1.Clinical Observation on Rheumatoid Arthritis Treated by Bushen Tongluo Pill:A Report of 54 Cases

Objective To observe the therapeutic effect of Bushen Tongluo Pill (Pills for reinforcing kidney and removing obstruction of meridians) on rheumatoid arthritis (RA).Methods Totally 100 RA patients were randomized into treatment group and control group with 50 cases in each group.The treatment group (50 cases) was administered Bushen Tongluo Pill and the control group (50 cases) was prescribed Celecoxib capsules and Methotrexate tablets for 3 months.The changes after treatment in the clinical symptoms including joint pain,swelling,joint functions,hand grip strength,and in the laboratory indices including erythrocyte sedimentation rate (ESR),rheumatoid factors (RF),and C-reactive protein (CRP),and the untoward effect as well were observed.Results The total effective rate of the treatment group was higher than that of the control (P0.05).Conclusion Bushen Tongluo Pill is effective for RA,the effect was superior to that of Celecoxib capsules and Methotrexate tablets.
2.Effect of Nalmefene Hydrochloride on Expressions of Pulmonary β-endorphin and Interleukin-17 in Rats with Lung Ischemia-reperfusion Injury

Objective To study the protective effect of nalmefene hydrochloride on lung ischemia-reperfusion injury and its mechanism.Methods 40 rats were randomly divided into model group,high dose of nalmefene group,low dose nalmefene group and sham operation group equally(n =10).The lung ischemia-reperfusion model was established by occlusion of the left pulmonary hilum.The intravenous injection of nalmefene (20,10 μg·kg-1) was applied at 10 minutes before occlusion of the left pulmonary hilum in the high dose of nalmefene group and the low dose of nalmefene group,respectively.The sham operation group without occlusion of the left pulmonary hilum was not given any treatment.At 2 h after reperfusion,all rats were detected arterial blood gas value and then sacrificed.The specimens from the upper lobe of the left lung tissue were preserved to observe pulmonary lesions,detect the ratio of wet / dry weight and the expressions of β-endorphin and interleukin(IL)-17.Results Compared with the model group,the value of PCO2,the degree of pulmonary lesions,the ratio of wet / dry weight and the expressions of β-endorphin and IL-17 in lung tissue were significantly decreased (P < 0.01),while the value of PO2 was significantly increased (P < 0.01) in the low dose of nalmefene group.Compared with the low dose of nalmefene group,thevalue of PCO2,the degree of pulmonary lesions,the ratio of wet/dry weight and the expressions of β-endorphin and IL-17 in lung tissue were significantly decreased (P < 0.01),while the value of PO2was significantly increased (P < 0.01) in the high dose of nalmefene group.Conclusion Nalmefene hydrochloride may prevent lung ischemia-reperfusion injury in a dose dependent manner by reducing the production of β-endorphin and inhibiting the expression of IL-17 in lung tissue.
3.Analysis of the monitor results of Marmota sibirica plague foci from 2004 to 2008

Objective To master of the characteristics of development and variation in plague natural foci of Marmota sibirica, and to analyze the plague monitoring results from 2004 to 2008. Methods From May to September every year, we monitored plague in Manzhouli, Chenbaerhuqi, Xinbaerhuzuoqi, Xinbaerhuyouqi,Ewenkeqi and Yakeshi city. The monitored area was 20 000 - 40 000 hm2 in every county. The density of Marmota,ground squirrel and moonlighting rats was surveyed by path method, one-day bow-clip method and a clamp was placed every five meters, respectively. The classification and identification of ectoparasite fleas were done by using low power lens. The fleas in the ground squirrel hole were collected by flannel rubber stick, then classified and identified. According to National Standard of Plague Diagnostic Criteria(GB 15991-1995), the collected rats and fleas were detected by isolation and cultured Yersinia pestis, the serums of collected rats were tested by indirect hemagglutination test. Results In five years, the density of Marmota and ground squirrel was 0.010/hm2 and 0.602/hm2, respectively. The capture rate of moonlighting rats was 2.69% (258/9600). The flea infection rate of Marmota was 17.54%(10/57) and the fleas index was 2.54. The fleas infection rate of ground squirrel was 28.40%(213/750) and the fleas index was 1.01. Flea infection rat of ground squirrel hole was 5.60%(46/822) and the fleas index was 0.17. The total number of various hosts of Yersinia pestis detected was 1351, the groups number of the variety of cultured fleas was 127, and the pathogen test results were negative. The number of serum tested was 1064, positive number was 43, and the detection rate was 4.04%(43/1064). The highest positive titer was 1 : 1280. Other than 2006, the remaining four years were found positive for blood clotting material; positive serum was found in a total of three regions, they were Manzhouli, Xinbaerhuzuoqi, and Xinbaerhuyouqi; and 30, 12 and 1 copies of hemagglutination-positive sera were detected. Conclusions The epidemic of plague natural foci of Marmota sibirica is in a active state, and gradually expands the scope. We must continue to strengthen the inspection of the bacteria, bearing in mind the replacement of the region's main host, make every effort to prevent and control of human plague.
4.The clinical values of 64-slice computed tomography coronary angiography

