2.Acute Insular Infarct Syndrome: 20 Cases Report

Objective To investigate the symptoms, signs and distribution of focus in acute insular infarct syndrome, as well as the effects on cardiovascular system. Methods The clinical manifestations of 20 patients with first-event acute insular infarct syndrome were analyzed retrospectively. Results All the patients revealed the intellectual deterioration, aphasia, hemiparalysis, sensory disorder and vestibular-like symptoms. The temporal lobes were mostly involved (65%). Electrocardiogram showed ST segment depressed mostly. The supraventricular premature beats were the most frequence finding by Holter monitoring in left insular infarction except atrial fibrillation, and their heart rate variability indicated that the standard deviation of normal-to-normal intervals (SDNN) decreased, but root mean square successive difference (RMSSD) increased. Conclusion The insular infarct syndrome can be charactered as a group of symptoms of intellectual deterioration, aphasia, hemiparalysis, sensory disorder and vestibular-like symptom. Left insular infarct could be complicated with arrhythmia, which increases the risk of adverse cardiac outcome. The characteristics of arteries to Sylvian angle may be associated with the syndrome.

'Social beings' model of standardized training program for residents in Shanghai is one of the important measures in the health care reform. According to the requirements of the standardized training of residents in thoracic surgery, the article preliminarily explored the standardized training model for residents from four aspects such as individualized treatment of different professional backgrounds, focus-ing on the training needs of residents with different starting point of social beings, developing training programs for different training time and combining the master's degree graduate education. And based on the characteristics of thoracic surgery, it also summed up the past experience and explored the clinical practice teaching.
4.Effect of Early Multidisciplinary Rehabilitation Therapy on Stroke Patients

Objective To observe the effect of early multidisciplinary rehabilitation therapy on stroke patients.Methods 80 cases of acute stroke were randomly divided into the rehabilitation group (n=40) and control group (n=40). All patients of two groups were treated with routine treatment, and those of the rehabilitation group were added with early multidisciplinary rehabilitation therapy. The degrees of clinical neural function defect, motor function and activities of daily living (ADL) of all patients in two groups were assessed by the clinical neural function defect assessment (NIHSS), Fugl-Meyer Assessment (FMA) and Barthel Index (BI) before and one month after treatment.Results The scores of NIHSS, FMA and BI of patients in two groups were not different before treatment ( P>0.05). After treatment, the scores of patients of the rehabilitation group were significantly higher than that of the control group ( P<0.01).Conclusion Early multidisciplinary rehabilitation therapy can improve the neural function, ADL and raise the quality of life of post stoke patients.
5.Generation of Japanese Encephalitis Virus-like Particle Vaccine and Preliminary Evaluation of Its Protective Efficiency.

The cDNA fragment of JEV prME gene was cloned into the baculovirus shuttle vector (bacmid) to construct a recombinant baculovirus vector, defined as AcBac-prME. Then the recombinant baculovirus Ac-prME was obtained by transfecting Sf9 cells with AcBac-prME. Western blot analysis and immunofluorescence results indicated that both prM and E proteins were efficiently expressed in Sf9 cells. Electron microscopy suggested that prME was assembled into JEV-VLPs. To further evaluate the potential of JEV-VLPs as vaccine, the mice were immunized with JEV-VLPs and then challenged with lethal JEV. The results of mice survival and pathological changes demonstrated that the JEV-VLPs performed complete protection against JEV-P3 strain and relieved pathological changes in the mice brain significant. This study suggest that JEV-VLPs would be a potential vaccine for Japanese encephalitis virus.

6.Study on the current employment situation of targeted admission medical graduates in rural areas

Objective: In order to describe the employment status of the first group of targeted admission medical graduates, evaluate the effects of targeted admission medical education program, find out the existing problems and provide relevant policy recommendations.Methods: This study conducted follow-up survey among 305 medical graduates and 315 clinical medical graduates in four medical colleges of the central and western regions.Data analyses of the contract fulfillment, their employment status, and their participation in the standardized residency training are carried out.Results: The contract fulfillment rate of rural-oriented medical graduates is 99.3%, while the percentages of rural-oriented medical graduates and clinical medical graduates are 90.7% and 2.8%, respectively.Their average monthly incomes are respectively 2,011 and 2,774 yuan, and their proportions of participating in standardized residency training are 77.8% and 43.3% respectively.From the findings it can be seen that the employment situation of the first group of targeted admission medical graduates in four medical colleges and universities is quite good, but the policy implementation process in terms of permanent post and income is less satisfactory.However, the way how medical graduates should participate in the standardized residency training is not yet clearly regulated.This paper recommends that medical colleges and universities should well coordinate with local health administrative departments and human resources and social security departments to systematically deploy medical graduates.At the same time, full consultations should be conducted to make appropriate arrangement for medical graduates to participate in standardized residency training.
7.Value of pressure and respiratory rate in predicting success of weaning from mechanical ventilator in patients

