1.Research of MSCT scan postprocessing on Focal Nodular Hyperplasia

Objective To investigate and evaluate the pathological features and diagnostic significance of MSCT findings in scan postprocessing for focal nodular hyperplasia (FNH).Methods A total of 24 patients with FNH who underwent MSCT scan post processing were investigated.The FNH were pathologically and clinically confirmed.Results There are single focus in 22 cases and multiple focus in 2 cases.On plane scan 16 lesions were hypodensity and other 7 isodensity,4 hyperdensity( cases with fatty liver).After contrast,17 lesions were evenly high enhanced on arterial phase;13 lesions showed still hyperdensity and other 10 lesions isodensity,4 lesions hypodensity on parenchymatous phase.Only 5 lesions were hyperdensity but 9 lesions isodensity and 13 lesions hypodensity on delayed phase.Twelve lesions in 27 were detected with central stella-formed cicatrix.There were thickening tumor feeding arteries in 24 lesions,the artery of which were entered integrated into parenchymatous lesions with soft course.Conclusion Synthesizing the charcteristic appearance on MSCT scan postprocessing,doctor could make correct diagnosis and differentiated diagnosis for FNH.
2.Inhibition of microRNA195 attenuates high-glucose induced neonatal cardiomyocytes hypertrophy in vitro.

OBJECTIVETo investigate the effects of micro(mi) RNA-195 on high-glucose induced neonatal cardiomyocyte hypertrophy and to explore the related mechanism.
METHODSThe potential target gene of miRNA-195 (Smad7) was predicted by TargetScan5. 1 software. Cardiomyocytes were isolated from neonatal SD rats and cells were then randomly divided into three groups: cells treated by culture medium containing 5 mmol/L glucose (control group) , by culture medium containing 25 mmol/L glucose (high glucose group) and treated by culture medium containing 25 mmol/L glucose and miRNA-195 inhibitor transfection (miRNA-195 inhibitor group). After 24, 48, or 72 h of in vitro culture, the morphology of cardiomyocytes was examined under phase contrast microscope. Micrographs were captured and the cell surface was calculated. The mRNA expressions of miRNA-195 and myosin heavy chain β (β-MHC), a biomarker for cardiomyocyte hypertrophy, in cardiomyocytes were detected by RT-PCR. The protein expression of Smad7 was determined by Western blot. The concentration of transforming growth factor-β1 (TGF-β1) in the supernatant of culture medium was measured by ELISA.
RESULTSCross-sectional area of cardiomyocytes, expression of miRNA-195 and β-MHC and secretion of TGF-β1 were significantly increased in high glucose-treated cells (P < 0.05 vs. normal control). The protein expression of Smad7 was significantly downregulated in cells exposed to high glucose for 48 h (P < 0.05 vs. normal control). Downregulation of miRNA-195 partly reversed the high glucose-induced effects. The expression of Smad7 was negatively correlated with miRNA-195 in high glucose control group (correlation coefficient: -0.945, P < 0.05).
CONCLUSIONOur results demonstrate that Smad7 could be the target gene of miRNA-195. miRNA-195 might play a crucial role in the development and progression of diabetic cardiomyopathy possibly through downregulating the expression of Smad7 and modulating TGF-β/Smad pathways.

3.Perospirone versus Risperidone in the Treatment of Schizophrenia: A Systematic Review

OBJECTIVE:To systematically evaluate the therapeutic efficacy and safety of perospirone versus risperidone in the treatment of schizophrenia,and to provide evidence-based reference for clinical treatment.METHODS:Retrieved from Wanfang database,VIP,CJFD,CBM and PubMed,randomized controlled trials (RCTs) of perospirone(trial group) vs.risperidone (control group) in the treatment of schizophrenia were collected.After data extraction and quality evaluation of included studies according to modified Jadad evaluation criteria,Meta-analysis of response indexes was performed by using Rev Man 5.0 statistical software.RESULTS:A total of 12 RCTs were included,involving 1 050 patients.Results of Meta-analysis showed,the cure rate of 2 groups had no statistical significance [OR=0.99,95%CI(075,1.30),P=0.93].The incidences of extrapyramidal reactions [OR=0.63,95%CI(0.43,0.92),P=0.02],the incidences of prolactin levels increasing [OR=0.23,95%CI(0.14,0.38),P<0.001] and the incidences of weight gain [OR=0.23,95%CI(0.13,0.39),P<0.001] in control group were significantly higher than trial group,with statistical significance (P<0.05).CONCLUSIONS:The therapeutic efficacy of perospirone and risperidone is similar in the treatment of schizophrenia,but the safety of perospirone is better than that of risperidone.
4.Evidence-based Pharmacoecomic Evaluation of Venlafaxine and Escitalopram in the Treatment of Depres-sive Disorder