Objective To evaluate the clinical values of(M-slice computed tomography coronary angiography(CTCA) in contrast to conventional coronary angiography(CCA).Methods Assessment of the major and minor branches of coronary arteries by 64-Slice CTCA,showed the information of 100 patients with 64-slice CTCA,comparing with CCA regarded as the golden standard recently implemented.Results Among the 100 patients,920 coronary arteries were evaluated with 370 lesions found.Of them ,348 lesions were consisted with CCA,22 lesions were falsely positive and 10 lesions were found by CCA not picked up by 64-slice CTCA.The sensitivity,specificity,positive predict values and negative predict values were 97.2% ,96%,94% and 98.1% respectively for the detection of coronary artery disease(CAD),Further statistical analysis using a 2-related X~2 test showed no obvious difference in the discovery of CAD between 64-slice CTCA and CCA.Comparing both imaging modalities,there was concordance in assessing lesion severity in 311 lesions,and accuracy approaches 86.8%.Conclusions 64-slice CTCA has excellent sensitivity and specificity in detecting CAD,and also can give fairly accurate assessment of stenosis severity in most patients as compared to CCA.As a alternative method it is a useful non-invasive for detecting coronary arteries disease in symptomatic patients
6.Induction of BAG2 in proteasome inhibitor-induced apoptosis in human thyroid carcinoma cells

Background and purpose: For neoplasms with different sources, proteasome inhibitors can inhibit their growth and promote the cell apoptosis. Its mechanism may be associated with Bcl-2-associated athanogene 2 (BAG2). We aimed to investigate the involvement of in thyroid cancer cell death induced by proteasome inhibitors. Methods:A panel of thyroid cancer cells (ARO, FRO, KTC1, KTC2, KTC3, 8305C and 8505C) were treated with vehicle or proteasome inhibitor MG132;BAG2 mRNA and protein levels were analyzed using quantitative real-time polymerase chain reaction (qRT-PCR) and Western blot, respectively. Results:MTT results indicated that FRO and KTC2 cell lines were the most sensitive to proteasome inhibitors. MG132 induced BAG2 mRNA and protein expression in the panel of thyroid cancer cells with various degree (P<0.01), for FRO and KTC2 cell line, the BAG2 mRNA level was increased in 20 to 25 times, meanwhile the protein expression level also increased signiifcantly;Sensitive FRO cells demonstrated quicker induction of BAG2 compared with insensitive ARO cells. Moreover, the extents of BAG2 induction in FRO cells were higher than that in ARO cells. Conclusion: BAG2 is a novel molecule induced by proteasome inhibitor, which might function as a proapoptotic molecule in thyroid cancer cell death mediated by proteasome inhibitor.
7.Oral acyclovir induced acute renal failure

BACKGROUND: The study aimed to investigate the clinical characteristics of acute renal failure (ARF) caused by oral acyclovir. METHODS: A 45-year-old Chinese male patient with acyclovir-induced ARF suffered from abdominal pain for one day. The pain was extended to the epigastric area from the right lower quadrant. Transient oliguria was seen in addition to microscopic hematuria and proteinuria. The serum creatinine concentration was 304 μmol/L. Eight days before the occurrence of ARF, the patient took oral acyclovir for facial neuritis. RESULTS: His renal function was restored completely following the discontinuation of acyclovir, with continuous renal replacement therapy for 54 hours and some symptomatic treatment. CONCLUSION: The presentation of acute renal failure caused by acyclovir can be diverse, but the prognosis is good after active treatment.
8.Efficacy and safety of fixed combination of nitrendipine and atenolol in treatment for patients with mild to moderate essential hypertension and their optimal dosage matching

Objective To evaluate efficacy and safety of fixed combination of nitrendipine and atenolol in treatment for patients with mild to moderate essential hypertension and their optimal dosage matching.Methods Totally,275 patients with essential hypertension were selcted from seven hospitals in Shanghai,Nanjing and Suzhou,China and randomized into five groups with same proportional probability in a double-blind,double-dummy,parallel active-controlled,multi-center clinical trial,receiving fixed combination of nitrendipine and atenolol at three different dosage matching (nitrendipine/atenolol 5/12.5 mg,5/10 mg,5/7.5 mg for groups 1,2 and 3),and nitrendipine (10 mg for group 4) or atenolol (25 mg for group 5),respectively for eight weeks.Results Mean reduction of diastolic blood pressure (DBP)was (17±7) mm Hg,(18±9) mm Hg and (17±7) mm Hg for groupl,2 and 3,respectively from the baseline,significantly greater than that in groups 4 and 5[(13±7) mm Hg and (12±6) mm Hg,respectively].Mean reduction of systolic blood pressure (SBP) was (21 ±11)mm Hg,(24±12) mm Hg,(23±11) mm Hg,(19±13) mm Hg and (18±9) mm Hg,respectively for the five groups from the baseline,and the reduction in group 2 was significantly greater than that in group 5,with an overall efficacy of 94.4%,98.1% and 88.2% for groups 1,2 and 3,respectively,all statistically higher than that in group 5 (71.4%) with P<0.01,eight weeks after treatment.The ratio of patients with increased dose of antihypertensive agents in week 5 was lower in group 2 than that in the other four groups,with mild adverse reaction only,no obvious change in laboratory biochemical examinations,and no needs in special management.Conclusions Fixed combination of atenolol and nitrendipine with an optimal doses of 5 mg and 10 mg respepctively was effective and safe for mild and moderate hypertension with good tolerance.
9.Health related quality of life in patients with early Parkinson' s disease