Objective:To evaluate the performance of the combinative index-a product of pressure (P) and respiratory rate (RR), to predict the outcome of weaning.Methods:Single-center, case-control study method was used to prospectively collect the data of 76 patients who had been mechanically ventilated for at least 24 h in ICU of the Affiliated Changshu Hospital of Xuzhou Medical University from November 2017 to April 2019, excluding the patients with central respiration repression and primary neuromuscular disease. The spontaneous breath test (SBT) was performed for 1 h after the patient was ready to be weaned. All patients were divided into two groups according to whether they were successfully weaned from the mechanical ventilation: 50 cases in the success group and 26 cases in the failure group. Clinical data and values of peak inspiratory pressure (P peak), driving pressure (DP) and RR of two groups were collected. The independent sample t test and Mann-Whitney U test were used for the comparison between the two groups. The differences between enumeration data were assessed by Chi-square test. Finally, receiver operating characteristic (ROC) curve analysis was used to evaluate the predictive value of each index. Results:Of the 76 patients studied, 26 patients failed extubation. The values of P peak×RR and DP×RR in the success group were significantly less than those in the failure group; P peak×RR (cmH 2O·breaths/min): 291.8±76.5 vs 519.8±108.8 ( P<0.01), DP×RR (cmH 2O·breaths/min): 131.6±34.2 vs 227.0±47.5 ( P<0.01). The AUC of P peak×RR, DP×RR was 0.960 and 0.941, respectively. A decrease in P peak×RR index less than 362 cmH 2O·breaths/min had a sensitivity of 96.2%, a specificity of 88.0%, a positive predictive value (PPV) of 97.8% and a negative predictive value (NPV) of 80.6%, to predict successful weaning. A decrease in DP×RR index less than 170 cmH 2O·breaths/min had a sensitivity of 88.5%, a specificity of 92.0%, a PPV of 94.0% and a NPV of 88.5%, to predict successful weaning. Conclusion:The combination of pressure and respiratory rate can accurately predict whether the patients can be successfully weaned from mechanical ventilation.
8.Retinal vessel density and retinal thickness as measured using optical coherence tomography angiography in open angle glaucoma

Objective:To investigate the discriminant function of optical coherence tomography angiography (OCTA) in the diagnosis of primary open angle glaucoma (POAG) and to evaluate the correlation between vessel density, as measured by OCTA, and visual field mean defect (MD) in patients with POAG.Methods:A case control study was designed.Ninety-five eyes of 54 patients with POAG and ninety-six eyes of 48 healthy subjects in Changsha Aier Eye Hospital were included in this study.The vessel density in the radial peripapillary capillary (RPC) network, the macular vessel density, the ganglion cell complex (GCC) layer thickness, the macular thickness, and the retinal nerve fiber layer thickness were tested for correlation with MD.The aera under the receiver operating characteristic (ROC) curves (AUC), as well as sensitivities at fixed vessel density specificities in the RPC network, and macular regions were analyzed.ROC regression was used to evaluate the effect of covariates on the diagnostic abilities.The study protocol was approved by the Ethics Committee of Changsha Aier Eye Hospital.Written informed consent was obtained from all subjects prior to entering the study cohort.Results:Compared with the normal control group, the macular area and vessel density in the RPC network were significantly decreased in the POAG group ( P<0.001). Also in the POAG group, except for the capillary density inside the disc, all other measurements including the density of superficial capillaries in the macula, the density of deep capillaries in the macula, the density of RPC layers around the optic disc, the average capillary density of the optic disc RPC layer, the capillary density of the optic disc RPC layer (above, below, nasal, and temporal), the thickness of the retinal nerve fiber layer, the thickness of the whole macular layer, and the thickness of the GCC layer of the macula area were positively correlated with the MD value ( r=0.586, 0.352, 0.610, 0.592, 0.598, 0.589, 0.445, 0.587, 0.578, 0.530, 0.609; all at P<0.01). The AUCs of vessel densities ranged from 0.623 to 0.927.The highest diagnostic efficiency was the RPC vessel density (whole image), the AUC value was 0.927. Conclusions:The OCTA parameters have a high diagnostic ability for POAG, except for the capillary density of inside the disc, all other vascular parameters are positively correlated with the MD value.

10.The macular vascular density and the area of foveal avascular zone in the follow eyes of monocular retinal vein occlusion patients

Objective:To observe the macular vascular density and the area of foveal avascular zone(FAZ) in the follow eyes of monocular retinal vein occlusion (RVO) patients.Methods:Retrospective case-control study. From May to November 2018, 78 follow eyes of 78 monocular RVO patients who were clinically diagnosed in Changsha Aier Eye Hospital were included in the study. Among them, 44 were male and 34 were female. The average age was 53.17±10.12 years. There were 42 patients with central retinal vein occlusion(CRVO group) and 36 patients with branch retinal vein occlusion (BRVO group). Forty-two eyes of 33 gender and age matched healthy volunteers were selected as the control group. Among them, 17 were male (22 eyes) and 16 were female (20 eyes), with the mean age of 53.48±10.84 years. OCT angiography was performed on all eyes in CRVO group, BRVO group and control group. The scanning region in the macular area was 6 mm× 6 mm. Macular vascular density and FAZ area in the superficial and deep retinal capillary plexi were measured.Results:The mean overall vascular density measured in the entire scan was lower in the CRVO group( t=-4.26, -4.93) and BRVO group ( t=-4.79, -4.74) compared with the control group in both the superficial and deep capillary plexus ( P<0.05). The reduce degree of vascular density in the deep capillary plexus (CRVO group:5.51%, BRVO group:4.58%) was higher than that in the superficial plexus (CRVO group:4.13%, BRVO group:3.50%). In the CRVO group, the FAZ area decreased compared with the control group ( t=-3.43, P<0.05). There was no statistically significant difference in the area of FAZ between the BRVO group and the control group ( t=-0.10, P>0.05). Conclusions:The macular vascular density in the follow eyes of monocular RVO patients is lower than that of normal healthy eyes. The reduce degree of vascular density in the deep capillary plexus is higher than that in the superficial plexus. Compared with normal healthy eyes, the FAZ area in the follow eyes of monocular CRVO patients decreased, while it did not change significantly in the follow eyes of monocular BRVO patients.
