1.Application of rotatory hinge prosthetic knee replacement in treatment of giant cell tumors around knee joint:report of 31 cases

Objective To evaluate the clinical effectiveness of rotatory hinge prosthesis replacement for treatment of giant cell tumors around knee joint. Methods Thirty-one patients with giant cell tumors of bone around the knee from January 2001 to January 2008 in our hospital were reviewed. There were 13 males and 18 females. Their ages ranged from 19 to 56 and the mean age was 33.5. The anatomic site for the lesions were the distal femur (20 cases) and proximal tibia (11 cases). Using Companacci’s staging system: 7 patients were classified as grade Ⅱ and 24 as grade Ⅲ. The patients were grouped as follows: the tumor breaking through the cortex with an extraosseous mass in 2 patients,having pathologic fracture in 3,representing more biologically-aggressive lesions in 5,and having one or more local recurrences in 21. All patients had been treated with en block excision and reconstruction with rotatory hinge prosthesis. Results According to the follow-up for 12 to 96 months,all patients had no local recurrence or metastasis. The mean Musculoskeletal Tumor Society (MSTS) score was 24.0?2.5,with an excellent limb function in 23 patients,good in 6 patients,and fair in 2 patients. The overall excellent and good function was obtained in 93.5% of the patients. Conclusion Rotatory hinge prosthesis replacement is an effective method to treat giant cell tumors of bone around the knee joint. It also can reduce the local recurrence and obtain better the outcomes.

3.Current status of aflatoxin pollution and risk assessment of peanut and related products in China

Peanuts, corn and other food products are prone to aflatoxins (AF). AF was listed as a human carcinogen by the International Agency for Research on Cancer (IARC), especially aflatoxin B1 (AFB1), as it is considered the most prevalent and toxic. China is a big peanut producer, so carrying out pollution investigation and risk assessment in peanuts and their products is crucial to formulate prevention measures, protect export trade and maintain health. This paper summarizes the AF pollution of peanuts and their products in the Huang-Huai-Hai Basin, Yangtze River Basin, Southeast Coast and Northeast region of China from 2015 to 2021, and the application of dietary AF exposure risk assessment methods, providing a basis for strengthening the supervision of AF pollution in peanuts and their products, and ensuring food safety.
4.Evaluation of the target-organ damage in the elderly hypertension patients by ambulatory blood pressure monitoring

Objective To explore the prognostic value of 24 h ambulatory blood pressure monitoring (ABPM)for target organ damage (TOD) in the elderly hypertension patients. Methods Two hundred and thirty two elderly hypertension patients who were involved at least one target- organ damage experienced 24 h ABPM. All subjects were randomly divided into 3 groups according to the number of TOD and the clinical situation of cardiovascular, cerebrovascular and renal complications. 24 h ABPM recordings of each group were compared with one another. Objective To explore significance and impact of hypoxia inducible factor 1α,metastasis associated with colon cancer 1 and transcription factor FOXO4 in gastric carcinoma and clinical pathological characteristics and prognosis .Methods 110 samples diagnosed as gastric carcinoma and 46 samples diagnosed as pericarcinomatous tissue which was 5 cm far away from cancer were collected in first affiliated hospital of Anhui medical university from December 2006 to May 2008 .Immunohistochemical stai-ning was performed in these samples to detect the expressions of HIF-1α,MACC1 and FOXO4 .And the correlation between devel-opment of gastric carcinoma and expressions of HIF-1α,MACC1 and FOXO4 was analyzed with the help of patients′messages .Ka-plan-Meier method and Cox regression were used to analyze patients′5 years survival rate and the factors .Results The expression levels of HIF-1α(P<0 .001) and MACC1(P<0 .001) in gastric carcinoma tissues were significantly higher than pericarcinomatous tissues ,however the expression levels of FOXO4(P<0 .001) was significantly decreased in gastric carcinoma tissues .Statistical a-nalysis revealed that the expressions of HIF-1αand MACC1 were apparently increased with the lower degree of cell differentiation , the greater degree of tumor infiltration ,accompanied with lymph node metastasis and higher TNM scores ,but lowered the expres-sion of FOXO4 factor .There were significant correlations between expressions of the three factors in gastric carcinoma .The expres-sions of HIF-1α,MACC1 and FOXO4 had an apparent influence on the patients′5 years survival rate .And they were independent factors on the prognosis of gastric carcinoma .Conclusion HIF-1α,M ACC1and FOXO4 are abnormal expression in gastric cancer tissue ,and the clinical pathological features of gastric cancer and the prognosis of patients with gastric cancer are closely related .

