or not. Methods The serum amino acid profiles of 30 enterostomy infants including small intestinal enterostomy 17 cases of enterostomy and 13 cases of colon enterostomy and 20 cases of control infants in Wuhan Children's Hospital,Tongji Medical College University of Science & Technology from January 2016 to June 2018 were performed by using tandem mass spectrum, and the related amino acid with the bowel absorbing ability-citrulline was screened out.The Pearson correlation of the serum citrulline levels with the gain weight of the infants was studied. Results The weight of small intestinal enterostomy was significantly lower than colon enterostomy and control infants [(3.22 ± 1.00) kg vs.(5.63 ± 0.66) kg and (6.09 ± 0.85) kg],and there was statistical significance (F =57.23,P < 0.05).Serum citrulline levels were significantly lower in small intestinal enterostomy compared with colon enterostomy and controls [(10.65 ± 5.82) μmol/L vs.(16.57 ± 4.33) μmol/L and (16.52 ± 4.20) μmol/L],and there was statistical significance (F =8.29,P < 0.05).In enterostomy infants serum citrulline was positively correlated with gain weight everyday(r =0.65,P < 0.05).Serum citrulline level < 12 μmol/L had 81.3% sensitivity and 71.4% specificity for underweight. Conclusions Serum citrulline is a simple and accurate biomarker for evaluation of the absorptive function and adaptation of the enterostomy infants.Serum citrulline level < 12 μmol/L is a strong predictor of PN-dependence in infants with temporary enterostomy.
