1.Diagnostic value of serum thyroglobulin autoantibody on recurrence and/or metastasis following surgery in patients with differentiated thyroid carcinoma

Objective To evaluate the clinical significance of serum thyroglobulin autoantibody(TGAb) in thyroglobulin(TG)-negative and TGAb-positive patients with differentiated thyroid carcinoma(DTC)after thyroid ablation and ascertain the optimal operating point(OOP)of TGAb.Methods A total of 169 patients with histologically confirmed DTC and thyroid remnant ablation showed TG-negative and TGAb-positive results which were assessed by electrochemiluminescence immunoassay(ECLIA).The cases were divided into group A(59 cases)and group B(110 cases)with or without evidence of recurrence or metastasis,respectively.The receiver operating characteristic(ROC)curve and positive likelihood ratio of different threshold values were analysed according to their serum TGAb level.Results Serum TGAb level(1368?1343)IU/ml in group A was significantly higher than that(154?539)IU/ml in group B(P1000 IU/ml was 1.12 times that of 204 IU/ml≤TGAb≤1000 IU/ml,4.03 times that of 100 IU/ml≤TGAb
2.Evaluation of thyroglobulin mRNA in peripheral blood of patients with differentiated thyroid carcinoma after remnant ablation

Objective The aim of this study was to evaluate the diagnostic value of thyroglobulin(Tg)mRNA in peripheral blood samples of patients with differentiated thyroid carcinoma(DTC)after remnant ablation.Methods Tg mRNA Was detected by reverse transcription-polymerase chain reaction(RT-PCR)in peripheral blood of 162 patients.SPSS 13.0 was used for date analysis.Results The Tg mRNA assay had higher sensitivity than the conventional serum Tg measurement[86.61%(97/112)vs 53.57%(60/112),X2=29.153,P<0.001]which Was the"gold standard"for the identification of recurrence or metastases.In the anti-Tg antibodies(TgAb)positive DTC patients,the sensitivity of Tg mRNA was higher than that of serum Tg[86.54%(45/52)vs 0]in identifying recurrent or metastatic thyroid disease(X2=79.322,P<0.001).There was a significantly positive correlation between Tg mRNA expression and the clinical stages(Kendall correlation coefficient=0.515,P<0.001).There was no difference of Tg mRNA expression in peripheral blood among ages,sex,pathological types and location of metastases,respectively(Kendall correlation coefficient=0.020,0.069,0.050 and 0.028;all P>0.05).Conclusions Circulating Tg mRNA is a more sensitive marker than serum Tg in identifying recurrent or metastatic DTC,particulady in patients during levo-thyroxine(l-T4)therapy and with TgAb-positive.The DTC patients who have positive expression of Tg mRNA indicates poor prognosis.
3.Relationship and conversion between persisters and antimicrobial resistant bacteria

Persisters are a sub-population of bacteria that can survive lethal concentrations of antibiotics.They contribute to recurrent or intractable chronic infections.Persisters can emerge as a result of multiple mechanisms which lead to drug tolerance.Unlike antimicrobial resistance which usually acquires genetic mutation, phenotypes of persisters are non-inheritable.Despite the distinct difference between persistence and resistance, recent studies have demonstrated that not only persistence can be observed in resistant mutants, but also persisters themselves can be regarded as an intermediate state which promotes the emergence of resistance.This review focuses on mechanisms of drug tolerance in persisters, phenomena of persistence in antimicrobial resistant bacteria and possible mechanisms of the conversion of persisters to resistant bacteria.Moreover, future research directions are also prospected is this review.
4.ANALYSIS OF PYROLYSIS GAS CHROMATOGRAPHY OF SALMONELLA TYPHI

The chromatograms of pyrolysis gas chromatography (PGC) of fifteen strains of Salmonella typhi were reported to see if there is any possible relation of the graphs to serological typing and drug-resistance spectra of the bact. In the fifteen strains there were fourteen clinical strains and one reference strain (Hg01). The results indicated that the clinical strains might be divided into group A and B (7 strains each) by PGC. The graphs drawn from group A organisms were entirely the same in shape while those of group B showed only basic similarity. According to the diversity of graphs of group B organisms, the group B organisms, may be subdivided into group B1 (4 strains), B2 (2 strains) and B3 (1 strain). The graphs within each subgroup were completely the same and those of subgroup B1 exhibited close similarity to that of the reference strain. Albeit no regular connection of chromatograms with serological types and drug-resistance could be seen, the graphs yet provided useful reference for founding a bank of chromatograms, compaed with which salmonella typhi graph could then be identified.
5.Treatment of multiple level noncontiguous thoracolumbar fractures with posterior pedicle screw