Objective To investigate the clinical characteristics of general paresis of insane(GPI) and diagnostic value of laboratory and neuroimaging examinations.Methods The clinical data of 2 patients with GPI were analyzed retrospectively and the pertinent literatures were reviewed.Results The main clinical characteristics consisted of insidious onset,progressive dementia,mental disorder and epileptic seizure.These 2 patients were originally misdiagnosed.Both rapid plasma reagin test and treponema gelatin agglutination test in serum and cerebrospinal fluid were positive.Cranial MRI showed diffuse cerebral atrophy.Conclusions The clinical characteristics of GPI are varied and misdiagnosis rate is high at early stage of this disease.The diagnosis of GPI mainly depends on clinical characteristics,serum and cerebrospinal fluid examinations.

8.Analysis of the monitor results of Marmota sibirica plague foci from 2004 to 2008

Objective To master of the characteristics of development and variation in plague natural foci of Marmota sibirica, and to analyze the plague monitoring results from 2004 to 2008. Methods From May to September every year, we monitored plague in Manzhouli, Chenbaerhuqi, Xinbaerhuzuoqi, Xinbaerhuyouqi,Ewenkeqi and Yakeshi city. The monitored area was 20 000 - 40 000 hm2 in every county. The density of Marmota,ground squirrel and moonlighting rats was surveyed by path method, one-day bow-clip method and a clamp was placed every five meters, respectively. The classification and identification of ectoparasite fleas were done by using low power lens. The fleas in the ground squirrel hole were collected by flannel rubber stick, then classified and identified. According to National Standard of Plague Diagnostic Criteria(GB 15991-1995), the collected rats and fleas were detected by isolation and cultured Yersinia pestis, the serums of collected rats were tested by indirect hemagglutination test. Results In five years, the density of Marmota and ground squirrel was 0.010/hm2 and 0.602/hm2, respectively. The capture rate of moonlighting rats was 2.69% (258/9600). The flea infection rate of Marmota was 17.54%(10/57) and the fleas index was 2.54. The fleas infection rate of ground squirrel was 28.40%(213/750) and the fleas index was 1.01. Flea infection rat of ground squirrel hole was 5.60%(46/822) and the fleas index was 0.17. The total number of various hosts of Yersinia pestis detected was 1351, the groups number of the variety of cultured fleas was 127, and the pathogen test results were negative. The number of serum tested was 1064, positive number was 43, and the detection rate was 4.04%(43/1064). The highest positive titer was 1 : 1280. Other than 2006, the remaining four years were found positive for blood clotting material; positive serum was found in a total of three regions, they were Manzhouli, Xinbaerhuzuoqi, and Xinbaerhuyouqi; and 30, 12 and 1 copies of hemagglutination-positive sera were detected. Conclusions The epidemic of plague natural foci of Marmota sibirica is in a active state, and gradually expands the scope. We must continue to strengthen the inspection of the bacteria, bearing in mind the replacement of the region's main host, make every effort to prevent and control of human plague.
10.Application and histological assess of endoscopic saphenous vein harvesting

Objective To observe the influence of endoscopic harvesting technique on the prevalence of leg-wound complications after coronary artery bypass grafting, and to assess histologically the potential trauma to the vein. Methods From August,2003 to August, 2005, 256 patients undergoing CABG had saphenous vein harvested by endoscopic harvesting system. About 4 mm proximal and distal vein end segment of 10 patient undergoing endosocopic and conventional harvesting respectively were examined with light and electro-microscope. Results The mean number of vein grafts of ESVH was 2.2 and the mean endoscopic harvest time was (45?20) minutes. There was no complication (incision infection, edema of lower extremity, lymphangitis and fat necrosis) occurred. Stay in bed time was 2~3 days. There was no difference in result of light and electro-microscopy. Conclusion Endoscopic vein harvesting in coronary artery bypass grafting can decreases the prevalence of postoperative leg-wound infections, postoperative pain, lying time and hospital stay, and increase the postoperative mobility ability, especially in patient with obesity and diabetes mellitus. Furthermore, the endoscopic harvesting technique may do no additional trauma to the saphenous vein.