Objective To investigate the characteristics of health related quality of life(HR-QOL) in Chinese patients with early Parkinson' s disease(PD), to identify the motor and non-motor factors that are associated with a poorer quality of life in patients with early PD. Methods All 391 patients with early PD were identified in a clinical-based study. Motor functions were measured by Unified Parkinson' s Disease Rating Scale (UPDRS) and Hoehn-Yabr Scale. Non-motor variables were assessed by Center of Epidemiological Survey Depression Scale (CES-D) for depressive symptoms, Pittsburg Sleep Quality Index (PSQI) for sleep disturbance, Fatigue Severity Scale (FSS) for fatigue, Alzheimer' s Disease Assessment Scale-Cognitive Sections (ADAS-Cng) for cognitive function, and Constipation Severity Scale for constipation. HR-QOL was measured by SF-36. Motor and non-motor variables were collected at the baseline assessment of a clinical trial and determined during a structured interview and by clinical examination by movement disorder specialists. The results were compared with those in healthy elderly people. Multiple regression analyses were used to determine which variables were strongly associated with lower levels of quality of life. Results Patients with early PD had a lower score on all dimensions of SF-36, except bodily pain dimension. Motor factors, particular physical disability and disease severity, contributed to decreased HR-QOL, but to a lesser extent. The motor score of the UPDRS (23. 8±11.8), Hoehn-Yahr stage(2. 0± 0.7), together with the rigidity score (4.4 ± 3.1), only accounted for 18.9 % (R2=0. 189) of the variance of SF-36 total score. The variables that most strongly predicted a low total SF-36 score were non-motor factors, particularly depressive symptoms, sleep disorders and fatigue. When the CES-D, FSS, and PSQI score were included in the model, the R2 increased from 0. 189 to 0.617, indicating that 61.7% of the variance in HR-QOL could be explained if additional CES-D, FSS and PSQI scores were known. Depressive symptoms, as measured by CES-D, had an overwhelming impact on HR-QOL. When CES-D score was included, the R2 change was 0.433, which indicated that additional 43.3% of the variability in HR-QOL could be explained by adding depressive symptoms. Conclusions PD has a substantial impact on HR-QOL, even if in its early stage. Depressive symptoms, sleep disorders and fatigue correlated strongly with lower quality of life. Depressive symptoms appeared to be the strongest determinant of HR-QOL in early PD patients. Every effort should be made to recognize and treat these conditions, thus improving all aspects of PD and giving these patients as good a quality of life as possible.
10.Percutaneous endovascular treatment of below-the-knee arterial bifurcation lesions with coronary bifurcation stenting techniques

Objective To report the preliminary results of drug eluting stents (DES) in infrapopliteal bifurcation lesions after failed angioplasty.Methods From April 2011 to December 2013, 21 patients with critical limb ischemia and infrapopliteal atherosclerotic disease underwent DES placement in tibial bifurcation using 4 techniques: balloon and stent (single stent), T-shape double-stent, Crush double-stent and Culotte double-stent.Clinical and angiographic end points included: Amputation-free survival, target lesion revascularization (TLR)-free survival, target vessel revascularization (TVR)-free survival.Angiographic primary and secondary patency of DES, 2-vessel primary patency (2VPP), and 1-vessel primary patency (1VPP).Results Technical success was achieved in all patients (100%) without major intraoperative complications.ABI increased from 0.23 ± 0.07 to 0.67 ± 0.24 (P < 0.05) after 1 week.The mean follow-up intervals were 23 ± 7 months;amputation-free survival, TLR-free survival and TVR-free survival were 100%, 90.5% and 79.1%, respectively, at 1 year.8 patients underwent angiography reevaluation.The angiographic primary and secondary patency of DES were 62.5%, 87.5%, respectively.The primary patency of 2VVP and 1VVP were 28.6%, 57.1%, respectively at 1 year.Conclusions Bifurcation stenting techniques utilized in coronary arteries can be used in the infrapopliteal arteries.Although provisional stenting in the origin of tibial bifurcation cannot inhibit restenosis in distal lesions treated with balloon angioplasty, clinical outcomes and limb salvage rates are acceptable for these techniques.