OBJECTIVE:To systematically evaluate the pharnacoeconomic effect of venlafaxine and escitalopram in the treat-ment of depressive disorders. METHODS:Usingvenlafaxineescitalopramdepressiondepressive episodeantidepressantas key words,retrieved from CNKI,VIP and Wanfang database during base creation date to Dec. 2015,related literatures about venlafaxine and escitalopram in the treatment of depressive disorders were selected. The cure rate of two therapy plans for depres-sive disorders and HAMD score were analyzed statistically by cost-effectiveness analysis of pharnacoeconomics. RESULTS:There was no statistical significance in cure rate and HAMD score between 2 groups(P>0.05),so minimum-cost analysis was adopted. The costs of venlafaxine and escitalopram in the treatment of depressive disorders were 772.03 yuan and 935.77 yuan. CONCLU-SIONS:Therapeutic efficacy of venlafaxine is similar to that of escitalopram in the treatment of depressive disorders,but the for-mer scheme is more economical.
5.Systematic review of tandospirone and buspirone in treatment of patients with generalized anxiety disorder

Objective To evaluate the therapeutic effect and safety of tandospirone and buspirone in the treatment of generalized anxiety disorder.Methods The clinical control study oftandospirone and buspirone in the treatment of generalized anxiety disorder was earched by PubMed,CBM,CNKI,VIP,and Wanfang Data,with deadline from January 2000 to June 2016.Meta-analysis was carried out using RevMan5.0 software to each effect index.Results A total of seven RCTs were included involving 615 patients were identified.Meta-analysis showed that the significant efficiency rate[P=0.34,OR=1.19,95%CI (0.83~1.69)] and HAMA[P=0.80,MD=-0.08,95%CI (-0.72~0.56)] of tandospirone and buspirone had no significant difference.Two groups of adverse drug reactions,dizzy,dry,constipine,insomnia,anorexia,and nausea had no significant difference (P>0.05).Conclusion The efficacy and adverse reaction oftandospirone and buspirone in the treatment of generalized anxiety disorder are fairly.
6.Development and evaluation of questionnaire of mental health knowledge for non-psychiatric clinicians

Objective: To develop the mental health knowledge questionnaire in order to know about the awareness of mental health knowledge in non-psychiatric clinicians. Methods: A sample of the 641 non-psychiatric clinicians from 2 tertiary general hospitals, 2 second-class general hospitals, and 1 community hospital were used for item analysis and internal consistency test; 38 cases were selected for the re-test interval of 2 weeks to test the test-retest reliability. And totally 99 psychiatrists finished the survey as controls. The awareness rates between the non-psychiatric and psychliatric clinicians were compared. Results: The questionnaire included 46 items and consis-ted of 6 subscales, involving anxiety, schizophrenia, depression, dyssomnia and psychological counseling, or-ganic brain disorder, and recognition for mental health service situation in general hospital The results of confirma-tory factor analysis showed that the ratio of chi-square and degree of freedom was below 3. The fitting indexes except GFI and AGFI were more than 0.90. The Cronbach's alpha coefficients ranged from 0.73 to 0.89, and test-retest Pearson correlation coefficient of reliabifity ranged from 0.59 to 0.78. Awareness rate was 47.6% in general hospital and 90.9% in psychiatric hospitals. Conclusion: The mental health knowledge questionnaire has good psychometric properties and may be applied in clinical studies.
7.Correlation of vascular endothelial cell dysfunction to vascular restenosis following coronary artery stent implantation