Objective To explore the clinical effect of posterior pedicle screw internal fixation in the treatment of multiple level noncontiguous thoracolumbar fractures.Methods Thirteen patients with multiple level noncontiguous thoracolumbar fractures were treated by posterior pedicle screw internal fixation.The Frankel score,percentage of vertebral compression and Cobb angle of the injured vertebral segment were analyzed to evaluate the surgery efficacy.Results All patients were followed up from 12 to 24 months ( averaged 15 months ).All cases achieved bone fusion with no implant failure.The Cobb angle of the injured vertebral segment was corrected from preoperative(22.2 ±5.3) degree to postoperative(5.3 ±3.5) degree and(6.2 ±3.6) degree at the last follow up.The percentage of vertebral compression was corrected from preoperative (45.7 ± 14.1 )% to postoperative ( 6.1 ± 3.8 ) % and ( 7.2 ± 3.9 ) % at the last follow up.All improvements showed significant differences when compared statistically( t =15.03,t =12.05,Ps <0.05 ).The spinal cord function was improved 1 to 2 degree in all patients except 2 patients of grade A.Conclusion The posterior fixation with pedicle screw is a secure,safe and effective method in treating multiple level noncontiguous thoracolumbar fracture.

8.Progress of recurrence of venous thromboembolism in cancer patients

Cancer patients complicated with venous thromboembolism (VTE) face treatment problems, very high recurrence rate and bleeding rate. Cancer itself and the antitumor treatment plan are the important factors causing the recurrence. After the relapse, the prognosis of the patients is extremely bad, and more than 50% of the patients died within 1 year. Until now, there is no unified effective treatment plan for the relapse. This paper reviews the recurrence of VTE and the related research progress.
9.Clinical practice of video-assisted thoracoscopic surgery in children.

OBJECTIVE To estimate the value of video-assisted thoracoscopic surgery (VATS) in diagnosis and treatment of children with chest diseases. METHODS From May 1997 to October 2001, forty-one children (25 boys and 16 girls) with chest diseases received VATS under general anesthesia in our hospital. Their average age was 6.9 years (range 9 days to 16 years) and their median body weight was 22.5 kg (2.8-54.0 kg). Operative procedures included fibrinopurulent empyema with debridement and decortication in 15 children, biopsy and(or) resection of mediastinal tumor in 11, bullectomy and cystectomy of the lung in 6, lobectomy with huge cyst of the lung or sequestration in 5, clearance of hemothorax in 2, and exploration, and repair of diaphragmatic hernia in 2. RESULTS The mean operative time was 74 minutes (range 30 to 220 minutes). The lost blood volume was 33 ml (range 10 to 150 ml). The mean duration of chest drainage and hospital stay after surgery was 2.4 days and 7.0 days (range 4 to 15 days) respectively. One infant born after 9 days with congenital diaphragmatocele died of respiratory failure due to left pulmonary hypoplasia 10 days after operation. Postoperative morbidity was 7.3% (3 patients). Forty patients were followed up for an average of 15.6 months, 38 patients lived and developed normally, and 2 received chemotherapy. CONCLUSION Video-assisted thoracoscopy is a safe and effective diagnostic and therapeutic procedure for children with chest disease, and this approach has an important place in pediatric thoracic surgery.

10.Clinical study on velopharyngeal function after maxillary advancement.

OBJECTIVETo explore possible alterations in velopharyngeal function after maxillary advancement.
METHODSTen patients (3 secondary deformity of cleft palate, 7 maxillary retrusion) had maxillary advancement surgery. Pre- and post-operative examinations consisted of lateral cephalometric radiography, nasopharyngoscopy and speech recording.
RESULTSThere was no significant changes on speech or velopharyngeal competence after maxillary advancement. Cleft palate patients appeared slight hypernasality and nasal emission.
CONCLUSIONSGood velopharyngeal function pre-operation is not affected after the surgery of maxillary advancement.