BACKGROUND: Dysfunction of endothelial cells is independently associated with coronary atherosclerotic heart disease. Correlation of dysfunction of endothelial cells to restenosis after stent implantation is not yet clearly determined.OBJECTIVE: To evaluate the correlation of vascular endothelial dysfunction to restenosis after stent implantation. DESIGN, TIME AND SETTING: A case control study was performed at the Department of Cardiology and Department of Heart Ultrasound, Sichuan People's Hospital from March 2005 to January 2007.PARTICIPANTS: After review, coronary angiography showed that 11 patients who occurred restenosis at the lesion region after stent implantation were included in a restenosis group, and an additional 15 patients who did not develop in-stent restenosis were included in a control group. Patients in the following conditions were excluded: over 70 years old, histories of long-term smoking, diabetes mellitus, multivessel disease, long coronary lesion and chronic total occlusion, heart failure (Killip's class Ⅲ or above), severe hepatic and/or renal insufficiency.METHODS: High-resolution ultrasound was used to assess the percentage of flow-mediated dilatation of brachial artery. Differences in endothelial function were compared between both groups.MAIN OUTCOME MEASURES: Ultrasound parameter of the brachial artery in both groups; Partial correlation analysis on control variable including sex, age, blood lipid level, diseased region and stent type.RESULTS: No significant difference was found in basic diameter of brachial artery in both groups. During reactive hyperemia, inner diameter and its absolute variation of brachial artery were smaller in the restenosis group than in the control group (P<0.01). The percentage of brachial artery flow-mediated dilatation was lower in the restenosis group compared with the control group (P=0.013). The partial correlation coefficient between the percentage of flow-mediated dilatation of brachial artery and in-stent restenosis was 0.47 (P=0.04).CONCLUSION: The endothelial dysfunction significantly decreases in patients with restenosis compared with controls following stent implantation. There is a correlation between endothelial dysfunction and restenosis.

Mitochondria play a key role in various cell processes including ATP production, Ca homeostasis, reactive oxygen species (ROS) generation, and apoptosis. The selective removal of impaired mitochondria by autophagosome is known as mitophagy. Cerebral ischemia is a common form of stroke caused by insufficient blood supply to the brain. Emerging evidence suggests that mitophagy plays important roles in the pathophysiological process of cerebral ischemia. This review focuses on the relationship between ischemic brain injury and mitophagy. Based on the latest research, it describes how the signaling pathways of mitophagy appear to be involved in cerebral ischemia.

10.Evaluation of Liver Metastases Using Contrast-Enhanced Ultrasound: Enhancement Patterns and Influencing Factors.

BACKGROUND/AIMS: To evaluate the enhancement patterns of liver metastases and their influencing factors using dynamic contrast-enhanced ultrasound (CEUS). METHODS: A total of 240 patients (139 male and 101 female; 58.5±11.2 years of age) diagnosed with liver metastases in our hospital were enrolled in this study to evaluate tumor characteristics using CEUS. A comparison of enhancement patterns with tumor size and primary tumor type was performed using the chi-square test. The differences between quantitative variables were evaluated with the independent-sample t-test and one-way analysis of variance. RESULTS: The enhancement patterns of liver metastases on CEUS were categorized as diffuse homogeneous hyperenhancement (133/240, 55.4%), rim-like hyperenhancement (80/240, 33.3%), heterogeneous hyperenhancement (10/240, 4.2%), and isoenhancement (17/240, 7.1%). There were significant differences in the enhancement patterns during the arterial phase based on the nodule size (p=0.001). A total of 231 of the nodules showed complete washout during the portal phase, and 237 nodules were hypoenhanced during the delayed phase. The washout time was correlated with tumor vascularity, with a longer washout time observed in hypervascular metastases compared to hypovascular metastases (p=0.033). CONCLUSIONS: Diffuse homogeneous hyperenhancement followed by rapid washout was the most common enhancement pattern of liver metastases on CEUS and was affected by the nodule size and tumor vascularity. Small metastases were prone to show diffuse homogeneous hyperenhancement. Hyper-vascular metastases showed a significantly longer washout time compared to hypovascular metastases.
